진동법에서 가변 특성 비를 이용한 혈압 추정 알고리즘의 개발 (Development of Blood Pressure Estimation Algorithm Using Variable Characteristic Ratios on Oscillometric Method)

  • In this paper, variable characteristic ratio algorithm based on oscillometric method is proposed to enhance the accuracy of blood pressure measurement. We combined the slope-based approach and fuzzy inference technique to change the characteristic ratios of height-based method. The proposed algorithm was assessed on 255 measurements from 85 subjects and compared with the conventional height-based algorithm. The testing results showed that the developed algorithm achieved an overall grade A for both systolic and diastolic blood pressures according to the BHS protocol. And, mean standard deviation between the observers and the developed algorithm were 5.71mmHg and 6.29mmHg for systolic and diastolic pressures respectively, which also fulfilled the AAMI criteria. In conclusion, this algorithm was successfully developed and recommended for further clinical trials with the wider adult population.

임상실습시 간호학생이 지각하는 힘 북돋우기 (empowerment)와 실습 만족도에 관한 연구 (A Study on Relationship between Empowerment and Satisfaction of Clinical Practice in Nursing Students)

  • The purpose of this study was to explain the relationship between empowerment and satisfaction of clinical practice in nursing students. The instrument of this study was a structured questionnaire included empowerment and satisfaction of clinical practice. The data was collected from september 28, 1999 to March 31, 2000 and analyzed by Cronbach alpha, descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient and simple regression by using SPSS Win program. The result of research were as follows: 1. There was significant difference according to the general characteristic factor. The total empowerment was predicted by meaning of nursing, university and college, relationship of peer, clearance of study purpose. Satisfaction of clinical practice was predicted by university and college, clearance of study purpose. 2. There was a significant correlation between empowerment and satisfaction of clinical practice(r=0.442, p=0.000). 3. Empowerment was the highest factor predicting satisfaction of clinical practice. A total of 19.5% of the variance was explained in the total satisfaction of clinical practice. It was found that the higher the degree of perceived empowerment, the higher the degree of satisfaction of clinical practice. Therefore, nursing faculty should plan interventions to inspire satisfaction level of clinical practice by empowerment.

시뮬레이션을 이용한 임상자료의 샘플링 방법 연구 (Study for the sampling method using simulation in clinical data)

  • 여러 분야에서 자료를 수집하기 위하여 모집단에서 표본을 추출하는 표본설계를 하고 있다. 특히 실험군과 대조군이 존재하는 임상자료에서는 모집단에서 집단별 일반적 변수들의 특성이 표본에 잘 반영되어야 하므로 더욱더 표본설계가 중요한 문제이다. 즉 모집단에서 집단별로 일반적 변수들이 가지는 빈도, 중심척도 그리고 산포척도 등이 표본에서도 동일하게 나타나야 한다. 그러나 주로 이루어지는 표본설계는 매우 복잡하고 어려워 일반 연구자가 사용하는데 있어 어려움을 겪는다. 따라서 본 논문에서는 시뮬레이션을 이용하여 집단별로 일반적 특성을 유지할 수 있는 임상자료의 샘플링 방법을 연구하였다. 또한 중환자실에 있는 환자자료에 적용하여 모집단과 표본의 일반적 변수의 특성값을 계산하여 보았고 통계적 가설검정을 이용하여 모집단과 표본집단에서 일반적 변수들의 값의 차이 여부를 비교하여 보았다.

네트워크 분석을 이용한 불면의 무작위임상시험 해외 연구 동향 (Research Trends of Randomized Clinical Trial for Insomnia Using the Network Analysis)

  • 본 연구는 불면과 관련된 무작위임상시험의 해외 연구 동향을 살펴보기 위하여 시계열적 키워드 비교 분석 및 네트워크 분석을 실시하였다. 분석에 사용된 자료는 'Insomnia'와 'Randomized Clinical Trial'의 연관 키워드를 사용하여 ClinicalTrials의 379건, Web of Science의 132건을 선정하였다. 분석 결과, ClinicalTrials에서는 불면과 약물요법을 중심으로 인지행동요법, 우울 등의 키워드가 네트워크를 이루고 있었다. WOS에서는 ClinicalTrials의 결과에 멜라토닌 키워드가 추가적으로 나타났다. 또한, 연구 대상자 중 특징적으로 노인과 여성에 대한 연구가 꾸준히 진행되는 것으로 파악되었다.

An Osteolytic Meningioma en Plaque of the Sphenoid Ridge

  • Meningioma en plaque (MEP) is a rare tumor characterized more by its clinical and biological behavior than its histological appearance. Hyperostosis of the skull is one of the characteristic signs of MEP. This bony change can produce clinical symptoms and signs in MEP by pressing against adjacent structures. The authors report a rare case of an osteolytic MEP extending from the sphenoid wing into the orbital wall, middle fossa, and temporalis muscle.

