  • Objectives : A case of cervical dystonia improved after Korean medical treatments by using measurements of Toronto Western Spasmodic Torticollis Rating Scale (TWSTRS), and Cervical Dystonia Impact Profile (CDIP-58) to evaluate the changes of symptoms. Methods : Korean medical treatments for relieving symptoms of cervical dystonia including acupuncture, herb medication, cupping therapy, and physical therapy were performed. To evaluate the effect of the treatments, TWSTRS and CDIP-58 were used. Results : TWSTRS decreased from 55 to 24.5 and CDIP-58 also decreased from 79.3 to 36.2. Conclusions : This report suggests that Korean medical treatments can be effectively used for patients with cervical dystonia. Further clinical studies are needed to verify the findings.

DYNAMIC WEDGE의 임상 적용 가능성에 관한 고찰

  • Dynamic wedge system has been introduced to modify the beam profile and to make homogeneous isodose curves in the mass of irregular shape. Before the clinical use of dynamic wedge, several factors such as wedge transmission factor, dose profile, percent depth dose, and wedge angle have to be measured quantitatively. Film dosimetry is used to evaluate these factors in this study. A comparison of the result of the dynamic wedge to physical wedge system is made. A positive result for the application of the dynamic wedge to clinic is derived even though there is a limitation in accuracy of the dosimetry system used. To measure all factors quantitatively, more accurate dosimetry systems are required.

Correction of palatally displaced maxillary lateral incisors without brackets

  • This article describes the orthodontic treatment of a 25-year-old Korean female patient with anterior crowding, including palatally displaced lateral incisors. Her facial profile was satisfactory, but 3.5 mm of maxillary anterior crowding was observed. To correct this crowding, we decided to minimize the use of the conventional fixed orthodontic appliances and employed a less bulky and more aesthetic appliance for applying light continuous force. We determined the final positions of the maxillary teeth via a working model for diagnostic set up and achieved space gaining and alignment with simple Ni-Ti spring and stainless steel round tubes. Tooth alignment was achieved efficiently and aesthetically without the conventional brackets.

Screening of Differentially Expressed Genes among Various TNM Stages of Lung Adenocarcinoma by Genomewide Gene Expression Profile Analysis

  • Background: To further investigate the molecular basis of lung cancer development, we utilize a microarray to identify differentially expressed genes associated with various TNM stages of adenocarcinoma, a subtype with increasing incidence in recent years in China. Methods: A 35K oligo gene array, covering about 25,100 genes, was used to screen differentially expressed genes among 90 tumor samples of lung adenocarcinoma in various TNM stages. To verify the gene array data, three genes (Zimp7, GINS2 and NAG-1) were confirmed by real-time RT-PCR in a different set of samples from the gene array. Results: First, we obtained 640 differentially expressed genes in lung adenocarcinomas compared to the surrounding normal lung tissues. Then, from the 640 candidates we identified 10 differentially expressed genes among different TNM stages (Stage I, II and IIIA), of which Zimp7, GINS2 and NAG-1 genes were first reported to be present at a high level in lung adenocarcinoma. The results of qRT-PCR for the three genes were consistent with those from the gene array. Conclusions: We identified 10 candidate genes associated with different TNM stages in lung adenocarcinoma in the Chinese population, which should provide new insights into the molecular basis underlying the development of lung adenocarcinoma and may offer new targets for the diagnosis, therapy and prognosis prediction.

외측연결형 임플란트 고정체의 경부 디자인이 임플란트 주위조직에 미치는 영향에 대한 전향적 예비 임상연구 (Effect of neck design on peri-implant tissue responses in external connection type implant : a prospective pilot clinical study)

  • This clinical study was conducted to evaluate the clinical effects of a concave neck of external connection type implant fixture designed for platform switching on the peri-implant tissue responses. Two types of implants with different neck designs were implanted in 20 patients. For the experimental group, the bioseal(BS) implant fixtures with 's' shaped concave profile on the neck were used, and non-bioseal(NBS) implant fixtures with a straight profile on the neck were used as control(Total of 40 implants, NBS: n = 19, BS: n=21). During the one-year period after implant placement, implant survival rate, marginal bone resorption, bleeding, plaque, and complications were evaluated. The survival rate of NBS and BS group was 94.74% and 90.48%, respectively. There was no significant difference on marginal bone resorption, bleeding and plaque between the two groups (P>.05). Within the limits of the present study, implants with a concave neck design showed similar clinical results to implants with a straight neck design on the peri-implant tissue responses. Longitudinal clinical studies are necessary to confirm more effective clinical results.

