미국의 보완대체의학 제도와 정책 - 면허제도와 의료보험급여를 중심으로 - (Policies on Complementary and Alternative Medicine in the United States - Focusing on Licensing and Insurance -)

  • In recent decades, as the utilization of complementary and alternative medicine in the United State have been growing rapidly, regulatory controls surrounding complementary and alternative medicine(CAM) aims to ensure patient protection against unproven practices and to provide safe and effective treatments. Regulation and policy method on licensing CAM practitioners varies across the states. Over 85% of the states have the licensing system for acupuncturists, chiropractors, and naturopaths. For acupuncture, although the requirements for formal education are various across the states, a unified written examination has been adopted by almost entire states which have acupuncturist licensing law. Medicare, the public medical insurance, does not cover CAM practices except chiropractic and biofeedback. In some states, however, Medicaid programs cover some CAM therapies including acupuncture, naturopathy, and massage therapy. 67% of Health Maintenance Organizations, the private health plans, provide at least one modality of CAM services. In conclusion, government policies have been strengthened to ensure patient protection, and will continue to integrate CAM practices that are proven to be safe and effective into mainstream health care system.

만성 질환을 가진 소아, 청소년에게 시행되는 보완대체요법의 실태 연구 (Complementary and Alternative Medical Therapies in Children and Adolescent with Chronic Disease : Utilizations and Patterns)

  • 목 적 : 본 연구는 소아, 청소년에게 행해지는 보완대체요법 사용실태에 대한 구체적이고, 다각적인 연구의 필요성이 있었기에 보완대체요법을 접할 기회가 많은 만성질환을 가진 소아, 청소년에서 보완대체요법이 어느 정도 사용되고 있는지, 보완대체요법의 종류에 따른 사용실태를 알아보고, 보완대체요법 사용에 영향을 미치는 인자가 있는지 알아보고자 하였다. 방 법 : 2004년 3월부터 8월까지 관동의대 부속병원 소아과에서 지속적 추적관찰을 해오던 입원 경험이 있는 환아들 중에 만성질환(기관지 천식, 아토피, 간질, 뇌성마비 등)을 앓고 있는 환아(160명)를 대상으로 하였고, 환아와 동행한 보호자에게 설문양식에 따라 면담설문을 실시하였다. 결 과 : 설문 조사에 응답한 만성질환을 앓고 있는 환아는 252명이었고, 그 중 보완대체요법을 시행한 160명(63.5%)을 대상으로 삼았다. 각 질환별 보완대체요법 이용률은 선천성 근병증(100%), 아토피성 피부염(91.1%), 만성 반복성 복통(73.5%), 기관지 천식(72.5%), 만성 설사(50.0%), 뇌성 마비(42.1%), 만성 기관지염(37.5%), 간질(17.1%) 순으로 이용률의 차이를 보였다. 시행한 종류는 식이요법(31.1%), 생약요법(23.4%), 수치요법(16.3%), 마사지(14.2%), 방향요법(8.9%), 침술요법(4.6%), 동종요법(1.3%) 순이었다. 64명(40.0%)이 2가지 이상의 보완대체요법을 동시에 시행하였고, 보완대체요법을 권유받은 경로는 비의사처방에 의한 경우가 133명(83.1%)이었으며, 99명(61.9%)이 보완대체요법의 효과에 대해 긍정적으로 답변했다. 보완대체요법 시행군에 있어서 보완대체요법 사용에 영향을 미치는 인자들 중 부모 연령, 자녀 연령이 통계학적 의의가 있었으며, 부모의 연령이 높을수록 시행경험이 많았고(P=0.001), 자녀의 나이가 어릴수록 보완대체요법을 시행하지 않는 경향을(P=0.002) 보였다. 결 론 : 만성질환을 앓는 환아들은 치료에 있어서 현대의학에 의존하는 반면, 상당수가 보완대체요법을 이용하고 있었으며, 2가지 이상을 동시에 사용하는 경우가 많았고, 그 효과에 대해 긍정적으로 평가하고 있었다. 이와 같은 결과는 국내에서 보완대체요법의 접근성이 용이하고, 긍정적인 측면이 강조되고 있기 때문이므로 무분별한 보완대체요법의 사용을 피하기 위해서는 보완대체요법의 효과와 부작용에 대한 현대의학적인 연구가 더 필요할 것으로 사료된다.

