ACCURACY OF TRI AUTO ZX® IN LOCATING APICAL FORAMEN WITH ROTARY FILE (전동 file을 장착한 Tri Auto ZX®의 치근단공 인지 정확도 평가)

  • The purpose of this experiment was to determine: (1) the safe automatic apical reverse setting that prevents overinstrumentation of the root canal, using Tri Auto ZX$^{(R)}$ and (2) the effect of various irrigant on such instrumentation. The instrumentation was carried out with the automatic apical reverse setting of 0.5, 1.0, 1.5, and 2.0. The root canal irrigants used in usual manner were normal saline(0.9%), NaOCl(2.5%), and RC Prep$^{(R)}$. For each reverse setting and each irrigant, ten teeth were used with the total of 120 teeth. The distance between the file tip and the apical constriction was determined by stereomicroscope using the point that the file began to rotate in reverse direction. When the reverse setting mode was set to 0.5, 18 of 30 were overinstrumented. If these were discriminated by irrigant, 10 of 6 with 0.9% saline, 10 of 6 with NaOCl, and 10 of 6 with RC Prep$^{(R)}$ has the file tip located 0.57${\pm}$0.30mm, 0.73${\pm}$0.39mm, and 0.26${\pm}$0.25mm beyond the apical constriction respectively. In 1.0 setting 15 of 29 were over the apical constriction, and the distribution was 6 in saline, 5 in NaOCl, and 4 in RC Prep$^{(R)}$. The mean distance over the apical constriction was 0.28${\pm}$0.13mm with saline, 0.75${\pm}$0.61mm with NaOCl, and 0.25${\pm}$0.17mm with RC Prep$^{(R)}$. When the autoatic reverse mode was set to 1.5, and 2.0, 5, and 1 teeth were found to be overinstrumented in respective settings. But there were large variations in overinstrumented distances when an attempt was made to compare the effect of irrigants on this overinstrumentations and they were meaningless for the small sample size. When all of the autoreverse setting were combined to compare the number of overinstrumented teeth with each irrigant, there were no significant differences (14 for normal saline, 12 for NaOCl, 13 for RC Prep$^{(R)}$). When 0.5 or 1.0 automatic apical reverse setting mode was used the Tri Auto ZX$^{(R)}$ in clinical application, the possibility of overinstrumentation beyond the apical constriction exists in 55.9% of cases. Therefore 1.5 or 2.0 setting is safer for the preparation inside the canal but this type setting needs additional apical hand preparation of the root canal because the accuracy is lower than 0.5 or 1.0 setting.

Contributions of CYP2C9/CYP2C19 Genotypes and Drug Interaction to the Phenytoin Treatment in the Korean Epileptic Patients in the Clinical Setting

  • We examined the contribution of CYP2C9 and CYP2C19 genotypes and drug interactions to the phenytoin metabolism among 97 Korean epileptic patients to determine if pharmacogenetic testing could be utilized in routine clinical practice. The CYP2C9 polymorphism is a wellknown major genetic factor responsible for phenytoin metabolism. The CYP219 polymorphism, with a high incidence of variant alleles, has a minor influence on phenytoin treated Koran patients. Using a multiple regression model for evaluation of the CYP2C9 and CYP2C19 genotypes, together with other non-genetic variables, we explained 39.6% of the variance in serum phenytoin levels. Incorporation of genotyping for CYP2C9 and CYP2C19 into a clinical practice may be of some help in the determination of phenytoin dosage. However, because concurrent drug treatment is common in patients taking phenytoin and many environmental factors are likely to play a role in drug metabolism, these factors may overwhelm the relevance of CYP polymorphisms in the clinical setting. Further investigations with an approach to dose assessment that includes comprehensive interpretation of both pharmacogenetic and pharmacokinetic data along with understanding of the mechanism of drug interactions in dosage adjustment is warranted.

