Nursing Case Management Program Development for the Elderly at Hospital (입원노인환자의 사례관리 프로그램 개발 연구)

  • Purpose: This study was performed to develop the case management program that can provide the qualitative health care service to satisfy the increasing needs for health care elderly and prevent the increase in medical cost cased by prolonged and repetitive hospitalization. Method: The survey was completed as follows. First, 290 elderly patients who would be soon discharged in 4 hospitals are participated in the survey on needs of nursing care and case management programs. Second, 70 professionals working at 5 hospitals including doctors, nurses, social workers and so on are participated in the survey on the case management team operation. Result: Level of needs to Nursing care and case management programs showed so high. Professionals suggested on the case management team operation. Finally, based on the survey and literature review, the case management program was developed. Conclusion: In this study, the specific models for the five case management services were suggested. It is thought that the service provision by the case management team based on the multi-disciplinary case management program model suggested in this study would be suggested as a constituent of the effective health care delivery system for the hospitalized elderly patients.

Effects of Community based case Management Program on Functional Status and Caregiver Burden of Stroke Patients (뇌졸중 환자에 대학 지역사회 중심 사례관리 효과)

  • Purpose: The purpose of the study was to analyze the changes in functional status and caregiver burden after a community based case management program for stroke patients. Method: This study was designed as a one group pre and post test. A total of 61 stroke patients and their caregivers were sampled in three urban areas and two rural areas. Face to face interviews were conducted as a pre test one week before intervention, and a post test two weeks after intervention. The case management program consisted of four home visits and two telephone counseling sessions for assessment, education and providing information during the eight weeks. The collected data were analyzed with paired t test. Results: First, significant differences in functional status, and specifically mobility and cognition, were found between pre test and post test. Second, caregiver burden, in particular, objective burden was significantly decreased after 8 weeks. Third, the positive response for the services increased after 8 weeks. Conclusion: The community based case management program was effective to improve the functional status of stroke patients and to decrease the caregiver burden.

Effectiveness of Community-based Case Management for Patients with Hypertension

  • Purpose: The purpose of this study was to evaluate the effectiveness of case management for patients with hypertension on their health status and medical service utilization. Methods: This study was a secondary analysis of data collected for a larger study of chronic disease management in 2008 using the National Health Insurance Corporation database. A total of 12,944 patients who received case management for hypertension were included in this analysis. The subjects of case management were classified into subgroups, namely, over-use, under-use, and non-use groups according to the amount of medical service utilization. To compare the medical service utilization, a control group was selected randomly. The data were analyzed through descriptive statistics, McNemar test, and ANOVA. Results: All the subgroups displayed significant differences in blood pressure, self-management, social support, and their characteristics of medical service utilization. The total medical expense of the under-use and non-use groups increased after case management. However, there was no decrease in the medical expense of the over-use group. Conclusion: This finding suggests that there is a need to re-examine why patients overuse medical services and to supplement specific strategies for encouraging appropriate medical service utilization, and enhancing case management efforts for the over-use group.

The Effects of Case Management for Clients with Clonorchiasis in Riverside Areas (강유역의 간흡충 감염양성자를 위한 사례관리 적용효과)

  • Purpose: In order to lower the infection rate and the reinfection rate of Clonorchiasis in high-risk areas, we performed and analyzed a case management on people with chlonorchisis. Methods: The data was collected from April 2010 to March 2011. A community health practitioner was selected as a case manager based on our training program. The intervention group had 58 participants with three months of case management and the control group had 144 participants handled with traditional methods. Results: the pre and post-test in the intervention group and the control group showed us improved knowledge of Clonorchiasis, attitudes and behavior toward eating freshwater fish. The results of post-test between the intervention group and the control group showed statistically significant differences in all categories, except a hand-washing category. However, one year after a case management, there was no significant difference in the failure rate of treatment. Conclusion: The case management was effective for people infected with Clornorchiasis near riverside areas; however, in order to lower the rates of reinfection and treatment failure, it is necessary to perform continuous monitoring and regular evaluations.

  • The purpose of this study is to investigate how the local community case management council perceives the expected outcome of the council and case management service by participating actors in order to establish the identity and direction of the activities of the council through concept mapping. A total of 12 practitioners from participating organizations freely expressed and shared their opinions about the case management service performance with the local community case management council in a brainstorming manner, producing a total of 42 statements. Through concept mapping, participants were empowered in the decision-making process, and their opinions were not alienated, but were accepted and rationally handled. It can be said that it is important to provide an opportunity for the participants to discuss on an equal footing in the decision-making process. Through this, it can be seen that the agreement between the case management council and the case management service was quickly reached, and the direction for subsequent activities was clearly set.

  • This study intends to find a solution through the activation of community business as a business as a business model of the creation of real satisfaction for the region's residence. In this study focuses on finding out how community building project, which is recently emerging as a new paradigm of regional development. In the past, we have lived community units, but nowadays many communities have been destroyed by the capitalization and the urbanization even in the rural area. This report studies essential elements needed for successful 'Rural Community Business' through case studies of 'Dandelion Village' in Gyoengsangnamdo. In order to do so, we analyzed this villages according to a theoretical 'Sustainable Community Business Model.' We constructed successful business model of 'Rural Community Business' based on six groups - 'Community Leader,' 'Teamwork,' 'Sustainable Population Influx,' 'Sustainable Voluntary Participations of Community Members,' and 'Sustainable Networking.

Distribution and Application of Community-based Disaster Risk Information : Lessons from Shiga Prefecture in Japan

  • Purpose - This paper aims to explore the distribution and application of community-based disaster risk information and employ a case study as a qualitative research method to make some implications and suggestions for disaster management in the future. Research design, data, and methodology - This research has basically adopted an idiographic approaches to examine the basic policy of integrated flood risk management of Shiga prefecture in Japan. The methodology is based on a retrospective analysis, which starts from critical events and traces backwards processes to find out what goes well or wrong. Results - The results of this paper support that the multiple stakeholders in a community have to share and distribute disaster risk information in the proper time. The distribution and application of community-based disaster risk information cannot be overemphasized in that the local communities are culturally rich in traditional flood management knowledge, have voluntary organizations and have enjoyed mutual support and human network to cope with floods. Conclusions - The study results also imply that local residents of the community will be abe to have an important role in coping with natural disasters, which involves more proactive actions than passive actions for the enhancement of disaster management.

  • This study was to define the possibilities for the community resource development. Focused on the community organization this study analized 3 cases of the community organization. Based on the theoretical concepts and the analized case studies this study concluded some scheme as follows : 1. For the effective community organization the Healthy Families Center has to give emphasis on the establishment of the concrete objects, the system of communication and decision-making. 2. The professionals of the Healthy Families Center have to possess the knowlegement of the organization management. And this point porposed the necessities for enlargement of family resource management.

  • This study aims to develop the case management activities inventory relevant to community-based mental health settings. The comprehensive literature review was conducted to define the concepts and activities for the case management process. After the content analysis from the professional scholar and the mental health-related worker, 83 items were constructed. A survey was conducted to 265 case managers in community mental health centers. Through exploratory factor analysis, reliability and validity analysis, finally 8 factors and 43 items were identified.
