Blindness to and evaluation of the condition of the nu turing environment at infant/toddler child care centers by principals, teachers and mothers (원장, 교사, 어머니의 어린이집 영아반 보육환경에 대한 불인지 비율과 평가)

  • This research was performed to examine the ratio of blindness to the child care environment and compare the evaluation of it among principals, teachers and mothers. Data were collected from questionnaires completed by respondents of this research were 37 principals, 37 teachers and 261 mothers of infants and toddlers at child care centers in Incheon, Korea. Cross tabulation, one-way ANOVA and Duncan's Multiple Range Test were utilized to analyze the data. In almost all of the items about the child care environment, mothers showed higher degrees of blindness than principals and teachers. In most of the items, mothers'evaluations were higher than those of principals and teachers. As a result, the researchers recognize a pressing need far programs far parents to improve the quality of child care.

Influencing Factors on Self-care in the Elderly with Essential Hypertension (노인본태성고혈압 환자의 자가간호에 영향을 미치는 요인)

  • Purpose: The purpose of this study was to investigate influencing factors on self-care in the elderly with essential hypertension. Method: The research instruments included the Self-Care Scale, the Knowledge Scale Related to Hypertension, the Self-Efficacy Scale and the Family-Support Scale. Results: The self-care was significantly associated with knowledge (r=.510, p=.000), self-efficacy (r=.708, p=.000), and family support (r=.403, p=.000). The major factors that affected subjects' self-care were knowledge and self-efficacy which explained 58.7% of self-care. Conclusion: Hypertension knowledge and self-efficacy are important factors for nursing interventions in caring for elderly patients with hypertension. It is needed to develop nursing programs for elderly patients with hypertension and to provide nursing interventions to improve knowledge and self-efficacy and, thereby, improving the self-care of elderly patients with hypertension.

An Innovative Wellness Program Promoting Participation in Physical Activity of Community-Dwelling Frail Elderly

  • Background: Given the potential benefits of wellness programs promoting physical activity of the community-dwelling frail elderly, it is recommended that comprehensive wellness programs combined with the component of physical activity. This may improve overall health and potentially lower the health care cost of the frail elderly. In general, the frail elderly residing in community or those after being discharged from hospitals are often committed to enhancing the status of participation in physical activity. Objects: The purposes of this study were to identify specific shortcomings of current wellness programs as part of continuum of community rehabilitation services and to propose alternatives for the care of the community-dwelling frail elderly. Through this study, geriatric health care professionals may be able to implement assessments and programs to successfully promote an effective continuum of care for the frail elderly. Methods: Article reviews were summarized and evaluated. Results: A model elaborating the relationship between components of successful wellness program and participation in physical activities for the community-dwelling frail elderly are recommended. First, periodic monitoring the levels of physical activity by the use of online measurement system should be considered. Second, individualized adaptive technologies for selecting optimal physical activities for the elderly may be better fit to individuals' current status of physical activity. Conclusion: The current status of physical activity in community-dwelling frail elderly can be monitored by online assessment systems. Through the innovative measurement system, elderly may assess his/her physical activity status overtime, select optimal physical activities matching the status, and create the elderly's own adaptive wellness programs that match to the status while residing in his/her community.

Development of a Community-based Preventive Health Care Model for the Elderly in Korea through the Evaluation of a Japanese Counterpart (일본의 노인건강관리체계 검토를 통한 한국 지역사회노인 예방 건강관리 방안모색 연구)

  • Purpose: Through a thorough examination of the CCSC (Community Comprehensive Support Center) system in Japan, this study suggests a scheme to provide community-based preventive health care services for the elderly in Korea. Methods: The study inquired into the applicability of the Japanese model by reviewing the data related to the CCSC project, aided by both in-depth interviews with staff in the field and consultations with specialists. Results: Rearrangement of the Visiting Health Management Project system is needed to manage the collective or individual visiting care management for frailty prevention of the elderly in communities. The delegated service system for preventive care in the community, including direct management by one of the public health centers, also needs to be reviewed and the application of stricter standards for the selection of the agency or corporation to run the delegated service is necessary. Long-Term Care Insurance, along with national and local grants, is to be considered as a financial resource for the community-based preventive health care model for the elderly. By making active use of education rooms at district offices, senior citizen centers in neighborhoods for the elderly with easy access can be created. The project needs to raise active supports from communities, develop programs which can be absorbed into particular local cultures, and promote the understanding of the preventive project in local communities. The preventive program should focus on first solving the problems of depression, seclusion, and lack of mobility of the elderly. Second, the program should instruct physical self-management for exercise-nutrition-dental maintenance, and third, the program should strengthen the cognitive abilities of the elderly. In addition, it is necessary to systematize and implement counter-plans of the family and community to protect the elderly who has mental and cognitive problems. Finally, by establishing a network of public health welfare resources based upon research on a community level, assessment and planning for the health of the elderly should be one with their family, and comprehensive consultation and recommendations should be provided to the family. Conclusion: Taking into consideration the experience Japan has had with respect to a similar project, it is appropriate to develop and implement a service system which would combine the Visiting Health Management Project system which has already been established and a preventive health care model for the elderly on a community level.

