The opportunities of virtual reality in the rehabilitation of children with attention deficit hyperactivity disorder: a literature review

  • Attention deficit hyperactivity disorder (ADHD) is one of the most common psychiatric disorders in childhood. This disorder, in addition to its main symptoms, creates significant difficulties in education, social performance, and personal relationships. Given the importance of rehabilitation for these patients to combat the above issues, the use of virtual reality (VR) technology is helpful. The aim of this study was to highlight the opportunities for VR in the rehabilitation of children with ADHD. This narrative review was conducted by searching for articles in scientific databases and e-Journals, using keywords including VR, children, and ADHD. Various studies have shown that VR capabilities in the rehabilitation of children with ADHD include providing flexibility in accordance with the patients' requirements; removing distractions and creating an effective and safe environment away from real-life dangers; saving time and money; increasing patients' incentives based on their interests; providing suitable tools to perform different behavioral tests and increase ecological validity; facilitating better understanding of individuals' cognitive deficits and improving them; helping therapists with accurate diagnosis, assessment, and rehabilitation; and improving working memory, executive function, and cognitive processes such as attention in these children. Rehabilitation of children with ADHD is based on behavior and physical patterns and is thus suitable for VR interventions. This technology, by simulating and providing a virtual environment for diagnosis, training, monitoring, assessment and treatment, is effective in providing optimal rehabilitation of children with ADHD.

교통사고로 인한 외상 후 스트레스 장애에서 증상 정도에 따른 인지기능의 차이 (The Difference of Cognitive Function in Posttraumatic Stress Disorder after Traffic Accident According to Severity of Symptom)

  • 연구목적: 본 연구의 목적은 외상후 스트레스 장애에서 증상의 정도에 따른 인지기능의 차이가 어떠한지에 대하여 연구한 것이다. 방 법: 교통사고 관련 외상후 스트레스 장애로 진단된 90명의 외래, 입원환자를 대상으로 외상후 스트레스 장애 증상의 심각도(MMPI 우울증 척도, 강박 척도, 정신분열증 척도)와 인지기능(K-WAIS)을 측정하였다. 그들을 MMPI 우울증 척도, 강박증 척도, 정신분열증 척도 점수에 따라 상위 30%에 해당하는 집단과 하위 30%에 해당하는 집단으로 구분하여 두 군간의 지능과 소검사 점수 차이에 대하여 각각 검증하였다. 결 과: 우울증 척도의 상위군과 하위군 사이에 유의한 차이는 없었다. 그리고 강박 척도에서 하위군이 상위군보다 기본지식, 어휘문제, 공통성문제, 빠진곳 찾기, 차례맞추기가 유의하게 높았다. 그리고 정신분열증 척도에서 하위군이 상위군보다 전체지능, 언어성지능, 동작성지능, 기본지식, 어휘문제, 이해문제, 산수문제, 빠진곳 찾기, 차례맞추기, 토막짜기, 모양맞추기, 바꿔쓰기가 유의하게 높았다. 결 론: 본 연구 결과, 외상후 스트레스 장애에서 증상의 정도에 따른 인지기능의 차이가 있었다. 따라서 증상의 정도가 심한 집단에서 인지기능 장애가 더 심하다는 것을 알 수 있었다. 그러므로 외상후 스트레스 장애는 불안장애이기도 하지만 인지기능 또한 증상에 의해 많은 영향을 받는다는 것을 알 수 있었다. 향후 외상후 스트레스 장애를 치료함에 있어서 약물치료 뿐만이 아니라 다양한 인지적 치료 접근이 필요할 것으로 생각된다.

외상후스트레스장애 환자의 신체증상 지각 특성과 인지적 정서조절전략 (Characteristics of the Perception of the Somatic Symptoms and the Cognitive Emotion Regulation Strategies in Patients With Posttraumatic Stress Disorder)

