Decision Making Experience of VIP Ward Nurses in the Clinical Practice Adjustment Process (특실병동 간호사의 실무적응과정에서의 의사결정 경험)

  • Purpose: The purpose of this research was to describe the decision making experience of VIP ward nurses in the clinical practice adjustment process. The research question was about "how nurses adjust their clinical practice to nursing situations and develop decision making process in a VIP ward." Method: The methodology of collecting and analyzing the data was based on the grounded theory of Strauss and Corbin(1998). The data was collected through in-depth interviews with 10 nurses from July to November, 2007. Results: The core category of nursing care decision making process was named as "adjusting with flexibility and deepened insight." The clinical practice adjustment process in nursing care decision making has progressed through four preceding interlocking phases: 1) dependent phase, 2) defensive phase, 3) independent phase, 4) integrative phase. These phases were classified by the level of nurses' dependency, proactivity, presupposition and integration. Conclusion: The result of this study indicated that nurse's decision making depended on their experiences and the nature of social context in which nursing occurs. Therefore, it is important to elaborate an effective training program for nurses to develop the phases of nursing care decision making.

Role Expectations and Cooperation of Staff Nurses and Clinical Research Nurses who take care of Clinical Research Patientser (임상연구환자를 돌보는 병동간호사와 임상연구간호사의 역할기대와 협력 인식)

  • This was an investigative study to identify the role expectations for clinical research related tasks that ward nurses can participate in and the awareness of cooperation between ward nurses and clinical research nurses (CRNs) and the obstacles for the same. The subjects were 66 ward nurses and 65 CRNs. The data were analyzed using descriptive statistics, the two-sample t-test, and the 𝑥2 test on SAS 9.3. As a result of this study, 4.56±0.41 ward nurses and 3.83±0.60 CRNs were found to have high role expectations of each other and statistically significant role expectations for ward nurses (p<.001). Significant differences were seen in the five categories of cooperation recognition between the two groups. Among the factors for cooperative disabilities, excessive work and lack of awareness of clinical research were ranked high. High role expectation becomes an element of role conflict. To organize the role of ward nurses and clinical research nurses in the new clinical research nursing work, procedures, establishing of effective practices, and the proper operation of an official system are expected to be necessary. To promote cooperation between the two groups, a culture of mutual trust and cooperation should be formed to enable each group to think of the other as partners and to recognize their expertise.

A Study on the Space configuration and Area of the Standard Ward in General Hospital in China (중국 종합병원 표준 병동부 공간구성 및 면적에 관한 연구)

  • Purpose: The ward division is a representative part of the hospital, where a variety of user activities are performed. Users can be broadly categorized as patients and carers, visitors, and medical staff (doctors and nurses). The relationship between these two is a major issue with ward planning as the patient's place of life centers around the hospital room and the task of the clinical workforce centers around the nursing station (NS). Against this backdrop, the study divides the ward units of the General Hospital of China into patient areas, medical team areas, supply areas, and public domain, with the focus on the ward areas affecting most significantly in the hospital patients. Methods: The theoretical advance studies of the standard ward unit are identified by the associated guidelines, articles, and documentation. Results: This study is a summary analysis of relevant regulations, reference literature, and drawing data from the target hospital. Further work is expected to be undertaken, including further surveys and observational surveys, to produce more detailed results. Implications: It is expected that the research in this paper will provide an effective reference for the future research of China General Hospital Ward department, so as to promote and improve the work system of China General Hospital Ward department.

A Study on the Development of Clinical Workstation (임상용 워크스테이션의 개발에 관한 연구)

  • A clinical workstation has been developed for the use in clinical ward. This system is connected to the Picture Archiving and Communication System previously installed. It provides the capabilities of image display, image processing, diagnostic report review, clinical lab data review and analytic diagrams presentation for the hospitalized patients in clinical ward. The performances of this system were evaluated during the 9 months. These results show typical aspects of clinical patterns in data transmissions, image studies and clinical lab data studies. These typical aspects should be counted in the further development of workstation for the clinical use.

Development of Nurse's Handover Standards between Hospital Units using SBAR (SBAR를 이용한 간호단위간 인수인계 표준항목 개발에 관한 연구)

  • Purpose: The purpose of this study was to develop the adapted standard items of situation, background, assessment, recommendations (SBAR) processing for handover between nursing units in Korean hospitals and evaluate the validity and relevance of the standard items. Methods: A delphi method with 33 experts was used to evaluate content validity of the standard items. Then, 1,175 nurses working in general hospitals of more than 500 beds were recruited to evaluate the validity and relevance of the standard items for clinical implication. Results: Content validity was higher than 0.8. The highest scores for relevance among items in handover standards were state of consciousness in the assessment domain for a ward to a ward transfer ($3.82{\pm}0.40$), for a ward to an ICU ($3.85{\pm}0.38$), an ICU to a ward ($3.81{\pm}0.39$) and an ER to a ward ($3.85{\pm}0.37$). Congenital malformation was the highest relevance score for handover from a delivery room to a neonatal unit ($3.91{\pm}0.30$). Conclusion: This study evaluated validity and relevance of the essential contents for handover standards between units to improve communication quality among nurses. The findings of this study should also be applied in clinical nursing areas and the quality of information and effectiveness of usage of the standard should be evaluated.