설문조사를 통한 전문가 대상 예방접종 후 이상반응 관련 우선순위 연구주제 선정 기준모델 제시 (Selecting the Priority Research Topics Addressing Unmet Post-immunization Safety Needs: A Model for Generating Research Proposal and Designing a Questionnaire for the Clinical Experts)

  • Objective: To gather inputs from clinical experts on selecting and prioritizing research topics, to address unmet vaccine safety needs. Methods: A questionnaire containing 15 vaccine safety assessment research proposals was sent to 28 vaccine experts chosen from various domestic medical association boards, and the rationale for each of the proposals was provided by presenting the following information: 1) a brief summary of the clinical safety studies on the vaccine, conducted by the Clinical Immunization Safety Assessment (CISA) project group, supervised by the United States Center for Disease Control (U.S. CDC), and 2) a summary of recently published studies that address vaccine safety issues. The experts were instructed to select and rank 5 topics in the order of preference, and the preference score for each proposed topic was calculated by assigning points on a scale of 1 to 5. Results: All 28 experts responded to the questionnaire, and the following topics were selected according to their calculated preference scores: 1) Human papillomavirus vaccine safety profile in the Korean female adolescents; 2) A signal detection of adverse events following Influenza vaccination: comparison between the US and South Korea; 3) Incidence of anaphylaxis following National Immunization Program vaccines between 2008 and 2017; 4) Safety of quadrivalent influenza vaccines compared to trivalent influenza vaccines; and 5) Pneumococcal vaccine safety profile in the general population. Conclusion: Five research topics addressing vaccine safety were selected, for which well-constructed research protocols need to be promptly developed to address current unmet vaccine safety needs in South Korea.

The G115 standardized ginseng extract: an example for safety, efficacy, and quality of an herbal medicine

  • Ginseng products on the market show high variability in their composition and overall quality. This becomes a challenge for both consumers and health-care professionals who are in search of high-quality, reliable ginseng products that have a proven safety and efficacy profile. The botanical extract standardization is of crucial importance in this context as it determines the reproducibility of the quality of the product that is essential for the evaluation of effectiveness and safety. This review focuses on the well-characterized and standardized ginseng extract, G115, which represents an excellent example of an herbal drug preparation with constant safety and efficacy within the herbal medicinal products. Over the many decades, extensive preclinical and clinical research has been conducted to evaluate the efficacy and safety of G115. In vitro and in vivo studies of G115 have shown pharmacological effects on physical performance, cognitive function, metabolism, and the immune system. Furthermore, a significant number of G115 clinical studies, most of them double-blind placebo-controlled, have reinforced the findings of preclinical evidence and proved the efficacy of this extract on blood glucose and lipid regulation, chronic obstructive pulmonary disease, energy, physical performance, and immune and cognitive functions. Clinical trials and 50 years of presence on the market are proof of a good safety profile of G115.

