Globozoospermia를 가진 불임환자에서 미세정자주입술을 이용한 체외수정시술 후 임신과 분만 1례 보고 (Successful Clinical Pregnancy and Livebirth was Achieved after Intracytoplasmic Sperm Injection in a Globozoospermic Patient: A Case Report)

  • 목 적: Globozoospermia는 남성불임환자의 0.1%에서 발병되는 극히 드문 정자의 형태학적 이상 증상으로 본원에서 이 증례를 경험하여 문헌고찰과 함께 보고하는 바이다. 연구방법: 2회 반복적 임신에 실패한 불임기간은 6년인 32세의 부인, 36세의 남편 부부가 본원에 내원하였으며, 정액 특수염색결과 희소정자증을 동반한 globozoospermia로 판별이 되어 미세정자주입시술을 실시하였다. 결 과: 14개의 난자가 회수되었으며 이 중 총 12개를 미세정자주입시술을 이용 수정시도를 하였고, 이 중 5개가 수정이 되어 3일 배양 후 이식하였다. 이식 결과 임신이 되었으며, 임신 39주에 건강한 남아를 출산하였다. 결 론: Globozoospermia의 경우 ICSI(미세정자주입시술)방법을 실시하더라도 수정율이 낮다. 이에 수정율 및 임신율을 높일 수 있는 방법의 개발이 필요한 것으로 사료된다.

Efficacy of a Cue-Mate Intravaginal Insert and Injection of Prostaglandin $F_{a{\alpha}}$ for Synchronizing Estrus in Hanwoo Cattle

  • The present study was performed on farm animals to test the effectiveness of progesterone-releasing intravaginal device (Cue-Mate$^{(R)}$ 1.56 g) and injection of prostaglandin $F_{2{\alpha}}$ ($PGF_{2{\alpha}}$) for synchronization estrus in Hanwoo cattle. The cattle were at random stage of the estrus cycle. The cows were artificially inseminated at day 7 after Cue-Mate withdrawal, using commercial semen from Korean native bulls. There was a season effect on the estrus synchronization rate. It was higher in spring (94.3%) followed by winter (93.3%), autumn (90.4%) and summer (67.2%). In summary, The results of this study revealed that season has influences on estrus behavior of cattle with no significant effect on pregnancy rate. In summary, we suggest summer reproductive management to alleviate the effects of heat stress. It should be based on intensive cooling combined with hormonal treatment. Given that different subgroups of cows benefit differently from the treatments, selective hormonal administration should be considered.

임신과 수유기간 동안 Alcohol과 저 Copper 식이가 어미와 새끼 쥐 간의 Copper 수준에 미치는 영향 (Influence of Alcohol and Low Dietary Copper on Copper Utilization of Maternal and Offspring Liver)

  • 임신과 수유 기간 동안 alcohol의 섭취가 어미와 새끼 쥐의 간내 copper 유용에 미치는 영향을 연구하기 위하여 cthanol(0 혹은 30%의 Kcal)과 copper(0.75 혹은 3.75mg/l 식이) 함량에 차이를 둔 식이조성으로 alcohol과 copper의 상호작용을 규명하는 factorial experiment를 수행하였다. 어미 쥐의 열량 섭취와 체중은 식이들에 의해 영향을 받지 않았다. 식이내 copper양이 적을 경우 alcohal을 섭취하지 않은 실험군과 비교하여 alcohol을 섭취한 실험군인 어미 쥐의 간 copper농도가 더욱 감소됨으로써 alcohol과 copper의 상호작용을 보여주었다. 새끼 쥐에게서는 식이내 copper 수준에 관계 없이 어미쥐의 alchol 섭취가 간의 총coppernein의 copper 농도에 반영되었다. Metallothionein의 zinc 함량은 metallothionetin의 copper함량과 반비례적인 관계로 보여주었다. 이상의 결과로 임신과 수유기 동안 식이내 copper함량이 적을 경우 alchol 섭취는 어미와 새끼 모두에게 copper 결핍을 초래할 수 있다는 것을 시사해준다.

