The Effects of Modified Constraint Induced Therapy on Upper Extremity Functions of Children With Hemiparesis (수정된 건측 상지 운동 제한 치료가 편마비 아동의 손 기능 향상에 미치는 효과)

  • The purpose of this study was to investigate the effect of Modified Constraint-Induced Therapy (MCIT) on the effected upper extremity of children with hemiparesis. Four children with hemiparetic upper extremity caused by brain injuries were trained by MCIT for ten weeks. During the same period, all of the subjects were also involved in thirty-minute regular physical therapy and occupational therapy. During the treatment period, the unaffected upper extremities of the subjects were restrained by a specially designed hand splint or a mitten for five hours a day, five days per week. For two hours out of the five-hour restraint period, the affected upper extremities were intensively trained by performing various functional tasks, which were individually structured to emphasize use of the affected arm. A single-subject design with A-B-A reversal was employed in this study. The affected limb motor ability was evaluated by Melbourne Assessment, measuring the time to grasp and release nine pegs, and measuring grasping power. As a consequence of this study, the affected limb motor test scores of all four subjects in the baseline period were improved during the treatment period. Furthermore, the treatment effect was maintained during a one-month follow-up period. The results of this study support the assumption that MCIT is an effective therapeutic method to improve the sensory and motor abilities of hemiparetic children. It also increases the frequency of functional use of the hemiparetic hands of brain-injured children. Based on the results of this study, it can also be assumed that the modified CIT method is especially beneficial to these children by reducing the negative emotional effects of forceful restraint of the unaffected upper extremity. To optimize the functional recovery of the paretic upper extremity by CIT, the restriction period per day should be decided individually, according to the characteristics of the individual.

The Effects of Trunk Stabilization Training on a Stroke Patient's Postural Control and Activities of Daily Living: A Sing Subject Design (뇌졸중 환자의 체간안정화 훈련이 자세조절과 일상생활동작에 미치는 영향: 단일사례연구)

  • Objective : This study intends to suggest an effective clinical intervention method for stroke patients by applying a trunk stabilization training program to a single stroke patient and investigating and analyzing the effects of the program on postural control and activities of daily living. Methods : The subject of this study was a 75 year-old female stroke patient hospitalized in C Hospital located in Gyeongju, Gyeongsangbuk-do. As for the research design, A-B-A reversal design was used with a single subject design; and the research period was a total of five weeks from April 21, 2014 to May 23, 2014. As for the research process, a total of 15 sessions were carried out including three sessions of baseline(A), nine sessions of an intervention period(B), and three sessions of maintenance(A'); and the trunk stabilization program was applied during the intervention period. To evaluate the participant's postural control and activities of living life, the Postural Assessment Scale for Stroke(PASS) and the modified Barthel Index(MBI) were used. Visual analysis was used in which collected data were plotted on a graph, using Microsoft Office Excel 2013. Results : In the postural control test, the mean scores improved from 11.7 in the baseline phase to 14.8 in the intervention phase and 15.3 in the maintenance phase. In the evaluation of activities of daily living, the score improved from 66(medium dependence) during the baseline period to 80(low dependence) after intervention and at the maintenance phase. Conclusion : It was found that the trunk stabilization training program showed effects on the stroke patient's postural control or alignment, and furthermore a beneficial improvement in the patient's performing the activities of daily living, as well.

Research Trends in Driving Rehabilitation for the Disabled in South Korea since 2000 (국내 장애인 운전재활 연구동향: 2000년도 이후)

  • Objective : This study aims to review research trends in driving rehabilitation for the disabled in South Korea since 2000 suggesting research directions for clinicians and researchers. Methods : Fifty eight articles in 16 journals listed in accredited or candidate journal lists of National Research Foundation of Korea from January, 2000 to December, 2016 were reviewed. 'Driving rehabilitation' and 'driving for disabled' were used as search terms. Descriptive statistics were used to classify articles according to study methodology, levels of evidence, study participants, research topics, and academic associations or official journals. Results : Fifty percent of analyzed researches have been published since 2012. Twenty-two studies (37.9%) were published as group comparison and correlational research. Only seven studies (12.1%) were included in evidence level I. There were 19 studies (38.8%) conducted with brain-injured patients among 49 studies including participants. The Korean Society of Occupational Therapy Journal, having published 15 studies (25.9%) about driving rehabilitation, ranked first among the analyzed journals. In research topic, 15 (25.9%) studies were performed about clinical evaluation. Conclusion : The present study showed that the quality of driving rehabilitation-related studies has been increasing, but more intervention-based researches need to be carried out and it is also necessary to carry out various researches in related fields in order to establish efficient driving rehabilitation in Korea.