질병 부담이 증가하는 암의 한의학적 치료 접근 (An Approach of Traditional Korean Medicine to Cancer, the Leading Cause of Disease Burden)

  • Objective: The purpose of this study was to investigate the disease burden of cancer, to investigate how many cancer patients use CAM (complementary and alternative medicine) therapies with or without conventional medicine, to know reasons for use of cancer CAM therapies, and finally to discuss viewpoints on treating cancer with traditional Korean medicine (TKM). Method: In order to know the disease burden of cancer, the prevalence and mortality of cancer patients of Korean, American, and WHO reports were investigated, and the usage of cancer CAM therapies was investigated in several reports. The viewpoints about cancer treatment with TKM were suggested with the characteristics of TKM. Results: One hundred thousand Korean people were diagnosed [in time period] as new cancer patients, and cancer patients are gradually increasing every year. Cancer is a leading cause of deaths in Korea and worldwide. From 21% to 63% of Korean cancer patients, 80% of American cancer patients, and 35.9% of European cancer patients used CAM therapies, and the most common therapy was an herbal therapy. Conclusions: TKM has been used to treat cancer since initial use of acupuncture and herbs with basic viewpoints on human health and disease. First, TKM regards occurrence of cancer as an environmental problem of the whole human being. Second, the pathologic concept centered on antipathogenic Qi (Zheong Qi) can reinforce the functioning of the patient's own natural vitality to overcome cancer. Third, TKM deals with cancer patients through mind, body, and spirit based on the monoism of mind and body.

Herbs for the Treatment of Insomnia

  • Pharmacological approaches have been included in conventional medical treatment for insomnia or sleep disorders. However, long-term use of frequently prescribed medications can often lead to habituation, critical withdrawal symptoms and/or side effects. Some individuals with insomnia or trouble sleeping have used complementary and alternative medicine (CAM) therapies to treat their conditions. Recently, CAMs or herbs have been attractive alternative medications to many patients with sleep disorders who may be averse to using conventional drugs. We reviewed the most widely available sleep-promoting herbs commonly used in the western and oriental countries.

한국 성인의 보완대체요법 이용에 관한 조사 (A Study on the Use of Complementary and Alternative Therapies in Korean Adults)

  • Purpose: This descriptive study was conducted to identify the use of complementary and alternative therapies, and the perception and attitudes toward CAT in Korean adults. Methods: The 1,010 study subjects were recruited from ten regions in South Korea. Data were collected from July to September, 2004. Results: The 43.2% of the sample(n=436) had an experience of using CAT. The subjects using CAT was more likely to have high score in the CAT perception compared to the subjects with no experience of CAT(p=.001). The primary source of information about CAT was mass media(59.7%). The 52.4% of the sample answered that CAT was supplementary and the 27.7% answered CAT was applicable to chronic pain or musculoskeletal disorders. Subjects(34.8%) pointed out that the greatest problem was no scientific informant in the use of CAT. The primary reason of using CAT was to maintain their health or to prevent disease(44.0%), and the 59.0% of the subjects responded that CAT was effective. Majority of subjects were satisfied with CAT(63.3%) and experienced no side effects of CAT(90.6%). Conclusion: This study suggests that professional counseling and education for CAT is needed for Korean public as well as the research approach to test the effects of CAT.

뇌졸중 환자의 보완대체요법 이용실태 (Utilization of Alternative Complementary Remedies of Stroke Patients)

  • Purpose: This descriptive survey was conducted to investigate out the utilization of alternative complementary remedies for stroke patients. Method: The subject for this study were 194 patients, selected from inpatients department of 11 major medical center at Busan and Ulsan. Data were collected from August 19th to November 12th, 2002 through interview schedule designed by the investigator. Data was analyzed with frequency, percentage, and ${\chi}^2$-test by using SPSS Win 10.0. Results: 71.6% of objects has used alternative complementary remedies. The use of alternative complementary remedies according to disease-related features was statistically significant in concurrent diseases(${\chi}^2=15.03$, p=0.001), rehabilitation treatment(${\chi}^2=6.341$, p=0.012) and the level of ADL(${\chi}^2=19.63$, p=0.000). The most frequently used therapies were diet and nutrition(31.8%). The patients with less than 3month onset and in the mid-period of treatment frequently used remedies. The reason for using remedies were the belief in it's effects on treatment, but most patients responded was not effective(30.9%) or not much improvement after using remedies. Most of the patients were recommended to use remedies by their neighbors(27.3%) and family members(25.2%). The family members were most supportive in therapy(56.1%). The 28.1% of patients responded that the remedies were effective in promoting blood circulation and 95% of the patients had not experienced side effects. Conclusion: These results could be the basic material in developing nursing intervention for cerebral apoplexy patients.

