  • Objectives : Evidence supporting the cold-heat symptom and sasang constitution type, which are diagnostic items of traditional Korean medicine, is needed to manage sleep disturbances, which is a typical symptom of mibyeong (subhealth). This study examined the association between each cold-heat symptom and sleep disturbances according to each sasang constitution type. Methods : This research was a cross-sectional study of 5,793 subjects from the Korean Medicine Data Center (KDC) community cohort survey. The association between each cold-heat symptom and sleep disturbances was analyzed by logistic regression analysis adjusted for several demographic variables. Subgroup analysis was then performed for each type of sasang constitution. Results : The soeum and soyang types were 1.53 and 1.26 times more likely to have sleep disturbances than the taeum type. Sleep disturbances were associated with 'coldness of the abdomen', 'watery mouth' in the cold domain items, and 'body feverishness', 'flushed face and eye', 'thirst', and 'scanty dark urine' in the heat domain items. The soeum and soyang types were 1.55 and 1.39 times more likely to sleep less than five hours per night than the taeeum type. In addition, the associations of those showed a different pattern for each sasang constitution type. Conclusions : Sleep disturbances are associated with specific cold-heat symptoms, and the associated cold-heat symptoms differ according to the sasang constitution type. These results may help traditional medicine specialists select customized interventions for patients with sleep disturbances.

Methods: As a prospective cross-sectional study based on medical records, 9213 patients, who visited Kyung Hee University Medical Center from May in 2007 to June in 2010, were included. To diagnose the constitution, Revised Questionnaire for the Sasang Constitution Classification II and examinations by oriental medical doctors who majored in Sasang Constitution Medicine were performed. Based on the World Health Organization criteria for Asian obesity assessment, body mass index (BMI) was classified into 3 groups; more than 25 as obesity, 23 to 25 as overweight, and less than 23 as normal. Ordinary symptoms questionnaire, which consists of 46 questions in 8 categories, were given to the subjects. One-way ANOVA test in continuous variables and chi-square test in categorical variables were carried out to analyze statistical significance. Results and Conclusions: 1) In general characteristics, we could find out the significant differences in age, sex, height, weight, BMI, waist circumference, blood pressure, total cholesterol, triglyceride, low density lipoprotein cholesterol, high density lipoprotein cholesterol, fasting blood sugar, and hemoglobin A1c among the 3 Constitutional groups. 2) Soyang Constitution of overweight and obesity has tendency to have higher appetite, like cool or cold water, defecate more, swear more, dislike heat, and have warmer or hotter extremities in the ordinary symptoms, compared with the control group. 3) Taeeum Constitution of overweight and obesity has tendency to have higher appetite, like cool or cold water, defecate more, swear more, dislike heat, and have warmer or hotter extremities in the ordinary symptoms, compared with the control group. 4) Soeum Constitution of overweight and obesity has tendency to have higher appetite and less fatigue in the ordinary symptoms, compared with the control group. 5) Soeum Constitution showed the slightest increase in appetite, preference about room temperature water, less number of feces, and feeling of chill and warmer extremities in ordinary symptoms, compared with Soyang and Taeeum Constitutions. Taeeum Constitution showed the distinct increase in appetite and sweating in ordinary symptoms, compared with Soyang and Soeum Constitutions.

  • Objectives In Sasang Constitutional medicine(SCM), it is an important factor to assess the degree of cold/heat and healthy status. We purposed to check the agreement among the experts on the assessment of cold/heat and healthy status. Methods Six SCM experts and 10 healthy volunteers were recruited for this study. The SCM experts had an interviewed with each volunteer in the similar condition like that of the actual clinic. And then the expert evaluated each volunteer's cold/heat and healthy status. Intraclass correlation coefficient(ICC) was used to measure the interrater reliability and interrater agreement among the experts. Results Two volunteers are excluded due to the missing data, therefore eight persons' data (3 males and 5 females) were included in our analysis. The mean of age and BMI are 21.73(${\pm}2.32$) and 29.63(${\pm}4.66$), respectively. In health status, cold status and heat status, the ICC was 0.789, 0.904, 0.925, respectively. Conclusions We found that the assessment of cold/heat and healthy status was continued reliable among SCM experts. The assessment of cold/heat status was more consistent than the assessment of healthy status.

  • 1. Objective The object of this study was to understand the clinical characteristics of the Taeeumin Symptomatic Pattem. 2. Methods The patients visiting the department of Sasang Constitution of Traditional Korean Medicine clinic from June to November of 2008 were used in this study. Those who showed a favorable turn after over ten days of medication and who showed up for at least two follow-up sessions were chosen among first-time visitors. Of these, 75 patients identified and verified to be of the TE type were chosen for the study sample. The Taeeumin were verified into two groups according to clinical symptoms into Exterior Cold Disease and Interior Heat Disease groups, of which the Interior Heat Disease group was further subdivided into the Galgeun-medicament group and the Cheongshimyeonja-tang group, based on questionnaire results. The Exterior Cold Disease group was set as the control group in order to contrast and compare it with the study group, the Interior Heat Disease group. 3. Results and Conclusions The questionnaire items shown to differentiate the Exterior Cold Symptomatic Patterm and the Interior Heat Symptomaic Pattern (indicating Heat-affceted Liver Symptomatic Pattern) were related to "dryness of stool," "dark urine colorm," "dryness of nose," and "dryness of lips." The items shown to differentiate the Galgeun-medicament group and the Cheongshimyeonja-tang group (of the Dry - Heat Symptomatic Pattern) were related to "nausea," "stifling sensations and palptation," "disturbed sleep," and "dizziness." Therefore, the sleep pattern, palpitation, dizziness, and nausea can be said to be indicators of the Interior Heat Symptomatic Pattern, and Dry-Heat symptoms are better distinctions between the Chongshimyeonja-tang group and the Galgeun-medicament group than Heat-affected Liver symptoms.
