A Improved Case of Post Cerebral Infarction Dizziness and Gait Discomfort after Treated with Korean Medicine Treatment and Vestibular Rehabilitation Practice (한의복합치료와 전정재활운동을 병행하여 뇌졸중 후 어지러움과 보행불안 증상이 개선된 증례 보고)

  • The aim of this study was to report the effect of Korean medicine treatments and vestibular rehabilitation practice on an stroke patient with dizziness. The patient received Korean medicine treatments, such as acupuncture and herbal medication, and vestibular rehabilitation practice. The effects of treatments were assessed with the Korean dizziness handicap inventory, Korean activities-specific balance confidence scale, Korean vestibular disorders activities of daily living scale. Researchers conducted gait analysis for evaluation patient's gait disturbance's improvement during treatments. The intensity of dizziness were significantly reduced after the complete treatment. After treatment with traditional Korean medicine and vestibular rehabilitation practice, the patient showed significant improvements in symptoms and gait parameters. This case proved that combined therapy of Korean medicine treatment and vestibular rehabilitation practice can be useful for stroke patients who suffered from dizziness. however, studies of larger populations are required in the future.

  • Dose calculations which are a crucial requirement for radiotherapy treatment planning systems require accuracy and rapid calculations. The conventional radiotherapy treatment planning dose algorithms are rapid but lack precision. Monte Carlo methods are time consuming but the most accurate. The new combined system that Monte Carlo methods calculate part of interesting domain and the rest is calculated by neural can calculate the dose distribution rapidly and accurately. The preliminary study showed that neural networks can map functions which contain discontinuous points and inflection points which the dose distributions in inhomogeneous media also have. Performance results between scaled conjugated gradient algorithm and Levenberg-Marquardt algorithm which are used for training the neural network with a different number of neurons were compared. Finally, the dose distributions of homogeneous phantom calculated by a commercialized treatment planning system were used as training data of the neural network. In the case of homogeneous phantom;the mean squared error of percent depth dose was 0.00214. Further works are programmed to develop the neural network model for 3-dimensinal dose calculations in homogeneous phantoms and inhomogeneous phantoms.

Study of a Brain Tumor and Blood Vessel Detection System Using Multiple Fluorescence Imaging by a Surgical Microscope (수술현미경에서의 다중형광영상을 이용한 뇌종양과 혈관영상 검출 시스템 연구)

  • In this paper, we propose a microscope system for detecting both a tumor and blood vessels in brain tumor surgery as fluorescence images by using multiple light sources and a beam-splitter module. The proposed method displays fluorescent images of the tumor and blood vessels on the same display device and also provides accurate information about them to the operator. To acquire a fluorescence image, we utilized 5-ALA (5-aminolevulinic acid) for the tumor and ICG (Indocyanine green) for blood vessels, and we used a beam-splitter module combined with a microscope for simultaneous detection of both. The beam-splitter module showed the best performance at 600 nm for 5-ALA and above 800 nm for ICG. The beam-splitter is flexible to enable diverse objective setups and designed to mount a filter easily, so beam-splitter and filter can be changed as needed, and other fluorescent dyes besides 5-ALA and ICG are available. The fluorescent images of the tumor and the blood vessels can be displayed on the same monitor through the beam-splitter module with a CCD camera. For ICG, a CCD that can detect the near-infrared region is needed. This system provides the acquired fluorescent image to an operator in real time, matching it to the original image through a similarity transform.

  • Odontogenic keratocyst (OKC) is a epithelial developmental cyst which were first described by Phillipsen in 1956. The frequency of OKC has been reported to vary from 3% to 11% of odontogenic cysts. The most characteristic clinical aspect of OKC is the high frequency of recurrence. The mechanism of recurrence is thought to be related to residues of cyst epithelium and an intrinsic growth potential following excision. And since the lining of the OKC is thin and friable, removal of the cyst in one piece may sometimes be difficult. Complete removal of the cyst lining without leaving behind remnants attached to the soft tissue or bone is necessary to avoid recurrence. Therapeutic approaches vary in different studies from marsupialization and enucleation, which may be combined with adjuvant therapy such as cryotherapy or Carnoy's solution, to marginal or radical resection. The recurrent rate varies from approximately 20% to 62%. And OKC in the angle-ramus region of the mandible had a higher tendency to recur, because of the difficulty in accessing and removing OKC from the ramus. By employing a sagittal splitting of the mandible a good surgical access was provided and cyst could be removed completely. We present an illustrative case of a small, lobulated OKC that involved ramus on mandible, and a review of the contemporary literature.

