The Clinical Efficacy of the Low-dose FSH Regimen for Intrauterine Insemination (인공수정 시술시 저용량 FSH(Low-dose FSH) 용법의 임상적 효용성에 관한 연구)

  • Objective: This study is to investigate the clinical efficacy of low-dose FSH regimen, comparing with clomiphene citrate and human menopausal gonadotropin (CC/hMG) regimen. Methods: Retrospective study of the ovulatory factor infertility 39 patients who had been treated by intrauterine insemination (IUI). The 31 cycles of 21 patients were stimulated by CC/hMG regimen, the 22 cycles of 18 patients were stimulated by low-dose FSH regimen. We compared the rate of clinical pregnancy, multiple pregnancy and ovarian hyperstimulation syndrome (OHSS) of both group. Results: The rate of clinical pregnancy of the CC/hMG group was 25.7% per cycle, and that of the low-dose FSH group was 54.5% per cycle. The low-dose FSH group showed a higher rate of clinical pregnancy per cycle than CC/hMG group (p=0.028). However, no differences was found statistically in the rate of multiple pregnancy and OHSS between CC/hMG group (22.2%, 5.7%) and low-dose FSH group (33.3%, 13.6%). Conclusion: This study showed that the low-dose FSH regimen is superior to CC/hMG regimen in getting clinical pregnancy, but dose not reduce the ovulation induction complications.

Influencing factors of and satisfaction with clinical practice between college and university dental hygiene students (학제에 따른 치위생과 학생의 임상실습 만족도와 관련 요인)

  • Objectives: This research aimed to identify the influential factors that affect the satisfaction of dental hygiene students' clinical training between college and university and contribute to improvement in clinical practice quality. Methods: A self-reported questionnaire was completed by 210 dental clinical hygienists from June 10 to August 30, 2018. Data were analyzed using SPSS 22.0. An independent t-test, one-way ANOVA, Scheffe's post-hoc test, and Pearson's correlation coefficient were employed, and a stepwise multiple regression analysis was performed. Results: Regarding the influential factors of satisfaction with clinical practice among dental hygiene students, in the case of college students, dental hygienist practice instructor dummy, self-efficacy, satisfaction of school life, interpersonal relation was the increasing order. In the case of university students, dental hygienist practice instructors, subjective health, satisfaction with school life, and interpersonal relations was the increasing order. Conclusions: The most influential factor of dental hygiene students' clinical practice satisfaction between college and university was the ability to form interpersonal relationships. Therefore, to increase the satisfaction of dental hygiene students with clinical practice between college and university, external sources and efforts to emphasize internal sources are required.

A Study on Menstrual Pain, Clinical Practice Stress and Clinical Competence Among Nursing Students (간호대학생의 월경통증, 임상실습 스트레스 및 임상수행능력에 관한 연구)

  • The purpose of this study was to investigate the menstrual pain, clinical practice stress, and clinical competence and identify influencing factors of clinical competence of 3rd nursing students who start clinical practice for the first tim. The survey was conducted on 155 nursing students from June 1 to October 30, 2020. Data were analyzed using t-test, ANOVA, Scheffe test, Pearson correlation coefficients and multiple regression analysis. The degree of influence menstrual pain was 5.01points, clinical practice stress was 2.82points, clinical competence was 3.42points. Menstrual pain was positive correlated with clinical practice stress(r=.319, p=.000), and menstrual pain was negative correlated with clinical competence(r=-.279, p=.000). Clinical practice stress was negative correlated with clinical competence(r=-.333, p=.005). Menstrual pain was main factor that affects clinical competence. The model explained 25.0% of the variables. Therefore, intervention education is needed to reduce menstrual pain in order to improve the clinical competence of nursing students.