The Experiences of Nursing Student on Alcoholism Care (간호대학생의 알코올중독 환자간호 실습경험에 관한 연구)

  • Purpose: The aim of this study was to describe the experiences of student nurses during their clinical practice periods in an alcoholic ward. Methods: A descriptive phenomenological study design was employed. Individual interviews were conducted and audio-taped. Colaizzi's method of data analysis was applied to inductively determine themes and formulated meanings. Data saturation was achieved and methodological rigor was established. Results: Four themes were identified and these themes included anxiety of nursing practice in unfamiliar setting, sympathy with patients, learning about the inpatient alcoholics care, and self-reflection. Conclusion: Nursing students had positive and negative experiences in their clinical practice. The results of this study had important implications regarding clinical practice and research for nursing clinical education.

Correlations Between Mother's Personality Characteristics and Children's Diagnostic/Clinical Pathology (어머니의 인성특성과 아동의 병리적 특성 및 진단유형의 연관성)

  • This research explored links between mother/child personality characteristics based on their diagnostic and clinical pathology. The 232 participants, selected from mother-child pairs receiving psychological care in a hospital setting, were tested and interviewed by clinical practitioners. Correlations were found between mothers' Psychopathic Deviate, Paranoia, Psychasthenia, and Schizophrenia and sons' Psychasthenia, aggression, alienation, depression, and anger. Mothers classified with Hysteria were likely to have aggressive children. Mothers with low self-esteem, repression, frustration, and strong levels of extroversion had daughters with anger/rage issues. Sons of mothers diagnosed with Hypochondriasis, Depression, Hysteria, Psychopathic Deviate, Paranoia, and Psychasthenia had high rates of Oppositional Defiant Disorder and ADHD. Children of mothers diagnosed with depression had high rates of ADHD. Specifically, mothers with Hypochondriasis, Hysteria, and Psychopathic Deviate had daughters with high rates of ADHD.

New Antimicrobial Agents for Children (소아청소년 영역에서의 새로운 항미생물제)

  • There are relatively few novel antimicrobial agents despite the dramatic increase in antimicrobial resistance and multiple drug resistance of clinical isolates worldwide. Vancomycin is still the most widely used antibiotic for treating resistant Gram-positive coccal infections in children, especially for methicillin-resistant Staphylococcus aureus. For children with Gram-positive coccal infections where vancomycin is not effective or older therapeutic agents cannot be tolerated, linezolid, quinupristin-dalfopristin or daptomycin may be useful in the appropriate clinical setting. For Gram-negative bacterial infections, new carbapenems await clinical application. Tebipenem pivoxil is a novel oral carbapenem undergoing clinical trials for acute otitis media in pediatric patients. Antiviral drug development is now progressing at the pace of antibiotic development 30 years ago. Newer antiviral agents used for the treatment of herpes viruses and hepatitis C virus infections in children are included in this review.

Augmented renal clearance

  • Adding to the complexity of caring for critically ill patients is the fact that many of them have a creatinine clearance that exceeds $130mL/min/1.73m^2$. This phenomenon, termed augmented renal clearance (ARC), has only recently been widely recognized and its pathogenesis remains incompletely understood. However, ARC has been shown to result in increased dose requirements for drugs that are primarily eliminated by renal excretion, including many antimicrobial agents and enoxaparin. Recognition of ARC is hampered by the fact that the standard creatinine-based equations used to estimate renal function are not accurate in this clinical setting and the diagnosis is best established using both serum and urine creatinine measurements to calculate clearance. So a high index of clinical suspicion and awareness is usually required before this step is taken to confirm the diagnosis of ARC.

  • Human papillomavirus is a virus that is distributed worldwide, and persistent infection with high-risk genotypes (HR-HPV) is considered the most important factor for the development of squamous cell cervical carcinoma (SCC). However, by itself, it is not sufficient, and other factors may contribute to the onset and progression of lesions. For example, infection with other sexually transmitted diseases such as human immunodeficiency virus (HIV) may be a factor. Previous studies have shown the relationship between HPV infection and SCC development among HIV-infected women in many regions of the world, with great emphasis on low- and middle-income countries (LMICs). Brazil is considered a LMIC and has great disparities across different regions. The purpose of this review was to highlight the current knowledge about HPV infection and cervical abnormalities in HIV+ women in Brazil because this country is an ideal setting to evaluate HIV impact on SCC development and serves as model of LMICs and low-resource settings.