극저주파 자기장의 소아백혈병 발생 위험 고찰 (Review on the Association between Exposure to Extremely Low Frequency-Magnetic Fields (ELF-MF) and Childhood Leukemia)

  • Background: The association between exposure to extremely low frequency-magnetic fields (ELF-MF) and childhood leukemia has been controversial. There is a need to clarify this relationship by summarizing key conclusions from systematic review articles. Objectives: The major aim of this study is to summarize key conclusions from systematic review articles on the association between exposure to ELF-MF and childhood leukemia based on childhood exposure to ELF-MF, proximity from childhood household to high voltage cables, and parental occupational exposure to ELF-MF. Methods: This study was conducted through a brief literature review focusing on systematic, meta-analysis, and pooled analysis methods. We conducted a literature search in PubMed using the key words "ELF-MF" and "childhood leukemia" singly or combined. Results: In 2002, the World Health Organization (WHO)'s International Agency for Research on Cancer (IARC) reviewed two manuscripts to conduct pooled analysis and concluded that there is a significant association between exposure to >0.3 μT or 0.4 μT and childhood leukemia. We found a total of four manuscripts for systematic or pool analysis that have been published since the IARC's conclusion. They consistently concluded that there was a significant association between exposure to >0.4 μT and childhood leukemia compared to ELF-MF exposure to below 0.1 μT. The proximity of children's households to high voltage cable lines and occupational exposure by their parents to ELF-MF during certain periods prior to or during pregnancy were inconsistently associated with childhood leukemia. The study found that many EU countries have implemented precautionary policies to prevent potential childhood leukemia due to exposure to ELF-MF. Conclusions: This study recommends implementing a precautionary policy that includes legal exposure limits for ELF-MF to minimize exposure to ELF-MF.

비폐쇄성 무정자증 환자에서 고환의 조직병리학적 진단에 따른 체외수정시술 결과의 비교 (Effect of Testicular Histopathology on Pregnancy Outcomes in Non-Obstructive Azoospermia)

  • 목 적: 비폐쇄성 무정자증 환자에서 고환의 조직병리학적 진단에 따라 고환조직내 정자채취술 (Testicular sperm extraction, TESE) 후 난자세포질내 정자주입술 (Intracytoplsmic sperm injection, ICSI)의 체외수정시술 결과를 알아보고자 하였다. 연구방법: 비폐쇄성 무정자증으로 고환조직내 정자채취술 후 난자세포질내 정자주입술을 이용하여 배아 이식을 시행한 122주기를 분석하였다. 고환의 조직병리학적 진단에 따라 Germ-cell aplasia (GA, 40주기), Maturation arrest (MA, 32주기) and severe hypospermatogenesis (S-HS, 50주기)로 구분하여 체외수정시술 결과를 비교하였으며, 이들 결과를 난자세포질내 정자주입술을 이용한 폐쇄성 무정자증 환자의 체외수정시술 결과와 비교하였다. 결 과: 고환조직내 정자채취술 후 난자세포질내 정자주입술시 수정율은 각각 58.1% in GA, 42.2% in MA and 48.0% in S-HS로 조직병리학적 진단에 따른 차이는 없었으며, 폐쇄성 무정자증 환자의 72.9%에 비해 유의하게 낮은 수정율을 보였다 (p<0.001). 고환조직내 정자채취술시 채취된 정자 (spermatozoa, 94주기)로 난자세포질내 정자주입술을 시행한 주기의 배아 이식 후 임신율은 각각 22.6% in GA, 29.4% in MA와 26.1% in S-HS이었으며, 출생률은 각각 16.1%, 29.4%와 19.6%로 조직병리학적 진단에 따른 차이는 없었다. 정자세포 (spermatid, 16주기)를 사용하여 난자세포질내 정자주입술을 시행한 주기의 임신율은 각각 0.0% (0/3 주기), 9.1% (1/11주기)와 0.0% (0/2주기)이었으며, 출생률은 각각 0.0%이었다. 정모세포 (spermatocyte, 12주기)를 사용한 주기의 임신율은 각각 0.0% (0/6주기), 0.0% (0/4주기)와 0.0% (0/2주기)이었으며, 출생률도 각각 0.0%이었다. 결 론: 비폐쇄성 무정자증환자의 배아이식을 시행한 주기에서 고환의 조직병리학적 진단에 따른 난자세포질내 정자주입술시 수정율은 차이가 없었으며, 폐쇄성 무정자증 환자에 비해 유의하게 낮은 수정율을 보였다. 비폐쇄성 무정자증환자에서 고환조직내 정자채취술시 정자를 채취하여 난자세포질내 정자주입술을 시행한 주기의 체외수정시술 결과는 고환의 조직병리학적 진단에 따라 차이를 보이지 않는다.

