아동병동 간호사의 임상의사결정 능력이 직무만족도에 미치는 효과: 아동병동 간호사-환아 부모 간 파트너십 매개효과를 중심으로 (Effects of Clinical Decision-making on Job Satisfaction among Pediatric Nurses: The Mediating Effect of the Nurse-Parent Partnership)

  • Purpose: The purpose of this descriptive survey study was to characterize the relationship between clinical decision-making and job satisfaction among pediatric nurses and to elucidate the mediating effects of the nurse-parent partnership on that relationship. Methods: The subjects of the study were 174 nurses who had worked in a pediatric ward in a university hospital, general hospital, or children's hospital. Data were collected from June 20, 2016 to August 10, 2016 and analysed using descriptive statistics, the t-test, analysis of variance, the Pearson correlation coefficient, and three-step mediated regression analysis in SPSS version 22.0 for Windows. Results: The nurse-parent partnership had significant effects on clinical decision-making and job satisfaction, with an explanatory power of 19% and 26%, respectively. The nurse-parent partnership had a partial mediating effect on the relationship between clinical decision-making and job satisfaction (Sobel test: Z=4.31, p<.001). Conclusion: The nurse-parent partnership had a partial mediating effect on the relationship between clinical decision-making and job satisfaction among pediatric nurses. Therefore, in order to improve the job satisfaction of pediatric nurses, it is necessary to develop effective educational programs and strategies to address their clinical decision-making and their experiences of the nurse-parent partnership.

장애인에 대한 임상간호사의 옹호간호 영향요인 (Factors influencing clinical nurses' advocacy for people with disability)

  • Purpose: Clinical nurses are the ideal health care providers to advocate for vulnerable and underserved populations such as people with disability. This study aimed to understand factors influencing nursing advocacy for people with disability among clinical nurses. Methods: The subjects of this study were 186 clinical nurses who were working in three hospitals in B and D cities. Self-report questionnaires were used to collect the data. Data were analyzed using IBM SPSS Statistics 25 with descriptive statistics, t-test, one-way ANOVA, Pearson's correlation coefficients, and multiple regression analysis. Results: Clinical experience and esthetical nursing competency have positive correlations with nursing advocacy, and optimism-human rights has a negative correlation with nursing advocacy. Factors influencing nursing advocacy for people with disability were identified as patient directivity (β=0.36, p=.001), optimism-human rights (β=-0.18, p=.008) and clinical experience (≥10) (β=0.14, p=.036). The final model consisting of these factors explained 19% of the variance of nursing advocacy (F=14.99, p=.001). Conclusion: The findings of this study indicate the importance of developing and implementing nursing interventions that can improve patient directivity and optimism-human rights toward people with disability among clinical nurses. These nursing advocacy interventions can be provided as part of continuing education as well as the nursing curriculum.

임상간호사의 아동학대 신고 지식, 아동학대 인식 및 도덕적 민감성이 아동학대 신고 태도에 미치는 영향 (Influencing Factors of Clinical Nurses' Knowledge of Child Abuse Reporting, Perception of Child Abuse, and Moral Sensitivity on the Attitude toward Reporting Child Abuse)

  • Purpose: The purpose of this study was to identify the factors affecting clinical nurses' attitude toward reporting child abuse. Methods: The participants in this study were 200 clinical nurses. Data were collected as structured self-report questionnaires through the online portal site for nurses from November 24 to December 7, 2021. The questionnaires included general characteristics, knowledge of child abuse reporting, perception of child abuse, moral sensitivity, and attitude toward reporting child abuse. The SPSS/WIN 25.0 program was used for data analysis which included descriptive analysis, t-test, ANOVA, Scheffé test, Pearson correlation coefficients, and multiple linear regression. Results: As knowledge of child abuse reporting, perception of child abuse and moral sensitivity were increased, the attitude toward reporting child abuse was significantly increased. Multiple regression analysis showed that knowledge of child abuse reporting (β=.32) and perception of child abuse (β=.21) were significant influencing factors of attitude toward reporting child abuse. Conclusion: These findings implied that knowledge of child abuse reporting and perception of child abuse would be related to attitudes toward reporting child abuse among clinical nurses. Therefore, it is necessary to develop education programs and public policies to improve the knowledge and perception of child abuse reporting among clinical nurses so that attitudes toward reporting child abuse can be improved.

