A Clinical Experience of Direct Extension to Frontal Sinus of Orbital Dermoid Cyst (전두동을 침습한 안와부 피부모양기형낭의 치험례)

  • The dermoid cyst is the one of common space occupying orbital lesion. This lesion is regarded as a non-invasive tumor, but infrequently causes destruction of adjacent bony structure and displacement of adjacent tissue. We experienced a characteristic ovoid orbital dermoid cyst that occupied in the frontal sinus and causes displacement of the eyeball with well-defined lining. This 55-year-old male presented a mass in left orbit, which rapidly increased in size for past 12 months, and patient could not open left eyelid. We removed this tumor totally then reconstructed the orbital roof and frontal sinus with an iliac bone graft and polyethylene sheet(Medpor Newnan, USA). This patient was followed up for 12 months and patient obtained satisfactory result without any complication suck as recurrence or infection.

  • Insufficiency fracture occurs when normal or physiological stress applied to weakened bone with demineralization and decreased elastic resistance. Recently, many studies reported the development of IF after radiation therapy (RT) in gynecological cancer, prostate cancer, anal cancer and rectal cancer. The RT-induced insufficiency fracture is a common complication during the follow-up using modern imaging studies. The clinical suspicion and knowledge the characteristic imaging patterns of insufficiency fracture is essential to differentiate it from metastatic bone lesions, because it sometimes cause severe pain, and it may be confused with bone metastasis.

  • Fiber reinforced materials have favorable mechanical properties. Moreover, the strength to weight ratios of this material is superior to those of most alloys. Comparing to the metals, it showed many other advantages as well, including non-corrosiveness, translucency and easy repair characteristic. Since, it has the potential for the chair-side and laboratory fabrication, it is not surprising that fiber reinforced composites offer the potential for use in various applications in dentistry. To make the well-fitted restorations, Fiber reinforced composite (FRC) has been suggested as an alternative framework material for the implant supported fixed prosthesis. Two fixed partial denture fabrication procedures were tried. Vectris fiber was pressed to the EsthetiCone gold cylinder on the implant positioned cast. And then, Targis were added on it. In the other method, we used the customized component using UCLA abutment. The beads for retaining the Vectris fiber were added on the abutment. If careful laboratory and clinical techniques were done, these two techniques would fulfill the demands of the esthetics and strength.

A case of Merkel cell carcinoma of the head and neck

  • Merkel cell carcinoma (MCC) is a relatively rare and aggressive cutaneous neuroendocrine malignancy. It is characterized by high rates of recurrence and metastasis, both to regional lymph nodes and to distant locations. Its characteristic clinical manifestation is a single, painless, hard, erythematous nodule on a sun-exposed area, particularly in older men. Surgical management of both the primary site and the sentinel lymph node is the standard of care. In this article, we describe the diagnosis and treatment of a case of MCC in the left cheek.

  • Deletion of the short arm of chromosome 6 is relatively rare, with the characteristic features of craniofacial malformations, hypotonia, and defects of the heart and kidney, with hydrocephalus and eye abnormalities. Here author reports a premature girl with bilateral anophthalmia, bilateral hydrocephalus and marked hypotonia, whose chromosome analysis revealed a 46, XX, del(6)(p23) chromosome constitution.

Posterior reversible encephalopathy syndrome caused by presumed Takayasu arteritis

  • Takayasu arteritis (TA) is a chronic inflammatory disease of unknown etiology that affects mainly the aorta, main aortic branches, and pulmonary arteries. Diverse neurological manifestations of TA have rarely been reported in children. Posterior reversible encephalopathy syndrome (PRES) is a neuroradiological condition that presents with headache, seizure, visual disturbances, and characteristic lesions on imaging. Inflammatory condition and severe hypertension in TA can cause PRES. We report of a 5-year-old girl with presumed TA who presented with PRES and chronic total occlusion in the renal artery. The findings on magnetic resonance imaging suggested PRES. Left nephrectomy was performed for total occlusion of the left renal artery, and the confirmatory diagnosis of TA was based on the pathologic findings of the renal artery.

  • Objective : To clarify the features of needling method in the Zhenjiu Jiayijing, the needling depth and needling treatment time were sorted out. Method : According to the analysis, we can identify the crucial factors of the needling features. Result : Compared with the needling related content of Huangdi Neijing(黄帝内经), the historical outline of the needling development could be found and the ancient acupuncturing process was reappeared with a historical perspective. This research will provide guidance for the literature research and clinical application. Conclusion : Zhenjiu Jiayijing, also an important and valuable literature, will provide more evidence or clues for depth and treatment time of needling, which will be helpful and meaningful in clinic.

  • Cleidocranial dysplasia is an autosomal dominant heritable skeletal disorder. The characteristic features of cleidocranial dysplasia (CCD) may include hypoplasia of the clavicle, delayed closure of frontanelles, late tooth eruption, and other skeletal disorders. This case report describes clinical and radiographic manifestations at the age of 11 and 29 of a CCD patient, investigates the mutation of core-binding factor A1 (CBFA1) based on gene analysis, and illustrates successful oral reconstruction with fixed prosthesis and dental implant after the extraction of multiple teeth.

  • The author analyzed the clinical and radiographic findings of 109 malignant tumors of epithelial origin occured in the jaws of the patients visited the infirmaries of Dentistry, Chosun University and several university in Korea during 1978 to 1988. The observed results were as follows: 1. It appeared that 93 % of the total 397 cases diagnosed as oral malignant tumors were squamous cell carcinomas. 2. The incidence ratios between nodular type and ulcer type were 4 to 1 in maxilla and 3 to 1 in mandible. 3. In nearly 50% of all patients complained of pain due to impingement of tumor mass or ulcer. 4. Most of carcinomas of maxilla eventually invaded into maxillary sinus and palate. 5. Characteristic features on the radiographs were the lesion with ill-defined border, the direct destruction of the alveolar bone and anatomical landmark without displacement of the involved teeth and the gray shadow of the tumor mass in the lesion.

  • Hereditary hemorrhagic telagiectasia (HHT), which is characterized by the classic triad of mucocutaneous telangiectases, arteriovenous malformations (AVMs) and inheritance, is an autosomal dominant disorder. The characteristic manifestations of HHT are all due to abnormalities of the vascular structure. This report deals with the case of a 14-year-old girl with typical features of HHT that include recurrent epistaxis, mucocutanous telangiectases, pulmonary and cerebral AVMs and a familial occurrence.
