Artificial Neural Network Analysis for Prediction of Community Care Design Research in Spatial and Environmental Areas in Korea

  • This study aims to empirically confirm the effect and impact of community care design research centered on domestic space and environment on health promotion, diagnosis treatment, disease management, rehabilitation, and mitigation through the year of publication and perspective. To this end, based on 1,227 space and environment design studies from 2,144 community care design research data conducted for about 20 years from 2002 to 2022, when care services began in earnest through the long-term care system for the elderly, SPSS 26.0 was used to create a 'Multi-layer Perceptron' artificial neural network structure model was predicted and neural network analysis was performed. Research Results First, as a result of checking studies in each field of health care by year, there is a significant difference with the number of studies related to health promotion being the highest. Second, the five perspectives are region, time, dimension, function, and content perspective. As a result of inputting these variables as independent variables and analyzing their importance in the artificial neural network, the function perspective had the most influence, followed by the region > content > dimension > time perspective.

Survey on the Care Burden and Quality of Life in Family Caregivers of Patients Using Home Mechanical Ventilator in Yeongnam Region, Korea (영남권역에서 가정용 인공호흡기를 사용하는 환자 가족간병인의 간병 부담과 삶의 질)

  • Objective : The aim of this study was to investigate the care burden and life quality in family caregivers of community-dwelling patients using home mechanical ventilator(HMV) in Yeognam region. Methods : Survey performed to family caregivers of the patients using HMV in Yeognam region, Korea. The questionnaire is composed with patient care and the burden on caring. Korean version of Short Form Zarit Burden Interview(K-ZBI-12) and 3-Level version of EuroQol-5 Dimension applying Korean weight(KEQ-5D-3L) were also investigated. Statistical significance was accepted for p<.05. Results : A total 98 out of 150 questionnaires were analyzed. The K-ZBI-12(33.08±10.34) had a correlation with KEQ-5D-3L(0.71±0.25) negatively(p=.038). Patients' age, duration of HMV, financial burden and professional caregivers' care time had correlations with K-ZBI-12 positively(p<.05). KEQ-5D-3L correlated duration of HMV negatively(p=.017). Invasive ventilator group had lower KEQ-5D-3L than the non-invasive ventilator group(p=.008). K-ZBI-12 was lower in more than one caregiver care of patients than in one(p=.001). Conclusion : This study revealed high care burden and low quality of life in family caregivers of the patients with HMV in Yeongnam region, Korea. Efforts are needed to continually identify the needs of patients and their families, and the socioeconomic support and medical services associated with HMV.

The Effects of Driving Rehabilitation Program on Cognitive Function in Elderly (운전재활프로그램이 노인의 인지기능에 미치는 효과)

  • Purpose : The purpose of this experiment is to find out the effectiveness which exert influence on cognitive skills by using the Driver Rehabilitation program for senior citizens who are over 65 years old and live in Busan. Method : From July first, 2014 to August 28th, 2014, we researched the 60 elderly people who are over 65 and go to community relief center which is in Busan. - 30 are experimental group and the other is control group. In the experimental group, we used Nintendo wii's driving simulation program and RC Car driving program in the model road. For estimation, we used MVPT-3(Motor-Free Visual Perception Test-3), Trail Making Test - 1, Trail making Test - 2 and LOTCA(Loewenstein Occupational Therapy Cognitive Assessment). Result : Nintendo wii's driving simulation program and RC Car driving program in model road results efficient visual perception ability. This programs results effectively in visual perception ability and space perception ability. This programs results effectively in motor apraxia ability. This programs results effectively in control ability for visual perception. This programs results effectively in thinking operation. Conclusion : Nintendo Wii's driving simulation program and RC Car driving program in model road positively influence improving for visual perceptual ability and cognitive function of elderly people. Also it is considered as being more efficient for improving visual perceptual ability and cognitive function to implement basic rehabilitation training with driving rehabilitation program than basic training itself.