이직을 고려해 본 임상간호사의 상처와 용서 경험 (A Phenomenological Study on the Experience of Hurt and Forgiveness of Clinical Nurses in Korea after Loss of Employment)

  • Purpose: The purpose of this study was to understand and analyze the experience of hurt and forgiveness of clinical nurses in Korea. Methods: A phenomenological research method guided data collection and analysis. The subjects were a total of 5 nurses who had experienced hurt and forgiveness. Data was collected through individual in-depth interviews. All interviews were audio taped and transcribed verbatim. Coding was used to establish different concepts and categories. Results: The following three common constituents have been found as a retrospective focus based on the primary hurt related to the clinical situation; recognizing their personal values, obtaining their view about forgiveness, as well as their view of self. Conclusion: The results of this study may contribute to health professionals working in various clinical settings to understand Korean nurses with hurt and forgiveness experiences.

신생아중환자실 간호사가 인식한 간호업무의 중요도와 수행 자신감 (Analysis of the importance of nursing care and performance confidence perceived by nurses in the neonatal intensive care unit)

  • Purpose: Neonatal nurses are expected to have clinical competency to provide qualified and safe care for high-risk infants. An educational intervention to enhance nurses' clinical competence is often a priority in the nursing field. This study was conducted to explore nurses' perceived importance and performance confidence of nursing care activities in neonatal intensive care units. Methods: One hundred forty-one neonatal nurses from seven hospitals across South Korea participated in the online survey study. The scale of neonatal nursing care activity consisted of 8 subdomains including professional practice (assessment, diagnosis, planning, intervention, evaluation, education, research, and leadership). The Importance-Performance Matrix was used to analyze the importance of and confident performance in each of the nursing subdomains. Results: Both importance and performance confidence increased as nurses' age (p=.042 and p<.001) and clinical experience (p=.004 and p<.001). Participants scored relatively higher in importance and performance confidence in the professional practice subdomains (assessment, intervention, evaluation), but scored lower in the education and research subdomains. Conclusion: To provide evidence-based nursing care for high-risk infants in neonatal intensive care units, educational interventions should be developed to support nurses based on the findings of the research.

  • 본 연구는 임상간호사의 바람직한 간호 인성의 본질과 내용을 확인하여 임상간호사로서 간호역량 확보와 양질의 간호 제공을 위한 기초 자료를 제공하고자 한다. 임상 간호현장에서 기대하는 바람직한 간호사의 인성에 대해 포괄적으로 탐구하기 위하여 포커스그룹 인터뷰 분석을 시행한 질적 연구이다. 2019년 11월 30일~ 2020년 1월 20일까지 경력 5년 이상 D지역 대학병원의 경력 간호사 6명을 대상으로 총 4회 진행하였으며, 내용 분석 방법을 통해 분석하였다. 임상 간호사에게 요구되는 인성은 '환자 간호를 위한 자기 통제와 노력의 과정', '인간 중심의 간호를 위한 구성요인', '건강한 인간관계를 만드는 영향력', 간호'조직을 구성하고 운영케 하는 영향력', '조직의 변화와 성과를 이끌어내는 영향력' 총 5개의 주제와 17개의 하위 주제로 분석되었다. 본 연구를 통해 간호현장에서 바람직한 인성의 요소를 확인하였으며, 추후 전문직 간호역량 확보와 양질의 간호 제공을 위한 간호사 대상의 임상간호 인성 교육 프로그램 개발 연구를 제언한다.

