The current status of Korean hospitals on foodservice hygiene was evaluated by a survey in the March of 2000 from 96 hospitals in terms of general sanitation management, education and training, and from 35 hospitals on the observance of hygienic practices in the whole stages of foodservice, personal hygiene, and kitchen equipment and facility hygiene. The questionnaire was filled directly by the dietitian working in the subject hospital. The status of general sanitation management was satisfactory overall; however, the record-keeping or documentation of the practices was the weak point which needs to be improved. only 4% of the subjects appeared to have implemented HACCP for the hospital foodservice. Comparison of the hospitals in their observance rate of hygienic practice by the type of hospital, the location of hospital, the number of bed, and the separation of dietitian's duties as clinical nutrition and foodservice showed characteristic trends among the groups. General hospitals often showed better scores than tertiary hospitals in their observance rates. As the location of hospital moved from large cities to small towns, the observance rate for hygienic practice decreased. Hospitals with entrusted foodservice system showed better scores in hygienic practice than those with owner-operated one in the stage of planning and equipment/facility hygiene (p<0.05). And the scores of the hospitals having dietitian's duties separated into clinical and food services were higher than the ones with un-separated duties. Detailed information obtained in this study would serve for the development of guidelines or programs to improve the hygienic level of Korean hospital foodservice.
Purpose: In this study, we tried to evaluate the clinical characteristics or circumstances that lead to unintentionally the delay in the diagnosis of intussusception or to the wrong direction that prevent the proper management early. Methods: All the patients of intussusception with delayed diagnosis in the department of pediatrics or emergency room at Gyeongsang National University Hospital from 1990 to 2003 were enrolled and reviewed retrospectively. Results: There were 8 boys and 6 girls and their median age was 8 months (range 2 months to 10 years). Their initial symptoms and signs were vomiting, seizure, diarrhea, lethargy, irritability, bloody stool, palpable abdominal mass, foul odor of urine and tachycardia. Clinical diagnosis or impressions at admission consisted of acute gastroenteritis, shigellosis and toxic encephalopathy, convulsive disorders, urinary tract infections, sepsis, abdominal mass and intestinal obstruction. Eight patients were luckily diagnosed due to the delayed manifestations of cyclic irritability or currant jelly stool. Six patients were not paid attentions for the possibilities of intussusception and diagnosed serendipitiously by the abdominal sonography or CT during the evaluation of the abdominal mass or distension. Only five of 14 cases (35.7%) were successfully managed by barium or air reductions. The other 9 cases needed surgical operations. Conclusion: Delayed diagnosis of intussusception arise when doctors initially diagnose the patients incorrectly due to the unusual presentations or when they overlook the newly arising symptoms or signs suggestive intussusception after the admission because they are ardently attached to the first impressions or initial clinical diagnosis.
Objectives: This study investigated the relevance of dietary behavior and oral management for oral heath in adults to extend the recognition of the importance of food intake in oral health and to provide basic information for oral health management. Methods: A survey and clinical assessment were conducted for 119 adults over 20 years of age. The chi-square test and logistic regression analysis were carried out using SPSS Statistics 20.0. Results: Gingivitis was influenced less when ham and sausage in the sub-area of meat, fish, egg, and bean were consumed once a week rather than the cases when consumed rarely (3.47 times) or when consumed two times or more a week (7.87 times) (p<0.05). Periodontitis was influenced more in the groups of 30s and 40s age than in that of over 50 years of age (14.97 times, 7.73 times) and in the group without drinking experience than in group with that (4.17 times) (p<0.05). Conclusions: The results verified the close relationship between food intake frequency and oral health in patients with periodontitis and gingivitis. Thus, it would be necessary to study the relationship between dietary behavior and oral health continuously.
