  • Autistic disorder and other PDD are currently viewed as a largely genetically determined neurodevelopmental disorder, although its underlying biological causes remain to be established. In this review, we examine the available neurodevelopmental literature on autistic disorder and discuss the findings that have emerged. Typical neuropathological observations are rather consistent with respect to the limbic system (increased cell packing density and smaller neuronal size), the cerebellum (decreased number of Purkinje cells) and the cerebral cortex ($>50\%$ of the cases showed features of cortical dysgenesis). However, most of the reported studies had to contend with the problem of small sample sizes, the use of quantification techniques, not free of bias and assumptions, and high percentages of autistic subjects with comorbid mental retardation or epilepsy. Furthermore, data from the limbic system and on age-related changes lack replication by independent groups. It is anticipated that future neuropathological studies held great promise, especially as new techniques such as design-based stereology and gene expression are increasingly implemented and combined, larger samples are analysed, and younger subjects free of comorbidities are investigated.

Korean Wechsler Intelligence Scale for Children-Fourth Edition Profiles in Child and Adolescent with Attention-Deficit Hyperactivity Disorder : Retrospective Study (주의력결핍 과잉행동장애 아동청소년의 Korean Wechsler Intelligence Scale for Children-Fourth Edition 프로파일 : 후향적 의무기록 분석)

  • Objectives : The aim of this study was to investigate Korean Wechsler Intelligence profiles and specific abilities related to attention problem of children with attention-deficit hyperactivity disorder (ADHD). Methods : The Korean Wechsler Intelligence Scale for Children-fourth edition (K-WISC-IV) and Advanced Test of Attention (ATA) were administered to 91 children and adolescents (age $8.5{\pm}2.6$ years, 73 boys) with ADHD. Pearson correlation and independent t-tests were used. Results : The means of Working Memory Index (WMI) and Processing Speed Index (PSI) showed a score of low average in K-WISC-IV. WMI scores for the K-WISC-IV showed clinically significant correlations with omission errors, commission errors, and response time variability on auditory ATA. PSI scores also showed significant correlations with response time and variability on visual ATA. In addition, significantly lower digit span backward scores were observed in hyperactive-impulsive/combined subtypes compared to inattentive subtype (t=3.60, p<.001). Conclusion : Children with ADHD showed significantly lower scores in WMI and PSI which were clinically correlated with ATA scores, and hyperactive-impulsive/combined subtypes showed poorer working memory functions in WMI. Follow-up studies are proposed.

Review on the Kampo Treatment and Education Program for Children with Developmental Disabilities in Japan (일본에서 발달장애아를 대상으로 Kampo치료 및 교육프로그램의 제공에 대한 연구동향)

  • Objectives This study reviewed the effects of a combined treatment with Kampo and Western medicine for Developmental disability in Japan, and the provision of education programs in clinical care. Methods The search database includes J-STAGE. To narrow the search, the following key words were used: 'pervasive developmental disorders, Attention-Deficit/Hyperactivity Disorder, Learning Disorders or Learning Disabilities, Intellectual Disability, and Kampo'. The search was limited to the publication date from 2001 to 2019. Results 1. Japan analyzed five sections: The usage of the Kampo medicine ranges from 25.2% to 71.6%, and the Kampo medicine was highly used in large cities. 2. In Japan, the educational programs were provided for the caregiver and special educational programs were available for children with disabilities. 3. In Japan, there were 9 studies regarding developmental disability treating with herbal remedies. There were seven clinical trial reports, and two were published in a review or report form. 4. The results showed benefits of using Kampo for patients with lack of Yin in blood in treatment of developmental disorder. It is also important to control the liver qi and Yin in blood. 5. Seven papers reported no side effects or abnormal findings. They have reduced the use of antipsychotics. Conclusions These review studies in regards to the combined treatment of Kampo and Western medicines can be helpful to improve long term side effects of the antipsychotics used in developmental disorders.

Acupuncture and Electroacupuncture Treatment for Temporomandibular Joint Disorder Focused on Recent Randomized-Controlled Studies: A Systematic Review and Meta-Analysis

  • This study aimed to evaluate the clinical efficacy and safety of acupuncture and electroacupuncture for temporomandibular joint disorder (TMD). We searched 11 databases to find randomized-controlled trials (RCTs) published between January 2018 and January 2023 that used acupuncture and electroacupuncture to treat patients with TMD. We performed a meta-analysis of 18 RCTs involving 1,109 patients. Nine studies reported that the treatment and control groups showed improvement in the evaluation index post-treatment. The results of the meta-analysis showed that the treatment groups receiving acupuncture combined with manual therapy had significantly improved visual analog scale score and dysfunction index than the control groups receiving manual therapy. Moreover, the treatment groups receiving acupuncture combined with Western medicine had statistically improved maximum mouth opening than the control groups using Western medicine. In conclusion, acupuncture and electroacupuncture have clinical effects on TMD patients. Given that 11 studies were obtained from the Chinese database, a regional bias would be considered. Furthermore, the included studies had high a risk of bias in terms of randomization, concealment, and blinding. Further studies are required to correct these limitations.

