Randomized Clinical Controlled Trials of a Herb Remedies in Korea - Systematic Review (한약을 이용한 한국에서의 무작위 임상시험 개괄적 검토)

  • Objective: The study aimed to review all randomized controlled clinical trials (RCTs) performed in Korea with herbal medicine, to produce further useful information for herbal-drug development. Methods: All papers reporting RCT with herbal drugs were collected via public database of Korea Science and Technology Integration Services, and various Oriental medicine journals. Then, the clinical question, herbal medicine, result, RCT design, and its conductor were analyzed. Results: A total of 20 RCTs were finally selected, and most of them were performed after 2003. The main subjects of RCT were related to heart and blood circulation, dermatitis, respiration disorder, Sasang constitution, or psychiatric disorders. Eight RCTs were done with a single herb while 12 RCTs were with multiple herbal formulae. In particular, three RCTs for atopic dermatitis with three formulae showed positive results. The quality of RCT was considered as better than average. Conclusions: This study revealed the current status of RCT using herbal drugs. This result would provide helpful information for research and drug development using traditional herbal medicine.

The Study on the Cost Analysis Based on ABC System in Clinical Laboratory (활동기준원가시스템을 이용한 임상병리과 검사 서비스 원가 분석)

  • The main purpose of this study is to compare the traditional cost system and ABC(Activity Based Cost) system of clinical laboratory department in a hospital. The study subject was 296 services in clinical laboratory from March, 1997 to August, 1997. In a new costing system, cost for a lab test consist of direct cost element, activity based cost element, and allocated common cost element. In a traditional cost system, cost elements included direct cost element and indirect cost allocated based on test volumes The major findings of this research were as follows. 1. In the application of ABC system, total cost was analyzed as follows. Direct cost was 39.3% of total cost. Activity cost and allocation were 20.9% and 39.8%, respectively. The results of analysis to use traditional cost system were as follows. Direct cost was 39.3% and it was as same as the result of direct cost of ABC system. Indirect cost was 60.7%. 2. Activities of clinical laboratory of subject hospital were registration, pre-test operation, test, test result handling, delivery, culture, post-test operation, technical support, management support, and educational support. 3. The differences of the case of higher number of test case being carried out, the cost of ABC system was lower than the cost of traditional cost system. Otherwise in the case of lower number of test case being carried out, the rests have not been appropriately evaluated, and effective management were needed in clinical laboratory.

Development of Evaluation Tool for Clinical Nursing Practice Competency of Nursing students (간호학생의 임상실무수행능력 평가도구 개발)

  • Purpose: The purpose of this study was to development and measure validity and reliability of evaluation tool for clinical nursing practice competency of Nursing students. Method: The participants in this study were 568 nurses who had worked for more than three years above in hospitals. A questionnaire of 79 items was administered to the nurses using a convenience sampling method. The data were collected from Sep. 12th, to Oct. 20th, 2004. Result: The derived outcome tool consisted of 6 factors of direct nursing actions and 6 factors of indirect nursing actions on the basis of 79 items. The 6 factors in direct nursing actions accounted for 76.84% of the variance and 6 factors in indirect nursing actions accounted for 79.68% of the variance on total scale. As a result of the item analysis, 79 items were selected and the internal consistency alpha coefficient was .9917. The value of Cronbach' alpha of direct nursing actions was .9640, indirect nursing actions was .9550. Conclusion: The results of this study show that useful application to the evaluation tool of clinical practice competency of Nursing students and further studies need to be done to verify clinical practice educational evaluation.

The Relationship Between Emotional Labor and Job Stress of Clinical Nurses in a Hospital (일개 병원 임상간호사의 감정노동과 직무스트레스에 관한 연구)

  • Purpose: The purpose of this research is to understand the relationship between emotional labor and job stress for clinical nurses. Method: The information for this research has been gathered in method of questionnaire for 202 nurses working in a hospital from January 30 to February 10, 2006. Results; The emotional labor for clinical nurses showed 2.43 on a scale of 4 and the job stress showed 49.84 out of 100. The correlation between the emotional labor and job stress showed that the emotional labor was higher, job stress was also higher. The result was that the factors that influenced the emotional labor were health status in a subjective point of view, working department, and the personality. The factors that influence job stress were self-efficacy, working department, and health status in a subjective point of view. Conclusion: The result of the research showed that emotional labor and job stress for clinical nurses were high and the strategy for reduction in those two areas is needed. Especially, diversified plans for job demand and occupational climate in an area of job stress that is correlated to emotional labor will be needed to minimize the emotional labor and job stress.