우수 한우의 수정란 생산 및 이식 (Embryo Production in Superior Hanwoo Donors and Embryo Transfer)

  • 본 연구는 MOET 기법에 의해 우수한 한우로부터 다수의 수정란을 회수하여 농가의 소에 이식하므로서 우수한 유전 자원을 확산시키고자 실시하였다. 80두의 우수 한우 공란우를 선발하여, 발정 확인 후 황체기에 성선 자극 호르몬($Folltropin^(R)-V$)을 투여하는 기존의 과배란 처리법과 임의의 발정주기에 CIDR를 삽입하고 성선 자극 호르몬을 투여하는 새로운 과배란 처리법에 의한 수정란 생산 비교와 CIDR 삽입한 한우 공란우에서 성선 자극 호르몬 ($Antorin^(R)R10$)의 투여 용량의 차이 (36 mg vs 28 mg) 에 따른 수정란 생산 성적을 비교하였다. 한편, 우수 공란우로부터 채란된 신선 수정란 또는 동결-융해 수정란을 농가 수란우 226두에 이식하였으며, 이식 결과 수태율에 대한 수정란 상태(신선 vs 동결-융해), 년도, 이식 시술자, 계절, 농가에 대한 영향을 조사하였다. 한우 공란우에서 기존의 과배란 처리 방법에 비해서 발정의 확인없이 CIDR를 삽입 후 과배란 처리 방법이 보다 많은 총회수 난자(6.5 vs. 5.8)와 이식 가능 수정란(3.9 vs.3.2)을 회수하였다(p<0.01). CIDR 처리 한우 공란우에서 성선 자극 호르몬($Antorin^(R)R10$) 용량을 36 mg으로 처리시가 28 mg 처리시 보다 많은 이식 가능 수정란(8.3 vs. 5.4)을 회수하였다(p<0.05). 수정란 이식후 수태율은 수정란의 상태(신선 43.9% vs 동결-융해 23.1%) 및 이식 시술자 (53.9 vs. $0{\sim}16.7%$)가 특히 중요한 영향을 미치는 것으로 나타났다(p<0.01). 본 연구의 결과는 CIDR를 이용한 과배란 처리 방법은 우수한 한우 수정란의 생산에 효과적으로 이용될 수 있음을 보여 주었으며, 수정란의 농가 수란우에 대한 이식은 동결-융해 수정란의 수태율 증진 및 이식 시술자의 기술이 향상될 때보다 효과적으로 이루어질 수 있을 것으로 보인다.

감음신경성난청(感音神經性難聽)의 원인(原因)에 관(關)하여 (Causes of Sensori-Neural Hearing Impairment in Korean Children)

