Changes in Blood Superoxide Dismutase Activities after Alcohol Withdrawal (주정금단 후 혈중 Superoxide Dismutase 활성도의 변화)

  • Objectives : The aims of this study were to evaluate changes in plasma superoxide dismutase(SOD) activities in alcohol depedence, to find out variables to influence on the SOD activities, and finally to identify the correlation of SOD activities with the alcohol-associated cognitive disorders. Methods : For 24 male alcoholics and 21 healthy male controls, plasma SOD activities were measured by spectrophotometry on 1-2 wks after alcohol withdrawal. Structured interviews and laboratory tests were also performed. Results : 1) Upon comparing SOD activities between controls and alcoholics, the SOD activities were significantly(p<0.01) lower in alcoholics($0.308{\pm}0.140$ units/mL) than in healthy controls($0.313{\pm}0.086$ units/mL). 2) Upon comparing SOD activities according to the presence of alcohol-related cognitive disorders, the SOD activities were significantly(p<0.05) lower in alcoholics with cognitive disorders($0.247{\pm}0.049$ units/mL) than in alcoholics without cognitive disorders($0.317{\pm}0.148$ units/mL). 3) Upon comparing SOD activities according to the presence of alcoholic polyneuropathy or alcohol withdrawal seizure, the SOD activities showed no significant differences between alcoholics with polyneuropathy or epilepsy and those without. 4) Upon analyzing variables influencing on the SOD activities in alcoholics, the SOD activities had the negative correlation with hemoglobin(${\gamma}=-0.433$) and severity of alcohol withdrawal symptoms(${\gamma}=-0.375$). 5) Upon comparing variables according to the presence of alcohol-related cognitive disorders, the occurrence of alcoholic polyneuropathy(p<0.05) and blood phosphorus concentrations(p<0.01) were significantly higher in alcoholics with cognitive disorders than those without. 6) Upon analyzing an association between SOD activities and variables in alcoholics with cognitive disorders, the SOD activities were positively correlated with the onset age(${\gamma}=0.995$), and negatively correlated with the severity of alcohol withdrawal symptoms(${\gamma}=-0.996$). Conclusions : Lower SOD activities in alcohol dependence suggested alcohol-associated cognitive disorders and alcohol withdrawal symptoms might be caused by oxidative stress.

  • Post traumatic stress disorder (PTSD) is an anxiety disorder that usually occurs after suffering from a psychological trauma that can not threatening events, which is one of the individual's subjective pain and their families and the larger society due to the chronic disease burden results. Treatment with a medication, as well as cognitive behavioral therapy (CBT), exposure therapy, prolonged exposure therapy (PE), eye-movement desensitizing, and reprocessing (EMDR) are preferentially performed, including psychotherapy. Korean medical psychotherapy consists of a wide range of manual therapy, but it does not have systematically screening limits. This study was developed by Korean medical psychotherapy program for PTSD, and it was carried out as a preliminary clinical trial for validity. The Korean medical psychotherapy program for PTSD consists of four stages as follows: building chapter of safety, flow and send, accept, and integration. Based on this preliminary clinical study of PTSD, seven patients were treated for 2 hours per week for 5 weeks. We report the meaningful results gained by observing the changes in IES-RK points, questionnaire on five-faceted mindfulness, quality of life, anxiety, depression, insomnia, hwabyung scale, HRV, and EEG, before and after the Korean medical psychotherapy program.

  • Obsessive compulsive disorder is a debilitating condition characterized by recurrent obsessive thoughts and compulsive reactions. A great portion of the obsessive compulsive disorder (OCD) patients are managed successfully with psychiatric treatment such as selective serotonin-reuptake inhibitor and cognitive behavioral psychotherapy, but more than 10% of patients are remained as non-responder who needs neurosurgical treatments. These patients are potential candidates for the neurosurgical management. There had been various kind of operation, lesioning such as leucotomy or cingulotomy or capsulotomy or limbic leucotomy, and with advent of stereotaxic approach and technical advances, deep brain stimulation was more chosen by neurosurgeon due to its characteristic of reversibility and adjustability. Gamma knife radiosurgery are also applied to make lesion targeting based on magnetic resonance (MR) imaging, but the complication of adverse radiation effect is not predictable. In the neurosurgical field, MR guided focused ultrasound has advantage of less invasiveness, real-time monitored procedure which is now growing to attempt to apply for various brain disorder. In this review, the neurosurgical treatment modalities for the treatment of OCD will be briefly reviewed and the current state of MR guided focused ultrasound for OCD will be suggested.

