An Intervention Study of Self-feeding for the Elderly in Nursing Homes (요양시설노인의 자립적 식사를 위한 중재 효과와 경험)

  • Purpose: This study aims to investigate the effects of an intervention of self-feeding for elderly residents who were eating with assistance or eating by himself/herself with spilling food. Methods: The Participants were 11 elderly people and 6 formal caregivers from 7 nursing homes in Korea. The intervention was to use the spoon and chopstick sets designed for compensating the weakened eating function. Both quantitative and qualitative data were collected through observation, structured questions, and in-depth interviews. Results: The mealtime was significantly increased by 3.2 minutes (p=.011) after the intervention. Three themes were extracted for the meaning of self-feeding expressed by the elders; fighting alone for self-feeding, pride of participating in the study, and burden for self-feeding and research participation. Caregivers expressed the meaning of the elder's self-feeding such as the regret of missed chances, facilitating rehabilitation, the increase of the eating pleasure and quality of care, and ambivalence. Conclusion: Self-feeding has become an opportunity to recognize life values for the elders in nursing homes; for the caregivers, to reconsider caring of the elderly. Posture and eating utensils were also important to improve self-feeding skills.

The Development and Effects of an Upper Extremity Exercise Program Based on Patterned Sensory Enhancement for Home-bound Stroke Patients (재가 뇌졸중 환자를 위한 패턴화된 감각향상(PSE) 상지운동 프로그램의 개발 및 효과)

  • Purpose: This study was conducted to develop and assess the feasibility of an upper extremity exercise program based on Patterned Sensory Enhancement (PSE) for home-bound stroke patients by measuring the level of upper extremity function, depression, and interpersonal relationship. Methods: The experiments were performed on stroke patients at three different rehabilitation centers in Incheon City. Twenty patients participated in the experimental group or in the control group. The program was executed three times a week for 6 weeks. Data were collected from August to October, 2006. Manual Function Test was used to quantify and evaluate upper extremity function. The depression level and the effectiveness of interpersonal relationship were quantified and measured using CES-D and Relationship Change Scale, respectively. Results: The effectiveness of upper extremity function of stroke patients who participated in the PSE upper extremity exercise program was higher than that of non-participating patients. The depression level decreased and interpersonal relationship improved in the stroke patients who participated in this program compared to those who did not. Conclusion: These findings prove that the PSE upper extremity exercise program for home-bound stroke patients was an effective strategy for enhancing upper extremity function, decreasing the depression level, and improving interpersonal relationships.

Assessment of Depression in Chronic Back Pain Patients in Urban-rural areas (도농 지역에서 만성 요통 환자의 우울증 평가)

  • This study was undertaken to investigate the severity of depression and to analyze various factors related to depression in chronic back pain patients in urban-rural areas. In this study, 30 patients who suffered from back pain more than 6 months, and 30 normal subjects who had similar demographic characteristics as the patient group were evaluated using the Back Depression Inventory(BDI). Various factors such as age, sex, causes of back pain, vocational history, pain continuity, visual analogue scale(VAS), duration of pain, type of management, and urban or rural residence were recorded for the study group. The results are 1) The study group revealed higher BDI scores than the control group(p<0.05). 2) The patients who were older and had higher VAS, longer duration of pain, or previous history of surgical management for back pain, and an urban 033residence revealed higher BDI scores(p<0.05). 3) The BDI scores were not significantly influenced by the subjects' sex, causes of back pain, pain continuity, and vocational history in the study group(p>0.05). From this study, we concluded that a Psychosocial approach is required for the management of chronic back pain patients. Also, it is necessary to assess the factors, which are making depression worse in other chronic diseases for comprehensive rehabilitation.