Association between Elevated Plasma Homocysteine and Low Skeletal Muscle Mass in Asymptomatic Adults

  • Background Homocysteine has been drawing attention with a closed linkage with skeletal muscle. However, the association of hyperhomocysteinemia with decreased skeletal muscle mass remains unclear. We aimed to investigate the association of hyperhomocysteinemia with low skeletal muscle mass (LMM) in asymptomatic adults. Methods This was a cross-sectional study of 114,583 community-dwelling adults without cancer, stroke, or cardiovascular diseases who underwent measurements of plasma homocysteine and body composition analysis from 2012 to 2018. Hyperhomocysteinemia was defined as >15 µmol/L. Skeletal muscle mass index (SMI) was calculated based on appendicular muscle mass (kg)/height (m)2. Participants were classified into three groups based on SMI: "normal," "mildly low," and "severely low." Results The prevalence of hyperhomocysteinemia was the highest in subjects with severely LMM (12.9%), followed by those with mildly LMM (9.8%), and those with normal muscle mass (8.5%) (P for trend <0.001). In a multivariable logistic regression model, hyperhomocysteinemia was significantly associated with having a mildly LMM (odds ratio [OR], 1.305; 95% confidence interval [CI], 1.224 to 1.392) and severely LMM (OR, 1.958; 95% CI, 1.667 to 2.286), respectively. One unit increment of log-transformed homocysteine was associated with 1.360 and 2.169 times higher risk of having mildly LMM and severely LMM, respectively. Conclusion We demonstrated that elevated homocysteine has an independent association with LMM in asymptomatic adults, supporting that hyperhomocysteinemia itself can be a risk for decline in skeletal musculature.

  • The world has become interested in aging problems. We will have many problems also in the PT field. Japanese aging rate was as high as 16.7 percent in the year of 1999. Its PT has dealt with the aging problem in various ways. This research helps you understand the background and history of Japanese PT from the both clinical and literal sides, which attaches importance to exercise treatment and local rehabilitation, through the PT charge and its related law. Japan and Korea are the neighbors and history, society and culture of both countries have much in common. Korea will also face the aging society soon. I hope it is a good way for Korea to deal with such problems in the future to know of the Japanese PT and refer to it.

The Immediate Effects of the Toe Spreader on the Gait of Children With Spastic Diplegic Cerebral Palsy (발가락 벌림 보조기가 양하지 뇌성마비 아동의 보행에 미치는 영향)

  • 본 연구는 발가락 벌림 보조기(toe spreader)가 긴장성 발가락 굽힘 반사(Tonic Toe Flexion Reflex: TTFR)가 있는 양하지 뇌성마비 아동의 보행에 미치는 영향을 알아보고자 하였다. 12명의 TTFR이 있는 양하지 뇌성마비 아동을 대상으로 같은 날에 맨발과 발가락 벌림 보조기를 착용한 상태에서 보행분석을 실시하였다. 시간 거리 보행변수로 활보장(step length), 보장(step length), 보폭(step width), 발가락 외전각도(toe out angle), 활보시간(stride), 입각기(stance phase), 유각기(swing phase), 분속수(cadence), 보행속도(speed)를 측정하였다. 발가락 벌림 보조기 착용 시 시간 거리 보행변수는 통계적으로는 유의한 차이가 없었다(p>.05). 양하지 뇌성마비 아동에서 TTFR을 억제하기 위한 발가락 벌림 보조기의 즉시적인 효과는 유의하지 않다고 볼 수 있으며, 추후연구로 장기간 일상생활에서의 지속적인 착용을 통한 연구가 필요하다고 본다.

Effects of the Group Task-related Program Training on Motor Function and Depression for Patient with Stroke (과제 지향적 그룹 운동 프로그램이 뇌졸중 환자의 운동 기능과 우울증에 미치는 효과)

  • Purpose : The purpose of this study is to assess the effect for gait, balance, and depression for stroke patients by group task-related program training based motor learning theory. Methods : The subjects of this study were administrated to the 11 stroke patients (9 male, 2 female) by 5 weeks, 3 times per week, 15 times. The group task-related program training were performed gait, balance, treadmill, muscle strengthening, and game program. Each program took 7~10 minutes and total time took 60 minutes including moving time. The difference of program training were compared using the paired t-test. Results : The results of this study revealed that Fugl-Meyer motor assessment, Chedoke-McMaster Stroke assessment of lower extremity and Berg balance scale were significantly correlated. However, impairment item of Chedoke-McMaster Stroke assessment, spatio-temporal gait parameters, Timed up and go test, and depression item of Minnesota Multiphasic Personality Inventory were not significantly correlated. Conclusion : These results support that group task-related program could be a useful treatment to improve the balance skills and motor function of lower extremity for the chronic stroke patients.

