Aim: To assess the efficacy of cyanoacrylate adhesive in the management of large perforations of the maxillary sinus membrane during sinus lifts. Material and methods: Eight rabbits were used in the study. Sinus membrane perforation(about 1.5cm) was repaired with cyanoacrylate adnesive on one side of the maxillary sinus and the opppsite side was used as a control. Histological evaluation was performed 4 weeks after the operation. Results: Histological studies showed normal healing of the sinus membrane across the site of previous perforation and no evidence of inflammation. Conclusion: Our results support the clinical use of cynoacrylate adhesive for repairing sinus membrane perforation.

Objective : Postoperative subgaleal cerebrospinal fluid (CSF) collection is considered as one of the common minor surgical complication which can lead to prolonged hospitalization. We introduce "galeal tack-up suture" to prevent postoperative subgaleal CSF collection. Methods : Galeal tack-up suture consists of various surgical techniques which aim to fix galea to cranium in order to prevent CSF pooling in subgaleal space. A total of 87 patients who underwent craniotomy were divided into two groups while closing the wound : group A with galeal tack-up suture and group B with routine wound closure without galeal tack-up suture. The patients were observed for postoperative subgaleal CSF collection. Results : Among 87 cranitomy cases, galeal tack-up suture was performed in 32 cases and routine wound closure was done in 55 cases. Postoperative subgaleal CSF collection occurred in 13 cases (15%) in which 12 cases occurred in group B patients and 1 case occurred in group A patients (p=0.026). Conclusion : Galeal tack-up suture is an easy and effective technique in wound closure to prevent postoperative CSF collection.
The omental pedicle flap[OFF] has been used for management of complicated problems in various fields of cardiovascular surgery. Its unique properties of enhancing neovascularity, relieving lymphedema, providing fibroblasts to promote healing, providing soft tissue coverage, & functioning in the face of existing infection make it ideal in managing many of the more complicated problems facing the thoracic surgeon. We have used omental pedicle for colosing of the bronchial fistula R esophageal fistula with filling the adjoining cavity after pneumonectomy. The successful closure of the bronchial stump with OFF were obtained in 2 cases, but one case was failed who was suffered from the esophageal fistula. The primary operation in each cases were right pleuropneumonctomy for tuberculous empyema in 2 cases & left pneumonectomy for chronic empyema in 1 case, We believe that the OFF is effective for closing fistula due to postoperative empyema k plombage procedures for dead space of infected thoracic cavity.
교정 치료에서 발치공간 폐쇄는 치열을 이루는 모든 치아의 연속적인 이동으로 이루어지므로 그 기전은 복잡하다. 특히 전치부 치축을 적절히 유지하면서 구치부 고정원을 조절하는 과정은 정교함을 요하기 때문에 입체적 분석을 통한 치아이동 양상에 대한 이해가 필요하다. 지금까지의 유한요소 분석은 초기 응력분포를 관찰하여 치아 이동양상을 예측해 보는데 그쳤지만 이러한 양상만으로 정확한 치아이동 결과를 추정하는 데에는 한계가 있었다. 따라서 본 연구에서는 3차원 유한요소 모델을 이용하여 상악 제1소구치 및 제2소구치 발치공간 폐쇄 시 전치부와 구치부의 입체적인 이동 양상을 단계별로 비교하여 그 기전을 규명하고자 하였다. 자연치의 크기 및 형태를 갖는 상악 치아들과 브라켓, 교정용 호선 및 치조골부를 3차원 레이저 스캐너로 스캐닝한 후 사면체 요소의 유한요소 모델을 제작하였다 $0.017"{\times}0.025"$ 스테인레스 강 호선에 제작된 bull 루프 후방에 $10^{\circ}$ gable bend를 부여하고 한 번에 2 mm씩 12회 활성화시켜 발치공간을 폐쇄시켰다. 그 결과 제1소구치를 발치한 경우 제2소구치 발치에 비해 전치부의 후방 이동량이 많았으며 구치부의 전방 이동량은 더 적게 나타났다. 전치부에서는 제f, 2소구치 발치 모두 비슷한 미약한 정출을 동반하였고 치축의 변화량은 제1소구치 발치에서 더 크게 나타났다. 또한 제2소구치 발치 시 고정원의 협측 이동량이 더 크게 나타났다
This study was conducted to investigate the influence that it has on the application pre and post balance index using method with spiral and reaction point balance taping application against 20 stroke patients. The taping application pre and post static eye opening balance index was statistically no significant difference in the Mean X Speed, and the Extent in X Dirction but in the Mean Y Speed, the Velocity Moment, and Extent in Y Direction, there were statistically significant difference(p<0.05). The taping application pre and post static eye closing balance inde was statistically no significant difference in the Mean X Speed, in Y Direction, there were statistically significant difference(p<0.05). From above results, the method of the applied taping was the equilibrium stability performance of the Y-axis than the stability of the balance of the X-axis, therefore it is considered to study the taping application method in the balance of detail stroke patients.