호주의 보완의학 현황에 관한 연구 (A Study on the Current Status of Complementary Medicine in Australia)

  • Objectives : This study aimed to investigate the history, current status, and regulation of complementary medicine in Australia. Methods : To investigate complementary medicine in Australia, we searched domestic and overseas academic databases, and websites of public and private organizations related to the Australian health care. Results : Complementary medicine consists of numerous services, among which massage and chiropractic care are significantly utilized by Australians. Since 2010, Australian healthcare practitioners, in the field of complementary medicine, have been supervised by the Australian Health Practitioner Regulation Agency (AHPRA). Those who AHPRA is responsible for managing acupuncturists, chiropractors, and osteopaths. Other professions are regulated by their own respective associations. Not only aforementioned services offered by specialists, but also consumption of oral supplements accounts for considerable portion of complementary medicine in Australia. Complementary medicine products, vitamins, and minerals are managed by the Therapeutic Goods Administration (TGA). In terms of insurance policy, the reimbursement of complementary medicine expenses in Australia is covered by the public healthcare insurance system, Medicare. Medicare covers acupuncture, chiropractic, and osteopathy services. Other complementary therapies are continuously reviewed to update their coverage under this scheme. Conclusion : In Australia, practitioner qualifications, education standards, and scope of procedures related to complementary medicine are systematically managed through legal regulations of the federal and state governments.

유방암 여성의 대체요법 이용과 영향요인 (Use of Complementary and Alternative Medicine and its Affecting Factors in Women with Breast Cancer)

  • Purpose: This exploratory study was to identify the use of complementary and alternative medicine(CAM) in women with breast cancer and to analyze which factor influence CAM use. Methods: 114 patients with breast cancer were identified and approached. The instrument used to gather data were the common types, main reasons and effects as well as stress, mood states and fatigue. Results: 42.7% of all participants reported the use of CAM. The most common types of CAM used were Phellinus linteus, elm tree, and vitamin C. The main reasons for CAM use were to boost the immune system and to prevent recurrence. Prayer, massage, Shitake mushroom, and Ganoderma lucidum were, identified as being more effective than others. 93.6% of the respondents informed their physicians of their use of CAM. Logistic regression analysis determined that stress, mood states, and duration after diagnosis were factors significantly associated with CAM use. Conclusion: CAM use is increasing among patients with breast cancer. Health care professionals are in the main position to identify what treatments patients are using and implement CAM therapies that can be helpful to relieve patient symptoms related to treatment and psychological distress.

중년여성의 보완대체요법 이용양상에 따른 갱년기 증상에 관한 연구 (A Study on Climacteric Symptoms Corresponding to the Use of Complementary and Alternative Therapy in the Middle Aged Women)

  • Purpose: This study was performed to identify climacteric symptoms corresponding to the use of complementary and alternative therapy(CAT) in middle aged women. Method: The subjects of the study were 603women(40-64years) and were chosen by convenient sampling from 1 April to 31 July 2004. The collected data was analyzed using descriptive statics, chi-square, t-test analysis by SPSS/WIN program. Results: 1. It was shown that 63.8% of subjects have utilized one or more types of CAT. 2. It was shown that the most frequently used types are health supplement(40.5%), gold acupuncture(31.2%), acupress(24.7%), hand acupuncture(21.8%) 3. There was significant difference in the using experience CAT by religion(p=.029), job(p=.018), hormone replacement(p=.016), exercise(p=.007), health state(p=.013). 4. There was significant difference in climacteric symptoms(p=.000) corresponding to the using experience of CAT. 5. There was significant difference in climacteric symptoms(p=.028) corresponding to the using experience hand therapy of CAT. Conclusion: We found that middle aged women used CAT more than others populations. Future research examining the determinants of CAT use, incorporating attitudinal and health conditions, as well as clinical efficacy, effectiveness, and health outcome studies of specific CAT therapies are warranted.

Merging the old with the new: a cybermedicine marriage for oncology interactions with traditional herbal therapies and complementary medicines

  • An oncology-specific database called OncoRx (http://bit.ly/cancerRx) was previously set up in cyberspace to aid clinicians in identifying interactions of anticancer drugs (ACDs) and chemotherapy regimens with traditional Chinese medicines (TCMs) and complementary and alternative medicines (CAMs). Since then, users have requested the drug-CAM interactions (DCIs) of 5 specific CAMs (cranberry, melatonin, co-enzyme Q10, huachansu, reishi mushroom) to be updated in the database. Pharmacokinetic properties (metabolism, enzyme induction/inhibition, elimination), TCM properties and DCIs of each CAM were collated with 117 ACDs using 9 hardcopy compendia and online databases as resources. Additionally, individual ACDs and CAMs were used as keywords for PubMed searches in combination with the terms 'anticancer drugs', 'drug interactions', 'herb-drug/drug-herb interactions', 'pharmacokinetic interactions' and 'pharmacodynamic interactions'. DCI parameters consisted of interaction effects, evidence summaries, proposed management plans and alternative non-interacting CAMs, together with relevant citations and update dates of the DCIs. OncoRx is also used as a case to introduce the "Four Pharmaco-cybernetic Maxims" of quality, quantity, relationship and manner to developers of digital healthcare tools. Its role in Hayne's "5S" hierarchy of research evidence is also presented. OncoRx is meant to complement existing DCI resources for clinicians and alternative medicine practitioners as an additional drug information resource that provides evidence-based DCI information for ACD-CAM interactions.
