Use of Platelet-Rich Fibrin in Oral and Maxillofacial Surgery

  • Platelet-rich fibrin (PRF) is a strong but flexible fibrin including a enrich platelet which contain growth factors and cytokines. PRF can be made very simply and requires no artificial additives unlike platelet-rich plasma. While PRF is remodeled and released in the tissue, this induces cell growth, vascularization, collagen synthesis, osteoblast differentiation and an anti-inflammatory reaction. Taking advantage of these functions, PRF can stimulate regeneration of bone and soft tissue in a diverse number of ways during the course of hemostasis, wound coverage, preservation, and reconstruction of alveolar bone. Moreover, the use of PRF to improve bone regeneration has become a recent technique in implantology. In this study, through a literature review of PRF's existing clinical applications, we classified a range of potential PRF oral and maxillofacial surgery applications including preservation of extraction sockets, guided bone graft, sinus lift, dressing and periodontal treatment. This trial gave us chance to confirm the usefulness of PRF. Recently, updated clinical studies results concerning skin and tendon wound healing have become available. These results suggest that the usage of RPF will gradually expand.

치위생과 학생들의 전공교육 및 직업관에 대한 의식 조사 연구 (A Study on Dental Hygiene Students' Consciousness about Course Education and their Occupation)

  • For the purpose of strengthening Dental Hygiene students' confidence and motivation in the Dental Hygiene Department and helping construct proper professionalism, survey on Dental Hygiene students' consciousness of attitude to and satisfaction of the course, career plan and occupation mind set was carried out. 530 three year students in 8 Dental Hygiene academies in Seoul and Kyonggi province were questioned. The results of the survery are as follows:. 1. Dental Hygiene students' motives consisted primarily of employment and a desire for professionalism 25% of them entered the course after one failure in the entrance examination and 17% had family members engaging in the dentistry field 84%, the largest portion, were from an academic high school. 24% had some knowledge of Dental Hygiene, which they had acquired from seniors, friends, and teachers. 2. Patient care and treatment assistance related matters were not considered important in the course. The weak points of the course turned out to be education in computers and foreign languages, but the weakest was the ability of patient care during clinical training. 53% had experiences had thought of changing their major while in the course because it didn't match their aptitude and interest. 3. As for a career after graduation, 49% worried about it Most students wanted to work at a dental hospital or general hospital, The most favored duty was coordination or reception or oral disease preventive work. They wanted to work untill they had a stable living. 68% answered they would get a job at an oral clinic and 70% said they would continue studying for self-realization. 4. Satisfaction with the major was high in students whose aptitude and interest matched the course, who had background knowledge of the major, and who. didn't think of changing the major but would continue studying resulting in statistically slight difference(p<0.001). As to satisfaction with the faculties, it was high in the students whose aptitude and interest matched the major and who didn't think about a career after graduation showing a slight difference(p<05, p<0l). As for satisfaction with clinical training, students whose aptitude and interest matched the major and who didn't consider changing the major answered positively showing a statistically slight difference(p<.001, p<.01). As to satisfaction with the course, it was high in the students who entered with aptitude and interest, who had preliminary knowledge, who didn't consider changing the major, and who didn't think about a career after graduation showing a statistically slight difference(p<.001, p<.05). 5. Occupation mind-set was positive for students who entered with interest and aptitude, who had preliminary knowledge, and who had not considered changing the major showing a statistically slight difference(P<.001). The higher the satisfaction with the major, faculty and clinical training was, the more positive the occupation mind-set was(p<.001).

  • The placement of different graft materials and/or the use of occlusive membranes to cover the extraction socket entrance are techniques aimed at reducing alveolar ridge resorption and enhancing bone formation. However, in spite of its clinical advantage, the use of graft materials in fresh extraction socket has been questioned because particles of the grafted material have been found in alveolar sockets with fibrous union. The purposes of this study were to evaluate whether alveolar ridge resorption following tooth extraction could be reduced and bone formation could be enhanced by the application of absorbable gelatin spongy or gelatin spongy soaked with platelet rich plasma(PRP) used as a space filler in clinical and radiographic aspects. Eighty patients who were scheduled for extraction of both third molars were participated and carried out by one experienced surgeon. Following extraction of teeth, one extracted socket were treated with gelatin spongy as an experimental group A and the other were treated with gelatin spongy and PRP as an experimental group B. The routine extracted socket were healed without any treatment as a control group. From the period of extraction to 12 weeks postoperatively, we examined the clinical course and radiographic evaluation on socket at regular interval. Both experimental groups showed faster wound healing process than control clinically. Vertical gingival height of the extraction socket were less changed statistically in both experimental groups than control. The horizontal width change of the extraction socket were not significant statistically in any group. Radiographic changes of the alveolar bone height were less changed in both experimental groups and bone density were showed higher than control. There were a little difference between experimental group A and B. In conclusion, absorbable gelatin sponge and with PRP were considered as having preservation effects of extraction socket and stimulation of bone formation process after extraction.

  • The present case illustrates an arteriovenous malformation of the cheek in a 25-year-old male. The clinical presentation. radiographic findings, differential diagnosis, treatment and histopathologic description are presented. Diagnosis of the lesion was confirmed by angiography, and the lesion was treated by angiographically controlled vascular embolization followed by complete surgical excision.

  • The present report describe the surgical therapy, clinical course, orthodontic treatment and morphological characteristics of an adenomatoid odontogenic tumor in the maxilla of an 11-year-old patient. The cystic tumor filled the maxillary sinus and involved a tooth. Marsupialization was accompanied by partial enucleation and applied traction to the affected tooth by a fixed orthodontic appliance. Healing was uneventful and no local recurrence was observed during a 1-year period of follow-up control.