Frailty is a clinical syndrome as an increased vulnerability to stressors, leading to a decrease in physiologic reserves and a decline in the ability to maintain a good homeostasis. This condition leads to an increased risk of hospitalization, disability and mortality. Frailty occurs due to various causes and requires a multidimensional approach. It is also important to detect and manage it early. Frailty is also deeply related to neuropsychiatric problems such as pain and depression. In evaluating frailty, it is desirable to comprehensively consider not only physical areas such as disease, nutrition, movement, and sensory functions, but also psychosocial areas, and representative scales include Fried's physical frailty phenotype and Rockwood's frailty index. Physical activity and appropriate protein intake are important for frailty management, and inappropriate drug use should be reduced and oral care, cognitive function, and falls should also be noted. Frailty and pain can affect each other, and pain can promote frailty. Evidence has been published that hormone and protein abnormalities, immune system activity and inflammatory response, and epigenetic mechanisms work in common in the field of frailty and pain. More extensive and high-quality research should be conducted in the future, and the quality of life will be improved if the results are applied to the suppression and treatment of old age and pain.
The purpose of this study was to investigate weight gain during pregnancy based on pre-pregnant Body Mass Index, to compare eating habits based on the recommended standards for weight gain presented by the Institute of Medicine, and to identify factors affecting the weight gain of pregnant women. The subjects were grouped into three categories according to the recommended weight gain standards presented by the Institute of Medicine: the inadequate group, the adequate group and the excessive group. The excessive group had a significantly higher pre-pregnant Body Mass Index than that of the two other groups. Based on a comparison of the eating habits and nutrient intake of the women to the recommended standards for weight gain, the excessive gain group had large meals and ate more frequently outside of the home when compared to the pre-pregnancy period. Based on an analysis of correlations between weight gain during pregnancy and diet factors, the adequate gain group had positive correlation between weight gain and a "balanced meal" and had negative correlation with "convenience meals". The excessive gain group had a positive correlation with "convenience meals" and had a negative correlation with "meal skipping." All the pregnant women were more frequent in the "overeating categories" and all ate more "fruit," as their weight gain was higher. In particular, the excessive gain group was frequently evaluated as "overeating". These results indicate that the factors affecting weight gain during pregnancy were the Body Mass Index before pregnancy, eating types, and eating habits. As the Body Mass Index before pregnancy was higher, the ingestion of a 'balanced meal' was less, and overeating and gluttony were more frequent. Therefore weight gain was enhanced.
This study was designed to secure the basis for developing the health tea that may help promote healthy blood vessels by natural herbal ingredients formulated in accordance with the basic principles of oriental medicinal materials. We investigated the vessels contracted by concentrations and safety assessment carried out by the cell viability of Taraaci Herba, Cnidii Rhizoma, Citri Percarpium, and Ophiopoginis Radix composition and concentration. We found cell survival rate was higher than the control group, showing a beneficial trend in the growth of normal liver and kidney cells. As a result, this study will be the basis to develop the health tea differentiated in the future Chinese medicine resources. Medicinal resources will be health tea based on clinical trials utilizing herbal western and oriental medicine convergence principle and vascular relaxation mechanism. And this study tried to make health tea industrialization possible.
In order to examine the relationship between cholesterol level on serum and depression in Korean adults, this study used data of the fourth Korea National Health and Nutrition Examination Survey (KNHANES). 834 persons with depressive symptoms were selected as an index group and 3,317 persons without depressive symptoms as a control group, and odds ratios were calculated by multivariate logistic regression. As a result, odds ratio of depressive symptoms was higher in low LDL group (OR=6.435, 95% CI; 2,358-17.557) than in high LDL group adjusted after monthly income, marital status, period of education, occupation, smoking status, and physical activity. Etiopathogenic mechanism must be found by convergent study in the future.
