Community Health Practitioners' Perception of Barriers to Research Utilization (일개 지역 보건진료원들이 지각하는 간호연구 활용의 장애요인)

  • Purpose: This study was done to identify Community Health Practitioner's (CHP's) perception of barriers to research utilization Method: Participants were 153 CHPs working in Chungbuk Province. Modified Barriers Scale was utilized in the research, and 4 sub-scales were nurse factor, organization factor, research factor and communication factor. Data was analyzed by using SPSS/WIN12.0. Results: Research factor showed the highest barrier score among four sub-scales, and nurse factor was the lowest. CHPs with membership in any nursing academic society showed lower scores on the four sub-scales. Those who had taken a course related to research were less likely to see the research factor or communication as barriers. Barriers to research utilization were not associated with work experience years or to demographics. Conclusion: It is highly recommended that CHPs be trained in research methods and evidence-based practice and that research articles be written more clearly.

Analysis of Factors related to the Utilization of a Community Health Posts Information System (보건진료소 업무전산 프로그램 사용실태 및 관련요인 분석)

  • A computerized Community Health Posts information system was developed in 1991 by Yonsei University, College of Nursing and disseminated to more than four hundred Community Health Posts. The content of was published in the journal, The Korean Nurses in 1995, volumne M, number 4. Successful adoption to the computer program is related to many factors, such as personal factors and environmental factors. For the computer program to succeed it must be significantly related to the users and therefore it is necessary to analyze these related factors. This study describes the status of the utilization of computer programs in Community Health Posts and analyzed the factors related to utilization of the computer program. Of the 801 community health practitioners, 656 responded and data were analyzed using SPSS computer programs. Responents were from six provinces and about 85% had been working as CHPs for more than five years and 84.1% had graduated from a community college. Of these 656, 42.1% had a computer and about 60% had funding aid from the community or government. The percentage using the community health post's computer program was 22.3%. Community health practitioners were most often using the following content of the computer program : treatment activities (65.5%). medication management(53.5%). counselling and education(46.4%). bookkeeping(39.5%). chronic disease management (28%), and insurance billing(15.9%), The factors which were significantly related to the computer utility were degree of support from province, years of working. and age. The Community health practitioners who had more support from the province. who had fewer working years. and were younger used the computer program more frequently.

A Study of the Relationship between Personality Traits and Job Satisfaction of Community Health Practitioners in a Rural Area (일부 보건진료원의 성격특성과 직무만족도에 관한 연구)

  • This study was attempted to examine relationship between personality traits and job satisfaction of community health practitioners(CHPs) working in remote rural area in order to suggest some methods to enhance their lob performance and the degrees of job satisfaction. The General Personality Test and the revised version of Job Satisfaction Questionnaire were administered to 200 of 348 CHPs in the Kwangju-Chonnam area and then the percentages, means, standard deviations and Pearson's correlation coefficients of these data were obtained, ANOVA and logistic analysis were used. The results of study were as follows : 1. CHPs without religion were more satisfied with their salary than those with religion. 2. CHPs who hoped for continuous education showed higher scores than the others on necessary job, professional pride and autonomy. Those who chose for independent job showed higher scores than the others on both necessary job and professional pride. Those who hope for long duration showed higher scores than the others on both necessary job and professional pride. Those who were satisfied with the present occupation showed higher scores than the others on pay satisfaction, necessary job, professional pride, interaction, autonomy and demand from organization. 3. Their autonomy scores differed significantly according to work status, both interaction and autonomy scores did so according to the fields of the past job in CHP, and their autonomy scores according to location of clinics. Their interaction scores differed significantly according to the frequency of home visits per mouth, both the degrees of salary satisfaction and professional pride scores did so according to the frequency of counseling education per mouth, and their professional pride scores did so according to total income per year. 4. The levels of their responsibility and self-confidence showed the highest of all personality traits variables. 5. The professional pride score of CHPs showed the highest of all job satisfaction variables. 6. Dominance were mostly correlated with autonomy and responsibility were mostly associated with professional pride. Both emotional stability and self-confidence were mostly related necessary job. In conclusion, religion, location of clinics, clinical experience, opportunity for education, dominance, self-confidence, the duration of services hoped for, satisfaction with the present occupation, the field of past job and administrative affairs were found to be the important factors in the degrees of their job satisfaction. Therefore, the methods to consider these variables will be necessary to develop for enhancing the efficiency of their Job performance and the degrees of job satisfaction.