Comparison of Level of Participation and Quality of Life in Stroke Patients and the Healthy Elderly (뇌졸중 노인과 정상 노인의 참여수준 및 삶의 질 비교)

  • Objectives : This study was to compare the level of activity participation(instrumental activities, leisure activities, and social activities of living) and quality of life for elderly people with and without stroke, and to analyze the correlation between the level of activity participation and quality of life in their scores on measures. Methods : The Korean Activity Card Sort(K-ACS) was conducted among 25 elderly people with stroke and 25 elderly people without stroke living in Busan, and their level of activity participation and quality of life were evaluated using the The Medical Outcome Study 36-item Short Form Health Survey(SF-36). Results : Elderly people with and without stroke both showed a significant difference in the retained level of activity participation, instrumental activities of daily living, leisure activities, social activities, and the SF-36 score. The scores of the elderly people with stroke for the retained level of activity participation and instrumental activities of daily living in the Korean Activity Card Sort had a significant correlation with the SF-36 score, whereas leisure activities and social activities did not show a significant correlation with the SF-36 score. Among the elderly people without stroke, there was a significant correlation between scores for the retained level of activity participation, leisure activities, and social activities of living in the Korean Activity Card Sort and the SF-36 score, but there was no significant correlation between instrumental activities of daily living and the SF-36 score. Conclusion : The level of activity participation and quality of life could be considered to correlate with one another, and an intervention to improve the activity participation of elderly people with stroke would help to enhance their quality of life.

Analysis of Intervention in Activities of Daily Living for Stroke Patients in Korea: Focusing on Single-Subject Research Design (국내 뇌졸중 환자를 대상으로 한 일상생활활동 중재 연구 분석: 단일대상연구 설계를 중심으로)

  • Objective : The purpose of this study was to confirm the characteristics and quality of a single-subject research that conducted interventions to improve activities of daily living (ADL) in stroke patients. Methods : 'Stroke,' 'activities of daily living,' and 'single-subject studies' were searched as keywords among papers published in the last 15 years between 2009 and 2023 among Research Information Sharing Service, DBpia, and e-articles. A total of nine papers were examined for the characteristics and quality before analysis. Results : The independent variables applied to improve ADL included constraint-induced therapy, mental practice for performing functional activities, virtual reality-based task training, subjective postural vertical training without visual feedback, bilateral upper limb movement, core stability training program, traditional occupational therapy and neurocognitive rehabilitation, smooth pursuit eye movement, neck muscle vibration, and occupation-based community rehabilitation. Assessment of Motor and Process Skills was the most common evaluation tool for measuring dependent variables, with four articles, and Modified Barthel Index and Canadian Occupational Performance Measure were two articles each. As a result of confirming the qualitative level of the analyzed papers, out of a total of nine studies, seven studies were at a high level, two at a moderate level, and none were at a low level. Conclusion : Various types of rehabilitation treatments have been actively applied as intervention methods to improve the daily life activities of stroke patients; the quality level of single-subject studies applying ADL interventions was reliable.

A Study on the Aging Anxiety and Self-Efficacy of the Elderly (노인의 노화불안과 자기효능감에 관한 연구)

  • This study was conducted to find out the difference between aging anxiety and self-efficacy according to the general characteristics of the elderly in the community and to find out the correlation between aging anxiety and self-efficacy. From April 2019 to August 2019, 284 responses were used in the final analysis, focusing on senior citizens' health centers, welfare centers for senior citizens, health centers, and senior citizen centers in Gwangju and South Jeolla Province, and the survey was conducted using an aging anxiety scale and a self-efficacy scale. Studies have shown that aging anxiety has shown statistically significant differences in age, economic level, health condition, and present concern, and self-efficacy has shown statistically significant differences in age, academic background, religion, economic level and health status. The higher the age anxiety, the lower the self-efficacy. It is thought that it can be used to establish specific strategic measures for successful aging that can keep healthy for a long time and maintain a safe life.

