가벼운 터치 자극을 이용한 균형훈련이 뇌졸중 환자의 자세동요와 보행속도에 미치는 영향 (The effect of balance training combined with light touch on postural sway and walking speed in patients with stroke)

  • Background: The purpose of the present study was to determine whether balance training combined with light touch improves the postural sway and gait speed in stroke patients. Design: Randomized Controlled Trial Study Methods: The current study included 40 stroke patients, who were randomly divided into two groups: the balance training with light touch (LT) group (n=20) and the balance training with heavy touch (HT) group (n=20). Both groups performed balance training on a stable and unstable ground. Additionally, the LT group provided light touch stimulation, and the HT group provided heavy touch stimulation. All the participants underwent 20 sessions of STS training (thirty minutes, five days per week for four weeks). In this study, postural sway was measured using a force platform, and walking speed was measured using a 10 meter walk test. Results: The improvements in postural sway and gait speed were observed to be significantly greater in the LT group, compared to the HT group (p<0.05). Conclusion: The results of the current study imply that balance training combined with light touch is a beneficial and effective therapeutic modality that can be employed to improve the postural sway and gait speed in stroke patients.

감각운동 훈련을 병행한 순환식 과제 지향 운동프로그램이 뇌졸중 환자의 보행 및 균형에 미치는 영향 : 예비연구 (The Effects of Task-Related Circuit Exercise Program Combined with Sensorimotor Training on Balance and Walking in Persons with Stroke : A pilot study)

  • Purpose: The purpose of this study was to identify whether task-related circuit exercise program combined with sensorimotor training for 4 weeks could improve the balance and gait in stroke patients. Method: Fifteen stroke patients who had agreed with the study were randomly divided into 3 groups categorized as task-related circuit exercise program combined with sensorimotor training group (experimental group 1, n=5), task-related circuit exercise program group (experimental group 2, n=5), and control subjects performed conventional physical therapy (control group, n=5). The balance and gait were assessed by BT-4 force platform system, Berg Balance Scale, 10meter Walk Test and Smart Step at before training and after training. Wilcoxon signed rank test was used to analyze change before and after intervention in intra-group. Kruskal Wallis H test, Mann-Whitney U test and Bonfferoni correction were used to analyze changes of all variables in inter-groups. Result: The experimental group 1 showed significant improvements in postural sway area, BBS scores, walking velocity and plantar pressures of affected foot, whereas the experimental group 2 showed significant improvements in BBS scores, and the control group were no significantly different in all variables following training. The changes of postural sway area and BBS scores in the experimental group 1 were significantly greater than them of the control group. The changes of postural sway area in the experimental group 1 was significantly greater than that of the experimental group 2. Conclusion: The result of this study suggest the task-related circuit exercise program combined with sensorimotor training is an effective intervention to improve balance and gait in stoke patients.

자기관찰훈련을 병행한 과제보행운동이 뇌졸중 환자의 다리 근활성도와 보행에 미치는 영향 (The Effect of Task Gait Exercise Combined with Self-observation Training on Leg Muscle Activity and Gait in Stroke Patients)

  • PURPOSE: This study examined the effectiveness of task gait exercise combined with self-observation training to provide basic clinical data by analyzing the factors that task gait exercise combined with self-observation training has on the leg muscle activity and gait improvement in stroke patients. METHODS: The subjects were assigned randomly to experimental Group I, which mediated task gait exercise combined with self-observation training, and experimental Group II, which mediated task-walking movement. They received 30-minute interventions three times a week for four weeks. The leg muscle activity and 10 m walking test (10MWT) were conducted as pre-intervention tests, and they underwent a post-test four weeks later in the same manner as the pre-test. RESULTS: A statistically significant difference was observed in all muscles and 10MWT (p < .01) in experimental group I (p < .05), while there were significant differences only in the rectus femoris, biceps femoris, and 10MWT (p < .05). In a comparison of the changes between groups, there were statistically significant differences only in the tibialis anterior, soleus muscle, and 10MWT (p < .05). CONCLUSION: Self-observation training in experimental group I was effective in increasing the leg muscle activity and improving walking speed by discovering and correcting incorrect movements and following a normal gait pattern using the ankle joint. Therefore, the task gait exercise combined with self-observation training should be introduced and actively utilized for the rapid social recovery of stroke patients.