The Paradigm Model of VIP Ward Nurses' Decision Making (특실병동 간호사의 의사결정 경험에 관한 패러다임 모형)

  • Purpose: The purpose of this research was to describe the decision making of nurses in a VIP ward. Method: The methodology of collecting and analyzing the data was based on the grounded theory of Strauss and Corbin (1998). The data were collected through an in-depth interview, which were audio-taped and transcribed. The data were collected from 10 nurses from July to November 2007. Results: The core category on VIP ward nurses' decision making was named as "adjusting with flexibility and deepened insight". The causal condition was established by 'the patients who wanted to be treated specially'. The contextual conditions included 'caring patients from various departments', 'differences depending on the nurses' clinical experience', and 'client-centered atmosphere in the VIP ward'. The intervening conditions included 'problem solving styles of nurses', 'attitudes of patients and family members', 'nurse-doctor relationships', and 'accessibility to information'. It was confirmed that nurses changed their action-interaction strategies depending on the intervening conditions, thus resulted in the nurses' role conflict and the need to expand their consciousness. Conclusion: The result of this study indicates that nurse's decision making depends on their experiences and the nature of social context in which nursing occurs.

Clinical Practice Performance According to Type of Practices and Satisfaction of Clinical Practice in Students at Child Health Nursing (아동간호실습 운영형태에 따른 간호학생의 임상실습수행도 및 실습만족도)

  • Purpose: This study was conducted to explore clinical practice performance according to type of practice in students in pediatric nursing and the relationship with satisfaction of clinical practice. Methods: The subjects consisted of 531 nursing students (307 at a pediatric ward and 224 at a nursery) from 5 colleges of nursing. Data collected from March to June 2012 were analyzed using descriptive statistics, t-test, AVONA and Pearson correlation coefficients. Results: Clinical practice performance on direct nursing activity was significantly different according to the course on pediatric nursing, grade point average, and satisfaction of nursing major. Observation practice was different according to type of college, the course on pediatric nursing, grade point average, duration of practice and clinical educator in the pediatric ward. In the nursery, direct nursing activity was significantly different according to the type of college, the course on pediatric nursing, hospital type, and duration of practice. Observation practice was different according to type of college, satisfaction of a nursing major, and the clinical educator. In addition, significant correlations were found between clinical practice performance and satisfaction of clinical practice. Conclusion: Our research can assist effective pediatric nursing practice planning for nursing students.

The Effectiveness of Forest Walking Program on Stress and Recovery of Schizophrenic Patients in a Closed Ward (숲산책 프로그램이 폐쇄병동 조현병 환자의 입원스트레스와 회복에 미치는 효과)

  • Purpose: This study was conducted to investigate the effects of forest walking program on stress and recovery among schizophrenic patients in a closed ward. Methods: A non-equivalent control group pre-posttest design was used. The participants were composed of 36 patients with schizophrenia admitted in two psychiatric hospitals. The experimental group (n = 18) received the forest walking program twice a week with a total of 6 weeks. Results: The study results revealed that the forest walking program was effective on stress(${\chi}^2=34.11$, p<.001), subjective recovery(${\chi}^2=22.40$, p<.001) and clinical recovery(F=28.34, p<.001) of hospitalized patients with schizophrenia. Conclusion: The findings indicate that the forest walking program for schizophrenic patients in the closed ward can be an effective nursing intervention to reduce stress and improve recovery ability. We suggest that mental health nurses may adapt this program to help patients with schizophrenia in a closed ward.

Comparative Study for the Clinical Nurses' Work Satisfaction, Job Stress and Turnover Intention - in General ward, Comprehensive nursing care service ward, and Anshim unit (일반병동, 포괄간호서비스병동, 안심병동 간호사의 직무만족도, 업무스트레스, 이직의도 비교연구)

  • The purpose of this study is to compare with work satisfaction, job stress and turnover intention of nurses who work in Comprehensive nursing care service ward, Anshim unit and general ward. The subjects are nurses of general ward, Anshim unit, Comprehensive nursing care service ward from two hospitals in Seoul and one hospital located in Gyeongi province. The summary of the research is as follows. Nurses in a general ward, a Comprehensive nursing care service ward and an Anshim unit don't have meaningful difference in job satisfaction(F=0.50, p=.602), job stress(F=1.12, p=.326), turnover intention.(F=0.02, p=.972). There were some statistic differences of Work satisfaction, job stress and turnover intention according to general characteristics. Henceforth it needs diverse consideration on these factors to reduce turnover intention of nurses.

Age-Related Bone Mineral Density, Accumulated Bone Loss Rate at Multiple Skeletal Sites in Korean Men (한국 남성의 연령에 따른 부위별 골밀도 변화 및 골 소실률)

  • This study examined the age-related bone mineral density (BMD), accumulated bone loss rate (ABLR) at different skeletal sites in Korean men using the data from the Korea National Health and Nutrition Examination Survey (KNHANES)(the 1st (2010), 2nd (2010) and the year at the 5th survey). The cubic regression model was found to be the best for describing the age-related changes in BMD. The lumbar spine, total hip, femoral neck, trochanter, Ward's triangle in the bone mineral density difference were analyzed by ANOVA. The peak BMD was at 20-24 years at the lumbar spine, total hip, femoral neck, trochanter, Ward's triangle, and the 75-79 years of age group had the highest Accumulated Bone Loss Rate. Therefore, intensive management will be necessary for men over 75 years, and a diagnosis of osteoporosis in Korean men should be made according to The International Society for Clinical Densitometry; ISCD.
