통합의학의 정의 고찰 및 국내 관련 연구 분석 (A Study on Definition of Integrative Medicine and Related Research Analysis in Korea)

  • Objective: Interests in integrative medicine and CAM(Complementary and Alternative Medicine) are on the rise. However, the term 'integrative medicine' is often confused with the concept of CAM. This paper aims at clarifying the concept of integrative medicine. Methods: The definition, concept, and research methods used to explain integrative medicine have been inquired into. Also, the differences between the researches on integrative medicine and those on CAM have been studied. Moreover, domestic researches regarding integrative medicine have been analyzed. Results: The concept of integrative medicine was born in order to create a more patient-centered medicine by actively accommodating the medical system classified as CAM. The biggest difference that discriminates integrative medicine from CAM is the fact that integrative medicine does not depend solely on establishing its grounds through the process of RCT(Randomized Controlled Trial). Conclusion: Domestic researches regarding integrative medicine naturally leads to the concept of unification of medicine, but they do not concern the original meaning of integrative medicine. It is only after active discussions on the meaning, philosophical backgrounds, and research methods of integrative medicine become possible that a truly patient-oriented medicine will be realized.

사망전 노인의 전체보건의료비용에서 보완대체요법 비용과 비용분율에 관한 연구 (A Study on the Cost and Proportion of Complementary and Alternative Medicine in Total Healthcare Cost among Elderly in the Last 6 Months of Life)

  • Objectives : To evaluate the cost and proportion of complementary and alternative medicines (CAM) in the total healthcare costs among the elderly in the last 6 months of life. Methods : The care-giving families of 301 persons older than 65 years, who died between July 1st and December 31st of 2001, and were also registered in Self-Employed Health Insurance Programs in Seoul, were interviewed. Results : The cost of CAM was 1.09 million Won, which as a proportion of the total healthcare cost was 38.1%. The elderly aged between 65 and 69 year-old, male, living with their spouse, Buddhist and having cancers had higher CAM costs in an ANOVA and simple regression analysis. After controlling of various factors, age was the only significant factor associated with the cost of CAM. The elderly above 80 years old, female, bereaved and Buddhist had higher proportional CAM costs, and the elderly having cancers or cardiovascular diseases had lower proportional CAM costs in an ANOVA and simple regression analysis. After adjusting for various factors, the elderly above 85 years old, female and Buddhist had higher proportional CAM costs, and the elderly having cancers had lower proportional CAM costs. Conclusion : The very old and Buddhist, and/or the ill with no clear diagnosis, may depend more on CAM. Further research will be needed on the meaning and impact of CAM and their costs to public health and the total healthcare system.

세계 전통의학 동향과 주요 한의학 R&D 분야에 관한 연구 (A Study on the Trend of World Traditional Medicine and Key Area of Traditional Korean Medicine(TKM) R&D)

  • The medical and healthy paradigm is changing by various reasons such as improving of life quality, the limit of western medicine, demographic changes and so on. And the demand, interest and market size of traditional medicine(TM) and complementary and alternative medicine(CAM) are increasing continuously and there is a good traditional medicine called TKM(Traditional Korean Medicine) in Korea. TKM have differentiated characteristics like nature-friendly, systematic, preventive and personalized medicine from western medicine. TKM with these characteristics have possibility to increase national competitiveness and contribute to growth of economy. At this time, it is necessary to select key area of TKM R&D to increase competitiveness of TKM and to create new growth engine. In this paper, we suggest key area of TKM R&D through SWOT analysis, the analysis of world traditional medicine and expert's counsel. As a result, 3 key area and 10 specific fields of TKM R&D was selected, which they are 'the standardization for TKM technology and technique', 'the development of generic technology based on TKM' and 'the establishment of infrastructure for TKM information and material'. This study may contribute to make plan for TKM research and policy such as TKM middle and long term plan and TKM promotion plan.

정신지체 및 자폐스펙트럼장애에서 다양한 치료방법의 사용실태 (Use of Various Treatment Modalities for Autism Spectrum Disorder and Mental Retardation)

  • Objectives : The purpose of this study was to investigate the use of various treatment modalities including pharmacotherapy, educational-behavioral therapy, and complementary alternative treatment for children with mental retardation (MR) or autism spectrum disorder (ASD) in Korea. Methods : The sample consisted of 50 parents who have children with MR (N=28) or ASD (N=22) : 38 boys, 12 girls ; mean age 14.06 (4.14) years old. A questionnaire was composed of the experienced modality, duration, cost, satisfaction, etc. Results : According to the results, 56.0%, 100.0%, and 36.0% of children with MR or ASD have experienced pharmacological treatment, educational-behavioral therapy and complementary alternative medicine (CAM), respectively. Children who experienced educational-behavioral therapy and CAM experienced 3.52 kinds of education-behavioral therapy and 2.78 kinds of CAM, respectively. Monthly cost of pharmacological treatment was lowest among three modality categories. Regarding treatment satisfaction by parental report, the lowest score was recorded for CAM. Conclusion : Parents who have a child with MR or ASD are trying many treatment modalities and feeling the burden of their treatment.