Cytotoxic Effect and Constituent Profile of Alkaloid Fractions from Ethanolic Extract of Ficus septica Burm. f. Leaves on T47D Breast Cancer Cells

  • The study aimed to investigate the profile of alkaloids in two ethyl acetate soluble fractions, namely fractions A and B from an ethanolic extract of Ficus septica leaves and cytotoxic effect on T47D breast cancer cells. Preparation of both fractions involved maceration of leaves with 70% (v/v) ethanol, filtration with $Al_2O_3$, precipitation with 0.1 N HCl, Mayer reagent, and 0.1 N NaOH, and also partition with ethyl acetate. Qualitative thin layer chromatography (TLC) was conducted to determine the profile of alkaloids in the two fractions, using alkaloid specific reagents such as Dragendorff, sodium nitrite, and Van Urk-Salkowski. Cytotoxic effects of both fractions on T47D cells were evaluated using MTT assay with a concentration series of 1.56; 3.12; 6.25; 12.5; 25 and $50{\mu}g/mL$. The TLC test showed that fractions A and B contained alkaloids with Rx values of 0.74 and 0.80 for fraction A and 0.74, 0.84, 0.92 for fraction B with regard to yohimbine using the mobile phase of n-buthanol:glacial acetic acid:distilled water (3:1:1 v/v/v). Moreover, an indole alkaloid was detected with Rx values of 0.80 and 0.84, respectively. Fractions A and B exhibited high cytotoxic effects on T47D cells with IC50 values of 2.57 and $2.73{\mu}g/mL$, respectively. In conclusion, overall the results of this study showed that fractions of Ficus septica contain alkaloids including indole alkaloid or its derivatives and possess a cytotoxic effect on T47D cells. This research supports the idea that alkaloids in F. septica have anticancer activity.

Comparative Study of Clinical and Radiological Outcomes of a Zero-Profile Device Concerning Reduced Postoperative Dysphagia after Single Level Anterior Cervical Discectomy and Fusion

  • Objective : This study analyzed clinical and radiological outcomes of a zero-profile anchored spacer (Zero-P) and conventional cage-plate (CCP) for single level anterior cervical discectomy and fusion (ACDF) to compare the incidence and difference of postoperative dysphagia with both devices. Methods : We retrospectively reviewed our experiences of single level ACDF with the CCP and Zero-P. From January 2011 to December 2013, 48 patients who had single level herniated intervertebral disc were operated on using ACDF, with CCP in 27 patients and Zero-P in 21 patients. Patients who received more than double-level ACDF or combined circumferential fusion were excluded. Age, operation time, estimated blood loss (EBL), pre-operative modified Japanese Orthopaedic Association (mJOA) scores, post-operative mJOA scores, achieved mJOA scores and recovery rate of mJOA scores were assessed. Prevertebral soft tissue thickness and postoperative dysphagia were analyzed on the day of surgery, and 2 weeks and 6 months postoperatively. Results : The Zero-P group showed same or favorable clinical and radiological outcomes compared with the CCP group. Postoperative dysphagia was significantly low in the Zero-P group. Conclusions : Application of Zero-P may achieve favorable outcomes and reduce postoperative dysphagia in single level ACDF.

어지럼증 환자(患者)의 MMPI 프로파일 특성과 한의학적(韓醫學的) 치료효과(治療效果) (The effects of Oriental medicine treatment and the characteristics of MMPI profile on Vertigo Patients)

  • This Study is attempted to analyze Minnesota Multiphasic Personality Inventory responses to Vertigo patients and remedial value of Oriental medicine treatment. Methods: 24 Vertigo patients and 28 control group were tested by MMPI and QSCCII. It is analyzed by cluster analysis, one-way ANOVA, chi-square test, t-test, Duncan method is used as ex-post test. Results and Conclusions: 1. The MMPI scores of Vertigo patients were elevated in Hs, Hy clinical scales, 1-3 profile type, it is profile of conversional neurosis. In this case body condition where the psychological problem will get conversion is a notable feature. 2. All of the profiles can be classified three types of profile. Group 1(33.3%) showed 1-3-2 profile type and elevated Pt. scale, suggesting mainly depressive neurosis in the department of neuropsychiatry. Group 2 (37.5%) showed a normal profile. Group 3(29.2%) showed 1-3 profile type it is profile of conversional neurosis. With this result it is indicated that 62.5% Vertigo patients are related to emotional difficulties. 3. There was to a remedial value for the Vertigo patients, before treating from VAS score 10.00 after treating at VAS score 4.13. From this study, we know the possibility the Oriental medicine register remedial value for the Vertigo patients.
