Study on Clinical Education for Nursing in Hospitals in Korea (간호학 임상실습교육 실태 조사)

  • Purpose: The purpose of this study was to ascertain the current state of clinical nursing education at hospitals. Methods: Questionnaires were sent out to 236 hospitals which have over 300 beds. Out of these, 116 hospitals responded, and the collected data was analyzed by mean, frequency, t-test, and ANOVA test. Results: These hospitals have teaching agreements with 4.2 nursing colleges and clinical education lasts 8 months. Clinical education status regarding hospital characteristics, between a university hospital, non-university hospitals, among advanced general hospitals, general hospitals, and special hospitals showed statistical differences in colleges per hospital and nurses' degree. 37.9% of cases have no internal regulation for nursing education, and in 68.1% of cases, students' practice was limited to simple nursing care. The current primary guide for student's practice was head nurse (61.25%), and the course professor took charge of mainly the conference. The difficulties as an educational hospital are increases in work load, difficulties in teaching, excessive number of students, simultaneous practical training, complaints from patients, lack in training manpower, and stress. Conclusion: This study determined that the big hospitals are heavily burdened by nursing education and that it will be necessary to establish standards for educational hospitals to ensure higher quality education.

A Study on the Clinical Practice Experiences on Nursing Activities of Nursing Students (간호학생의 임상실습 중 간호활동 경험 정도)

  • Purpose: The purpose of this study was to identify the clinical practice experiences of nursing students who completed the courses of nursing curricula in college. Method: The data was collected using a questionnaire with 191 task elements in 8 nursing standards specified by the Korean Nurses Association(2003), from December 2004 to January 2005. Three hundred thirty-three nursing students from five three-year and five four-year colleges of nursing were selected for respondents. Result: The nursing activities which over 70% of the students had performance experiences with were 23 items(12.04%) practiced frequently in clinical settings, simple, low in malpractice risk and noninvasive in characteristic. In addition, the nursing activities which under 30% of students had performance experiences with were 87 items(45.55%). The nursing activities which over 70% of students had only observation experiences with were 20 items(10.47%). Also, the nursing activities that over 30% of students didn't have any experiences with were 19 items(9.95%) not frequently seen in the clinical setting. Conclusion: It is concluded that the students are not fully qualified as the new nurses of the future. Therefore, every effort to develop a more effective clinical nursing education in school, clinical settings and in students themselves is needed.

Detection of Extended-Spectrum β-Lactamase Producing Klebsiella pneumoniae by Multiplex Polymerase Chain Reaction (Multiplex Polymerase Chain Reaction을 이용한 Extended-Spectrum β-Lactamase 생성 Klebsiella pneumoniae 균주의 검출)

  • The production of extended-spectrum ${\beta}$-lactamases ($ESBL_S$) is the main mechanism of bacterial resistance to third-generation cephalosporins and monobactams, whose prevalence varies depending on the different geographical areas. In the last years it has increased notably to the point of being considered a health problem of great importance. The characterization of the ESBLs producing Klebsiella penumoniae strains present in clinical isolates is time-consuming. I describe here the development of a new system, which consists of a multiplex PCR. I found 51 K. pneumoniae strains to be presumptive strains ESBLs producers by clinical and laboratory standards institute (CLSI) guidelines. The double disc synergy test showed 47 positive K. pneumoniae, which were K. pneumoniae isolates. All ESBLs producing K. pneumoniae strains were resistant to antibiotic amikacin, gentamicin and ciprofloxacin. By multiplex PCR analysis, $bla_{TEM}$ gene in 17 strains 44 $bla_{SHV}$ genes and $bla_{CTX}$ genes in 33 strains were identified. In this study, the multiplex polymerase chain reaction (PCR) assay was a good method to detect and differentiate ESBLs producing K. penumoniae strains in clinical isolates.

Development and Validation of Primary Method for the Determination of Glucose in Human Serum by Isotope Dilution Liquid Chromatography Tandem Mass Spectrometry and Comparison with Field Methods

  • Glucose is a common medical analyte measuring in human serum or blood samples. The development of a primary method is necessary for the establishment of traceability in measurements. We have developed an isotope dilution liquid chromatography tandem mass spectrometry as a primary method for the measurement of glucose in human serum. Glucose and glucose-$^{13}C_6$ in sample were ionized in ESI negative mode and monitored at mass transfers of m/z 179/89 and 185/92 in MRM, respectively. Glucose was separated on $NH_2P$-50 2D column, and the mobile phase was 20 mM $NH_4OAc$ in 30% acetonitrile/70% water. Verification of this method was performed by the comparison with NIST SRMs. Our results agreed well with the SRM values. We have developed two levels of glucose serum certified reference material using this method and distributed them to the clinical laboratories in Korea as samples for proficiency testings. The expended uncertainty was about 1.2% on 95% confidence level. In proficiency testings, the results obtained from the clinical laboratories showed about 3.6% and 3.9% RSD to the certified values. Primary method can provide the traceability to the field laboratories through proficiency testings or certified reference materials.

Grope for a Summary Program about a Traditional Medical Database for Intellectual Property Protection in WIPO (지적재산권 보호를 위한 전통의학 DB 구축에 관한 고찰)

  • Objectives : In the WIPO conference about the protection of intellectual property for traditional knowledge (TK) and genetic resources (GR), there was a demand concerning the necessity of recognizing the invention of TK and GR and the foundation of a related database. Methods : In the standard of the foundation of a database, the content and resource identification standards, the technological standards and the security and transmission standards were discussed. In a decision on intellectual property, it means the protection and recognition as a technology precedent of the materials of the database. Results : Massive material on traditional medical care technology in Korea has existed in many areas. However, it is hard to make a database because it is difficult to organize and find the data. In addition, a database demanded by WIPO is not only the material of images or texts of simple technology information, but also the use of patent decisions about traditional knowledge technology and the public sharing of information in related areas. A traditional medical care database would consist of the core of a subject and the related areas. The core part is the medical herb resource and Oriental clinical technology. Conclusions : These are necessary to detail the kind of medicines and prescriptions, their use, and their processing. In the related areas, it needs to detail PIAS and clinical data in regarding the industrialization. These DB have to protect the TK and the GR of Korea.

