Impacted teeth are a frequent phenomenon encountered by every clinician. The artificial eruption of embedded teeth is the process of directing an impacted tooth into normal occlusion. This procedure is currently attracting attention, with the aim of finding the best technique to use according to each case. This article presents key information regarding impacted incisors, canines, and premolars. In addition, we describe the most common techniques to use for artificial eruption, the open and closed techniques. We review the literature concerning these techniques and outline how clinicians can manage every type of impacted tooth.

The impacted maxillary canine is a common problem of which all dental practitioners should be aware. The surgical orthodontic treatment of impacted canines is replaced correction position in dental arch without periodontal damage. Many treatment possibilities have been considered for this goal; window procedure, apically positioned flap, closed eruption technique and tunnel traction. Prognosis for these treatment may be very uncertain in many case(infraosseous impacted tooth). Other steps are required to achieve a satisfactory periodontal outcome. Satisfactory results could be expected if the physiologic eruption pattern is restored by tunnel traction, because permanent tooth erupts through the gingiva near the crest of the ridge so that periodontal damage is reduced. This article report that the surgical orthodontic treatment using tunnel traction is obtained proper position and reduced periodontal damage in facially impacted maxillary canines.

It is a relatively common clinical experience to see a impacted maxillary central incisor Most often, the central incisor is impacted labially. The labial impaction has been indicated as the most difficult to manage. The labial impactions have been found to be associated with mucogingival recession, reduced attached gingiva and periodontal pockets. For the successful treatment, clinicians should avoid loss of attached gingiva in surgical exposure. The most common methods of uncovering labially impacted maxillary anterior teeth are gingivectomy, apically positioned flap and closed eruption technique. If gingivectomy will not leave enough attached gingiva, then an apically positioned flap may be the treatment of choice. If the tooth is impacted in the middle of the alveolus or high in the vestibule near the nasal spine, the closed eruption technique may be the treatment of choice. Closed eruption technique was used in one case, apically positioned flap was used in two case. As the result in two cases of impacted maxillary central incisor, apically positioned flap provide the adequate width of attached gingiva. In case of impacted maxillary central incisor, through the clinical and radiologic examination to select correct surgical operation for reduce the complications.
Impaction with a severely dilacerated root is seldom reported, especially in the maxillary incisor. It is probably because of the high clinical difficulty associated with bringing the dilacerated tooth into proper position, and the high chance of failure due to ankylosis, external root resolution, and root exposure after orthodontic traction. Even the successful cases may need periodontal surgery to improve the unesthetic gingival shape. However, it has previously been reported that an impacted maxillary central incisor was successfully treated by proper crown exposure and orthodontic traction. This article presents a case of an invertedly impacted maxillary right central incisor with a developing dilacerated root, which was aligned into proper position after orthodontic traction composed of two stages of a closed eruption technique.

The impacted teeth were surgically exposed by the closed-eruption technique and orthodontically retracted. The subject of this study are 24 patients(Mx. insiors; 10 Mx. canines; 14) who finished their treatment. The periodontal condition of the impacted teeth and the normally erupted proximal and opposed teeth were compared and analysed. The results are the followings ; 1. When the gingival index, plaque index, pocket depth and attached gingiva in periodontal evaluation were compared, there was no significant difference between the study group and the control group (P>0.05). 2. When the alveolar bone support of the mesial and the distal surface of the maxillary central incisors and proximal teeth was compared, there was no significant difference between the study group and the control group (P<0.05). 3. When the alveolar bone support of the maxillary canines was compared, there was no significant difference between the retracted teeth and the normally erupted teeth(P>0.05). The results above showed that the surgical exposure by closed-eruption technique followed by the orthodontic retraction of the impacted teeth has a positive influence on the regeneration of gingival tissue in clinical practice and is esthetically more stable. And it is considered that the clinicians should give attention to the direction of retraction power and the maintenance of normal shape of the alveolar bone in treatment of maxillary central incisors.

Dilaceration is most common in maxillary central incisor. Impaction by dilacerated tooth cause orthodontic problem such as tilting of adjacent tooth, midline deviation. Dilaceration is caused by trauma, cyst, other origin and tooth shape, degree of root curvature, location in the alveolar bone, ability of spontaneous eruption should be considered whether treat or not. Labially and lingually dilacerated tooth is difficult to spontaneous eruption. Thus, Prosthetic treatment alternative with extraction, auto transplantation, orthodontic traction with surgical exposure is included within the treatment alternative. These cases are about impacted central incisor with dilacerated root. We use closed eruption technique and guide impacted tooth into normal position by orthodontic traction.