Functional Improvement Following Revision Surgery in a Patient with the Dual Disability of a Complicated Residual Limb and Contralateral Hemiplegia: A Case Report

  • The number of reported cases with dual disability is increasing for the past few decades. Currently, dual disability of lower limb amputation and motor weakness after stroke became a strong issue in public concern. The functional levels of patients have shown in the wide range from independent community ambulation to non-ambulation. Thus, it indicates that favorable outcomes for dual disability may depend upon adopted rehabilitative strategies. We present the case of a man with left below-knee amputation and severe right-sided weakness following a huge putaminal hemorrhage. He had suffered from extreme pain and misfit of the prosthetic socket and the complicated residual limb for three years prior to the stroke. Forty days post-stroke, we performed a revision surgery to resolve the complications of bony overgrowth, verrucous hyperplasia, and neuroma and applied an ankle foot orthosis (AFO). Two years post-stroke, he was able to ambulate outside his home and negotiate stairs using a cane. This is the first case with the dual disability of lower limb amputation and contralateral hemiplegia to undergo revision surgery. The results suggest that an early revision surgery and use of an AFO are crucial for achieving a higher level of mobility in such cases.

The pros and cons of entry restrictions: are entry restrictions really effective in preventing the spread of SARS-CoV-2?

  • Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has rapidly spread worldwide, leading the World Health Organization to declare coronavirus disease 2019 (COVID-19) a pandemic. To curb the unchecked spread of SARS-CoV-2 infection, most countries have enforced travel restrictions. However, it is debatable whether such restrictions are effective in containing infections and preventing pandemics. Rather, they may negatively impact economies and diplomatic relationships. Each government should conduct an extensive and appropriate analysis of its national economy, diplomatic status, and COVID-19 preparedness to decide whether it is best to restrict entering travelers. Even if travelers from other countries are allowed entry, extensive contact tracing is required to prevent the spread of COVID-19. In addition, governments can implement "travel bubbles," which allow the quarantine-free flow of people among countries with relatively low levels of community transmission. An accurate evaluation of the benefits and losses due to entry restrictions during the COVID-19 pandemic would be helpful in determining whether entry restrictions are an effective measure to reduce the spread of infection in future pandemics.

Communities' Perception of the Effect of Ecosystem Services on the Forest Rehabilitation of Abandoned Mine Areas: A Case Study in Taebaek-si and Jeongseon-gun (강원도 폐광산 산림복구지의 지역사회 생태계서비스 인식조사: 태백시 및 정선군을 중심으로)

  • Rehabilitation of mining areas can reduce damage to ecosystems. However, the effects of rehabilitation on ecosystem services (ESs) and its contribution to local communities are not well known. Thus, the aims of this study were to clearly identify the ES beneficiaries affected by mining activities, to determine how the beneficiaries profit from surrounding areas in cooperation with local stakeholders, and to manage the rehabilitation areas for the ESs that the beneficiaries want. This study chose 18 ESs (4 provisioning, 7 regulating, 5 cultural, and 2 habitat services) based on The Economics of Ecosystems and Biodiversity. A semi-structured questionnaire survey using an 11-point Likert scale was conducted among 87 community residents to investigate social awareness and identify key ESs. The survey results from two local communities showed high awareness and demands mainly on cultural (mental and physical health, aesthetic appreciation, and recreation) and regulating services (local climate and air quality, and moderation of extreme events). These services were related to the daily lives of residents in local communities, provided positive benefits, and potentially improved the residents' future livelihoods. However, the average questionnaire scores were limited to 6-7 points, indicating that the benefits to local communities were meager. The residents' awareness of provisioning service was negative, even if it provided goods and profit opportunities. This indicated a disconnection between local communities and provisioning services due to forest rehabilitation that did not consider local communities that traditionally relied on specific provisioning services before the onset of mining activities. Future forest rehabilitation in abandoned mine areas must consider the welfare of local communities for sustainable use of rehabilitated forests and enhancing ESs. In this study, only a qualitative evaluation based on frequency analyses was conducted. The quantification and valuation of key ESs are warranted in the future to promote ESs from forest rehabilitation in abandoned mine areas. The study results would be useful for developing site-specific ES promotion strategies for reforesting mine areas.