근긴장도 조절기법을 병행한 트레드밀 보행훈련이 만성 뇌졸중 환자의 보행능력에 미치는 영향 (Effects of Treadmill Gait Training Combined with Muscle Tone Control Technique on Gait Ability in Patient with Chronic Stroke)

  • Purpose : Stroke patients exhibit considerable variations in gait patterns. Stroke patients generally show abnormal muscle tone and gait. This study was performed to evaluate the effects of treadmill gait training combined with muscle tone control technique (TM) on gait ability in patient with chronic stroke. Methods : A single-blind, randomized controlled trial was conducted with 36 patient with chronic stroke. They were randomly allocated 2 groups; treadmill gait training combined with muscle tone control technique group (TM group; n=18) and conservative treatment group (CG group; n=18). The TM group received 15 minutes muscle tone control technique and 15 minute treadmill gait training. In the conservative treatment group received 30 minutes conservative physical therapy. Each group performed 30 minutes a day 3 times a week for 8 weeks. The primary outcome gait ability were measured by gait measurement system (Optogait, Microgate, Italy) and 10 m walking test (10MWT). An independent t-test was used to statistically analyze the pre-test and pos-ttest gait ability results. Results : Both groups demonstrated significant improvement of outcome in gait ability during intervention period. TM group showed significant differences in gait ability as compared to the CG groups (p<.05). TM group showed significant differences in 10MWT as compared to the CG groups (p<.05). Our results showed that TM was more effective on gait ability in patients with chronic stroke. Conclusion : Our findings of this study confirmed that the treadmill gait training combined with muscle tone control technique provided significant improvements on gait ability in patient with chronic stroke. Therefore treadmill gait training combined with muscle tone control technique may positive influenced gait ability. This study will be able to be used as an intervention data for recovering gait ability in patients with chronic stroke.

스쿼트를 병행한 옆으로 걷기 훈련이 뇌졸중 환자의 균형 및 보행에 미치는 영향 (Effect of Side Walking Training Combined Squat on Balance and Gait Ability in Patients with Stroke)

  • Purpose: This study investigated the effects of side walking training combined with squats on the balance and gait ability of stroke patients. The purpose of this study was to provide fundamental data regarding the use of side walking training combined with squats among stroke patients. Methods: Thirty patients with stroke were randomly divided into an experimental group (n=15) that underwent side walking training combined with squats and a control group (n=15) that performed general rehabilitation exercises. Both groups performed their respective exercises for 30 minutes, five times a week for six weeks. Balance was assessed using the functional reach test and timed up and go test, while gait ability was evaluated using the 10-meter walk test. A paired t-test was performed to compare within-group changes before and after the intervention. Differences between the experimental and control groups were analyzed using an independent t-test. For all tests, the level of statistical significance was set at α=0.05. Results: After the exercise, significant within-group improvements in balance and gait ability were observed in both the experimental and control groups (p<0.05). There was also a significant between-group difference in balance and gait ability following the intervention (p<0.05). Conclusion: While general rehabilitation is commonly employed in treating stroke patients and is relatively effective, the application of side walking training combined with squats may offer additional benefits in terms of improving balance and gait ability in these patients.

수직편위를 동반한 간헐성외사시 소아에 대한 시기능 훈련 후 임상 사례 (A Clinical Case of Vision Training for the Pediatric Intermittent Exotropia Combined with Vertical Deviation)

  • Purpose: The purpose is to study the effects of the vision training, as a non-surgically treated vision therapy, on the objective and the subjective symptoms of the patients with intermittent exotropia combined with vertical deviation. Method: For the boys at the age of eight with intermittent exotropia, who report a long-distance blurred vision, dazzling, and intermittent diplopia more than ten times a day, the vision training had been practiced for seven weeks. The objective and subjective symptoms have been examined after six months from the end of the training. Results: The objective and the subjective symptoms have been improved after the seven weeks training and the improvements lasted after six months from the end of the treatment. Conclusion: Non-surgical treatment for the patients with intermittent exotropia, including intermittent exotropia combined with vertical deviation like this case, should be considered as an auxiliary treatment and on the preferential basis.

Effect of Strength Training Combined with Blood Flow Restriction Exercise on Leg Muscle Thickness in Children with Cerebral Palsy

  • Purpose: The purpose of this study was to investigate the effect of strength training combined with blood flow restriction on leg muscle thickness in children with cerebral palsy. Methods: Nineteen children with cerebral palsy, aged between five and 10 years of age, living in area N, were recruited. Ten participants were classified into a blood flow restriction group and nine into a strength exercise group. The experimental group performed strength training using a blood flow restriction cuff on the leg, and the control group performed strength training without blood flow restriction. A paired t-test was performed to confirm intragroup changes before and after five weeks of the experiment, and an independent t-test was performed to confirm intergroup changes, and the significance level was α=0.05. Results: The rectus femoris, gastrocnemius and gluteus medius muscles showed significant differences in the groups after five weeks (p<0.05). There was a significant difference between the groups in the rectus femoris and gastrocnemius after five weeks (p<0.05). Conclusion: As a result of this study, it was found that strength training combined with blood flow restriction had a positive effect on the changes in leg muscle thickness in children with cerebral palsy. This suggests the possibility of using it in the future as basic data for strength training methods and blood flow restriction exercises for children with cerebral palsy.