The Preliminary Study for Development of Occupational Therapy Model Focused on Improving Living Functions within the Community Care System (커뮤니티 케어 제도 내 생활기능 향상 중심의 작업치료 모델 개발을 위한 기초 연구)

  • Objective : This study conducted a preliminary study to develop a occupational therapy model focused on improving living functions within the community care system. Methods : From June to July, 2018, the literature on community care was researched, focusing on cases of Japan's Management Tool for Daily Life Performance (MTDLP), Sweden, United Kingdom, Germany and domestic S Elderly Care Centers and I Health Centers. Based on this information, a group of experts developed a occupational therapy model within the community care system. Results : Assessment tools such as occupation-based health promotional table, interest checklist, occupational goals for improving living functions, sheet for evaluation of living functions, survey of daily life time (weekday and weekend), and sheet for transition of living functions were developed to conduct evaluation for occupational therapy. The improving living functions program, analysis of activities based on ICF model, lifestyle redesign program, cognitive exercise therapy, the Lee Silverman Voice Treatment (LSVT), hospice, and home modification were also organized interventions already in place by occupational therapists. Conclusion : This study showed specific measures and models for the implementation of occupational therapy within community care systems. Occupational therapy is positioned as a specialized area that is essential to the client, and we look forward to the use of this model.

Prevalence of Clonorchiasis, Knowledge and Intention to Change Behavior of Village People living in the Catchment Area of Community Health Posts along the Geum River (금강유역 보건진료소 지역주민의 간흡충 감염 실태와 지식 및 행태변화 의도)

  • Purpose: The purpose of this study was to describe the prevalence of clonorchiais and analyze the knowledge level and intention of the participants to change their behavior related to clonorchiais. Method: The participants were 514 people from 7 community health posts along the Geum River. The formalin-esther sedimentation technique was used to detect the Clonorchis Sinensis (CS) eggs and a questionnaire for data on related factors. The study was carried on from December 2008 to January 2009. Results: The prevalence of CS averaged 9.3%, with a range from 0% to 24.6%. The significant factors were sex, habit of eating raw fish, and habit related to smoking and alcohol consumption. The level of knowledge was not high and not significantly different between the CS positive group and CS negative group. Intention to change their habits of eating raw fish showed various stages of change and attitudes to raw fish eating habit of others were not positive. Conclusions: These results suggest that a CS control program needs to be developed by community health services in the Geum River area to decrease the prevalence of clonorchiais.

Health promotion services of health care center at some universities in California, the U.S. (미국 대학보건실의 건강증진 서비스 제공체계 - 캘리포니아 주에 소재한 일부 대학의 운영사례를 중심으로 -)

  • Background: University health services have provided comprehensive medical care, counseling, health promotion, and public health services to their students and several other local institutions. To their faculty and staff, university health care centers have served occupational health services and employee assistant program. Purpose: We performed this study to review the health promotion services on two kinds of health care center with different style of university formate. Methods: We tried to collect the data by literature review and interview with executive and provider at health care center in University of California at Berkeley and San Jose State University. Results: Our results were as followed. First, students could use the medical services just as they would their regular doctor's office and urgent care center. Second, the health promotion unit offered programs and services for keeping students healthy and safe, including many opportunities for students to get involved in shaping the public health of the campus. Third, the health promotion recommendation offered from ACHA was useful guideline to improve health status of their member in university campus. Finally, the student satisfaction surveys were used for evaluation and quality improvement. Conclusions: The systematic approach to improve health status of students, faculty and staff can use to maintain a state of optimum health among the diverse student community in support of academic excellence. Coupled with health promotion and public health programs, university health service have to reach all segments of the healthy campus community. To achieve study goals in university, the health care center contributes to promote accountability and responsibility for the health and well being of the members in their campus.

