Alteration of Growth Factor Expression after Acute Ischemic Renal Injury (급성 허혈성 신손상 후 여러 성장인자 발현의 변화)

  • Purpose : Regeneration and repair after ischemic renal injury appears to be modulated by circulating or locally produced growth factors. This study examined the changes of serum insulin like growth factor(IGF-I) and renal expression of IGF-I and II, vascular endothelial growth factor(VEGF), transforming growth $factor-{\beta}$($TGF-{\beta}$), and connective tissue growth factor(CTGF) during the active regeneration period after acute ischemic injury. Methods : Sera and kidney tissue samples(whole kidney, cortex, outer medullae and inner medullae) were obtained before and after one, three, five and seven days of 40 minutes bilateral renal pedicle clamping. Acute renal failure was assessed by measuring the concentration of serum creatinine. Serum IGF-I level was measured by radioimmunoassay. The mRNA expression in kidney was measured by RT-PCR. The distribution of IGF-I and CTGF was detected by immunohistochemistry. Resuts : Serum IGF-I concentration after one day following acute ischemic renal injury was significantly decreased compared to preischemic value. The mRNA levels of IGF-I, IGF-II, $TGF-{\beta}1$ and VEGF in whole kidney were temporally decreased on day one of ischemic injury. IGF-I and IGF-II expressions in outer medullae were significantly decreased on day one after ischemic injury. $TGF-{\beta}1$, CTGF and VEGF expressions were markedly decreased in medullae after one day of ischemic injury compared to other kidney sections. IGF-I was markedly decreased in cortical tubules on day one of uremic rat. CTGF was markedly increased on tubule within three days of ischemic injury. Conclusion : These findings suggest that IGFs, $TGF-{\beta}1$ and CTGF may involve in the pathogenesis or the recovery from acute ischemic renal injury.

Reliability and Validity of the Clinical Judgment Rubric on Simulation Practice with a Post-operative Rehabilitation Case (수술 후 재활 사례에 적용한 임상 판단력 평가도구의 신뢰도 및 타당도)

  • Purpose: The purpose of this study was to evaluate the reliability and validity of the clinical judgement rubric on simulation practice with a post-operative rehabilitation case. Methods: Methodological study design was used to evaluate the reliability and validity of the clinical judgement rubric on simulation practice. The participants were 35 students in a college of nursing. Results: There were showed reliability and validity of the clinical judgement rubric on simulation practice with a post-operative rehabilitation case. In terms of internal consistency, the Cronbach's ${\alpha}$ for all columns was .821. Factor analysis showed that planning, intervention and evaluation domains with Eigen values of 69.87% were bound, unlike the original tool where four factors including noticing, interpretation, responding and reflecting domains were bound. Convergent validity was established by the correlation between the total clinical judgement score and critical thinking disposition before and after simulation practice. Discriminative validity was established by the correlation between the total clinical judgement score and simulation effectiveness score. Conclusion: The findings of this study suggested that clinical judgement rubric on simulation practice is reliable. Repeated studies are needed to objectively evaluate the clinical judgement rubric of simulation practice for applying various clinical situations.

Prescribing Patterns of Antidepressants and Their Associated Factors in Breast Cancer Patients (유방암 환자의 항우울제 처방 현황 및 영향요인 연구)

  • Purpose: The aim of this study was to investigate the current state of antidepressant prescriptions in breast cancer patients and factors affecting the prescription of antidepressants. Methods: This study targeted female breast cancer patients who were prescribed antidepressants by a psychiatrist at least once between August 2010 and July 2011 at the Asan Medical Center in Seoul. The prescription history of each study subject was investigated to analyze the current state of antidepressant prescriptions in breast cancer patients. Results: The analysis of the prescription histories of 136 subjects in the antidepressant group determined that escitalopram, mirtazapine, and trazodone were the three most commonly prescribed medications with an average of 1.54 antidepressants prescribed per patient. A logistic regression analysis showed a statistically significant increase in antidepressant prescriptions in patients who were divorced or widowed, had sleep disturbances, or had undergone oncologic surgery for the breast cancer (p<0.050). In contrast, the prescription rate was lower for patients with tumour sizes greater than 50 mm (p<0.050). Conclusion: The sociodemographic factor of marital status, clinical factors of sleep disorders and tumour size, and a treatment-specific factor of the use of surgical therapy were identified as affecting the prescription of antidepressants in female breast cancer patients.

Implant survival and risk factor analysis in regenerated bone: results from a 5-year retrospective study

  • Purpose: The aims of this study were to evaluate the 5-year cumulative survival rate (CSR) of implants placed with guided bone regeneration (GBR) compared to implants placed in native bone, and to identify factors contributing to implant failure in regenerated bone. Methods: This retrospective cohort study included 240 patients who had implant placement either with a GBR procedure (regenerated bone group) or with pristine bone (native bone group). Data on demographic features (age, sex, smoking, and medical history), location of the implant, implant-specific features, and grafting procedures and materials were collected. The 5-year CSRs in both groups were estimated using Kaplan-Meier analysis. Risk factors for implant failure were analyzed with a Cox proportional hazards model. Results: In total, 264 implants in the native bone group and 133 implants in the regenerated bone group were analyzed. The 5-year CSRs were 96.4% in the regenerated bone group and 97.5% in the native bone group, which was not a significant difference. The multivariable analysis confirmed that bone status was not an independent risk factor for implant failure. However, smoking significantly increased the failure rate (hazard ratio, 10.7; P=0.002). Conclusions: The 5-year CSR of implants placed in regenerated bone using GBR was comparable to that of implants placed in native bone. Smoking significantly increased the risk of implant failure in both groups.

