The Effects of Community-based Exercise Program to Improve Leg Muscle Strength and Balance for Elderly Women (노인여성의 하지근력 및 평형성 향상을 위한 지역사회 운동프로그램의 효과)

  • Purpose : This study was aimed at determining the effects of community-based exercise program to improve leg muscle strength and balance for elderly women. Methods : This study was nonequivalent control group pre-post test design. Subjects were assigned to two different groups(exercise group=11, control group=7). Exercise group performed community-based exercise program for 8 weeks. Outcome measures included leg muscle strength, balance, walking velocity, activities of daily living(ADL), exercise self-efficacy and health-related quality of life(QoL). The collected data analyzed by independent t-test and paired t-test. Results : Leg muscle strength, walking velocity and mental component summary(QoL) were significantly improved in the exercise group. But balance and exercise self-efficacy were significantly worsened in the control group. There were no significant difference between groups after 8 weeks of community-based exercise. Conclusion : These results suggested that the community-based exercise program is effective for elderly women in improving leg muscle strength, walking velocity and mental component summary(QoL).

The Effectiveness of a Forest-experience-integration Intervention for Community Dwelling Cancer Patients' Depression and Resilience (숲 체험 통합 중재가 재가 암 환자의 우울과 회복탄력성에 미치는 효과)

  • Purpose: This study examined the effectiveness of a forest-experience-integration intervention in community dwelling cancer patients. Methods: The study was done with a nonequivalent control group pretest-posttest design. The subjects of the present study were 53 community dwelling cancer patients who were registered in a community health center in Gyeongsangbuk-do. The subjects were divided into an experimental group (n=26) who participated in the forest-experience-integration intervention and a control group (n=27) who did not participate. Data were collected from May to June in 2011. Collected data were analyzed using SPSS PC+ 19.0 through $x^2$ test, and independent t-test. Results: There were significant differences between the groups in depression (t=-4.51, p<.001), self-regulation resilience (t=6.95, p<.001), interpersonal resilience (t=10.10, p<.001), positivity resilience (t=9.67, p<.001), and total resilience (t=13.93, p<.001) measurements. Conclusion: The forest- experience-integration intervention delivered to community dwelling cancer patients was an effective method for relieving depression and enhancing self-regulation resilience, interpersonal resilience, positivity resilience, and total resilience, and can be utilized as an effective nursing intervention for community dwelling cancer patients.

Predictors of Tobacco-Control Activities of Community Health Practitioners: Report from a National Survey

  • The involvement of health-care professionals in tobacco-control activities is essential to prevent smoking-related morbidity and mortality. The purposes of this predictive correlational study were to examine tobacco-control activities and to identify the predictors of such activities of community health practitioners (CHPs). Of the 1,813 members of the Korean Association of CHP, 1,247 participated in this study. A mailed survey was conducted to collect data. The majority of CHPs supported tobacco-control policies and recognized tobacco-control activities as an important role for them. Only $44.3\%$ of CHPs were confident in their knowledge and skills regarding tobacco-control activities, and only $30.8\%$ had received professional tobacco-control education. While the majority of the CHPs 'asked, advised, and assessed' their clients, only a small number 'assisted or arranged'. The tobacco-control activities of CHPs were predicted by their attitude toward it, age, experience of tobacco-control education, educational level, and general perception of the risk of smoking; these variables accounted $13.5\%$ of variance in the tobacco-control activities of CHPs. These findings provide the basis for developing a continuing education program for CHPs. CHPs should be encouraged to integrate tobacco-control activities into their routine practice, and CHP education programs should be adjusted to increase the time spent on the tobacco-control intervention techniques.