The Effect of Psychotherapy Using Forest Environment on Depressive Symptoms in Patients with Major Depressive Disorder : a Preliminary Study (산림활동 심리프로그램이 우울증 환자들의 우울감에 미치는 영향 : 예비적 연구)

  • The use of natural environments to change lifestyle and health has been long recognized. In particular, forests, trees, and open space have been shown to promote mental health. In this study, we examined the effectiveness of the structured psychotherapeutic program using forest environment ("forest activity program") to improve the symptoms of nine patients with Major Depressive Disorder (MDD) who were taking variable doses of antidepressants. We assessed the depressive symptoms, quality of life, and autonomic nerve regulation among the MDD patients. Hamilton Rating Scales for Depression (HRSD) scores significantly decreased after the forest therapy (13.56 vs. 5.56, p=0.003), and some subscores of Short Form 36 health survey questionnaire (SF-36) and heart rhythm coherence are improved as well. Combined with antidepressant pharmacotherapy, the structured psychotherapeutic program using forest environment showed an improved health status for MDD patients and thus has potential as an adjuvant treatment for MDD, especially for rehabilitation and relapse prevention.

The Effects of the Thoracic Mobilization Exercise Using Kaltenborn on the Convergence Pulmonary Function of 20's Normals (칼텐본을 접목한 흉추가동운동이 20대 대학생의 복합적 폐활량에 미치는 영향)

  • The purpose of this study was to examine the effects of a thoracic mobilization exercise using the Kaltenborn on the convergence lung function. The study was conducted on 20 university students in their 20s over a four-week period. The 20 subjects were randomly selective assigned to a Kaltenborn thoracic mobilization exercise group and diaphragm exercise control group. The experimental group performed a Kaltenborn thoracic mobilization exercise for 30 minutes. The control group performed a diaphragmatic breathing exercise for 30 minutes. Each exercise program was performed three times a week for four weeks. When comparing the breathing capacity of the experimental and controls before and after the experiment, the experimental group showed significant changes in TV, IRV. In testing the differences between the experimental and controls in their changes after the experiment, TV, IRV showed a significant change. Based on the results of this study, a thoracic mobilization exercise using the Kaltenborn thoracic mobilization exercise may have positively affected the extrementals lung function. Therefore, if a thoracic mobilization exercise using the Kaltenborn is combined with a respiratory exercise program in the future, it will likely become a more effective treatment technique.

Prospective phase II trial of regional hyperthermia and whole liver irradiation for numerous chemorefractory liver metastases from colorectal cancer

  • Purpose: A prospective phase II trial was conducted to evaluate the effectiveness and toxicity of regional hyperthermia and whole liver irradiation (WLI) for numerous chemorefractory liver metastases from colorectal cancer. Materials and Methods: Enrolled patients had numerous chemorefractory hepatic metastases from colorectal cancer. Five sessions of hyperthermia and seven fractions of 3-gray WLI were planned. Health-related quality of life (HRQoL) was determined using the Korean version of the European Organization for Research and Treatment of Cancer quality of life questionnaire C-30 and the Functional Assessment of Cancer Therapy-Hepatobiliary version 4.0. Objective and pain response was evaluated. Results: A total of 12 patients consented to the study and the 10 who received WLI and hyperthermia were analyzed. WLI was completed as planned in nine patients and hyperthermia in eight. Pain response was partial in four patients and stable in four. Partial objective response was achieved in three patients (30.0%) and stable disease was seen in four patients at the 1-month follow-up. One patient died 1 month after treatment because of respiratory failure related to pleural metastasis progression. Other grade III or higher toxicities were detected in three patients; however, all severe toxicities were related to disease progression rather than treatment. No significant difference in HRQoL was noted at the time of assessment for patients who were available for questionnaires. Conclusion: Combined WLI and hyperthermia were well tolerated without severe treatment-related toxicity with a promising response from numerous chemorefractory hepatic metastases from colorectal cancer.

Injection-Acupuncture Combined by Conventional Treatment in Canine Demodicosis : Case Report (개 모낭충증에 있어서 약침과 기존 치료의 병용 : 증례 보고)

  • Therapeutic effect by injection-AP with antibiotics plus conventional treatment was investigated to establish a new treatment method for canine demodicosis. Three patients diagnosed into canine demodicosis were used in the present study. Injection-AP with enrofloxacin (5 mg/kg, twice a week) was made into the acupoints such as Fei Shu(BL13), Wei Zhong (BL40), Da Zhui (GV14), Zu San Li (ST36), He Gu (LI04), Qu Chi (LI11) and Sang Yin jiao(SP06) for 5 weeks. Further, they were additionally received with conventional treatments including injection of ivermectin (0.5 mg/kg, twice a week, SC) and 0.05% amitraz rinse (twice a week) for 5 weeks. The clinical symptoms (pruritus, excoriation, erythema and alopecia) were much improved by injection-AP therapy plus conventional treatment, compared by those of pre-treatment. Case 1 became to normal after 4 weeks, Case 2 became to mild after 5 weeks and Case 3 became to normal after 5 weeks. Total WBC revealed decreasing tendency with treatment in all cases, however, N/L ratios didn't show regular pattern of change with treatment. Notable changes were not histopathogically found in skin lesions of Case 1 and Case 3 except Case 2 with mild change after 5 weeks treatment, compared by those of pre-treatment, respectively. In conclusion, the present patients were cases with canine demodicosis which revealed favorable therapeutic response by injection-AP with antibiotics plus conventional treatment.