Laboratory Diagnosis of Invasive Candidiasis

  • Invasive candidiasis is associated with high morbidity and mortality. Clinical diagnosis is complicated by a lack of specific clinical signs and symptoms of disease. Laboratory diagnosis is also complex because circulating antibodies to Candida species may occur in normal individuals as the result of commensal colonization of mucosal surfaces thereby reducing the usefulness of antibody detection for the diagnosis of this disease. In addition, Candida species antigens are often rapidly cleared from the circulation so that antigen detection tests often lack the desired level of sensitivity. Microbiological confirmation is difficult because blood cultures can be negative in up to 50% of autopsy-proven cases of deep-seated candidiasis or may only become positive late in the infection. Positive cultures from urine or mucosal surfaces do not necessarily indicate invasive disease although can occur during systemic infection. Furthermore, differences in the virulence and in the susceptibility of the various Candida species to antifungal drugs make identification to the species level important for clinical management. Newer molecular biological tests have generated interest but are not yet standardized or readily available in most clinical laboratory settings nor have they been validated in large clinical trials. Laboratory surveillance of at-risk patients could result in earlier initiation of antifungal therapy if sensitive and specific diagnostic tests, which are also cost effective, become available. This review will compare diagnostic tests currently in use as well as those under development by describing their assets and limitations for the diagnosis of invasive candidiasis.

The Effects of Clinical Learning Environment on Nursing Students' Powerlessness and Self-Efficacy Related to Clinical Practice (간호대학생의 임상실습교육환경이 임상실습 관련 무력감과 자기효능감에 미치는 영향)

  • Purpose: The purpose of this study was to identify the effect of the clinical learning environment on nursing students' powerlessness and self-efficacy related to clinical practice. Methods: Participants were 149 nursing students from a university who had completed their clinical practicum in B city. Data were collected using self-report questionnaires and analyzed using t-test, one-way ANOVA, Scheffe's test, Pearson's correlation coefficients, and hierarchical multiple regression. Results: The mean score of the clinical learning environment was 2.99 out of 5 points, the powerlessness related to the clinical practice, 2.96 out of 5 points, and the self-efficacy related to clinical practice, 73.69 out of 100 points, respectively. The regression analysis showed that the clinical learning environment explained 11% of the powerlessness related to clinical practice and 3% of the self-efficacy related to clinical practice in the nursing students after controlling for the general characteristics. Conclusions: The result of this study indicated that it is necessary to improve the clinical learning environment for decreasing the feeling of powerlessness and increasing the self-efficiency related to clinical practice among the nursing students.

The Influence of Nursing Students' Emotional Labor, Clinical Practice Stress on Satisfaction of Clinical Practice (간호대학생의 감정노동, 임상실습 스트레스가 임상실습만족도에 미치는 영향)

  • This study was a descriptive study to investigate emotional labor, clinical practice stress and satisfaction of clinical practice in nursing college students. The data were collected using an self-reporting questionnaire from June 1 to July 12, 2015. Collected data were analysed on SPSS Win 20.0. The study results were summarized as follows : The students' emotional labor was $3.13{\pm}0.54$ in average on a scale of 5. The clinical practice stress was $3.65{\pm}0.47$ in average, and students' satisfaction of clinical practice was $3.41{\pm}0.43$. The emotional labor has positive correlation significantly with clinical practice stress(r=0.353, p<.001), and negative correlation with satisfaction of clinical practice (r=-0.170, p<.007). In stepwise multiple regression to put control variables, emotional labor and clinical practice stress were no significant influence toward satisfaction of clinical practice. According the result, to improve the satisfaction of clinical practices emotional labor should be released. Moreover it is necessary to attention to the health care of nursing students and it is needed to boost up the students satisfaction with major, more friendly relations should be built with co-trainees.

