  • Objective : We performed this study to understand the trend of recent medical treatment on chronic prostatitis. Methods : We analyzed 20 manuscripts contributed to the Chinese medical journals from 1998 to 2004 that presenting report of recent medical treatment on chronic prostatitis. Results : The results are summarized as follows 1. In clinical cases, we investigate the cause of chronic prostitis was 'ha cho sup yol' 'ki wool' and 'hyul er'. also it combined complex causes. 2. Deep stimulation acu-therapy near prostate was more effective than other acu therapy. 3. External medical treatment was very efficient than other therapy that was rectal injection therapy, fumigation therapy etc. Conclusion : We conclusion that Rectal Injection therapy was clinical effect on chronic prostatitis.

  • Purpose : This rearch aims to prove effects of an oriental medical complex therapy on vitiligo patients who have the symptoms in the hands. Method : The rearch conducted its investigation targeting these four patients who have the symptoms in the hands. The study resolved the findings through the comparison on the pictures before and after the therapy. Result : After making a comparison on the pictures, the study confirmed positive changes in the conditions of the patients. The oriental medical complex therapy turned out to be effective enough to treat the patients who have the symptoms in the hands. Conclusion : Such result leads the study to a conclusion that the oriental medical complex therapy can be an effective method to treat these patients who have the symptoms in the hands. In addition, more case studies and research will be necessary.

  • We present a case of intracranial aneurysm located in the P1 segment of left posterior cerebral artery in the context of tetralogy of Fallot. Complex variations included right aortic arch with abnormal branching. Also, the bilateral vertebral arteries were absent, with a type I persistent proatlantal intersegmental artery of the left side. The aneurysm was treated with endovascular intervention with a Tubridge flow diverter and was noted to be completely cured on 6-month follow-up. We discuss the many considerations in this patient including developmental and modern-era treatment.

Complex Korean Medical Treatment of Postoperative Ankylosis in Septic Arthritis of the Knee: A Case Report (화농성 무릎관절염의 수술 후 관절강직에 대한 한의복합치료: 증례보고)

  • A 50-year-old Korean male tour guide without any medical or family history complained of left knee pain. After receiving umbilical cord blood injection treatment, the pain gradually worsened. After being diagnosed with septic arthritis in the left knee, arthroscopic lavage, debridement, antibiotic treatment, and routine rehabilitation therapy were performed, but the symptoms persisted. In the hospital, acupuncture, pharmacopuncture, acupotomy, Chuna manual therapy, and cupping therapy were performed in addition to the usual treatment for 59 days. To evaluate the patient's improvement, the numeric rating scale, EuroQol 5-dimension, pain disability index, and Lysholm knee scoring system were used. After treatment, the symptoms improved in all assessment tools, swelling, and range of motion of the joint. Through this study, it was found that complex Korean medical therapies may be effective for postoperative ankylosis in septic arthritis of the knee, and further studies are needed to clarify the therapeutic effect.

Primary Total Elbow Replacement for Treatment of Complex Distal Humerus Fracture: Outcomes of Short-term Follow-up

  • Background: High complication rate after open reduction and internal fixation can lead to use of primary total elbow replacement (TER) in treatment of complex distal humerus fractures in elderly patients. The purpose of this study was to investigate the short-term outcomes and complications after primary TER in patients with complex distal humerus fracture. Methods: Nine patients with acute complex distal humerus fracture were treated by primary TER using the semiconstrained Coonrad-Morrey prosthesis. The mean age of patients was 72.7 years (range, 63-85 years). Clinical and radiographic outcomes were evaluated over a mean follow-up of 29.0 months (range, 12-65 months) using visual analog scale (VAS) score for pain; Mayo elbow performance score (MEPS); Quick Disabilities of the Arm, Shoulder, and Hand (Quick-DASH) score; and serial plain radiographs. Complications were also evaluated. Results: At the final follow-up, mean VAS, MEPS, and Quick-DASH scores were 1.2, 80.5, and 20, respectively. The mean range of motion was 127.7° of flexion, 13.8° of extension, 73.3° of pronation, and 74.4° of supination. There was no evidence of bushing wear or high-grade implant loosening on serial plain radiographs. Three complications (33.3%) comprising two periprosthetic fractures and one ulnar neuropathy were observed. Conclusions: Primary TER for treatment of complex distal humerus fractures in elderly patients yielded satisfactory short-term outcomes. However, surgeons should consider the high complication rate after primary TER.

  • Idiopathic scoliosis is a complex three-dimensional deformity of the spine. There is two ways in treatment of scoliosis, normally. In order to prevent a progressive course of the disorder, conservative treatment is started in selected patients. And sometimes, operation is performed to severe patients. But, application standard is vague. So We reviewed the latest medical theses. English language articles in Medline database between 2003 and 2005. 8. 31. were reviewed. The searching words was Idiopathic scoliosis, conservative treatment, brace. exercise. We classified 27 articles. The results of this research demonstrate that conservative treatment is more effective than observation until operation.