  • This paper presents the results of a survey for the causes of sensori-neural hearing impairment in Korea, The subjects were 1,676 children of total 2,928 enrolled in 16 Deaf Schools; two schools in each area of Seoul, Busan, Kyoungbook, Kyoungnam, Kyounggi and Chunbug, and each one in Chungnam, Chungbug, Chunnam and Jaeju. The data were collected by questionaire with 28 items distributed to their parents. The filling in the check lists were performed by their class teachers, interviewer, for 18 months from September, 1975 to february, 1976. The questionable or missed problems were reaffirmed. The results obtained were as follows. Most of the reasons, 78.5% were acquired characters that could be developed during pregnant period, the time of delivery and the time of after birth. The pure hereditary reasons except the cases complexed with one or two were only 11.3%. Those who could not be defined with any reasons were 10.2%. Among the acquired causes, 5.8% of total subjects were developed for pregnancy: 3.3%, during delivery; and 69.7%, after birth. In the pregnant period, the drug intoxications were 2.4% of total subjects, several diseases such as influenja, bleeding, surgical operation, venereal diseases and rubella etc. were about one percent, and the accompanied with some symptoms of pregnancy intoxication and traumatic events were 2.4%, During time, the cases with delayed rhythmical pain were 16 persons, the immaturities were 11, the asphyxial cases were nine, the errors of forceps delivery were seven, the cases of low body weight inspite of full term were four, the cases with cesarian section were three, the head injuries were two, and the accompanied with three kinds of above reasons were three. During after birth, the cases with acute communicable diseases were 35.4% of total subjects, the fever unknown origin were 16.1%, the chronic otitis media were 3.7%, the meningitis were 3.5%, the gastric and nutritional diseases were 3.5%, the drug intoxications were 4.8%, the blood diseases were 0.3% and the other causes were 2.2%. Here by acute communicable diseases, some importants were measle, 10.1% of total subjects; meningitis, 7.3%; convulsion with some reasons, 4.9%; poliomyelitis. 3.2%; encephalitis, 2.4%; and mumps, rubella, pertusis, scarlet fever, and small pox were somewhat played a role in. Among 59 cases with train diseases, 53 were concussion by the accidents, such as traffic and falling or sliping down etc., the cerebral paralysis and hydrocephalus were two, respectively. And the blood diseases were severe newjaundice in all five cases. If we were summarized with the above mentioned, most of the hearing impairments were introduced by the combined reasons with familial or hereditary factors and the acquired, than by a simple disease. Among the congenital or hereditary hearing impairments classified to now a day, we suppose that the many cases with the acquired causes during pregnancy, delivery and after birth were complexed. Subsequently, the maternal and child health should be more and more developed in our country, also.

자궁강내인공수정시 황체기 보강으로서 경구 미분화 프로게스테론과 디드로게스테론의 비교 (Comparison of Oral Micronized Progesterone and Dydrogesterone as a Luteal Support in Intrauterine Insemination Cycle)

  • 목 적: 과배란유도 후 자궁강내 인공수정 시술시 황체기 보강으로서 경구 미분화 프로게스테론 투여법과 경구 디드로게스테론 투여법의 임상적 효용성을 비교하고자 하였다. 연구방법: 2007년 1월부터 2009년 8월까지 시행된 과배란유도 후 자궁강내 인공수정 시술 183주기를 후향적으로 분석하였다. 과배란유도는 성선자극호르몬 단독 또는 클로미펜과 성선자극호르몬의 병합요법을 사용하였다. 136주기에서는 황체기 보강으로서 경구 미분화 프로게스테론을 하루 300 mg으로 투여하였고 47주기에서는 디드로게스테론을 일일 20 mg으로 투여하였다. 결 과: 여성의 연령, 불임 인자, 성숙난포수 ($\geq$16 mm), 총운동성정자수, triggering 날의 자궁내막 두께는 두 군간 유의한 차이가 없었다. 자궁내 태낭이 확인되는 임상적 임신율은 미분화 프로게스테론 투여군에서 21.3%, 디드로게 스테론 투여군에서 19.1%로 차이가 없었다 (p=0.92). 유산율은 미분화 프로게스테론 투여군에서 다소 높은 경향을 보였으나 통계학적인 차이는 없었다 (34.5% vs. 11.1%, p=0.36). 결 론: 황체기 보강으로서 경구 디드로게스테론 투여법은 경구 미분화 프로게스테론 투여에 비하여 비슷한 임신율과 유산율을 보였다. 그러나 상대적으로 디드로게스테론 투여군의 수가 적어 좀더 많은 환자를 대상으로 한 전향적 연구가 필요하다.

'아유르베다'($\bar{A}yurveda$)의 의경(醫經)에 관한 연구 (A Study of The Medical Classics in the '$\bar{A}yurveda$')