A STUDY ON THE REFERRAL PROGRAM FROM PRIMARY HEALTH CARE FACILITIES IN A RURAL AREA (일부 농촌지역의 의뢰환자 프로그램 운영에 관한 연구)

  • This study was conducted to follow the patient referral system operated from the primary health care facilities to the hospital in a rural area of Korea. The subject for this study was sampled from a community health development project carried out by the Korea University in Yeoju Kun, Kyonggi Province. The data of referred patients from primary health care facilities were collected during the period from January 1989 to December 1989. The data was sorted out by a computer system using Database package. The results of this study were summarized as follows: 1. Characteristics of the referred patients were: males $32.0\%$. and females $68.0\%$. The more elderly of the patients visited to the hospital after having been referred there by CHPs or public physicians, $25.9\%$ has been to hospital on at least one previous occasion as against $74.1\%$ for whom it was there first visit. 2. The majority of patients who were referred to a hospital where: medicine $44.3\%$ and orthopedics$16.4\%$, major diseases were : diseases of digestive system $(21.3\%)$ ; symptoms and ill defined conditions $(17.3\%)$ ; diseases of the muscular skeletal system and connective tissue$(14.2\%)$. chronic illness was $82.0\%$ and acute illlness was $18.0\%$. 3. From Community health practitioners more patients referred than the public physicians. Categoris of diseases of the referred patients were different between community health practitioners and public physicians. Due to the. respective differences between the medical restrictions put on the nursing staffs at the community health practitioners and public physicians. From this study it was recommended to define the reason of differences between ~he two groups in futher study. Study as to 1) why one group should be referring more for hospital treatment than the other. And 2) why the two agencies should be referring different diseases.

Predictors of Community Health Practitioners' Practice Regarding Urinary Incontinence (보건진료원들의 요실금 관련 간호행위에 영향을 미치는 요인)

  • Objectives: The purposes of this study were to investigate CHPs' knowledge, attitude, and practice regarding UI, and to identify predictors of their UI related practice. Methods: A descriptive-correlational study using self-administered questionnaire was conducted. A total of 330 members of the local Korean Association of CHPs were included in this study. A mailed survey was conducted to collect data. Findings: Of CHPs surveyed, 170(51.5%) returned completed questionnaire. Educational preparation of CHPs regarding UI was limited. The mean correct score of knowledge scale was 68.96%. Overall, CHPs exhibited positive attitudes toward UI. CHPs in this study were not actively participate UI related practice. Contrary to expectation, knowledge was not significantly related to CHPs' practice regarding UI. In the final analysis, CHPs' practice regarding UI was predicted by attitude toward the care of UI clients and educational needs for UI and these two variables explained 9% of variance of UI related practice. Conclusion: The results of this study suggest the importance of attitude and educational needs in shaping UI related practice among CHPs. To facilitate UI related practice among CHPs, efforts for attitudinal change related to UI among CHPs are needed. Moreover, continuing educational program for UI management should be developed, applied and evaluated.

A Study on the Development Strategy of Continuing Education Package for the Community Health Practitioners (보건진료원을 위한 보수교육자료의 개발방향)

  • The role of Community Health Practitioner(CHP) should be continuously adapted to the social changes and the needs for health care services. That is the reason CHP needs to be retrained through the continuing education program. This paper showed CHP's roles to be reinforced by analyzing his present . task performance and ability in seven task areas as well as the changes of the social environment. In addition, this paper presented retraining areas needed for the reinforcement of the CHP's role in the future, and the development strategy of related continuing education package. The major results are as follows: 1. CHP's main practice area is health care services and management & guidance, whereas the development of health information system is neglected. 2. As a result, CHP plays a role mainly as a health care supplier, a consultant and a health instructor. Therefore CHP's roles to be reinforced are management of the community health system, act as a spokesman and a team member, promotion, assessment, collection & maintenance of information, coordination and research. 3. The areas to be reinforced in CHP's continuing education are (]) aged people's health, (2)?drinking & smoking, (3)?young people's health(including drug and sexualissues), (4) rehabilitation, (5)?administration and management for community health, (6)?partnership & membership, (7) local residents' participation and community development, (8) collection & treatment of health information and (9) environmental issues for community health. 4. The priority in developing continuing education package should be given to the area, which is encountered often in rural area but important, and has a good opportunity to be resolved. The health management of aged people was selected as a top priority by members of the Community Nursing Academic Society. 5. It is recommended that the instruction materials be accommodated to the small scale workshop or seminar in order that CHPs can participate actively in the continuing education program.

An experimental Study on the Development of a Program for the Promotion of the Compliance with Sick Role Behavior in the Hypertensives (고혈압자의 환자역할행위 이행증진을 위한 프로그램개발에 관한 연구)

  • This experimental study was undertaken to gauge the possibility of application and extension of a program for hypertension care to be operated by Community Health Practitioners. Four community health posts were selected. Two places were experimental groups and the other two control groups. The study was carried out from April 1987 to March 1988. In this study the hypertensives were screened form a group of adults who were over 20 years old. The rate of prevalence was 10.7% in the experimental group, and 11.1% in the control group. The hypertension care program was composed of three parts : regular care by CHPs, reinforcement of education and family support for the changing of health beliefs. The data for this analysis is based on 109 the hypertensives, with 78 from the experimental group and 31 from the control group. After the program was completed, the results obtained were as follows ; 1) Sick role behavior compliance in the experimental group were significantly higher than the control group. 2) Blood pressures were decreased in both systolic and diastolic in the experimental group. Diastolic pressure was strikingly decreased from those of the control group and showed statistical significance (p<0.05). 3) In the experimental group, benefits, perceived family support and family support behavior were high, out benefits was significantly higher than those of the control group(p=0.000). Sensitivity, seriousness and barriers were high in the control group, but not statistically significant. 4) In conclusion, it is revealed that hypertension care program developed in this study has an effect of decreasing blood pressure and promoting sick role behavior compliance.