An Analysis of Research on Clinical Nursing Education (간호학 임상실습 교육에 대한 연구 논문 분석)

  • This literature review was designed to identify a trend of study in clinical nursing education, to propose the idea for further study on an improvement of teaching students in the clinical setting. Method: The researches reviewed were 36 (seven in Korea and twenty nine out of Korea) on clinical nursing education in baccalaurate program from 1996 to 2000 from Journal of Korean Academy of Nursing, Journal of Korean Education of Nursing, Journal of Nursing Education, and Nurse Educator. Result: The prevailing research design was the nonexperimental(N=21). Subjects were predominantly nursing students(N=24). Structured questionnaire(N=22) was used most often for data collection. Among clinical setting studied, specific area was none in Korea. Research variables in nonexperimental studies were 4 types of student, teacher and teaching method. Independent variables in experimental studies were 7 types of clinical teaching methods, and dependent variables were six types of competence and knowledge of student. Research theme of qualitative research was most in clinical experience of student. Conclusion: In Korean, there were the lack of researches in specific clinical area, clinical teacher and teaching method. Accordingly, future studies need to be focused on various clinical areas, clinical teacher, and teaching method to improve the clinical nursing education in Korea.

Effect on Preference of Clinical Practice Subjects

  • This study was a cross-sectional descriptive survey study that confirms the effect on subjects that prefer clinical practice in order to prepare basic data for efficient clinical practice guidance for nursing college students. The study participants were 201 students attending C University, and the data collection period was from October 1 to October 15, 2022. The collected data were analyzed using SPSS 18.0 as descriptive statistics, Pearson correlation, Chi square test, ANOVA test, and Multiple regression test. As a result of the analysis, it was found that clinical decision-making and critical thinking were correlated under the statistical significance level (r=.730, p<0.01). The most favorite clinical practice department was community nursing, and male students preferred community nursing the most (Male=45.6%, χ2=.000), female students were found to prefer similar levels of practical subjects with child nursing , adult nursing, and maternal nursing(χ2=000).Clinical decision-making was found to be higher in students who preferred community nursing at a statistical significance level than those who preferred child nursing (F=4.91, p<0.01). Critical thinking was higher among students who preferred adult nursing than those who preferred other subjects (F=4.65, p<0.01). Through the analysis results, it was found that general characteristics vary, but clinical decision-making ability and critical thinking affect the preference of clinical practice subjects. Therefore, based on the results of this study, the professor of clinical practice suggests the development of a program to foster clinical decision-making and critical thinking to make students interested in clinical practice subjects.

A study on the awareness and practice of hand washing according to the clinical practice experience of physical therapy students (물리치료 전공 대학생의 임상실습 경험에 따른 손 씻기의 인식도와 실천도에 관한 연구)

  • The purpose of this study was to provide basic data on hand washing of physical therapists by examining their awareness and practice of hand washing according to proper clinical practice. A total of 536 questionnaires were collected from physical therapy students in Gwangju metropolitan city. The general characteristics of the subjects were calculated by frequency analysis, and the awareness and practice of hand washing with and without clinical practice were analyzed by independent t-tests. As a result of analyzing hand washing awareness according to the clinical practice, there was a significant difference in hand washing up to the wrist (p<0.05), and a significant difference in 6 stages of proper hand washing (p<0.01). As a result of analyzing hand washing practice according to clinical practice, there was a significant difference in hand washing before eating and starting clinical practice (p<0.05), a significant difference in washing hands after contacting clinical practice equipment and washing your fingers from your thumb down (p<0.01). As a result of analyzing the correlation between awareness and practice, most showed positive correlation. This means it is important to raise the awareness of proper washing hands. Therefore, it is believed that the training school should repeatedly conduct its own hand washing training within the school and in clinical practice to increase awareness and improve handwashing in clinical practice.
