집중치료실 간호사의 진정요법 지식과 간호실무 역량요인의 상관성 분석 (Relationships between Sedation Knowledge and Nurse Practice Competences in Intensive Care Unit)

  • Purpose: For this study an examination was done of relationships between intensive care unit (ICU) nurses' knowledge related to sedation and their clinical competencies in sedation practice. Methods: Ninety one ICU nurses were recruited from two tertiary hospitals in South Korea. A self-report questionnaire was used to examine the levels of knowledge related to sedation and nursing practice competence based on Ajzen's theory of planned behavior. Descriptive statistics, independent t-tests, one-way ANOVA, and Pearson correlations were performed using the IBM SPSS 21.0 Results: ICU nurses are more knowledgeable about general information on sedation rather than up-to-date information. Continuing education on sedation was related to difference in knowledge levels. However, the levels of knowledge were not related to competence in sedation practice. Instead, a positive attitude toward sedation practice was significantly related to the subjective norms; orders and goals, perceived behavioral control, practice of sedation, and intention to use. Conclusion: The results of this study provide fundamental information on levels of knowledge related to sedation practices of Korean ICU nurses. Continuing education should emphasize up-to-date information on sedation practice and need for positive attitudes which influence all other competencies towards sedation practices to achieve optical care of sedation in the ICU.

임상간호사의 이직의도에 영향을 미치는 요인: 정서사건, 정서, 직무만족 (Factors Influencing Turnover Intention in Clinical Nurses: Affective Events, Affect, and Job Satisfaction)

  • 본 연구는 정서사건과 정서, 직무만족이 임상간호사의 이직의도에 미치는 영향을 확인하고 이직의도를 감소시키기 위한 중재 및 전략에 대한 지침을 제공하기 위하여 수행되었다. 정서사건과 정서, 직무만족, 이직의도의 관계는 경로분석을 활용하여 검증하였다. 총 296명의 임상간호사를 대상으로 구조화된 자기보고식 설문지를 통해 수집하였고, 수집된 자료는 SPSS Statistics 24.0과 AMOS 24.0 프로그램을 활용하여 분석하였다. 최종 경로모형은 권장 적합도 수준에 적합하였고, 경로분석 결과, 긍정과 부정사건, 긍정과 부정정서, 직무만족은 이직의도에 통계적으로 유의한 영향을 미쳤으며, 이 변수들의 설명력은 37.0%이었다. 본 연구는 임상간호사의 이직의도에 미치는 영향요인을 확인하였으며, 임상간호사의 이직의도를 감소시키기 위한 전략수립에 기여할 것이다.

응급실 간호사의 폭력경험, 소진 및 직무만족도간의 관련성 (Relationship of the Experience of Violence to Burnout and Job Satisfaction in Emergency Department Nurses)

  • Purpose: This study was done to identify the relationship between experiences of violence, and burnout and job satisfaction of emergency department (ED) nurses and to provide basic information to improve quality of life for nurses and quality of nursing care by increasing the ability of nurses to deal with violent situations and thus, improve job satisfaction. Method: Participants (n=153) were recruited in S and B cities from October 16 to December 22, 2007. Descriptive statistics, t-test, ANOVA, and Pearson correlation coefficient were used to analyze the data. Results: Level for ED nurse burnout was 2.54 out of a possible 4, and for ED nurse job satisfaction, 2.81 out of a possible 5. There were significant differences according to sex (t=-3.217, p=.002), total clinical career (F=5.805, p<.05), and clinical career in the ED (F=1.640, p<.01). There was a significant positive correlation between violence experience and burnout (r=.500, p<.001), and significant negative correlations between violence experience and job satisfaction (r=-.300, p<.001), and between burnout and job satisfaction (r=-.492, p<.001). Conclusion: The findings from this study indicate that experiences of violence in the ED decrease work efficiency of the nurses and increase burnout. So, it is important to reduce occurrences of violence in ED and to prepare programs for dealing with violence and providing emotional support in cases of violence directed at nurses.

임상간호사가 지각하는 업무 관련자와의 감정노동 (Perceived Emotional Labor of Clinical Nurse from the Persons Concerned)

  • Purpose: This study aimed to understand the degree of clinical nurses' emotional labor that they perceive in the relationships with people related to their work. Methods: This study was a descriptive research, and its subjects were 167 nurses in five university hospitals located in B and Y city. The degree of emotional labor was measured with Visual Analogue Scale (VAS). Data were collected from April to May in 2015, and analyzed by descriptive statistics, t-test, ANOVA, $Scheff{\acute{e}}$ and Games-Howell examination. Results: The score of the emotional labor of health care providers and visitors were higher than non-health care providers. Among health care providers, doctors had the highest emotional scores, and fellow nurse and nurses other departments were followed after that. Among visitors in hospital, patients and their family had the higher emotional labor scores than care givers. There was statistically significant difference in the emotional labor, according to the subjects' educational level, age, wages, job position, and working departments. Conclusion: Clinical nurses perceived emotional labor not only from the interaction with patients, but also from the interaction with the various other persons concerned such as health care providers, non-health care providers and visitors.