Factor Analysis of Low Back Pain Patients (일부 요통환자의 인자분석)

  • Objectives: The objective of this study was to investigate factor analysis of low back pain patients. Methods: The data were collected from hospital located in Daegu. Observation was made on 40 low back pain patients and factor analysis study was also performed in terms of their abnormal physical findings. Results: The low back pain patients occupied 20% of all clinical patients in male group and 20% in female group. The occurrence of acute low back pain in the first factor tended to be higher among in male group than among in female group. While it was significantly higher muscle weakness of the back in female group. The most prevailing abnormal finding among low back pain patients in the first factor were tender point on the back, While it was significantly lower Decreased tendon reflex in both sexes Conclusions: The factor pattern of abnormal physical findings in low back pain patients was much different between male and female patients, suggesting the different pattern of etiology. Although low back pain is one of the most common symptoms causing limitation activity, as yet there is no known specific effective treatment.

A Test to Compare the Water Resistance Sun Protection Factor of General Water, Artificial Seawater, and Natural Seawater of Sunscreen (자외선 차단제의 일반 물, 인공 해수, 자연 해수의 내수성 차단지수를 비교하기 위한 시험)

  • Sunscreen is a product that protects against ultraviolet rays by blocking and scattering ultraviolet rays, and has now become a daily necessity beyond cosmetics. Applying sunscreen is a common and easy way to prevent skin damage caused by ultraviolet rays. Due to its significance, the evaluation of sunscreen has evolved since its regulation by the FDA in 1978, progressing to standardized methods established by ISO. Additionally, to assess the loss of sunscreen due to activities such as water exposure or sweating, the Ministry of Food and Drug Safety in Korea and ISO have established protocols for evaluating the water-resistant sun protection factor (SPF). However, existing evaluations of water resistance have been mainly confined to test methods involving plain water, and methods accounting for the impact of seawater during activities like beach leisure, sports, and recreation are yet to be established. Based on the existing guidelines for testing the water-resistant UV protection index, this study compared the water-resistant UV protection index in water, artificial seawater (salt water) and natural seawater (sea water) to evaluate the UV protection index in real-world situations such as marine leisure, sports, and leisure activities. Through these results, we were able to compare the differences between water resistance sun protection index tests in ordinary water, artificial seawater, and natural seawater, and suggest a method for water resistance sun protection index tests using natural seawater.

Affecting Factors on Clinical Competence of Nursing Students (간호학생의 임상수행능력 영향 요인)

  • The purpose of this study was to investigate the level of clinical competence and influencing factors of clinical competence in nursing students. The data was collected using questionnaire from 207 students who running the 2nd grade courses of nursing curricula in college and was performed from August 25 to September 15. In clinical competence, there were significant differences in satisfaction of major, satisfaction of clinical experience. There was a positive correlation among clinical competence and professional self-concept, critical thinking disposition and self-directed learning ability. The most effective factor on clinical competence was self-directed learning ability, followed by professional self-concept and critical thinking disposition. The study findings suggest that to develop the effective learning program with achievement level of clinical competency for nursing students.

  • Purpose: This study was to evaluate clinical competence in relation to self-directed learning, critical thinking disposition, and participating in PBL(Problem-Based Learning) group activities of nursing students. Methods: Data were collected from 108 nursing students in Oct. 2006. Results: Clinical competence had a significant positive correlation with self-directed learning, critical thinking disposition, and participation in PBL group activities. There was a significant difference in clinical competence according to interpersonal relationship. Participation in PBL group activities was the most influential factor of clinical competence($R^2$=.34). Also, the influence of clinical competence increased with the addition of self-directed learning($R^2$=.42). Conclusion: It is essential to encourage the self-directed learning and participation in PBL group activities for the improvement of clinical competence. It is relatively important for clinical competence to consider the educational environment systematically.

Development of Decision Tree Program based on Web for Analyzing Clinical Information of Sasang Constitutional Medicine (사상체질 임상정보 분석을 위한 웹 기반의 의사결정 나무 프로그램 개발)

  • Sasanag Contitution Medicine(SCM) is the traditional medicine theory based on constitutional medicine in Korea. It is most import ant that a personal SCM type is determined accurately ahead of applying any Sasang treatments. For this, many researches have been studied to diagnose the SCM type using constitutional clinical data. The decision tree is a tree-structured data-mining methodology. Recently, in the Korean traditional medicine society, there have been several efforts to find diagnosing tools using the decision tree method. So, we developed a decision tree program based on web for analyzing constitutional clinical information. It can use various clinical data as input data, offer filtering function to select clinical data to be used. We can find useful factor to be influential on SCM types using this program.