  • The traditional application of night soil to vegetable gardens and rice paddies results in a most wide spread condition of parasitism, with a variety of helminths found in Korea. In addition to the above fact, the peculiar habit of the consumption of raw vegetables, fish, crustaceans and mammals provides a means of infestations of helminths. During the last sixty years numerous reports were found on the prevalence of helminths amongst the Korean population in different parts of the country, and it was generally recognized that ascariasis, hookworm disease, filariasis, clonorchiasis and paragonimiasis constitute the important helminthic disease in Korea. In practical measures of parasite control activities the main measures are summarized as mass-treatment, night-soil disposal and transmission control. Among the three, the mass-treatment has been commonly applied, however, no reduction of transmission has been obtained by treatment of a population. Therefore, the ultimate eradication of parasites will depend upon the application of comprehensive environmental sanitation measures. The basic environmental measures will be concerned with (a) the safe disposal of human excreta, (b) the provision of adequate and safe water supplies in such a way as to promote a higher standard of personal hygiene in the population, and (c) the prevention of food contamination by faecal material. Additional environmental measures will deal with the improvement of housing and housing hygiene and with general community development. Community development means social and cultural as well as economic development. The control measures on the parasitic endemic diseases, such as clonorchiasis and paragonimiasis are the good examples for community health development in Korea. The control of Clonorchis and Paragonimus infections are theoretically very simple, as the infection can only invade the human body by way of encysted metacercaria which are taken into the body when eating passive intermediate hosts(fishes, crabs and crayfishes). Although prophylactic measures in the case of the infections deal with above merely consist the fishes in cooking or submerging in hot water before eating them, it is exceedingly difficult to carry out such simple measures in face of century old traditions, to which the relatively primitive population clings with great tenacity. There is no one universally applicable method of control. The choice of methods must be dictated by the nature of the environment. the habit and custom of the people. the pattern of transmission and the resources of the country. There must exist a well organized public health infrastructure. Since a control programme is of necessity on a longterm basis and continuity in its implementation is essential. An investigation should be made on the prevalence of the diseases and its relationships to irrigation engineering, freshwater ecology, agricultural methods, hydro-electric schemes, and the development of communities in affected areas. In conclusion, however. the control of clonorchiasis and paragonimiasis in Korea is not an impossible task. A combination of efforts with major emphasis on health education and mass chemotherapy coupled with governmental aid in enforcing legislative public health measures could reduce the diseases. Health education in particular attempts following four things: (a) It supplies a person with enough general knowledge about a disease to make the preventive measures. (b) It makes a person feel sufficiently about the importance of his own health to make him alter his behavior and adopt these preventive measure. (c) It makes him concerned for the health others. (d) It tries to make him feel so strongly about the first three that be supports and even initiates preventive action by the community. Educational efforts should be directed primarily toward school children because it is during the early years that most persons become infected, and also because children are less entrenched in their food habits so that, the educational process should be involved at various levels in successive changes of knowledge, attitude, beha viour, habit and custom of their lives. The most parasitic endemic diseases are related to community diseases. In caring for a sick community. the first stage is to gather epidemiological data, the next is to make inferences from it-to make the community diagnosis. The third is to prescribe community treatment or community health action part of a community health action programme. The community health action is the sum of the steps decided upon to remedy the critical features revealed by the community diagnosis. Action takes various forms; health education is the most important.

Bacterial Community Shift during the Startup of a Full-Scale Oxidation Ditch Treating Sewage

  • The oxidation ditch (OD) is one of the most widely used processes for treating municipal wastewater. However, the microbial communities in the OD systems have not been well characterized, and little information about the shift of bacterial community during the startup process of the OD systems is available. In this study, we investigated the bacterial community changes during the startup period (over 100 days) of a full-scale OD. The results showed that the bacterial community dramatically changed during the startup period. Similar to the activated sludge samples in other studies, Proteobacteria (accounting for 26.3%-48.4%) was the most dominant bacterial phylum in the OD system, but its relative abundance declined nearly 40% during the startup process. It was also found that Planctomycetes proliferated greatly (from 4.79% to 13.5%) and finally replaced Bacteroidetes as the second abundant phylum in the OD system. Specifically, some bacteria affiliated with genus Flavobacterium exhibited remarkable decreasing trends, whereas bacterial species belonging to the OD1 candidate division and Saprospiraceae family were found to increase during the startup process. Despite of the bacterial community shift, the organic matter, nitrogen, and phosphorus in the effluent were always in low concentrations, suggesting the functional redundancy of the bacterial community. Moreover, by comparing with the bacterial community in other municipal wastewater treatment bioreactors, some potentially novel bacterial species were found to be present in the OD system. Collectively, this study improved our understandings of the bacterial community structure and microbial ecology during the startup of a full-scale wastewater treatment bioreactor.

Development and Application of a Multidimensional Suicide Prevention Program for Korean Elders by utilizing a Community Network (지역사회 네트워크를 이용한 한국노인의 다차원적 자살예방 프로그램 개발 및 적용)

  • Purpose: The purpose of this study was to develop a multidimensional suicide prevention program for Korean elders by utilizing a community network and to evaluate its effect. Methods: A non-equivalent control group pretest-posttest design was used. The subjects were recruited from two different elderly institutions located in D city and K province, Korea. Nineteen subjects in the control group received no intervention and 20 subjects in the experimental group received a multidimensional suicide prevention program. Results: There were more significant decreases in depression, suicide ideation, and increases in life satisfaction in the experimental group compared to the control group. Conclusion: According to the above results, the multidimensional suicide prevention program for Korean elders decreased stressful events like depression, and suicide ideation and increased life satisfaction through the community network. These findings suggest that this program can be used as an efficient intervention for elders in a critical situation.