Unerupted maxillary anterior teeth is not a common case, However it may present practitioners with management problem. The cause of impaction is considered to be multifactorial, and local cause is the most common. These impacted teeth require surgical intervention, removal, transplantation, or surgical exposure, with or without orthodontic traction to align the malpositioned tooth. The preferred option is surgical exposure and orthodontic correction. Surgical intervention and orthodontic correction should not be delayed to avoid unnecessary difficulties in aligning the tooth in the arch. Surgical exposure should be performed with the intent of providing sufficient attached gingiva rather than simply uncovering the crown, which results in only alveolar mucosal attachment. Attached gingiva is essential to secure the gingival tissues to the adjacent teeth at the dentogingival junction. Thus preventing loss of periodontal tissues as a result of the pull of the surrounding soft tissues and facial muscles. Labially impacted maxillary anterior teeth uncovered with an apically positioned flap technique have more un- esthetic sequelae than those uncovered with a closed-eruption technique. In the case of severly displaced impacted teeth, autotransplantation ensures preservation of the alveolar bone and will facilitate future placement of an osseointegrated implant once growth has ceased or if ankylosis/resorption of the transplant occurs.

Ectopic eruption and impaction of canines is a frequently encountered clinical problems. The incidence of impaction ranges between 1-3%. The cause of canine impaction can be the result of localized factors or can be a polygenic multifactorial inheritance and associated with other dental anomalies. The general dentist and pediatric dentist should know how to properly diagnose and manage potential disturbances in the eruption of maxillary canine. Diagnosis of impacted canine at age 8-10 years can significantly reduce serious ramifications, including surgical exposure and orthodontic traction as well as root resorption of the lateral incisors. Extraction of primary canine would be one of the method to prevent the impaction. The surgical procedure should be designed to minimize the destruction of periodontal tissue of impacted canine. Closed eruption technique is thought to be optimal method of surgical exposure compared with other methods. An overview of the incidence, sequela as well as the surgical periodontal, and orthodontic consideration in the management of impacted canine was presented.

Impaction is defined as a cessation of the eruption of a tooth caused by a clinically or radiographically detectable physical barrier in the eruption path or by an ectopic position of the tooth. The reasons for impaction of the maxillary central incisor are supernumerary tooth, odontoma, ectopic position of tooth germ, dilacerated tooth and so force. Impacted tooth cause space loss due to proximal movement of adjacent tooth, malocclusion, root resorption of adjacent tooth, cyst formation, so careful observation and early detection is important and exact treatment should be applied to prevent these results. The treatment options of impacted tooth include induction an eruption through extraction of deciduous tooth or surgical exposure, reposition of impacted tooth by surgical method or orthodontic treatment. Orthodontic traction is recommended when an eruption does not happen after removal of barrier or surgical exposure, when eruption path is too transpositioned to be corrected spontaneously so eruption does not expected. In these cases, traction of impacted maxillary central incisor was carried out using orthodontic method with closed eruption technique and it showed good clinical results so we report these cases.

Maxillary central incisor impactions occur infrequently. Their origins include various local causes, such as odontoma, supernumerary teeth, space loss, and disturbances in the eruption path, also trauma and apical follicular cysts. Impacted teeth can cause serious dental and aesthetic difficulties as well as psychological problems especially in anterior regions. Although the impaction of maxillary incisor occurs less frequently than that of the maxillary canine, it is of concern to parents during the early mixed dentition stage because of the uneruption of the tooth. Forced eruption of impacted teeth should be considered in young patients because this technique can lead to suitable results from a periodontal, occlusal, and esthetic perspective at an earlier stage better than with other treatment options. This report presents the surgical and orthodontic treatment of cases with horizontally impacted and dilacerated maxillary central incisors. For each patient, we used the closed eruption method, placed an attachment on the impacted tooth on surgery, and fully closed the flap. Traction was applied immediately. The impacted tooth erupts through the healed tissue in a manner resembling normal eruption.