  • 연구목적 본 연구는 외상후스트레스장애 환자들의 신체증상 지각 특성과 인지적 정서조절전략을 탐색하기 위해 시행하였다. 방 법 본 연구는 2019년 7월 1일부터 2021년 2월 28일까지 48명의 외상후스트레스장애 환자들과 48명의 정상인을 대상으로 시행하였다. 환자군에 대해 임상가용 외상후스트레스장애 척도, 신체감각 증폭 척도, 인지적 정서조절전략 척도, Hamilton 불안 척도를 평가하였고 정상대조군에 대해 신체감각 증폭 척도를 평가하였다. 신체감각 증폭 척도에 대해 집단간 차이를 검증하였고 환자군에서 척도간 상관 관계를 확인하였다. 결 과 신체감각 증폭 정도는 외상후스트레스장애 환자들이 정상인보다 평균적으로 높았으나 유의하지는 않았다. 외상후스트레스장애 환자군에서, 외상후스트레스장애 증상 심각도는 반추, 파국화의 인지적 정서조절전략과 유의한 양의 상관관계를 나타냈고, 신체감각 증폭 정도는 외상후스트레스장애 과각성 증상과 파국화, 타인비난의 부적응적 인지적 정서조절전략과 유의한 양의 상관관계를 보였다. 결 론 외상후스트레스장애 환자들은 반추, 파국화와 같은 부적응적인 인지적 정서조절전략을 가지고 있으며, 신체감각의 증폭은 외상후스트레스 증상 중 과각성 증상 및 파국화, 타인비난의 인지전략과 관련이 있을 것으로 보인다. 또한 반추, 파국화, 타인비난, 신체감각 증폭과 같은 부정적인 인지를 줄여주는 것이 외상후스트레스장애 증상과 신체증상에 도움이 될 수 있을 것이다.

정상인에서 메칠페니데이트가 학습에 미치는 영향 (Effect of Methylphenidate on Learning in Normal Population)

  • Methylphenidate is a widely used stimulant for treatment of attention-deficit hyperactivity disorder (ADHD). In addition to core symptoms of attention and behavioral symptoms, methylphenidate is even effective for executive functions. However, abuse and misuse of stimulants, including methylphenidate, for the purpose of cognitive enhancement is an issue of concern worldwide. Some prejudices and misunderstandings against methylphenidate are popular ; however, little attention has been given to the neuropsychiatric evidence of methylphenidate for enhancement of cognitive function among healthy populations. In this article, our aim was to conduct a review of previous studies investigating the effect of methylphenidate in healthy populations. Findings from several recent studies have demonstrated the effectiveness of methylphenidate for enhancement of cognitive function in healthy populations. The mechanisms of cognitive enhancement are thought to be associated with motivation and the reward circuit in the brain. However, when considering the risk to benefit, an official discussion of the use of methylphenidate among healthy members of the population would be premature. Instead, investigation of epidemiological studies of the prevalence of misuse of stimulants among healthy members of the population, particularly adolescents and college students, is needed. In addition, based on achievements reported in previous studies, investigation of the effect of an approach using non-pharmacological enhancing motivation, which will in turn result in increased cognitive function would be helpful.

반응성 애착 장애의 치료 (TREATMENT OF REACTIVE ATTACHMENT DISORDER)

  • 반응성 애착 장애에 관한 미국과 영국으로부터의 대부분 논문들은 국외에서 입양한 탈억제형(Disinhibited Type) 반응성 애착장애를 보고하였다. 터키와 한국에서만 반응성 애착장애의 억제형(Inhibited Type) 아동들에 대한 보고를 하고 있는데 대부분 친부모에 의해 양육되고 있으며 그들의 증세는 자폐장애와 매우 유사하다. 반응성 애착장애의 치료, 특히 억제형의 치료에 대한 논문은 매우 드문 상태이기 때문에 저자는 1987년부터 한국의 반응성 애착장애 아동들을 치료해 온 경험을 이 논문에 자세히 기술하였다. 반응성 애착장애 환자와 그 부모들을 치료하기 위해서는 세가지 중요한 영역이 반드시 포함되어야 한다:1) 반응성 애착장애 아동에게 치료적인 환경을 조성해 주어야 한다. 2)개별 놀이치료, 엄마 놀이치료, 형제 놀이치료, 또래 놀이치료, 집단 놀이치료를 통해 반응성 애착장애 아동과 그 엄마와의 사이에 애착을 형성시켜 주어야 한다. 3) 언어치료, 인지치료, 치료교육등을 통하여 뒤진 발달을 촉진시켜 주어야 한다. 이 치료방법은 미국이나 영국을 포함한 서구에서의 다른 치료 방법보다 우리나라 환자들에게 좀더 쉽게 좀더 효과적으로 적용될 수 있을 것이다.