Recent researches on adipocytes in human and mice model have reported that the adipocytes are not only the fat depots but having role in maintenance of physiology and metabolism through adipokines released by them in accordance to their anatomical location. Ayurveda scholars too have mentioned different tissues like Vasa (inter muscular fat), Meda (visceral fat) and Majja (bone marrow) which are predominantly rich in adipocytes similar to adipose tissues, with a different sites, functions, compositions and pathological outcomes. The metabolic effect of Meda and Majja Dhatu on other tissues like muscle (Mamsa Dhatu), bone (Asthi Dhatu) and reproductive tissue (Shukra Dhatu) shows their functional interdependence. The detailed description of therapeutic indications of Vasa and Majja under Snehakarma (oleation therapy) illustrates that clinical physiology of these tissues have been elaborated rather than general physiology. This article is an attempt to comprehend the physiological aspect of Vasa, Meda and Majja retrospectively on the basis of their therapeutic indication for the management of variety of disorders, in the form of Sneha through different therapeutic procedures. An effort has been also taken to distinguish Vasa, Meda, Majja based on the functional peculiarities of adipocytes present in different sites of body like omentum, muscle and bone marrow. Critical observation of explanations of Vasa, Meda and Majja in Ayurveda compendia and advanced research in field of adipocytes reflected that Ayurveda scholars had deep insights regarding the various dimensions of adipocytes, most of which are in consistent with the advanced physiology and biomolecular studies of adipocytes.
A variety of livestock feed resources were used in Korean dairy farm due to a lack of the endemic feed. However, there is inadequate real farm data to support farmers' decisions on the choice of options. The main objective of this study was to evaluate the nutritional value of total mixed ration (TMR) as well as the metabolic diseases status in Korean dairy farms. TMR samples were collected from nine feed companies and eight selected self-formulated by the dairy farms. The nutrient contents were examined by AOAC methods. The frequency of metabolic diseases such as ketosis and hypocalcemia were surveyed. The average moisture content was 36.2% although the min. and max. value were varied from 21.7% and 50.6% among farms. The mean${\pm}$standard deviation of crude fiber (CF), crude ash (CA), ether extract (EE), and crude protein (CP) were $21.4{\pm}2.5$, $4.6{\pm}0.4$, $3.2{\pm}0.5$ and $9.8{\pm}1.7$, respectively. However, the average ADF and NDF was $17.3{\pm}3.7$ and $31.0{\pm}5.7$, respectively. The compositions of TMR were varied significantly among the dairy farms. The frequency of clinical Ketosis (CK), subclinical ketosis (SCK) and hypocalcemia were higher in early lactation period with 4.5%, 11.0% and 3.0%, respectively. Also, the frequency of SCK was higher than CK and hypocalcemia throughout the lactation. Periodic TMR nutrient analysis based on herd production or physiology change would maximize the effects of TMR feeding. Furthermore, the study results would be useful to the farm practitioner and producer for their farm management.
Objective: Endometritis is a major disease, that causes infertility in cattle, and is usually categorized as clinical or subclinical endometritis (SCE). The nutritional condition during the dry period is important for recovery after the last stage of the lactation period, and for postpartum production and reproduction. This study aimed to clarify the relationship between nutritional and metabolic characteristics in the dry period, and the risk of postpartum SCE. Methods: Multiparous Holstein dairy cows (n = 25, raised in a tied stall) were used. Endometrial cytological analysis was performed around 30 days post-partum, with 5% to 14% polymorphonuclear (PMN) as a cut-off point to define SCE. Serum levels of glucose, non-esterified fatty acids, β-hydroxybutyric acid (BHBA), blood urea nitrogen, total cholesterol, aspartate aminotransferase, γ-glutamyl transpeptidase, calcium, phosphorus, and magnesium were measured in the cows at the dry period to evaluate energy status, protein metabolism, and mineral metabolism. Results: The incidence of SCE in the cows was 60.0% (n = 15/25) and the mean PMN% in postpartum cows diagnosed as SCE was 8.05%±2.6%. Overall, 17 and 8 samples were collected from the cows in the far-off and close-up periods, respectively. The serum concentration of BHBA in the far-off period and serum glucose concentration in the closeup period were correlated with postpartum PMN% (r = 0.62, p<0.01; r = -0.74, p<0.05, respectively). Serum levels of calcium and magnesium in the dry period were associated with the incidence of postpartum SCE (healthy vs SCE cows, p<0.05). Conclusion: Blood levels of glucose, BHBA, calcium, and magnesium in dry periods could be useful parameters for predicting the risk of postpartum SCE. The present study also suggests that management in the close-up period is essential for promoting recovery from calving fatigue.