Relationships Among Cognitive Functions, Activities of Daily Living, Participation and Quality of Life in Community-dwelling Elderly (지역사회 거주 노인의 인지기능, 일상생활활동, 참여 및 삶의 질의 관계)

  • Objective : The purpose of this study was to identify the relationships among cognitive function, activities of daily living (ADL), participation and quality of life in community-dwelling elderly. Methods : Cognitive function, ADL, participation and quality of life were assessed in the 144 elderly people residing in the community. We used MMSE-K for measuring cognitive function, K-ADL for ADL, K-ACS for participation, and GQOL for quality of life. Results : Cognitive function was significantly correlated with social activity of participation and quality of life (physical health, psychological health, social relations and economic level). ADL was significantly correlated with participation (instrumental ADL, leisure activity) and all sub-areas of quality of life. Social activity of participation were significantly correlated with all subarea of quality of life except physical health. Among the variables, the most relevant variable was leisure activity. Conclusion : It was related to quality of life in the order of participation, ADL, and cognitive function. Among the sub-areas of participation, leisure activity was the most correlated with quality of life. This study, thus, aimed to provide basic data for successful elderly life by examining the correlation between cognitive function, daily life activity, participation and quality of life in community-dwelling elderly.

Awareness and Needs for Community based Telerehabilitation Service: A Survey for Users of Centers for Independent Living (장애인 자립생활센터 이용자를 대상으로 한 원격재활서비스에 대한 인식 및 요구도)

  • The purpose of this study is to analyze the awareness and needs for community based Telerehabilitation(TR) service by carrying out a online survey targeting individual with disabilities. By using the survey, we attempt to utilize the results as the basis for the health promotion and effective rehabilitation service to the disabilities in the community. Survey was conducted on the 162 target users, who use center for independent living, are registered as disabled. There are 34 questions in total. The results of the study are as follows. First, 74.7% of respondents said 'they had never heard of TR service'. 68.3% of the respondents said that 'they want to use the service' and 78% are positive about the service. Second, 84% of them said 'they have an intend to participate the service', 87.1% of the respondents answered that they needed 'very necessary' and 'necessary' in the question of necessity. Third, as a results of searching the requirements for TR service, it was found that there are requirements such as service system and construction considering the characteristics of the disabled, service provision of rehabilitation experts, and various program composition. Based on the results of this study, we suggest the development of a differentiated TR system program to improve the quality of rehabilitation for individuals with disabilities by considering the reality of rehabilitation treatment of community disabled people and considering the knowledge and needs of TR system.

Model Development of an Elderly Health Promotion Center: The Effect of a Social Support Program at a Community Health Center (노인 건강증진센터 모형개발을 위한 연구: 일 지역보건소에 적용한 사회적지지 프로그램의 효과)

  • Purpose: The purpose of this study was to develop a model of an elderly health promotion center after witnessing the effect of a social support program on dependent variables in older adults at a local community health center. Method: The subjects were 60 female adults over 65 years dwelling in a rural area, and they were divided into experimental and control groups each with 30 people. A social support program was implemented 6 hours a day, 3 times a week, for 4 months in the experimental group. Included was health assessment, health education, counseling, consultation, exercise, physical & occupational therapy, primary care, recreation, lunch & transfer service. Data was collected from May 1stto September 14th, 2002 by questionnaires, and analyzed by $x^2$-test, t-test, Pearson's correlation coefficient and stepwise multiple regression using SAS. Result: The social support program in the elderly was very effective on all dependent variables of physical health (t=4.68, p=.001), health knowledge (t=3.60, p=.001), life satisfaction (t=8.65, p=.001), and health promoting behaviors (t=5.23, p=.001). Conclusion: The Social Support Program at a Community Health Center was effective on health promoting behaviors in the elderly.

Health Problems and Support from the Health & Welfare Service in the Elderly at Home (재가노인의 건강문제와 보건ㆍ복지 서비스 지원 실태에 관한 연구)

  • Purpose: This study was conducted to identify health problems and support received from the health and welfare service using MDS-HC(Minimun Data Set for Home Care) in the aged living at home. Method: Eighty-one elderly persons were selected from those listed in community-welfare service centers in Seoul and Kyunggi Province between December 2002 and January 2003. Result: Eleven health problems per elderly person on average were identified, and the frequent care needs were in order: preventive health measure, health promotion, visual function, depression & anxiety, communication disorders, social function, pain, environmental assessment, oral health, cognition and falls. The number of health problems by the level of ADL was ‘ADL 1(Independence)’ 9.87, ‘ADL 2(Partial independence)’ 12.78, ‘ADL 3(Dependence)’ 13.73. Utilization of formal health & welfare services among the elderly was ‘meals on wheels’ 40.7%, ‘home helper’ 38.2%, ‘visiting of social welfare worker’ 21.0%, ‘physical therapy’ 19.6%, ‘day care center’ 12.3%, ‘volunteer's service’ 9.9%, ‘home visiting care’ 3.7%, ‘occupational therapy’ 3.7%, and ‘speech therapy’ 2.5%. Conclusion: The results suggest that using the MDS-HC 2.0 is applicable to help decide criteria for both health and welfare service supplied to the elderly.
