Purpose: The purpose of this study was to evaluate the validity and reliability of the Korean Version of the Nurses' Self-Concept Instrument (NSCI) geriatric hospital nurses in Korea. Methods: Bilingual nursing professionals performed translation and back-translation. Reliability and validity of the content and construction of the instrument were confirmed. Internal consistency reliability was determined. Construction and concurrent validity were verified using factor analysis and correlation coefficients. Results: The total 14 items for the Korean version of the Nurses' Self-Concept Instrument (NSCI) were retained through item analysis. In explanatory factor analysis, four subcategories were proposed with their names of each factor: 'Leadership', 'Staff relation', 'Knowledge', and 'Care'. The four factors accounted for 78.81% of the variances. The Cronbach's ${\alpha}$ regarding internal consistency were .77~.91 for the NSCI subscales. Correlation among four subcategories ranged .62~.84. Conclusion: The findings show that the Korean version of the Nurses' Self-Concept Instrument is reliable and valid for measuring professional Self-Concept of geriatric hospital nurses in Korea.
Purpose: The objective of this study was to understand the caring experience of families with terminal cancer patients. Method: This was designed to be an inductive and descriptive study. Forty-seven families with terminal cancer patients were interviewed in depth and collected data were examined through content analysis. Result: The main categories of difficulties found in this study were 'suffering of patient', 'emotional suffering of family', 'bereavement of patient', 'difficulties in coping', 'problems in treatment', 'incurable situation', 'family problems', 'relationship with other people', 'economic problems', 'spiritual problems', 'problems in the future', 'informing patients of their condition', 'preparing death', 'emotional unstability', 'meaninglessness', 'unkindness of medical teams', 'poor environment for treatment', 'difficulties in hospital environment' and 'economic burden'. Conclusion: The main point found from this result was that families taking care of terminal cancer patients are suffering emotionally from watching the patients' pains and had difficulties in coping with the patients' situation and treatment. In addition, they had negative experiences in medical teams' attitude and hospital environment. This result can be used as an important guide for nurses to assess families' needs in the terminal care setting.
Purpose: The purpose of this study was to try to understand the essence of the experience early turnover of the new nurse by applying the phenomenological method and to provide basic data for a community-based management program. Method: Phenomenological approach was used to identify subjects experience. Subjects were five new nurses, with less than one year of clinical experience in clinical practice. This study used in-depth interview. Results: there were 104 meaningful sentences or phrases, with 41 generally comprehensive thema. Finally, thema were classified into 12 thema clusters. Conclusion: Finally, based on the results, some suggestions regarding management of early turnover of new nurses are needed. First, we proposed a new characterized of hospital selection method for each hospital. Second, practical training in clinical practice in the school and the community is necessary in order to reduce the real impact of new nurses. New nurses require various support elements in order to mitigate the real shock the first time they encounter clinical practice. Third, addition of work-related training and promotion of a self-esteem program will be needed. Fourth, interview opportunities with seniors who adapted successfully in clinical should be provided for new nurses. In addition, continuous communication should be provided for new nurses.
Objective: Polypharmacy is one of the main causes of inappropriate medication use, adverse drug-related events and cost. It aimed to investigate the status of polypharmacy and potentially inappropriate medication (PIM), the factors affecting polypharmacy and cost in elderly outpatients. Method: A pharmacy claim data were retrospectively analyzed with elderly patients prescriptions at a pharmacy located near a top tier general hospital. The numbers of medications per person, prevalence of polypharmacy and PIM according to the 2012 Beers criteria and Korea PIM list, medication cost and the factors affecting polypharmacy were investigated. Results: Forty-six percentages of the elderly outpatients received polypharmacy and over 21% of them had medications listed in Beers or Korean PIM. In multiregressional analysis, we found that age, gender and insurance types were affective factors of polypharmacy. (p < 0.001, 0.047, 0.009, respectively). The cost of polypharmacy with PIM in elderly outpatients was increased with age. Various approaches of interventions would be further required.
Purpose: The purpose of this study was to verify the validity and reliability of the Korean version of the Pain in Older Adults Knowledge Survey (K-POAKS) to assess knowledge of pain in older adults including people with dementia for use in long-term care hospital nurses. Methods: Survey data were collected from a convenience sample of 179 nurses who have worked in long-term care hospitals in B, D and U cities. The Kuder-Richardson Formula 20 for internal consistency for test reliability was conducted. The content, criterion-related and construct validity were evaluated using SPSS/WIN 22.0. Results: The KR 20 was .75 and Content Validity Ratio (CVR) was a range of 0.84~1.00. The criterion-related validity was positively correlated with attitudes (r=.28, p<.001) and performance (r=.21, p=.004). The construct validity of K-POAKS was analyzed by conducting the principal component method using the exploratory factor analysis varimax rotation, and seven factors were derived above the eigenvalue of 1.0. The seven factors explained 58.5% of the total variation. Conclusion: The Korean version of the POAKS showed satisfactory internal reliability, content validity, criterion-related validity and construct validity. These results suggest that the K-POAKS could be used as a suitable tool to measure the knowledge of the aged people's pain, including that of dementia patients for long-term care hospital nurses.
The epidemiological information on 156 cases of acute drug intoxication treated at emergency room of Sin An Dai-Woo Hospital in Bi-Gum island during the year of 1981-1982 were reviewed and analyzed. Lyme disease, or Lyme borreliosis. is an infection caused by spirohete Borrellia burgdorferi. This disease was recognized in Lyme, Conneticut U.S.A. in 1975. The onset of the disease is usually heralded by the appearance of a pathognomic skin lesion, known as erythema chronicum migrans, and accompanied by flue like or meningitis like symptoms. Unless treatment is initiated early, the disease usually disseminated, often resulting carduac, neurologic, or joint manifestations. All stages of the disease are usually curable by appropriate antibiotic therapy, and can prevent severe late cardiac, neurologic, and joint complications. Lyme disease is typically defined by clinical evidence supported by serologic test. The diagnosis require serologic confirmantion of erythema chronicum migrans, occurring in patient in nonendemic countries. Determination of antibody titer against B. burgdorferi by enzyme linked immunosorbent assay(ELISA) currently the most practical diagnostic test. Currently Lyme disease occurs in U.S.A. Europe, and Australia. It has recently recognized in China, Japan, and Soviet Union also. In United States, Lyme disease is most common vector borne infection. There is no reported case of patients with this disease in Korea. But the vector of this disease, -Ixodes ticks- had been identified in Korea. And Korea is geographically closely related to China and Japan where Lyme disease is already reported. We expect first case of Lyme disease could he reported in near future. We review the clinical manifestations and diagnostic method of Lyme disease.