구내 표준 방사선사진을 이용한 구내방사선촬영기의 선질 평가 (Evaluation of the Beam Quality of Intraoral X-ray Equipments Using Intraoral Standard Films)

  • Purpose: This study was to evaluate the beam quality of intraoral X-ray equipments used at Yonsei University Dental Hospital (YUDH) using the half value layer (HVL) and the characteristic curve of intraoral standard X-ray film. Materials and Methods : The study was done using the intraoral X -ray equipments used at each clinical department at YUDH. Aluminum filter was used to determine the HVL. Intraoral standard film was used to get the characteristic curve of each intraoral X-ray equipment. Results: Most of the HVLs of intraoral X-ray equipments were higher than the least recommended thickness, but the REX 601 model used at the operative dentistry department and the X-707 model used at the pediatric dentistry department had HVLs lower than the recommended thickness. The slopes of the characteristic curves of films taken using the PANP AS 601 model and REX 601 model at operative dentistry department, the X-70S model of prosthodontic dentistry department, and the REX 601 model at the student clinic were relatively low. Conclusion: HVL and the characteristic curve of X-ray film can be used to evaluate the beam quality of intraoral X-ray equipment. In order to get the best X-ray films with the least radiation exposure to patients and best diagnostic information in clinical dentistry, X -ray equipment should be managed in the planned and organized fashion.

역류성 식도질환 환자의 생활습관 및 식습관 조사연구 (Survey of Life and Dietary Styles on Patients with Gastroesophageal Reflux Disease)

  • Objective: It was surveyed how to help patients get better treatment for their disease by making complements based on the survey outcomes when the pharmacists guide how to administer medicine. Methods: A total of 142 gastroesophageal reflux disease (GERD) patients answered the self-answering questions consist of 15 items about their general characteristic, lifestyle and dietary style. The frequency of survey materials was analyzed to find out specific figures of surveyed patient's general characteristic, lifestyle and eating habits. Results: Based on the outcomes on analysis there was no difference between female and male patients. The characteristic based on the age group, as ones grow older, the number of GERD patients also increased. In case on the characteristic in lifestyle, many patients had improper habit to treat GERD. The items were smoking habits, stress control habit, sleeping postures and the way in wearing outfits. In case of postures after having a meal, they had right lifestyle. Patients had improper habit in eating food. They enjoyed pungent food and drinks which can hinder the treatment. In case of the time of having meal, eating habit of meat and vegetables and eating food less than an hour before go to bed, this research showed that they have the right lifestyle. Conclusion: This research indicate that pharmacists give guidance GERD patients to stop smoking that is causative of GERD and guide patients have mental stability, and patients avoid pungent food and drinks such as coffee, soda, chocolates and mint candies.

Molecular Genetic Characteristics of Methicillin-Resistant Staphylococcus aureus Isolated from University Campus Environment and Students

  • Staphylococcus aureus (S. aureus) is known as a bacterium that can cause skin infections, respiratory system infections, and sinusitis; however, it can exist as a normal flora rather than a pathogen. Recently, methicillin-resistant S. aureus (MRSA) infections have emerged in the community as a new variant of community-associated (CA)-MRSA. In the present study, S. aureus and MRSA were isolated and cultured by collecting samples from facilities and environments where students and educational personnel have multiple contacts on university campuses; specifically, the nostrils and hands of college students were tested from July to September of 2019. The molecular properties of the isolated MRSA were analyzed, and the one MRSA strain was isolated from the university campuses. One MRSA that was isolated and cultured on campus was the mec complex group A and staphylococcal cassette chromosome (SCC) mec type II, which is a characteristic of healthcare-associated (HA)-MRSA, and SCCmec type V, which is a characteristic of CA-MRSA. This result was similar to other studies wherein the SCCmec type II was detected in SCCmec typing analysis in CA-MRSA. To confirm whether there is a new variant of CA-MRSA in the Republic of Korea, additional follow-up studies on the analysis of virulence factors of MRSA are needed by additionally separating CA-MRSA from the body parts of university students and educational personnel.

사기질모세포종의 세침흡인 세포학적 소견 - 1예 보고- (Fine Needle Aspiration Cytology of Ameloblastoma - A Case Report-)

  • Utilization of fine needle aspiration cytology (FNAC) in the diagnosis of odontogenic tumors seems to have little attention. Ameloblastoma is the most common epithelial odontogenic tumor. However, the cytologic findings of ameloblastoma are described recently. We report a case of ameloblastoma diagnosed by FNAC. The aspirated blood-tinged mucoid fluid from maxillary mass showed a few clusters of tumor cells with distinct two-cell population: basaloid cells with palisading arrangement and squamous cells with more abundant and eosinophilic cytoplasm. Nuclear atypism, pleomorphism or mitosis were absent. A few keratinized bodies in the tumor cell clusters and the third component, stellate cells were also characteristic.

유전성 암과 유전상담 (Hereditary cancer and genetic counseling)

  • Hereditary syndromes cause approximately 5 to 10% of overall cancer cases. Cancer related with genetic syndromes are found elsewhere, including stomach, breast, colorectum, ovary, brain and so on. Because hereditary cancers are due to germline mutations, these patients have unique clinical features distinct from sporadic cancer. Generally these features include (i) early age-of onset of cancer, (ii) frequent association with synchronous or metachronous tumors, (iii) frequent bilateral involvement in paired organs (iv) frequent association with other site tumors or characteristic clinical manifestation specific to each genetic syndrome. Due to these differences, the management strategy for patients with hereditary cancer is quite different from that for sporadic cancer. Additionally, there are important screening and surveillance implications for family members. Genetic counselling is prerequisite to these families for risk assessment by pedigree analysis, and guidance to clinical or genetic testing. The genes responsible for these syndromes has recently identified, as a result, genetic testing has become important determining factor in clinical decisions.