Use of Complementary and Alternative Medicine Among Breast Cancer Survivors in Taiwan

  • Aims: The purpose of this study was to investigate complementary and alternative medicine use among breast cancer survivors in Taiwan. Materials and Methods: This study employed a descriptive research design approach to detail the CAM use among the target population. Convenience sampling was used along with a structured questionnaire. Results: A total of 230 breast cancer survivors completed the use CAM scale. Prayer, reading books, taking antioxidants, eating various grains, and maintaining a vegetarian diet proved to be the five most frequently used CAM practices among patients in our study. More than 50.0% of the participants reported praying occasionally. More than 40.0% of participants read books occasionally, and 38.7% stated that they occasionally take antioxidants. Conclusions: These results provide more insight into CAM use for nurses who care for breast cancer patients.

Why do registered nurses choose to offer complementary and alternative medicine?

  • The use of Complementary and Alternative Medicine (CAM) is increasing in Norway. A growing number of nurses choose to offer CAM, and the purpose of the study presented in this article was to examine the reasons for their choices. Fieldwork including interviews with 20 nurses offering CAM was conducted. The results showed that the nurses in general are not satisfied with the public health service. They had four main reasons for their choice to offer CAM: 1. A desire to perform holistic nursing. 2. A tendency to value self-realization. 3. A wish to experience meaning in their work and develop a stronger professional identity. 4. A freedom to mix care and cure. The results of this study are also discussed in view of medicalization.

간암 환자의 보완·대체 식이요법 치유과정 경험 (Healing Experience of Liver Cancer Patients by Complementary and Alternative Diet Therapy)

  • Purpose: This study was to evaluate the importance of liver cancer patient's diet and to provide guideline materials for proper nursing intervention. Method: The hermeneutic phenomenological method of van Manen was applied for the in-depth interview of liver cancer patients and the cultural background studies including linguistic, literary and art works to enhance the insight and understanding, from which the meaning of the cognition and lesson of the experiences were extracted. The participants for this study were five male patients, who had been diagnosed with liver cancer, 5-15 years ago and had been treated with Transcatheter Arterial Embolization without chemotherapy. The repeated interview and close observation were carried out for nine months starting from January 2001 in Seoul, Korea. Result: Eight essential themes were emerged ; (1) confliction(frustration) with hospital treatment (2) trial of every possible remedies (3) liking unpolluted natural foods(4) faithful tolerance (5) experiencing diet effectiveness (6) discovering personal control methods (7) deepen their faith in God (8) searching for healthy new life. Conclusion: The alternative diet therapy influenced their life beyond the physical overcoming of cancer toward psychological and spiritual healing. The study evidenced the necessity for scientific research and education on the effectiveness and application of complementary and alternative diet therapy for the treatment of cancer in hospital practices.

Natural Standard Methodology를 이용한 암 보완대체의학의 근거 및 권고수준에 대한 연구 (Study on Level of Evidence and Recommendations of Complementary and Alternative Medicine for Cancer by Using Natural Standard Methodology)

  • Objective : We designed this study to analyse the Natural Standard methodology and apply the results to new study and evaluation methods. Methods : We investigated the main stream of complementary and alternative medicine (CAM), the methodology of Natural Standard, guideline in CAM, and concrete 35 cancer-related herbs and dietary supplements in 98 ones. Results : There are 35 graded herbs and supplements associated with cancer in natural standard contents: 9 foods, 17 herbs, 7 amino acids, enzymes and vitamins, and 2 herbal mixture formulas. Most of them get a grade of C: unclear or conflicting scientific evidence. Conclusion : Natural Standard aims to provide high-quality, reliable information about CAM therapies to clinicians, patients, and healthcare institutions. The more demands for and use of CAM grows, the more the needs for related studies increases. According to this international and multidisciplinary collaborative effort, we have to develop how to study and evaluate the results. We need guidelines in association with the evidence and recommendations of CAM.