The Effect of Sit to Stand Training Combined with Ultrasound on Spasticity, Muscle Strength and Gait Speed in Patients with Stroke

  • Objective: The purpose of the present study was to determine whether sit to stand training combined with ultrasound improves the spasticity, muscle strength and gait speed in stroke patients Design: Randomized controlled study Methods: The current study included 40 stroke patients, who were randomly divided into two groups: the sit to stand training with ultrasound (USTS) group (n=20) and the sit to stand training (STS) group (n=20). All the participants underwent 30 sessions of STS training (thirty minutes, five days per week for six weeks). Additionally, the USTS group received ultrasound therapy. The present study evaluated the spasticity of ankle plantar-flexors by the composite spasticity score. The muscle strength and gait speed were evaluated using the handheld dynamometer and the 10-meter walk test, respectively. Results: The USTS group and the STS group showed significant improvements in spasticity, muscle strength and gait speed after the intervention (p<0.05). Significant improvement in the spasticity, muscle strength, and gait speed were observed in the USTS group compared to the control group (p < 0.05). Conclusions: The results of the current study imply that sit to stand training combined with ultrasound is a beneficial and effective therapeutic modality that can be employed to improve the spasticity, muscle strength and gait speed in stroke patients.

12주간 복합트레이닝이 비만과 골다공증 지적장애인의 신체조성, 건강체력, 골밀도에 미치는 영향-사례연구 (The Effect of 12 Weeks of Combined Training on Body Composition, Health-Related Physical Fitness, and Bone Mineral Density of Obese and Osteoporotic Intellectual Disabilities-Case study)

  • 본 연구는 유산소와 저항성 운동이 접목된 복합트레이닝을 비만과 골다공증 진단을 받은 지적장애인 성인에게 12주간 적용한 후 운동의 효과를 알아보기 위해 신체구성검사, 건강관련체력 그리고 골다공증의 변화를 검증하였다. 연구대상들의 운동은 12주간 주2회 유산소운동과 근력 운동을 구성된 복합트레이닝 프로그램을 실시하였다. 유산소운동은 저강도에서 시작하여 점증부하원리를 적용하였고, 저항성 운동은 15회를 반복할 수 있는 강도로 설정하였다. 신체조성과 골밀도는 운동전, 6주, 12주 세 차례 측정하였고, 건강체력평가는 운동전과 운동 후 측정하였다. 체중, 체지방은 운동전에 비해 운동 후 감소하였고, 골밀도는 운동전에 비해 운동 후 증가하였다. 체력요인으로 근력과 근지구력, 유연성이 운동전에 비해 운동 후 개선되었다. 본 연구결과로 12주의 복합트레이닝은 하지근력을 증가시켜 걷기, 달리기 등의 기능을 향상시키고, 건강체력의 향상과 골밀도의 증가를 일으키는 운동임을 확인하였다.

12주간의 복합트레이닝이 국가대표 여자럭비선수들의 FMS(Functional Movement Screen) 점수에 미치는 영향 (Effects of Combined Training on the FMS Score in Woman Rugby National Players)

  • 이 연구는 FMS를 통하여 선수들의 상해에 대한 위험요인 등을 찾아내고 이를 복합트레이닝프로그램에 적용하여 선수들의 신체 불균형 및 기초체력, 근 기능을 개선함으로써 FMS 점수에 어떠한 영향을 미치는지를 규명하고 국가대표 여자 럭비선수들의 경기력을 향상시키고자 실시되었다. 제17회 인천아시안게임에 참가를 준비하는 국가대표 여자럭비선수 14명을 대상으로 하여 FMS 검사를 실시하였으며, 2014년 5월~8월까지 12주간 복합트레이닝프로그램을 실시하여(주4회/120분${\geq}$) 복합트레이닝 전 후의 FMS 점수를 paired t-test를 통해 검증하였다. 이 연구의 결과에서 Deep Squat와 Hurdle Step, Active Straight Leg Raise, Trunk Stability Push up의 항목에서는 12주간의 복합트레이닝 전보다 트레이닝 후에 FMS의 점수가 유의하게 증가되었으며(p<.05), Inline Lunge(p<.01)와 Rotary Stability(p<.001)에서도 트레이닝 후, 유의하게 증가된 것으로 나타났다. 또한 전체 항목의 점수 합산도 복합트레이닝 후에 유의하게 증가되었다(p<.001). 그러나 Shoulder Mobility 항목에서 유일하게 트레이닝 후에 약간 증가하는 경향을 보였지만 통계적 유의수준에는 도달하지 못하였다. 이상의 결과에서 복합트레이닝프로그램이 FMS 점수를 유의하게 증가시키는 효과가 있는 것으로 나타났으며 이는 국가대표 여자럭비선수들의 상해 위험성이 줄이는데 효과가 있음을 의미한다. 또한 주기적인 FMS의 평가는 선수들의 상해예방과 경기력 향상에 도움일 될 것으로 사료된다.