중환자실 간호사를 위한 자기주장훈련의 효과 (Effects of Assertiveness Training for ICU Nurses)

  • Purpose: This study was done to examine the effects of assertiveness training on Intensive Care Unit (ICU) nurses' assertive behavior, job stress, communication conflict, and self-esteem toward improving their communication skills and coping. Methods: The assertiveness training program was developed to a 10-hour program consisting of 3 sessions. The goals were to understand assertive behavior and learn how to practice assertive communication in general conflict situations and/or a difficult conflict situation in the ICU. Participants were 65 nurses recruited from a tertiary teaching hospital and of these, 27 nurses completed the program. Data were collected from Jan. 30th to Mar. 28th, 2009 using a questionnaire which included demographic characteristics, assertive behaviour scale, job stress scale, communication conflict inventory-specific, and self-esteem scale. Data were analyzed using SPSS v. 14.0 for $x^2$ test and t-test. Results: ICU nurses in the experimental group had a significant increase in aspects of assertive behavior and a decrease in job stress. Conclusion: The results of the study indicate that assertiveness training is effective in increasing ICU nurses' assertive behavior and decreasing job stress through reinforcement of assertiveness behavior.

임상간호사의 돌봄대상자로부터 언어폭력 경험 (Clinical Nurses's Experience of Verbal Violence by Care Recipients)

  • Purpose: The purpose of this study are to understand the meaning and essential structure of clinical nurses' experiences of verbal violence and find nursing intervention for them by applying phenomenological research method. Methods: The participants in this study were seven clinical nurses with experience of verbal violence. Data collection were conducted from August 2016 through January 2017 until data saturation. Data analysis was performed through a cyclical process of data collection and analysis by applying Giorgi's phenomenological method. Results: Four categories were identified: A weapon of the mind: the shock of verbal abuse; negative emotions that shake my heart; skepticism about the value and meaning of a job; an unforgettable wound. Conclusions: The results of this study provide a deeper understanding of the pain caused by verbal violence experienced by nurses from their care recipients. These results suggest that nursing interventions for nurses with experience of verbal violence will contribute to resolving the problem.

  • Purpose: This study aims to find the sources of nurses' conflicts and to find ways to eliminate them for improved nursing care. Method: This study is based on a phenomenological approach. All participants were woking at K-hospital, located in Seoul. The research was performed from September 2000 to February 2001. Data was collected through interviews and observations while participants were working. Interviews lasted a duration of one and half hours and data collection accured five to six times per participant. The data were analysed by Giorgi method and the results are as follows: Result: Essential themes in the professional conflict of clinical nurses emerged (1) The discords of human relationships ; (2) Dissatisfaction with working conditions ; (3) Lack of self-respect in one's professional expertise ; (4) Dissatisfaction with one's work ; (5) Depression accurring due to one's ability as a professional. Conclusion: The foregoing arguments suggest that nurses perform stressful environments in a hospital originating from the relationship among peer group, working conditions, and lack of self-respect as a nurses. Therefore, hospital authorities should strangly consider working conditions, interpersonal relationships, and working conditions in order to promote self-respect of the nurses hospitals.

임상간호사의 그릿, 직무만족도와 재직의도와의 관계 (Relationships among Grit, Job Satisfaction, and the Intention to Stay of Clinical Nurses in Korea)

  • Purpose: This descriptive study aimed to identify the relationships among grit, job satisfaction, and the intention to stay of clinical nurses in Korea. Method: Data collected from a total of 239 experienced nurses in four general hospitals of the Jeolla province. The t-test, ANOVA, and Pearson's Correlation Coefficient were performed using the IBM SPSS/WIN 21. Results: There were positive correlations between the grit and job satisfaction of the nurses (r=.31, p<.001), between their grit and intention to stay (r=.25, p<.001), and between their job satisfaction and intention to stay (r=.49, p<.001). Conclusion: Developing and adapting educational programs to enhance the grit enthusiasm, and patience can contribute to increasing the intention to stay of nurses in hospitals.