A Study on the Digital Therapeutics Based Community Care for Rehabilitation in Dysarthria in the Post-COVID-19 Era (Post-COVID-19 시대 마비말장애 재활을 위한 디지털 치료제 기반의 커뮤니티케어 방안)

  • The purpose of this study was to explore ways a digital therapeutics-based community care plans for the rehabilitation in dysarthria in a situation where non-face-to-face services are expanding in the COVID-19 era. To this end, a qualitative study was conducted on experts working in hospitals, speech-language pathology centers, and social welfare centers, and as a result of the study, 3 topics, 9 sub-themes, and 18 content units were derived. Based on the analysis results, the digital therapeutics-based community care model consisted of 9 types: remote diagnosis, telepractice, rehabilitation training program, peer supporters, clinical support, communication, psychosocial intervention, and care plan services. This study will be able to provide basic data for health care & welfare services using digital therapeutics and guidelines for establishing shared care plans based on multidisciplinary cooperation.

Outcome Assessment of a Demonstration Project on Integration of Home Visiting Health Care and Social Welfare Services (보건소 중심의 방문보건.재가복지 통합시범사업 성과)

  • Purpose: The objectives of this study was to measure the outcomes of interventions on the health and social welfare of the elderly in a rural community in Korea. The project involved integrating services of one public health center with that of one social welfare agency, which were under different administrative structures. Method: A single group pretest-posttest design was used for this research. Seventy-five elderly residents living alone in a rural community participated in the study. All of them had coverage of free basic medical care and social welfare services by the government. Major activities for the intervention included: developing partnerships among community leaders/institutes; forming committees of community residents; educating care providers and volunteers; developing 8 integrated service programs and instruments; and organizing the networks. The 20-month intervention was care-managed by a public health nurse whom collaborated with social worker, and was assisted by volunteers. The t-test was utilized to analyze the outcome variables including the elder's health, social welfare and quality of life. A major limitation of this study was the lack of a control group. Results: The outcome of the intervention was shown by improved elder's health, social welfare needs, and quality of life. Integrating the services of public health centers with those of social welfare agencies is an effective way to improve the health of the elderly in the community. Conclusion: Developing community capacity with such integrated services will pay an important role in improving the health of the elderly who live alone.

Factors Affecting Communication Satisfaction of Geriatric Caregiver : Focusing on the case of an urban-rural complex community (노인 돌봄서비스 제공자의 의사소통 만족도에 영향을 미치는 요인 : 도·농 복합 지역사회의 사례를 중심으로)

  • Objectives: The aim of this study was to identify factors influencing communication satisfaction between geriatric caregivers and older adults in urban-rural complex communities. The ultimate goal was to design local community educational programs and policies to enhance communication satisfaction among geriatric caregivers and improve the quality of care services for older adults. Methods: To identify factors influencing communication satisfaction between elderly caregivers and older adults, a survey titled "CCEP: Assessment of Communication Status between Elderly Care Service Providers and Recipients" was conducted from February to July 2020, focusing on rural-urban complex areas. The survey was administered based on providers of elderly healthcare services. The survey targeted 131 respondents involved in providing care services for older adults. The dependent variable of this study was the communication satisfaction reported by elderly caregivers in their interactions with the elderly. The independent variables included perceptions of older adults, factors associated with communication difficulties, and communication efforts. Additionally, gender, working environment, working experience, and the proportion of face-to-face interactions with older adults during caregiving were controlled for the hierarchical multiple regression analysis. Results: The analysis revealed that communication efforts with older adults significantly influenced communication satisfaction (β=.09, p<0.01). However, perceptions of the elderly and communication hindrance factors did not have a significant impact on communication satisfaction among geriatric caregivers. Conclusion: Effective communication between geriatric caregivers and older adults is crucial for identifying and meeting the needs and demands of caregiving services, and it plays a vital role in overall caregiving service satisfaction. To enhance communication skills and satisfaction among geriatric caregivers and ensure the appropriate fulfillment of elderly care needs in the local community, the development of community-centered, specialized health communication programs and other initiatives will be necessary in the future.