  • Through a simple study of the medical classics in the '$\bar{A}yurveda$', we have summarized them as follows. 1) Traditional Indian medicine started in the Ganges river area at about 1500 B. C. E. and traces of medical science can be found in the "Rigveda" and "Atharvaveda". 2) The "Charaka" and "$Su\acute{s}hruta$(妙聞集)", ancient texts from India, are not the work of one person, but the result of the work and errors of different doctors and philosophers. Due to the lack of historical records, the time of Charaka or $Su\acute{s}hruta$(妙聞)s' lives are not exactly known. So the completion of the "Charaka" is estimated at 1st${\sim}$2nd century C. E. in northwestern India, and the "$Su\acute{s}hruta$" is estimated to have been completed in 3rd${\sim}$4th century C. E. in central India. Also, the "Charaka" contains details on internal medicine, while the "$Su\acute{s}hruta$" contains more details on surgery by comparison. 3) '$V\bar{a}gbhata$', one of the revered Vriddha Trayi(triad of the ancients, 三醫聖) of the '$\bar{A}yurveda$', lived and worked in about the 7th century and wrote the "$A\d{s}\d{t}\bar{a}nga$ $A\d{s}\d{t}\bar{a}nga$ $h\d{r}daya$ $sa\d{m}hit\bar{a}$ $samhit\bar{a}$(八支集)" and "$A\d{s}\d{t}\bar{a}nga$ Sangraha $samhit\bar{a}$(八心集)", where he tried to compromise and unify the "Charaka" and "$Su\acute{s}hruta$". The "$A\d{s}\d{t}\bar{a}nga$ Sangraha $samhit\bar{a}$" was translated into Tibetan and Arabic at about the 8th${\sim}$9th century, and if we generalize the medicinal plants recorded in each the "Charaka", "$Su\acute{s}hruta$" and the "$A\d{s}\d{t}\bar{a}nga$ Sangraha $samhit\bar{a}$", there are 240, 370, 240 types each. 4) The 'Madhava' focused on one of the subjects of Indian medicine, '$Nid\bar{a}na$' ie meaning "the cause of diseases(病因論)", and in one of the copies found by Bower in 4th century C. E. we can see that it uses prescriptions from the "BuHaLaJi(布哈拉集)", "Charaka", "$Su\acute{s}hruta$". 5) According to the "Charaka", there were 8 branches of ancient medicine in India : treatment of the body(kayacikitsa), special surgery(salakya), removal of alien substances(salyapahartka), treatment of poison or mis-combined medicines(visagaravairodhikaprasamana), the study of ghosts(bhutavidya), pediatrics(kaumarabhrtya), perennial youth and long life(rasayana), and the strengthening of the essence of the body(vajikarana). 6) The '$\bar{A}yurveda$', which originated from ancient experience, was recorded in Sanskrit, which was a theorization of knowledge, and also was written in verses to make memorizing easy, and made medicine the exclusive possession of the Brahmin. The first annotations were 1060 for the "Charaka", 1200 for the "$Su\acute{s}hruta$", 1150 for the "$A\d{s}\d{t}\bar{a}nga$ Sangraha $samhit\bar{a}$", and 1100 for the "$Nid\bar{a}na$", The use of various mineral medicines in the "Charaka" or the use of mercury as internal medicine in the "$A\d{s}\d{t}\bar{a}nga$ Sangraha $samhit\bar{a}$", and the palpation of the pulse for diagnosing in the '$\bar{A}yurveda$' and 'XiZhang(西藏)' medicine are similar to TCM's pulse diagnostics. The coexistence with Arabian 'Unani' medicine, compromise with western medicine and the reactionism trend restored the '$\bar{A}yurveda$' today. 7) The "Charaka" is a book inclined to internal medicine that investigates the origin of human disease which used the dualism of the 'Samkhya', the natural philosophy of the 'Vaisesika' and the logic of the 'Nyaya' in medical theories, and its structure has 16 syllables per line, 2 lines per poem and is recorded in poetry and prose. Also, the "Charaka" can be summarized into the introduction, cause, judgement, body, sensory organs, treatment, pharmaceuticals, and end, and can be seen as a work that strongly reflects the moral code of Brahmin and Aryans. 8) In extracting bloody pus, the "Charaka" introduces a 'sharp tool' bloodletting treatment, while the "$Su\scute{s}hruta$" introduces many surgical methods such as the use of gourd dippers, horns, sucking the blood with leeches. Also the "$Su\acute{s}hruta$" has 19 chapters specializing in ophthalmology, and shows 76 types of eye diseases and their treatments. 9) Since anatomy did not develop in Indian medicine, the inner structure of the human body was not well known. The only exception is 'GuXiangXue(骨相學)' which developed from 'Atharvaveda' times and the "$A\d{s}\d{t}\bar{a}nga$ Sangraha $samhit\bar{a}$". In the "$A\d{s}\d{t}\bar{a}nga$ Sangraha $samhit\bar{a}$"'s 'ShenTiLun(身體論)' there is a thorough listing of the development of a child from pregnancy to birth. The '$\bar{A}yurveda$' is not just an ancient traditional medical system but is being called alternative medicine in the west because of its ability to supplement western medicine and, as its effects are being proved scientifically it is gaining attention worldwide. We would like to say that what we have researched is just a small fragment and a limited view, and would like to correct and supplement any insufficient parts through more research of new records.