The Effects of Community Ambulation Training on Gait, Depression and Self-efficacy in Chronic Stroke Patients (지역사회 보행훈련이 만성 뇌졸중 환자의 보행 및 우울증, 자기효능감에 미치는 영향)

  • PURPOSE: This study examine the effect of community ambulation training on gait, depression and self - efficacy of stroke patients in order to develop more effective training methods of community rehabilitation. METHODS: In the experimental group, community ambulation training combined with indoor and outdoor walking was performed for 30 minutes three times a week for six weeks. In the control group, general indoor walking training was performed. The physical factors were assessed by a 10-meter walking test, six-minute walking test and community gait test. Psychological factors were assessed by the Korean version of the Epidemiology Center Depression Scale and Activity-Specific Balance Confidence Scale. RESULTS: In the 10-meter walking test, the normal walking speed was significantly improved after the intervention in both the experimental group and the control group (p<.05). However, the fast walking speed was significantly improved only in the experimental group (p <.05). ln the community gait test, the experimental group showed significant improvement (p<.05), but the control group did not. Depression and self-efficacy were significantly improved in the experimental group (p<.05) but not in the control group. CONCLUSION: Community ambulation training may improve the gait ability of stroke patients and reduce their depression and improve self-efficacy.

The effect of Community Mapping based on Volunteered Geographic Information System on Smoking Prevention among Female Middle School Students (일개 여자중학교의 흡연예방을 위한 참여형 GIS(Geographic Information System) 기반 커뮤니티 맵핑 활동의 효과)

  • Purpose: The aim of this study was to identify the effects of community mapping based on volunteered Geographic Information System on smoking prevention among female middle school students. Methods: This study used a triangulation method which integrated quantitative data from a "pre-post" study on a nonequivalent control group and qualitative data from focus group interviews. Data was collected from 4 August 2015 to 10 January 2016. The experimental group (n=24) participated in community mapping along with education on smoking prevention and the control group (n=28) participated only in routine education. Both groups were measured on their knowledge and attitude related to smoking prevention through self-report questionnaires. The quantitative data was analyzed by descriptive statistics, $x^2$ analysis, and t-test using SPSS 23.0. The qualitative data was collected through focus group interviews to investigate the social-environmental effect of smoking prevention. Results: Knowledge related to smoking prevention was significantly higher (t=2.591, p=.013) in the experimental group than the control group. But attitude related to smoking prevention did not show significant differences between the two groups. When asked about their experiences of the community mapping program, it turned out to be a process where they could learn practical knowledge related to smoking prevention and experience their individual practices manifested as collective intelligence while working together with community members. The study found that community mapping had an effect on smoking prevention from a social and environmental aspect. Conclusion: To be more effective, school education on smoking prevention should be provided in connection with the community. It is also desirable to provide an opportunity where adolescents can experience discovering and solving practical problems along with their own community.

A study of the Relationship between Children's Self Esteem and Maternal Child Rearing Behaviors and Home Environmental Variables in Urban and Rural Community (도시와 농촌지역 아동의 자아존중감과 어머니의 양육행동 및 가정환경변인과의 관계에 대한 연구)

  • The purpose of this study was to investigate the relationship between the maternal child rearing behaviors and home environmental variables related with children's self-esteem in urban and rural community. Subjects were 435 boys and girls enrolled in grade 4, 5, 6 and their mothers from five public elementary schools in Chonan city and Naju city. The instruments for the study were coopersmith's Self Esteem Inventory Korean Maternal Behavior Inventory and questionnaire on home environmental variables. The data were analyzed by Pearson's one-way ANOVA t-test and Duncan post hoc test. The major findings were as follows: 1. Children's self esteem and 'Affection' and 'Achievement' of maternal child rearing behaviors were a positive correlation. 2. Children's self esteem of urban community was higher than that of rural community. Maternal child rearing behaviors of urban community was 'Achievement' of maternal child rearing behaviors were a positive correlation. 2. Children's self e teem of urban community was higher than that of rural community. Maternal child rearing behaviors of urban community was 'Active Involvement' and that of rural community were 'Authoritaran Control', 'Overprotection' and 'Achievement' 3. There were no differences in children's self esteem with family size. 4. There were significant differences in children's self esteem with parent's educational leveland monthly family income; the higher parent's educational level and the more monthly family income the higher children's self-esteem. 5. There were also significant differences in maternal child rearing behaviors 5. There were also significant differences in maternal child rearing behaviors with 'Achievement' by parent's age monthly family income and family size in 'Reasoning Guidance' and 'Active Involvement' by parent's educational level. in 'Authoritarian Control' and 'Overprotection' by monthly family income and in 'Reasoning Guidance' and 'Authoritarian Control' by family size.

The Effect of an Obesity Control Program on Body Composition and Blood Lipid Level among Middle-aged Obese Women in a Health Center (중년비만여성을 위한 보건소 비만관리 프로그램 효과)

  • Purpose: The Purpose of this study was to examine an obesity control program using dietary consult and physical exercise among middle-aged obese women in a health center. Method: This study used a pretest-posttest design. The subjects were 63 middle-aged women with over BMI $25(kg/m^2)$ or %BF 28 who participated in the obesity control program for 12 weeks. Results: After the program for 12 weeks, BMI(p= .02) and TC(p= .00) decreased significantly compared with those before the program. Conclusion: Obesity control programs in health centers can decrease the degree of obesity in middle-aged obese women. Therefore health centers should develop strategies for their members' continuous participation in such an obesity control program.