Effects of Dynamic Stretching Exercise Combined with Transcranial Direct Current Stimulation on Lower Extremity Muscle Activity and Jump Performance in Soccer Player (경두개직류자극을 결합한 동적 신장운동이 축구선수의 다리 근활성도와 점프수행력에 미치는 영향)

  • Purpose : The purpose of this study was to provide the effective method for exercise therapy in soccer player. Methods : 30 soccer players were recruited for subjects. The subjects were divided to dynamic stretching exercise combined tDCS (Group I), dynamic stretching exercise combined Sham-tDCS (Group II) of which 15 subjects were randomly allocated. The subjects were given 3 hour of soccer skill and performance and additionally given 20 minutes of dynamic stretching exercise combined tDCS and sham-tDCS for each group, five times a week, for 6 weeks. Their muscle activity and jump performance were analyzed before the intervention. After 6 weeks of the intervention, the mentioned parameters were measured once more for between-group analysis. Results : Comparative analysis of the muscle activity and jump performance between the groups I and groups II showed statistically significant difference. Conclusion : Such results revealed that dynamic stretching exercise combined with tDCS is effective in muscle activity and jump performance. Based on the current study, more effective program is to be proposed for neurologic and musculoskelectal disorder as well as soccer player. Based on the current study, studies that incorporates various combine of variable is required for development of effective tDCS program.

Recent Advances in Cognitive Behavioral Therapy for Panic Disorder (공황장애 인지행동치료의 최신 지견)

  • 30% of patients with panic disorder (PD) show treatment-resistant and chronic waxing and waning course. Therefore, adequate treatment strategies for PD by evidence based pharmacotherapy and combined cognitive behavioral therapy (CBT) are recommended. Regarding how and why CBT for PD works, three hypotheses include the cognitive theory, anxiety control theory, and behavioral theory were discussed. The recent findings that the altered activation in frontal lobe is normalized after CBT, suggest a reduction of an altered top-down fear processing in the neural correlates of CBT in PD. In order to improve accessibility to CBT, brief CBT and internet based CBT for PD were suggested. Despite limitations of sample sizes and study design, most of studies suggest that brief CBT is more effective than control conditions, and even as equally effective as standard CBT. The evidences suggest that internet based CBT may not be significantly different from face-to-face CBT in reducing anxiety. Several advances within the field of third-wave CBT for PD have led to the development of new techniques based on mindfulness, such as mindfulness-based cognitive therapy and acceptance and commitment therapy. Based on Korean algorithm project for panic disorder, especially the psychological education and cognitive reconstruction components were recommended in CBT with PD.

The Effects of Recovery Oriented Practices on Psychiatric Symptoms of People with Mental Disorder (회복지향실천이 정신장애인의 정신과적 증상에 미치는 영향)

  • This study examines the effects of recovery-oriented practices on psychiatric symptoms of people with mental disorder and further explores the impact of recovery-oriented practices in each subcategories on psychiatric symptoms, thereby demonstrating the intent of restoring the community mental health rehabilitation center. For this, a survey was conducted on 378 people with mental disorder who used community mental health rehabilitation center and correlation and hierarchical regression analysis were conducted. The study found that among the five subcategories of recovery, including age and duration of illness, 'Life Goal,' 'Involvement,' and 'diversity of treatment options' alleviate symptoms of people with mental disorder. Based on these findings, community mental health rehabilitation center has presented the combined significance of psychiatry and social welfare programs to facilitate the recovery of the mentally handicapped by reflecting the value and factors of recovery-oriented practice.

  • Recent breakthroughs in functional neuroimaging techniques have launched the quest of mapping the connections of the human brain, otherwise known as the human connectome. Imaging connectomics is an umbrella term that refers to the neuroimaging techniques used to generate these maps, which recently has enabled comprehensive brain mapping of network connectivity combined with graph theoretic methods. In this review, we present an overview of the key concepts in functional connectomics. Furthermore, we discuss articles that applied task-based and/or resting-state functional magnetic resonance imaging to examine network deficits in post-traumatic stress disorder (PTSD). These studies have provided important insights regarding the etiology of PTSD, as well as the overall organization of the brain network. Advances in functional connectomics are expected to provide insight into the pathophysiology and the development of biomarkers for diagnosis and treatment of PTSD.

  • With the analysis result about previous curative effect researchs, It has been reported that game play therapy is effective for enhancing social intelligence of handicappted children who have ADHD, tic disorder, communication disorder, or autism. This study was designed to develop a digilog game play therapy contents that was combined analog and digital. We infer that this contents is effective game play therapy not only for ordinary people but also for dementia old man, or intellectual disorder man. Also We are going to develop a new digilog play game system for preventing children's smartphone addiction.

Association between psychological factors and temporomandibular disorders in Korean adults : The fouth Korean national health and nutritional examination survey(2009) (한국 성인의 심리적 요인과 측두하악관절 장애와의 관련성 : 제4기 3차년도(2009) 국민건강영양조사)

  • Objectives : This study aimed to investigate relationship between the psychological factors, the prevalence, and perception of temporomandibular disorder in Korean adults. Methods : A total of 7203 data were derived from Korean national health and nutritional examination survey. All data were analyzed using R-COMMANDER(12.2 SDI) program. Results : The prevalence of temporomandibular disorder in Korean adults was 3.1% and 10.5% of adults with temporomandibular disorder (TMD) had more than one TMD related symptom. Women had higher prevalence rate of TMD related symptom than men. Prevalence rate in TMD was low in the younger age, the higher education level, higher income, and the professionals. Psychological factors including cognition of stress and depression was shown to be closely related with suicidal ideation and TMD prevalence. Conclusions : To prevent TMD, it is necessary to provide the systematic oral health education and to develop the combined psychological counseling with treatment program.