Auditory and Visual P300 in ADHD Children with Higher and Lower IQ : Pilot Study

  • Objectives : Intellectual impairment in children with attention-deficit hyperactivity disorder (ADHD) is often associated with relatively severe cognitive dysfunction. This study was designed to investigate cognitive function using auditory and visual event-related potential P300 in children with ADHD with relatively higher and lower IQ. Methods : A total of 20 children aged 6-12 years with DSM-IV-TR diagnosis of ADHD-combined type were recruited. For 10 children with lower IQ (${\leq}$100) and 10 children with higher IQ (>100), auditory and visual P300 using oddball paradigm (target 0.2, standard 0.8, in probability) were employed. Results : No significant differences were found in P300 amplitude and latency between ADHD children with higher and lower IQ in both modalities. However, auditory P300 amplitude in the right parietal area (P8 electrode) was negatively correlated with verbal IQ in ADHD subjects (R=-.50, p<.05). Visual P300 amplitude in the left parietal area (P3 electrode) was positively correlated with performance IQ in ADHD subjects (R=.57, p<.01). Conclusion : This study suggests intellectual impairment, as evidenced by lower IQ, could not be associated with cognitive dysfunction reflected in event-related potential P300 in ADHD children. However, cognitive function reflected in intellectual subcomponents and P300 might be processed in a stimulus modality-specific and asymmetric pattern.

Gaps-In-Noise Test Performance in Children with Speech Sound Disorder and Cognitive Difficulty

  • Background and Objectives: The Gaps-In-Noise (GIN) test is a clinically effective measure of the integrity of the central auditory nervous system. The GIN procedure can be applied to a pediatric population above 7 years of age. The present study conducted the GIN test to compare the abilities of auditory temporal resolution among typically developing children, children with speech sound disorder (SSD), and children with cognitive difficulty (CD). Subjects and Methods: Children aged 8 to 11 years-(total n=30) participated in this study. There were 10 children in each of the following three groups: typically developing children, children with SSD, and children with CD. The Urimal Test of Articulation and Phonology was conducted as a clinical assessment of the children's articulation and phonology. The Korean version of the Wechsler Intelligence Scale for Children-III (K-WISC-III) was administered as a screening test for general cognitive function. According to the procedure of Musiek, the pre-recorded stimuli of the GIN test were presented at 50 dB SL. The results were scored by the approximated threshold and the overall percent correct score (%). Results: All the typically developing children had normal auditory temporal resolution based on the clinical cutoff criteria of the GIN test. The children with SSD or CD had significantly reduced gap detection performance compared to age-matched typically developing children. The children's intelligence score measured by the K-WISC-III test explained 37% of the variance in the percent-correct score. Conclusions: Children with SSD or CD exhibited poorer ability to resolve rapid temporal acoustic cues over time compared to the age-matched typically developing children. The ability to detect a brief temporal gap embedded in a stimulus may be related to the general cognitive ability or phonological processing.

유산소 운동이 소아 및 청소년의 인지기능에 미치는 영향 : 문헌고찰 (Effect of Aerobic Exercise on Cognitive Functions in Childen and Adolescents : A Review)

  • The purpose of this review was to investigate the relationship between aerobic exercise and cognitive function as well as synthesize the effect of aerobic exercise intervention studies centered on psychiatric symptoms associated with general cognitive deficit. Prospective studies on aerobic exercise and improvement of cognitive function were reviewed and synthesized. In addition, this literature review provides significant positive results on the relationship between aerobic exercise and general cognitive deficit associated with psychiatric symptoms. Review of this literature suggests that there is a positive relationship between participation in aerobic exercise and cognitive function. In this text, there are at least three general pathways by which aerobic exercise may facilitate executive function in children : 1) cognitive demands inherent in engaging in physical activity, 2) cognitive demands inherent in cooperation of complex motor tasks, and 3) physiological changes resulting from aerobic exercise. Another main finding of this review is that physical activity has a stronger influence on cognitive deficit, including attention-deficit hyperactivity disorder, depression, and anxiety. Development of cognitive function is the most important factor for children and youth. Therefore, future research should prove relationship between physical activity and cognitive function using a more scientific and quantitative approach design.

사상의학적으로 치료하여 호전된 주요 신경인지장애 치험 2례 (Two Case Reports on Major Neurocognitive Disorder Patients Treated in Sasang Constitutional Medicine)

  • Objectives:This study was designed to report treatment by Sasang Constitutional Medicine to the patients with Major Neurocognitive Disorder(Dementia) from various causes. Methods:These two patients were diagnosed as Taeeumin and Soyangin according to the result of Sasang constitutional diagnosis, and treated by Sasang constitutional medications. The progress was evaluated with the Korean version of Mini-Mentel State Exam(KMMSE) and Global Disorientation Scale(GDS). Result:The symptoms like wandering, insomnia, cognitive disorder was improved in these patients. Also, KMMSE and GDS score were highly improved. the one patient was cured with Cheongsimyeonja-tang and and the other was cured with Jihwangbaekho-tang and Yanggyuksanhwa-tang. Conclusion:This cases show that Sasang constitutional herbal medications are an effective treatment for the patients with Major Neurocognitive Disorder.