Effects of Case-Based Learning on Clinical Decision Making and Nursing Performance in Undergraduate Nursing Students (사례기반학습이 간호대학생의 임상 의사결정 능력과 간호수행 능력에 미치는 효과)

  • Purpose: The aim of this study was to examine the effects of case-based learning (CBL) on clinical decision making and nursing performance. Methods: This research was conducted between September, 2011 and January, 2012 as a nonequivalent comparison group design. The participants were 55 third year nursing students who were enrolled in a college of nursing in a university in Korea. The intervention was the CBL procedures which involved role-play practice videoed by camera and watched on the computer by the students. Questionnaires were used before and after the intervention to measure clinical decision-making. Nursing performance tests were done after the intervention. Results: Statistically significant group differences were observed in clinical decision-making. Nursing performance was significantly higher in the CBL group than in the control group. Conclusion: CBL focused on the solving problem process and clinical cases which are based on clinical setting allowing students to develop efficiency in clinical practice and adaptation to the clinical situation.

The Observational Experience of Labor and Delivery by Student Nurses in the Clinical Setting (간호학생의 분만관찰경험)

  • The purpose of this study was to use phenomenological perspectives to identify the meaning and structure of the observational experience of labor and delivery by student nurses in the clinical setting. The participants were 115 junior and senior students from Christian College of Nursing in Kwang Ju. The students were asked to write about their observational experience in the delivery room. Colaizzi's method was used for phenomenological analysis. The investigator analysed the data to identify and categorize themes and basic structural elements. Twelve themes and eight basic structure were identified. The process of the observational experience in labor and delivery has three proposed phases : anticipatory, observational and post-observational. In the anticipatory phase, nursing students were related only to the event of anticipated labor and delivery of clients. Structure 1, feeling of expectation and fear, was identified. In the observational phase, students were related to the labor and delivery situation, a woman delivered and a newborn. Structure 2, shock and suffering, was identified in relation to the labor and delivery situation. Structure 3, need for caring and satisfaction, was identified in relation to the woman delivered. Structure 4, feeling of mystery and joy, was identified in relation to the newborn. In the post-observational phase, relations between students and the life of a human being, students and self in the future, students and their mothers and students and women and God appeared. And in these relations 〈structure 5, valuable〉, 〈structure 6, feeling of expectation and fear〉, 〈structure 7, love and appreciation〉 and 〈structure 8, greatness〉 were identified.

Content Validity of a Korean-Translated Version of a Fullerton Advanced Balance Scale: A Pilot Study

  • The purpose of this study were to translate the Fullerton Advanced Balance (FAB) scale into Korean and to verify the content validity by utilizing a back-translation method with a view to assessing balance function and the risk of falling in a clinical research setting. This research was conducted in six steps. First, three Korean physical therapists translated the FAB scale into Korean. Second, two bilingual professors of physical therapy and a physical therapist evaluated translation conformity of Korean-translated FAB scale. In the third and fourth steps, twelve physical therapists evaluated the degree of translation comprehension, and a translator back-translated the Korean FAB scale into the original language. Fifth, a bilingual professor of physical therapy and two native speakers evaluated the technical and conceptual equivalence between the original and translation versions. In this process, inappropriate translated items were revised using recommended substitute words or sentences, and all items were evaluated on the basis of three points or more on a rating scale in terms of translation comprehension, and the technical and conceptual equivalence of the back-translation. In the sixth and last step, the translation verification committee completed the final Korean version. The above process indicated that the content validity of the Korean-translated FAB scale was established by means of systematic translation methods, and it can therefore be used to assess balance function and the risk of falls in a clinical research setting.