Research Trends of the Traditional Korean Medicine Treatment for Atopic Dermatitis -Based on the Journal of Pediatrics of Korean Medicine- (아토피 피부염의 한의학적 치료에 대한 연구 동향 -대한한방소아과학회지를 중심으로-)

  • Objectives The goal of this review was to investigate clinical, experimental and literature studies of Korean medicine on atopic dermatitis (AD) of Korean children in Korean medicine literature, seeking for the better research methods for more effective treatments. Methods Electronic investigations were practiced on AKOP (The Association of Korean Oriental Pediatrics, http://www.akop.or.kr) to collect theses which were published by J Pediatr Korean Med. The key word 'Atopy' was used for searching to ensure that every related thesis was collected. The publication date was limited from 1996 to 2016. The selected literatures were assessed mainly according to distributions of study type, publication year, scale, treatment and its efficacy. Results 55 papers were selected from 59 studies. In these collected 55 related theses, there were 29 experimental studies (52.73%), 18 clinical studies (32.73%), 8 review studies (14.55%). In the 29 experimental studies, there were 20 studies (68.97%) using NC/Nga mouse as subjects. The mite antigen was used to trigger AD by 8 studies (27.59%). The studies have been utilizing DNCB and DNFB instead of mite antigen since 2011. All the experimental studies showed that the Korean medicine was effective in AD. In total 18 clinical studies, 3 studies (16.67%) were case reports, only 1 thesis (5.56%) was conducted by case control study and the rest were case series studies. 7 studies (38.89%) of 18 studies diagnosed AD according to Hanifin and Rajka's diagnostic criteria (1980), only 1 study (5.56%) according to the Korean standard and the rest 10 studies (55.56%) didn't mention diagnostic criteria. 12 clinical studies (66.67%) showed efficacy in treating AD. There were 10 studies (55.56%) only employing internal treatments and 1 thesis (5.56%) didn't mention the exact prescription. Naesowhajungtang-kamibang, Saenghyeoryunbue-um were used 3 times (16.67%) as the internal medicine respectively. 5 theses (27.78%) were combined with acupuncture therapy. Hegu (LI4), Sanyinjiao (SP6) were used in 4 theses, and Taichong (LR3), Quchi (LI11), Zusanli (ST36) in 3 theses. The number of acupoints chosen from The Spleen meridian of Foot-Taiyin was the largest. There were 8 review theses. 2 were about the foreign oriental medicine, 3 were about the external medicine or external treatment methods, and 3 other studies were about severity scoring systems, the methodological study in the latest clinical study, overview for pattern and results of herbal medicine-derived AD clinical researches. Conclusions The experimental studies and clinical studies showed the effectiveness of Korean medicine treatments. However, this study still needs improving by conducting more comparative studies and using better research methods, in order to find more effective treatments to improve clinical efficacy.

A Case Report of Non-Motor Symptoms Evaluated Using the Non-Motor Symptom Scale in a Patient with Secondary Parkinsonism Presumed to be Probable Lewy Body Dementia and Improved with Combined Treatment with Herbal Medicine and Acupuncture (루이소체 치매로 추정되는 이차성 파킨슨증 환자의 Non-Motor Symptom Scale(NMSS)로 평가한 비운동성 증상을 한약과 침의 복합치료로 호전시킨 증례보고 1례)

  • Parkinson's syndrome is a degenerative brain disease that presents characteristic motor symptoms of tremor, rigidity, and gait disturbance. In addition to these motor symptoms, Parkinson's syndrome also presents non-motor symptoms (NMSs) such as sleep disturbance and cognitive decline. NMSs reduce patient's quality of life and psychosocial functioning and cause economic burden on the patient, so appropriate evaluation and treatment are required. Lewy body dementia is one of the several diseases belonging to Parkinson's syndrome. Its symptoms such as cognitive function, memory impairment, and hallucinations occur with Parkinsonism. Although drug therapy is being used with drug treatment to treat non-motor symptoms, it has limitations such as side effects, which stimulated interest in other complementary treatment methods such as oriental medicine treatment, dance, and yoga. The patient in this case complained of tremor in the right upper extremity, muscle hypertension and pain, and persistent vision, memory, and cognitive decline. The patient was diagnosed with probable Lewy body dementia. The patient was hospitalized for 4 months and received acupuncture and herbal medicines. After treatment, the patient's NMS scale scores decreased from 90 to 63, and the Unified Parkinson's Disease Rating Scale scores (summed I, II, and III) decreased from 17 points to 8 points. The Beck Depression Inventory score decreased from 22 points to 13 points. In addition, the patient's subjective evaluation revealed improvement. In this case, a patient diagnosed with probable Lewy body dementia who did not respond to the standard treatment and did not want to take medications showed improvement in not only motor symptoms but also NMSs after integrative Korean medicine treatment.