Psychological Intervention Effects of Group Music Therapy on Patients with Parkinson's Disease (파킨슨병 환자에 대한 집단 음악요법의 심리적 중재 효과)

  • Purpose: Music therapy is a non-pharmacological treatment of the behavioral and psychological symptoms. This study was conducted to contribute to the rehabilitation of patients with idiopathic Parkinson's disease in the areas of functionality, emotionality and sociability. Methods: The study included 35 participants, aged between 52 and 77 years, who were diagnosed with idiopathic Parkinson's disease (IPD) and reported suffering from depression, chronic illness anticipated stigma, and worrying about quality of life. The patients were recruited from the department of outpatient neurology at School of Medicine, D University in B Metropolitan City. Group music therapy was performed for nine weeks. The results were analyzed by repeated measures ANOVA. Results: In terms of stigma, there were significant differences between two groups and time (p<.05). Analysis of the interaction between time and service method revealed significant differences (p<.05). Between time, and the interaction between time and service method revealed significant differences in terms of quality of life (p<.05). Conclusion: The results suggest that group music therapy will be effective on the psychosocial well-being of patients with Parkinson's disease living in assisted and independent living communities.

The Effect of the Solution-Focused Group Counseling on Self-esteem, Depression, and Hope of Unemployed Homeless Persons (해결중심 집단상담이 실직노숙자의 자아존중감, 우울 및 희망에 미치는 효과)

  • Purpose: To examine the effect of the solution-focused group counseling on self-esteem. depression, and hope of unemployed homeless persons, and to develop a nursing intervention program designed to help rehabilitation of the unemployed homeless persons. Method: This study was conducted from July 14 to September 29, 2000, and May 4 to August 17, 2001. The subjects were 30 unemployed homeless persons residing in 1 shelter in Seoul, Korea. This study used a quasi-experimental, one-group pretest-posttest design. The data were analyzed by frequency, percentage, and paired t-test using SPSS WIN program. Results: 1) Self-esteem of the subjects increased significantly after the solution-focused group counseling (t=3.31, p=0.002). 2) Depression of the subjects decreased significantly after the solution-focused group counseling (t=2.05, p=0.046). 3) Hope of the subjects increased significantly after the solution-focused group counseling (t=2.65, p=0.011). Conclusion: The solution-focused group counseling may be considered as an effective nursing intervention program for the unemployed homeless persons by improving their self-esteem and hope as well as reducing their depression. However, the effects of the program should be further addressed through, a review study.

Degradation of Lowland Forest Landscape and Management Strategy to Improve Ecological Quality in Mt. Baekja and Its Surroundings

  • The number of species and forest area has decreased as urbanization is progressed. The landscape degradation was examined by analyzing vegetation map, satellite image and characteristics of actual vegetation. The study was conducted in Mt. Baekja and its surroundings located on Gyeongsan city, southeastern Korea. As the result of landscape analysis, agricultural field was a characteristic attribute of the study area. Lowlands of this study area were occupied by agricultural field and various plantations. For 15 years from 1987 to 2002, forest area decreased from 2,072.9 ha to 1,853.2 ha, and shape index and fractal dimension of vegetation patches increased from 1.32 to 1.65 and from 1.05 to 1.09, respectively. Pinus densiflora Siebold & Zucco. community showed the highest species diversity, whereas Larix kaempferi (Lamb.) Carriere community showed the lowest species abundance. As forest management implications, monitoring of endangered plant species (Jeffersonia dubia (Maxim.) Benth. & Hook.f. ex Baker & S.Moore), and restoration of lowland forest from plantation to natural forest were discussed. Further, establishment of greenways utilizing existing streams, roadside, and public facilities were recommended.

  • Cerebrovascular diseases in Korea is an important health problem since mortality and mobidity have been increasing rapidly. Cerebrovascular diseases marked the 2nd rank of cause specific death rate in 1993. The ploblem of emotion after a stroke has received very little attention from the nursing profession until recently. Even the frequency of the emotional disorder after stroke is uncertain, and there has been very little research. Emotional disorder after stroke was related to limited social function, guilty conscience, helplessness, hopelessness, powerlessness, alienation, and damage of self-image. In the stroke patient, hope may be related to a rehabilitation or enhancing physical condition. Inspiring hope is necessary when stroke patients are unable to mobilize energy on their own behalf and perceive limited or no person choices available. Inspiring hope is an intervention that can be used with many nursing diagnoses, especially when feelings of helplessness, hopelessness, powerlessness, and depression are present. The nurse can inspire hope by understanding the hoping process. On the basis of this literature review, the following suggestions are prosed. 1) Qualitative studies on hope have been done to indentify variables that affect maintenance of hope in the chronically ill patients. 2) In the development of an instrument to measure hope, the validity, reliability, and cultural property of the hope have been estabilished.