Osteosarcoma of the mandible: A case report with an early radiographic manifestation

  • Osteosarcoma is a classical malignant bone-forming neoplasm which usually presents with an aggressive clinical course. The current case is presented with the radiographic feature of widening of the periodontal ligament space of the involved teeth, which is considered to be the earliest radiographic manifestation of osteosarcoma involving the jaw bone. The main aim of this case report was to focus on the importance of early diagnosis of this tumor based on clinical and radiographic examinations, and confirmation by histopathology. Considering the rarity of the disease type and particularly taking into account the fast progression and aggressiveness of this neoplasm, it is clear that the presentation of a clinical case represents a major contribution to better understanding of osteosarcomas involving the jaw bone.

치기공과 재학생의 성별에 따른 임상실습 만족도 조사에 관한 연구 (A Study on the Satisfaction Level of Clinical Training for Students in the Department of dental technology according to their gender)

  • Purpose: of this survey research was to investigate the satisfaction level of clinical training for 300 students in the department of dental technology. Methods: This survey was carried out from 1th of january to 20th of january and analyzed using the Statistical Package for Social Sciences 17.0 for Windows. As for the analysis methods, the study used the frequency analysis, percentage, mean, t-test, analysis of variance, multiple regression analysis. Results: The results of multiple regression analysis between the satisfaction level and the related factors indicated that the satisfaction of major and place of clinical training in the clinical training are the major component in the male students and the satisfaction of major and a school record and clinical training experience are the major component that contribute the satisfaction level in the female students. Conclusion: Regular course of training and pre-employment training also recommends to students in the department of dental technology. Faculty and clinical dental technicians should try in order to improve satisfaction level of clinical training.

Prophylactic antibiotics in intra-oral bone grafting procedures: a prospective, randomized, double-blind clinical trial

  • Objectives: This study was conducted in order to assess the efficacy of 1st generation cephalosporin as use as a single-dose preoperative prophylactic antibiotic for surgical wound infections resulting from intra-oral bone grafting procedures. Materials and Methods: A total of 23 patients who were to undergo intra-oral bone graft procedures participated in this study. After randomization, 2 grams of 1st generation cephalosporin was orally administered to both the experimental and placebo groups one hour prior to surgery in a double-blind fashion. Post-operatively, the experimental group (12 patients) was orally administered placebo three times a day for three days. The control group (11 patients) was orally administered 1st generation cephalosporin three times a day for three days. The postoperative course was observed for one month including the clinical parameters associated with infection. Results: Postoperative infections were noted in 1 out of 11 patients in the experimental group. No infections occurred in the control group. Conclusion: There was no significant difference in the incidence of postoperative infections between the two groups. Two grams of 1st generation cephalosporin administered orally one hour before surgery served as an effective prophylactic antibiotics therapy for intra-oral bone graft surgery.

치위생(학)과 임상실습 표준화 방안 (Study of Clinical Practice Standardization in Dental Hygiene)

  • 전국 82개의 치위생(학)과의 임상실습 교과목은 필수 교과목으로 운영되고 있으며, 임상실습 기관은 대학병원부터 개인 치과의원까지 범위가 넓고, 실습시간별 교육환경이 달라 임상실습 내용과 평가방법이 다양하게 이루어지고 있으며, 치위생학 교육과정과 실무와의 연계 및 전문성 제고를 위한 임상실습 표준화에 대한 요구도가 높아지고 있다. 이에 본 연구에서는 치과위생사 2차 직무기술서와 선행연구에서 파악한 임상실습 항목을 목록화 하여 총 156개 항목 중 74개(47.4%)의 임상실습 내용을 추출하였으며 각 임상실습 내용에 대해 필수 및 선택실습 여부, 실습시기, 활동방법을 구체적으로 제시하였다. 실습일정은 전반부(1주차), 중반부(2~3주차), 후반부(4주차)로 구분하고, 활동방법을 관찰, 준비, 수행, 교육으로 구분하여 임상실습 기관에서 학생들에게 임상실습을 지도할 수 있도록 시범운영을 진행하였다. 각 대학별 교과과정을 기반한 임상실습 내용 배치를 위해서는 임상실습 선수교과목으로 이수한 임상치의학 교과목을 중심으로 임상실습 내용을 구성하도록 하여 단계별로 임상실습 내용을 모두 달성할 수 있도록 해야 하며, 이를 위해서는 교육기관과 임상실습 기관의 유기적인 협력관계뿐 아니라 표준화된 임상실습 프로토콜이 제시되어야 할 필요성이 있겠다.

일부 4년제 치기공학과 학생이 인지한 현장실습교육환경(FLE)에 관한 연구 (A Study on Some 4-year Dental Technology Students Perceptions of Field Learning Environment(FLE))

  • Purpose: This research was to examine some 4-year dental technology students perceptions of field learning environment(FLE). Methods: This study was conducted from October 15th to November 30th, 2017. 163 copies were used as the final analysis data. We surveyed questionnaire 163 of some 4-year dental technology students. The collected data were analyzed using SPSS (Statistical Package for Social Science) 22.0 for windows, and frequency analysis, chi-square test analysis, and correlation analysis were performed. Results: Satisfaction of practicing educator 3.88, satisfaction of clinical practice 3.49, major satisfaction 3.34, and university life satisfaction 3.25. Factors influencing satisfaction with clinical practice were clinical practice place, number of clinical practice sites, work done in clinical practice, and difficulties (p < .05). There was a significant positive correlation between satisfaction with clinical practice and satisfaction with major (P < .05). Conclusion: The results of this study, we were able to check the current field learning environment(FLE) and actual condition of students. Through the investigation of the desired clinical practice, it is anticipated that it will be possible to provide a field learning environment suitable for the course of the field desired by the user.