Effect of Complex Korean Medicine Treatment on Obese Patient: A Retrospective Study (비만 환자의 한의복합치료 후 비만 개선 효과: 후향적 연구)

  • Objectives: The purpose of this study was to observe the effect of improving obesity after 4 week complex Korean medicine treatment in obese patients by retrospectively analyzing. Methods: A retrospective chart review was conducted on 44 patients who received obesity treatment at 23 Korean medicine clinics in Daejeon city. We analyzed weight, body mass index (BMI), body fat percentage, body fat mass, muscle mass, hip and waist circumference, EuroQol-5D (EQ-5D), Korean version of obesity-related quality of life scale (KOQOL), and satisfaction surveys at 4 and 8 weeks after treatment. Results: The weight, BMI change after 4 weeks and 8 weeks was decreased significantly. In addition, there were significant differences in fat mass, muscle mass, waist circumference, and hip circumference. Life quality scale EQ-5D and KOQOL showed decreased significantly. In the evaluation of overall satisfaction, safety, help level, recommendation intention, and revisit intention, more than 89% answered positively that they were satisfied. There were no serious side effects. Conclusions: It was significant effect on weight loss and obesity improvement after complex Korean medicine. And quality of life evaluation and satisfaction survey also show positive results.

Effect of Bee Venom Acupuncture Complex Therapy of Thoracolumbar Compression Fracture (흉(胸).요추압박골절(腰椎壓迫骨折)에 대한 봉약침(蜂藥鍼) 복합치료(複合治療) 효과(效果))

  • Methods : We investigated 28 cases of patients with thoracolumbar compression fracture. We divided patients into two groups : We treated one group with complex oriental medical treatment focusing on Bee Venom Acupuncture therapy, and did the other group with complex oriental medical treatment focusing on acupuncture therapy. To investigate the effectiveness of treatment applied for two groups, we used Visual Analog Scale(VAS) and Oswestry Low-back Pain Disability Index(ODI). We compared the VAS score and ODI score of two groups statistically. Results : 1. Female is prominent in distribution of sex, the average age bracket is 70's, and the largest group was Faccidental fall in the distribution of cause. 2. In regard to level of injury, T12 was found to be most prominent, followed in turn by L1, L2. In regard to grade of clinical symptoms, Grade III was most prominent, followed by Grade IV, Grade II. 3. As a result of evaluation by using VAS score and ODI score, treatment score at final was significantly different from that at the baseline in each group. 4. After treatment, Bee Venom acupuncture therapy group showed significant difference on VAS score and ODI score compared with acupuncture therapy group. Conclusions : Bee Venom acupuncture therapy can be available for relieving symptoms related with thoracolumbar compression fracture.

Experimental Evaluation of Proton Dose Calculations in Phantoms Simulating a Clinical Heterogeneity in Patients

  • In a treatment planning for actual patients with a complex internal structure, we often expect that proton beams, which pass through both a bolus and the heterogeneity in body, will form complex dose distributions. Therefore, the accuracy of the calculated dose distributions has to be verified for such a complex object. Then dose distributions formed by proton beams passing through both the bolus and phantoms simulating a clinical heterogeneity in patients were measured using a silicon semiconductor detector. The calculated results by the range-modulated pencil beam algorithm (RMPBA) produced large errors compared with the measured dose distributions since dose calculation using the RMPBA could not predict accurately the edge-scattering effect both in the bolus and in clinical heterogeneous phantoms. On the other hand, in spite of this troublesome heterogeneity, calculated results by the simplified Monte Carlo (SMC) method reproduced the experimental ones well. It is obvious that the dose-calculations by the SMC method will be more useful for application to the treatment planning for proton therapy.

Low doses of amitriptyline, pregabalin, and gabapentin are preferred for management of neuropathic pain in India: is there a need for revisiting dosing recommendations?

  • Background: Current therapy for the treatment of neuropathic pain is often unsatisfactory. Considerable variation in treatment pattern still exists in spite of availability of sufficient literature from various guidelines. Recent Indian market data suggested that the utilization (sale) of drugs such as amitriptyline, pregabalin, and gabapentin was more for low-dose unit packs than that of the high-dose unit packs, raising the belief that these drugs are prescribed at a lower dose than is actually recommended in the guidelines. To test this hypothesis, a survey was conducted across speciality throughout the country to observe the prescription pattern of these drugs amongst the health care providers in India. Methods: Three hundred fifty survey forms were distributed of which 281 forms were included for analysis. Results: It was observed that the commonly used initiation and maintenance dose for amitriptyline, pregabalin, and gabapentin was 5-10 mg/day, 50-75 mg/day, and 100-300 mg/day, respectively. The reason to select the lower dosages was to have a balancing effect to achieve good efficacy with minimum side effects. Care-givers reported no side effects/not many side effects as a reason in 22.2%, 16.88%, and 23.86% patients with amitriptyline, pregabalin, and gabapentin, respectively. Sedation and giddiness were commonly reported with all three drugs. Conclusions: Commonly prescribed drugs for management of neuropathic pain, such as amitriptyline, pregabalin, and gabapentin are preferred at lower doses in Indian clinical settings. Acceptable efficacy and low tolerance to the standard dosage is believed to be the reason behind the prescribed dose.