The Effects of Hand Acupoint Massages on ADL Ability and Grip Strength of Stroke Patients (수지경혈 마사지가 뇌졸중환자의 일상생활동작 수행 능력과 악력반응에 미치는 효과)

  • Purpose: The purpose of this study was to explore the effects of hand acupoint massages on ADL ability and grip strength of stroke patients. Method: This study was designed as a quasi-experimental nonequivalent control group for pre and post-test. Twenty-eight patients were assigned to the experimental group and thirty to the control group. The hand acupoint massage was applied to both hands of the experimental group for sixteen minutes each time, a total of 15 times during five weeks. The patient's ADL ability and grip strength were measured before and after the program. The data were analyzed with mean. percent, chi-square, a paired t-test and t-test. Result: The level of ADL ability was significantly higher in the experimental group than in the control group. Also, the level of grip strength was significantly higher in the experimental group of the plegic side than in the control group, but not in the normal side. Conclusion: We have found that the hand acupoint massage helps the stroke patients improve their ADL ability and grip strength. Through this result, the hand acupoint massage could be considered as an effective application for rehabilitation of hemiplegic patients after stroke.

Subjective Stress Level Associated with Health Characteristics among First-Year Students of University: Focusing on the Freshman Health Survey in an University (대학 신입생의 주관적 스트레스 인지수준에 영향을 미치는 건강 특성 분석: 일개 대학 신입생 건강조사를 중심으로)

  • Ojectives: This study performed to analyze health promotion factors related to subjective stress level among first-year students of university. Methods: To examine the association with subjective stress level and health promotion factors, health survey was conducted with 3,892 students who were first-year students in an university by self-reported questionnaire from February 26 to March 10, 2015 in an university. Multiple regression analysis performed to identify the difference of health promotion factors by subjective stress level. Results: 1,015 students (26.1%) were on high stress level and 2,110 students (54.2%) were in healthy status. The 10.8% of them was obesity group and the 40% was alcohol use disorder group. The 8.2% had experienced depression in the last 1 year. In multiple regression models, it remained significantly the difference of subjective stress level by sex, subjective health status, sleep hours in a day, eating frequency of fruit and vegetable in a day, depression experience and suicidal thought in the last 1 year(p<0.05, p<0.01). On the other hand, it was not significant the difference by age, BMI, alcohol use disorder, eating breakfast, regular exercise and current smoking. Conclusions: To improve health promotion of first-year students of university, it should provide the intensive mental health program to women on campuses.

An Analysis of Duty and Workplace of the Health Education Specialist through the Review of Literature (문헌고찰을 통한 보건교육사의 직무 및 활동영역 분석)

  • Objectives: The purpose of this study is to expand of workplace by analyzing the duty and workplace of health education specialist and by presenting professional duty for health education specialist. Methods: 22 papers related to health education experts were finally selected from 1993 to 2017(25 years). The selected literature analyzed the title, researchers, the publishing agency, the publication year, the main contents, and duty and workplace of the health education specialist. Results: The studies on health education specialists comprised 5 cases (22.7%) between 1993 and 1999, 3 cases (13.6%) between 2000 and 2009, 14 cases (63.6%) between 2010 and 2017. Health education specialists core duty were diagnosing health education needs, planning health education programs, developing health education methods and materials, performing and managing health education programs, evaluating and conducting research on health education programs, and health communication. The workplace were 11 for medical institutions (55.0%), 9 for healthcare organizations (45.0%) and 8 for schools (40.0%). Conclusions: In addition to the basic core duty, professional and differentiated duty capabilities such as managing the target, generating health information, encouraging involvement of the target, and mobile health care should be developed.