  • Neurofeedback, also known as EEG biofeedback or neurotherapy, is a new therapy differentiated from biofeedback in that brain waves are monitored to study brain itself as well as its functions. Needless to say, it has originated from biofeedback, however it has established itself as an independent and specialized therapy in the field of neurotherapy. In order to have a better understanding of neurofeedback we are going to have a look into its historical and theoretical background first and then we will explain how it can be used clinically. Neurofeedback was clinically introduced for epilepsy for the first time and its effect was proved. Since then it has been used successfully for a number of psychiatric disorders, such as attention deficit hyperactivity disorder(ADHD), anxiety, anxiety and mood disorder, sleep disorder, substance dependency, etc. Furthermore, it has been used for non-psychiatric or psychosomatic symptoms, such as headache, chronic pains, chronic fatigue symptoms, etc. Recently, it has been also used for non-medical areas, such as improvement of peak performance for sports and art activities, enhancement of cognitive function, etc, making the neurofeedback a very promising field for its potential and effectiveness.

Predictors of Hospitalization for Alcohol Use Disorder in Korean Men (알코올사용장애로 인한 한국성인남성의 병원입원여부에 미치는 영향요인)

  • Purpose: This study was done to identify the patterns and significant predictors influencing hospitalization of Korean men for alcohol use disorder. Methods: A descriptive study design was utilized. Data were collected using self-report questionnaires from 143 inpatients who met the DSM-5 alcohol use disorder criteria and were receiving treatment and 157 social drinkers living in the community. The questionnaires included Alcohol Use Disorders Identification Test (AUDIT), Alcohol Problems, Alcohol Expectancy Questionnaire (AEQ), Life Position, and The Korean version of the Children of Alcoholics Screening Test (CAST-K). Data were analyzed using descriptive statistics, t-test, ${\chi}^2$-test, F-test, Pearson correlation coefficients, and logistic regression with forward stepwise. Results: AUDIT had significant correlations with alcohol problems, alcohol expectancy, and parents' alcoholism. In logistic regression, factors significantly affecting hospitalization were divorced (OR=4.18, 95% CI: 1.28-13.71), graduation from elementary school (OR=28.50, 95% CI: 8.07-100.69), middle school (OR=6.66, 95% CI: 2.21-20.09), high school (OR=6.31, 95% CI: 2.59-15.36), drinking alone (OR=9.07, 95% CI: 1.78-46.17), family history of alcoholism (OR=2.41, 95% CI: 1.11-5.25), interpersonal relationship problems (OR=1.28, 95% CI:1.17-1.41), and sexual enhancement of alcohol expectancy (OR=0.83, 95% CI: 0.72-0.94), which accounted for 53% of the variance. Conclusion: Results suggest that interpersonal relationship programs and customized cognitive programs for social drinkers in the community are needed to decreased alcohol related hospitalization in Korean men.

A Case of Headache and Memory Disorder after Traumatic Brain Injury Treated with Modified Seogakjihwang-tang (외상성 뇌손상 후두통과 기억장애가 발생한 환자에 서각지황탕가감방(犀角地黃湯加減方)을 투여한 1례 보고)

  • The purpose of this study is to report the clinical effects of modified Seogakjihwang-tang on a patient who had headache and memory disorder after traumatic brain injury, which were considered as the result of blood stasis. The patient had headache and memory disorder after traumatic brain injury and treated with modified Seogakjihwang-tang three times a day for 60days. As a result, the numerical rating scale and pain rating score of headache decreased and the scores of mini-mental state examination-Korea, the Korean version of Montreal cognitive assessment and the mini-memory test increased during treatment with modified Seogakjihwang-tang. Therefore, this case supports a possibility that modified Seogakjihwang-tang might be an effective treatment to patients with headache and memory disorder after traumatic brain injury.

Clinical Utility of the Minnesota Multiphasic Personality Inventory-Adolescent Restructured Form in the Assessment of Internalizing and Externalizing Disorders in Adolescents: A Preliminary Approach

  • Objectives: This study investigated whether the Minnesota Multiphasic Personality Inventory-Adolescent Restructured Form (MMPI-ARF) can differentiate between two groups of adolescents, one diagnosed with internalizing disorders and another with externalizing disorders, and examined the clinical utility of the MMPI-A-RF by examining which subscales can significantly discriminate between these two groups. Methods: A total of 105 adolescents aged 13-18 years completed the MMPI-A-RF (53 internalizing disorder and 52 externalizing disorder groups). Independent t-test, chi-square test (χ2), and discriminant analysis were used to examine whether MMPI-A-RF can distinguish between the two groups. Results: Sixteen MMPI-A-RF scales best predicted differences between the groups with internalizing and externalizing disorders. Fourteen scales (Higher-Order Scale [Emotional/Internalizing Dysfunction], Restructured Clinical [RC] Scale [RC demoralization, Somatic Complaints (RC1), and Low Positive Emotions (RC2)], Personality Psychopathology Five Scale [Introversion/Low Positive Emotionality-Revised, Negative Emotionality/Neuroticism-Revised], Somatic/Cognitive Scale [Malaise, Head Pain Complaints, and Gastrointestinal Complaints], Internalizing Scale [Stress/Worry, Self-Doubt], Externalizing Scale [Negative School Attitudes], Interpersonal Scale [Social Avoidance, Shyness]) were associated with the internalizing disorder group, whereas two scales (Externalizing Scale [Conduct Problems, Negative Peer Influence]) were associated with the externalizing disorder group. Conclusion: The MMPI-A-RF can be an efficient assessment tool for a quick diagnosis as it can classify individuals with internalizing and externalizing disorders in clinical settings that lack a variety of assessment tools for children and adolescents.