Congenital coronary arteriovenous fistulas (CCAFs) are rare coronary artery abnormalities in which blood is shunted into a cardiac chamber or great vessel. If the fistula itself is large and tortuous, it is generally recommended to occlude the fistula to prevent several complications. In approaches of transcatheter occlusion, the transvenous approach is preferred over the transarterial approach. The transvenous approach would enable the cannulation of a relatively larger catheter or sheath without potential damage to the femoral vessels or normal coronary arteries, which can occur in the transarterial approach. The transvenous approach may also minimize the blind pouch after releasing the devices. Herein, we report the success of transvenous proximal closure of a CCAF using an Amplatzer vascular plug (AVP) in a 3-year-old patient with cardiomegaly. Complete occlusion was achieved by a single AVP and thrombus formation of the distal aneurysmal portion of the fistula. We suggest that this strategy of closing the proximal end with a dilated fistula using a single AVP by the transvenous approach may be a good option in treating CCAFs in a young child.
We present an augmented reality (AR) application for cell phone where users put a virtual pet on their palms and play/interact with the pet by moving their hands and fingers naturally. The application is fundamentally based on hand/palm pose recognition and finger motion estimation, which is the main concern in this paper. We propose a fast and efficient hand/palm pose recognition method which uses natural features (e.g. direction, width, contour shape of hand region) extracted from a hand image with prior knowledge for hand shape or geometry (e.g. its approximated shape when a palm is open, length ratio between palm width and pal height). We also propose a natural interaction method which recognizes natural motion of fingers such as opening/closing palm based on fingertip tracking. Based on the proposed methods, we developed and tested the AR application on an ultra-mobile PC (UMPC).
Pyoderma gangrenosum(PG) is an uncommon cutaneous vascular disease that typically presents as a painful and destructive ulceration on the anterior surface of the legs. The etiology of PG is currently unknown. But, the association with many immunologic disorders and its clinical response to immunomodulating agents suggest an immune etiology. A common feature of patients with PG is the presence of pathergy(the induction of lesion following injury of the skin). The trauma of surgery can be sufficient to induce pathergy, thus paradoxically limiting the usefulness of surgical treatment of PG. For that reason, medical treatments have been commonly used, while surgical treatments have been regarded not suitable. However, the use of the classic systemic agents is limited by their side effects and contraindications. Moreover, the large, problematic ulcers take too long to heal with medical management only. We present our experience in closing large wounds with the goal of decreasing morbidity, drug side effects and hospital stay by combination of medical and surgical therapy(split thickness skin graft). And authors advocate that surgical management is not a contraindication and may be considered as a selective modality in treatment of PG.
Methods for the operative management of patent ductus arteriosus are now well established, and in the vast majority of children the circulatory shunt can be safely and effectively abolished by dividing the ductus or by closing it in continuity with ligatures and transfixing sutures. In adults, however, closure of patent ductus arteriosus may pose important technical problems, particularly when there are associated pulmonary hypertension, calcification, aneurysm and infective endocarditis. Under these circumstance, division or ligation is unusually hazardous because the diseased vessels often fracture or tear when sutures are placed in them. Then we closed the patent ductus arteriosus by use of cardiopulmonary bypass in 18 patients and ligated the ductus via thoracotomy in 18 patients from Jan. 1986 to May 1990. And we compared the results between two different methods. We concluded that ligation of ductus had a problem of rupture and transpulmonary internal suture closure of PDA had a problem of injury of recurrent laryngeal nerve.
하악운동이상증(oromandibular dystonia, OMD)은 턱이나 구강조직의 반복적, 불수의적, 비정상적인 움직임으로 정의되며 저작기능과 교합에 문제를 초래하여 치과치료를 어렵게 하고 보철물의 유지곤란을 야기할 뿐 아니라 잦은 구강 연조직 손상이나 만성 안면통증의 원인이 되기도 하므로 치과의사의 관심이 필요한 질환이다. 본 연구는 단국대학교 치과대학 부속치과 병원 구강내과에 내원하여 OMD로 진단되어 치료를 받은 환자들의 병력기록부상의 관련 항목을 조사하여 OMD 환자들의 임상적 특징과 치료 유형을 분석하였다. 2007년 8월부터 2010년 4월까지 단국치대 부속병원 구강내과에 내원한 78명의 OMD 환자는 평균 72세의 고령이었으며 여자가 남자에 비해 3.6배 많았고, 평균 10개월 이상의 비교적 긴 병력기간을 보였다. 내원한 OMD 환자의 가장 흔한 주소는 턱의 통증이었으며 그 다음으로 빈도가 높은 주소는 턱과 구강조직의 조절 안 되는 반복적인 움직임과 구강조직의 통증이었다(p=0.000). OMD 유형은 페구형(jaw-closing dystonia)이 가장 많았고, 측방운동형(lateral jaw-deviation dystonia)과 복합형(combination dystonia)도 많이 발생하였다(p=0.001). 환자의 60% 이상은 OMD 발생과 관련하여 뚜렷한 원인을 인지하지 못했으나 약 19%의 환자들은 보철치료, 발치 등의 치과치료와 관련 있다고 보고하였다(p=0.000). 본원에 내원한 환자는 약물치료가 가장 먼저 시도되었고 진정제(clonazepam)가 거의 모든 환자(97%)에서 처방되었다. 이상의 결과로 볼 때, OMD는 뚜렷하게 노인, 특히 여성에게 많이 발생하는 질환으로서 턱과 구강조직의 통증을 유발하며 과반수 이상은 뚜렷한 원인을 모르지만 일부 환자들은 치과치료 후에 자신의 문제를 처음 인지하는 경향이 있다. 그러므로 치과의사는 환자를 치료할 때 OMD의 존재를 간과하지 않도록 관심과 주의를 기울일 필요가 있다.