Clinical Decision Making Patterns of Pediatric Nurses (아동간호사의 임상적 의사결정 유형에 관한 연구)

  • Purpose: The purpose of this study was to identify clinical decision making pattern of pediatric nurses and analyze how it shows the differences in types of decision making pattern by nurses characters. Methods: A self-administered questionnaire was used to pediatric nurses of 4 general hospitals in Seoul from February 2004 to April 2004. The data of 251 nurses was analyzed by varimax rotation factor analysis, t-test, and ANOVA. Results: 6 decision making patterns were identified: Individual Patient-oriented, Pattern-oriented Intuitive, Typical Nursing Knowledge-oriented, Nursing Model-oriented, Medical Knowledge-oriented, and Patient-Family-Nurse Collaborative. Individual Patient-oriented, Pattern-oriented Intuitive, Typical Nursing Knowledge-oriented, and Nursing Model-oriented decision making pattern got meaningful differences in age, marital status, total number of years in nursing practice, and number of years in pediatric nursing practice. Conclusion: We expect the result of this study can be applied for promotion of understanding the decision making of nurses that occurs in pediatric nursing practice and also can be used as foundation data for development and expansion of pediatric nursing practice.

Effects of Calcific lesion on Shockwave Therapy of the Tennis Elbow (체외충격파를 이용한 테니스 엘보우의 치료에서 석회화 병변의 영향)

  • Purpose: The aim of this study was to investigate effects of calcific lesion on shockwave therapy of the tennis elbow. Materials and Methods: twenty-four patients with refractory tennis elbow were treated with shock waves. The patients were evaluated by assessment of pain using visual analog scale (VAS) and simple elbow test (SET). Comparision of clinical outcomes for the patients with and without calcification in the extensor tendon and/or cortical irregularity of lateral condyle was tried to determine if this could be a possible prognostic factor in clinical settings. Overall clinical outcomes were evaluated by Roles and Maudsley score at 12 months after ESWT. Results: Significant improvement of symptoms were observed in 20 (83 %) patients at 12 months follow up according to Roles and Maudesley scores. The patients with calcification and/or cortical irregularity improved significantly better, when compared to the patients without calcification and/or cortical irregularity at follow up. Conclusion: This study suggests that shock waves therapy could be considered as effective and noninvasive treatment modality for refractory tennis elbow. Also calcific deposit in extensor tendon and/or cortical irregularity of the lateral epicondyle was seem to be good prognostic factor for shock wave therapy for tennis elbow

  • Objective: Clinical and Experimental Reproductive Medicine (CERM) converted its language to English only beginning with the first issue of 2011. From that point in time, one of the goals of the journal has been to become a truly international journal. This paper aims to identify the position of CERM in its scholarly journal network based on the journal's metrics. Methods: The journal's metrics, including citations, countries of author affiliation, and countries of citing authors, Hirsch index, and proportion of funded articles, were gathered from Web of Science and analyzed. Results: The two-year impact factor of 2013 was calculated at 0.971 excluding self-citation, which corresponds to a Journal Citation Reports ranking of 85.9% in the category of obstetrics and gynecology. In 2012, 2013, and 2014, the total citations were 17, 68, and 85, respectively. Authors from nine countries contributed to CERM. Researchers from 25 countries cited CERM in their articles. The Hirsch index was six. Out of 88 original articles, 35 studies received funds (39.8%). Conclusion: Based on the journal metrics, changing the journal language to English was found to be successful in promoting CERM to international journal status.

Associations between Clinical Characteristics and Plasma BDNF Levels of Panic Disorder (공황장애의 임상적 특성과 Brain-Derived Neurotrophic Factor 농도와의 관계)

  • Objective : Brain-derived neurotrophic factor (BDNF) is implicated in the pathophysiology of several neuropsychiatric disorders. However, there are few studies on BDNF of panic disorder. In this study, we investigated plasma BDNF levels in patients with panic disorder, and evaluated whether there are associations between clinical characteristics of panic disorder and plasma BDNF levels. Methods : We included 110 patients with panic disorder and 110 health controls in the current study. Plasma BDNF levels were measured by the enzyme-linked immunosorbent assay (ELISA). Plasma BDNF level differences were evaluated according to the clinical characteristics, such as duration of illness, recent stressful life event, agoraphobia, and insomnia. Results : The mean plasma BDNF levels of patients with panic disorder were significantly lower, as compared with those of controls (192.50 pg/mL vs. 693.75 pg/mL, t=8.838, p<0.001). The mean plasma BDNF levels of patients who had recent stressful life events were significantly higher, as compared with those who did not ($269.79{\pm}358.96pg/mL$ vs. $136.94{\pm}187.06pg/mL$, t=-2.525, p=0.013). Conclusion : These results suggested that BDNF plays a potential role in the pathophysiology of panic disorder.