A Case of Advanced Gastric Cancer Patient Treated with Sojeukjungwon-san and Allergen Removed Rhus Verniciflua Stokes(aRVS) (소적정원산(消積正元散) 및 옻나무 추출물 투여로 체중증가 및 일반활동도의 개선을 보인 진행성 위암환자 1례)

  • Advanced gastric cancer is the most common type of all cancers in Korea, which account for approximately 18.1% of all incident cancers, and mortality from advanced gastric cancer is estimated at nearly 15.6% of death that caused by all cancer. In the conventional medicine, treatments of advanced gastric cancer include chemotherapy such like FOLFOX. In this case report, we introduce a case of advanced gastric cancer with greater omentum, thyroid, supraclavicular lymph node metastasis who had received chemotherapy. Chemotherapy was stopped because of weight loss and neurologic symptom such like cognitive disorder. The patient visited $M{\cdot\mu}$ Integrative cancer center in 2008, and was treated by Sojeukjungwon-san and allergen removed Rhus verniciflua stokes(aRVS). There was no evidence of brain metastasis. The patient showed improvement of cognitive disorder and gained weight. Further case study will be needed in order to determine the effects of Sojeukjungwon-san and allergen removed Rhus verniciflua stokes(aRVS) for the quality of life of advanced gastric cancer patients after chemotherapy.

A Systematic Study on Occupational Therapy Interventions for Adult Attention Deficit Hyperactivity Disorder (성인 ADHD를 위한 작업치료 중재에 대한 체계적 고찰)

  • Purpose : This systematic review examined evidence published 2000-2021 for occupational therapy interventions for adult Attention Deficit Hyperactivity Disorder (adult ADHD). Methods : In order to find out the studies on the effect occupational therapy intervention on individuals with adult ADHD, studies published from 2000 to February 2022 were searched on Google Academic Search. The keywords used were "adult ADHD" AND "Occupational therapy". A total of 785 studies were searched, and from these, 3 studies were selected based on the application of the selection and exclusion criteria. The included studies comprised of 1 Quasi-experimental design study(one group pre-post test) and 2 randomized controlled trials design studies. The included studies were analyzed in PICO(patients, intervention, comparison, outcomes). Results : As a result of analysing the subjects of the included studies, subjects diagnosed with adult ADHD or classified as high-risk group in Adult ADHD Self-Report Scale test, the number of subjects was 97, and the average age was about 33 years old. Occupational therapy intervention methods for adult ADHD include Tailored Intervention, Computerized Cognitive Training, and Cognitive-Functional Intervention. The Adult ADHD Self-Report Scale was the most used to check the symptoms of ADHD, and the COPM was used the most to measure the effects of occupational therapy interventions. The dependent variables of occupational therapy intervention were occupational performance, quality of life, executive function (behavioral regulation, metacognition), self-regulation function, stress management, and sustained attention. The effect of the intervention was statistically significant in 78.5% of the total. Conclusion : It is expected that the results of this study can be used as basic data for evidence-based practice for occupational therapists to apply adult ADHD interventions. In the field of occupational therapy in Korea, research on various interventions that can improve the occupational performance of adults ADHD should be conducted.

A Study on Clinical Characteristics of Patients with Mild Cognitive Impairment at Convalescent Hospitals (요양병원에 내원한 경도인지장애환자의 진료특성에 관한 연구)

  • Dementia among the diseases occurring in the old age is recognized as a disease to be solved at the national level and systematic management is being carried out. However, the awareness and response of the mild cognitive disorder, which is the middle stage of normal aging and dementia, are insufficient and social costs are increasing every year. The subjects of the study were 303 hospitalized patients whose primary diagnosis was mild cognitive impairment (F067) in a nursing hospital located in one district in 2016. The number of days of hospitalization was shorter in autumn and longer in winter or in hospital discharge. The cost of treatment for patients with mild cognitive impairment is lower in autumn and higher in winter or in 'discharge' of medical treatment results. The total cost of medical treatment was high in the winter by season in the ADL 6-12 and ADL 13-20 groups. Difference in the cost of medical treatment according to the characteristics of medical treatment, the medical expenses were the highest when the number of corporal disease was 5 ~ 8. In the medical treatment results, the ADL 6-12 point group and the ADL 13-20 point group had high medical expenses in 'continue'. Considering the clinical characteristics of mild cognitive impairment and considering that 10-15% of the patients with mild cognitive impairment progress to dementia every year, early detection and treatment of mild cognitive impairment is an effective and potentially important task for reducing potential social and dementia patients.