A Comparative Study on National and International Hospital Accreditation Systems Focusing on Korea, the U.S., and Australia (의료기관인증제도의 국내외 비교 연구 -한국, 미국 및 호주 중심으로-)

  • The purpose of this study was to ascertain new policies fo the development of the Korean Hospital Accreditation Program (HAP) by comparing and reviewing the domestic HAP of Korea, the U. S. and Australia as well as the International Accreditation Program of the International Society for Health Care Quality. The results of this study are as follows. First, it is necessary to improve the standards of the HAP such as 'patient and community centered standards' and 'staffing related standards'. Second, it is necessary to strengthen patient safety through the HAP. Third, the linkage among the HAP, clinical indicators and patient experience evaluation should be strengthened. Fourth, the competencies of surveyors should be improved. Fifth, the HAP of small and medium hospitals also needs to be vitalized. Sixth, it is necessary to improve the disclosure of the healthcare quality evaluation results. The findings of this study can be used as a basis to improve the Korean HAP.

Evaluation of Absorption and Release, Survival Efficiency and Recovery Rate of Transport Medium according to the CLSI M40-A2 Standard (CLSI M40-A2 기준에 따른 수송배지의 흡수 및 배출, 생존 효율, 회수율 평가)

  • The absorption and release capacities, survival efficiency, and recovery rates of five kinds of transport media were evaluated based on the swab elution method (Quantitative) of CLSI (Clinical and Laboratory Standards Institute) M40-A2. Liquid media showed mostly better results than semi-solid media in the three evaluations. The flocked swabs had better ability to absorption and discharge bacteria than the standard swabs. The liquid medium (S4) had the best survival efficiency. Pneumococcal strains with poor growth had a higher survival efficiency and recovery rate in liquid media (S4, S5). The results of microbial recovery showed that S. pyogenes met all the CLSI standards in all media. S. pneumoniae was inadequate in the semi- solid media (S2, S3) and all the remaining media met the criteria. H. influenzae was unsuitable in semi-solid media (S1, S3) and met the criteria in semi-solid medium (S2) and liquid medium (S4, S5). The viability of the H. influenzae, pneumococcal strain causing respiratory disease, was poor in most media. Overgrowth of P. aeruginosa was observed at room temperature. The combination of liquid medium and flocked swab confirmed the best results in the three evaluation methods.

Nurses' Experience of Health Communication with Doctors in the Clinical Fields (임상실무에서 간호사가 겪은 의사와의 의사소통 경험)

  • Purpose: The purpose of this study was to describe nurses' experience of health communication with doctors in the clinical fields. Methods: Eight experienced clinical nurses working in various units of general hospitals participated. Data were collected through in-depth individual interviews and analyzed using the Colaizzi' method. Results: Five themes of nurses' experience of communication were identified: exploring the speaker seriously, becoming aware of emotions and controlling them, developing relationships after lining up for battle, having superficial and withered relationships with doctors, having few resources to promote communications with doctors. Conclusion: The results of the study indicate that humanistic and warm interpersonal relationships and genuine sharing of information are necessary to ensure effective communication. Thus, practice guidelines should be re-evaluated and developed to meet the proposed standards; safety-net programs and education for health communication should also be established. Nurse managers should create a high-touch work environment.

  • Korean magnetoencephalography (MEG) system had been developed and installed to hospital. The Korean MEG system contains helmet-shaped arrays of 152 first-order double relaxation oscillation SQUID (DROS) sensor. As a clinical application we have measured and analyzed evoked responses in patients with functional brain disease by outer stimulation as follows; 1) auditory evoked field in patients with hemifacial spasm, 2) somatosensory evoked fields in patients with tumor. We confirm that neuromagnetic data by Korean MEG system can provide useful information for pre-surgical planning or functional brain research.

Nurses and Nursing Students' Recognition of Good Instruction (간호사와 간호대학생의 좋은 수업에 대한 인식)

  • Purpose: The purpose of this study was to identify and assess from nursing students and nurses in the clinical field what constitute good instruction, through the review of nursing students' opinions and clinical field demands. Methods: The study design was used Creswell's exploratory sequential design by collecting and analyzing qualitative data obtained from interviews and then analysis of quantitative data. The participations were 79 seniors in nursing schools and 85 nurses with less than three years of clinical experience. The data were collected through individual interviews and analyzed based on Elo and Kyngas's content analysis method. The quantitative data were collected using the questionnaire developed based on qualitative results and analyzed by SPSS 23.0 program and Importance Performance Analysis (IPA). Results: The results showed that IPA extracted seven items with high importance but low satisfaction: "nursing fads and trends," "teacher-learner communication and reflection," "materials used in clinical settings such as monitoring results and test results," "special presentations by experienced practitioners," "instruction assures learners' comprehension," "accurate and detailed evaluation standards" and "feedback on homework and exam." Conclusion: The factors comprising good instruction were verified, and the necessity for additional efforts to improve high importance and low performance factors was noted. Therefore, this study can serve as a guide for nursing education facilities and educators in developing of a thorough education system with excellent instruction designed to achieve an ideal nursing education.