A Clinical Analysis of Thymoma (흉선종의 임상적 고찰)

  • Thymoma is defined as a neoplasm of the epithelialreticular framework cells of the thymus. The treatment of choice and the prognosis for patients with thymoma are still controversial subjects due to lack of a uniform histological classification and standardized criteria for typing thymic tumors. Between June 1985 and May 1989, eight patients underwent thymomectomy at the Department of Thoracic and Cardiovascular Surgery, Korea University Medical Center. A clinical analysis was performed and the following result was obtained. In histologic cell type, epithelial type was 2 cases, lymphocytic type 3 cases, and mixed type was 3 cases. The clinical stage was classified by Masaoka’s classification that Stage I was 4 cases, Stage II; 2 cases, Stage III; 2 cases and Stage IV was none. Myasthenia Gravis was associated with thymoma in 5 patients. In 8 patients, thymomectomy were performed. Adjuvant radiotherapy was performed in one patient with Stage I due to suspicious tumor remnant and in Stage II, III patients routinely. Combined chemotherapy was performed in one patient with Stage III due to local recurrence. We concluded that the most important prognostic factor of thymoma is extent of tumor invasion. And the presence of Myasthenia Gravis is no more prognostic factor. Surgical resection of thymoma is treatment of choice, If local invasion or distant dissemination is present, postoperative radiotherapy and /or combined chemotherapy is recommended.

Correlation Between EGFR Mutations and Serum Tumor Markers in Lung Adenocarcinoma Patients

  • Background: Mutations affecting the epidermal growth factor receptor (EGFR) are good predictors of clinical efficacy of EGFR tyrosine kinase inhibitors (TKI) in patients with non-small cell lung cancer. Serum carcinoembryonic antigen (CEA) levels are also regarded as predictive for the efficacy of EGFR-TKI and EGFR gene mutations. This study analyzed the association between EGFR gene mutations and clinical features, including serum tumor marker levels in lung adenocarcinomas patients. Patients and Methods: A total of 70 lung adenocarcinoma patients with complete clinical data and pathological specimens were investigated. EGFR gene mutations at exons 19 and 21 were assessed. Serum tumor markers were detected by protein chip-chemiluminescence at the corresponding time, and correlations were analyzed. Results: Mutations of the EGFR gene were detected in 27 of the 70 patients and the serum CEA and CA242 concentrations were found to be significantly associated with the incidence of EGFR gene mutations (P<0.05). The AUCs for CEA and CA242 were 0.724 (95% CI: 0.598~0.850, P<0.05) and 0.769 (95% CI: 0.523~0.800, P<0.05) respectively. Conclusions: Serum CEA and CA242 levels are associated with mutations of the EGFR gene in patients with lung adenocarcinomas.

Factors Affecting the Burnout of Clinical Nurses - Focused on Emotional Labor - (임상간호사의 소진에 영향을 미치는 요인 - 감정노동을 중심으로 -)

  • Purpose: This study was to identify the factors that influence the burnout of clinical nurses. Methods: Subjects were 279 nurses in 3 university hospitals at Seoul and Gyunggi province. Data were collected with self-administered questionnaires and analyzed using SPSS 16.0 by applying t-test, ANOVA, Pearson correlation, and Stepwise multiple regression. Result: There are significant differences between burnout and demographic characteristics, such as, age, marital status, education level, clinical career, and position. All variables except emotional labor were negatively correlated with burnout. The emotional labor was positively correlated with burnout. Job satisfaction, emotional labor, self-efficacy, and age had significant influence on nurses' burnout. The explained variances for burnout was 54.8% and job satisfaction was the most significant factor of burnout of nurses. Conclusion: Emotional labor is found to be a new factor that influences burnout. Therefore, it is necessary to establish new organizational culture through emotional education programs at the organizational level to reduce nurses' emotional labor.