The "$Ast\bar{a}nga$ Sangraha $samhit\bar{a}$" was translated into Tibetan and Arabic at about the 8th$\sim$9th century, and if we generalize the medicinal plants recorded in each the "Charaka", "$Su\acute{s}hruta$" and the "$Ast\bar{a}nga$ Sangraha $samhit\bar{a}$", there are 240, 370, 240 types each. 4) The 'Madhava' focused on one of the subjects of Indian medicine, '$Nid\bar{a}na$' ie meaning "the cause of diseases(病因論)", and in one of the copies found by Bower in 4th century C. E. we can see that it uses prescriptions from the "BuHaLaJi(布唅拉集)", "Charaka", "$Su\acute{s}hruta$". 5) According to the "Charaka", there were 8 branches of ancient medicine in India : treatment of the body(kayacikitsa), special surgery(salakya), removal of alien substances(salyapahartka), treatment of poison or mis-combined medicines(visagaravairodhikaprasamana), the study of ghosts(bhutavidya), pediatrics(kaumarabhrtya), perennial youth and long life(rasayana), and the strengthening of the essence of the body(vajikarana). 6) The '$\bar{A}yurveda$', which originated from ancient experience, was recorded in Sanskrit, which was a theorization of knowledge, and also was written in verses to make memorizing easy, and made medicine the exclusive possession of the Brahmin. The first annotations were 1060 for the "Charaka", 1200 for the "$Su\acute{s}hruta$", 1150 for the "$Ast\bar{a}nga$ Sangraha $samhit\bar{a}$", and 1100 for the "$Nid\bar{a}na$". The use of various mineral medicines in the "Charaka" or the use of mercury as internal medicine in the "$Ast\bar{a}nga$ Sangraha $samhit\bar{a}$", and the palpation of the pulse for diagnosing in the '$\bar{A}yurveda$' and 'XiZhang(西藏)' medicine are similar to TCM's pulse diagnostics. The coexistence with Arabian 'Unani' medicine, compromise with western medicine and the reactionism trend restored the '$\bar{A}yurveda$' today. 7) The "Charaka" is a book inclined to internal medicine that investigates the origin of human disease which used the dualism of the 'Samkhya', the natural philosophy of the 'Vaisesika' and the logic of the 'Nyaya' in medical theories, and its structure has 16 syllables per line, 2 lines per poem and is recorded in poetry and prose. Also, the "Charaka" can be summarized into the introduction, cause, judgement, body, sensory organs, treatment, pharmaceuticals, and end, and can be seen as a work that strongly reflects the moral code of Brahmin and Aryans. 8) In extracting bloody pus, the "Charaka" introduces a 'sharp tool' bloodletting treatment, while the "$Su\acute{s}hruta$" introduces many surgical methods such as the use of gourd dippers, horns, sucking the blood with leeches. Also the "$Su\acute{s}hruta$" has 19 chapters specializing in ophthalmology, and shows 76 types of eye diseases and their treatments. 9) Since anatomy did not develop in Indian medicine, the inner structure of the human body was not well known. The only exception is 'GuXiangXue(骨相學)' which developed from 'Atharvaveda' times and the "$Ast\bar{a}nga$ Sangraha $samhit\bar{a}$". In the "$Ast\bar{a}nga$ Sangraha $samhit\bar{a}$"'s 'ShenTiLun(身體論)' there is a thorough listing of the development of a child from pregnancy to birth. The '$\bar{A}yurveda$' is not just an ancient traditional medical system but is being called alternative medicine in the west because of its ability to supplement western medicine and, as its effects are being proved scientifically it is gaining attention worldwide. We would like to say that what we have researched is just a small fragment and a limited view, and would like to correct and supplement any insufficient parts through more research of new records.