전문간호사 간호행위에 관한 간호수가체계 개발 (Development of Nursing Costs by Nursing Activities in Clinical Nurse Specialist)

  • Purpose: This study was carried out to deveop nursing costs and to substantiate the application of appropriate activity-based nursing costs for the current clinical nurse specialists. Method: The study sample was a group of 8 home health care team of the tirtiary Hospital in Korea. The data was collected from September 2003 through December 2004. The statistical analysis was done by SPSS PC 11.0 program and calculated mean and the standard deviation. In Lee(2003)'s nursing activity analysis, nursing activities were classified into two major classification, 19 domains and 70 activities. Each activities was calculated using its work validity, physical effort, psychological effort, stress as a intensity, and the necessary time. The simple work costs was calculated the work wage per minute and the necessary time. The work load intensity was calculated using the work wage per minute and the necessary time work load intensity/100. Results: In this study, the work wage was 283 won per minute. The work validity ranged from 2.71-4.00. The highest simple work cost/work load intensity cost was 12,735won/47,374won for research activity, 10,700won/27,499won for bedsore care, and 9,727won/35,114won for deathbed care. The lowest simple work cost/work load intensity cost was 2,123won/2,038won for intramuscular injection, 2,210won/2,166won for hypodermic injection, and 2,210won/1,547won for a application of medicine(or ointment or cream). Conclusions: It revealed that the nursing cost should be considered validity, physical effort, psychological effort, stress as a intensity, and necessary time. Therefore, It is necessary to calculate nursing cost systematically based on activities.

이직을 고려해 본 임상간호사의 상처와 용서 경험 (A Phenomenological Study on the Experience of Hurt and Forgiveness of Clinical Nurses in Korea after Loss of Employment)

  • Purpose: The purpose of this study was to understand and analyze the experience of hurt and forgiveness of clinical nurses in Korea. Methods: A phenomenological research method guided data collection and analysis. The subjects were a total of 5 nurses who had experienced hurt and forgiveness. Data was collected through individual in-depth interviews. All interviews were audio taped and transcribed verbatim. Coding was used to establish different concepts and categories. Results: The following three common constituents have been found as a retrospective focus based on the primary hurt related to the clinical situation; recognizing their personal values, obtaining their view about forgiveness, as well as their view of self. Conclusion: The results of this study may contribute to health professionals working in various clinical settings to understand Korean nurses with hurt and forgiveness experiences.

간호사의 정보자원 접근 및 활용정도와 근거중심실무 장애요인 및 역량 (Nurses' Access & Use of Information Resources and Barriers & Competency of Evidence Based Practice)

  • Purpose: The purpose of this study was to explore nurses' access and use of information resources and to identify the barriers and competency to evidence based practice (EBP). This study used descriptive method to identify baseline data for the purpose of developing strategies for establishing EBP in clinical nursing practice. Methods: Participants in this study were 278 nurses from five hospitals in Daegu and Kyungsangbukdo. The data were collected by self administered questionnaires and SPSS/WIN 15.0 program was used to analyze the data with descriptive statistics, t-test, Pearson's correlation coefficients, and ANOVA. Results: Nurses reported $^*most$ frequently using paper and human resources. The mean score of barriers was $3.02{\pm}0.41$, and competency was $2.70{\pm}0.50$. The reported competency was correlated with nurse factors (r=-.31, p<.001) organization factors (r=-.20, p<.001) and research factors (r=-.12, p<.040) as the barriers to evidence based practice. Conclusion: To promote competency in EBP and to decrease the barriers, it would be necessary to develop the organizational culture that encourages nurses to be involved in research activities. In addition, the development of systemic methods to introduce and establish an education program for facilitating EBP in the clinical settings is needed.

실무교육에 대한 간호원의 학습요구 (Learning Needs of Registered Nurse for Insertive Education)

  • The advanced knowledge is increasing day by day due to the progress of medicine and tectonics, the increase of nursing research nowadays. In parallel with these, nursing profession has a need of the nursing care with improvement of new Knowledge, tectonics and nursing procedure through the insertive education. Therefore, this study was attempted to investigate that to grasp learning need of nurses about inservice educational play a pivotal role in the progress of inservice education. This study was made from 242 clinical nurse at 4 hospitals in Seoul and through question- are consisting of 1) ideological part 2) basic sciences 3) nursing part 4) administrative part 5) the others from 10 July '75 to 7 Oct. '75. The data were treated by percentage, Licker method, arid chi- square examination. Significant difference p is 0.05. The results of this studies are as follows : A. learning needs of nurses 1) Generally the learning needs of nurses is very high. 2) Of the learning need of inservice education for nurses, the highest learning need is as follow (Table 2 ) 1 st : Charting 2 nd : The ability of grasping patient's needs and problems. 3 rd ; The contents and the methods of the advanced nursing skill. 4 th ; The importance of team work 5 th ; The general knowledge of the various disease 6 th : The decision of a propriety nursing diagnosis under the general condition. 7 th ; The introduction of the new nursing theory. 8 th : The Nurses role and the responsibility in emergency and disaster. 9 th : The improvement of nursing skill for the perfect interpersonal relationship. l0th ; Cultural education: B . Verification of hypothesis 1. Hypothesis I , learning needs of nurses and educational level of nurse will be correlated, is rejected (Table 3 ) 2. Hypothesis II, learning need of nurses and duties of post will be correlated, is rejected (Table 4) 3. 병원간호사 역할갈등의 빈도와 심각성 (Frequency and Severity of the Nurses' Role Conflict in the Hospital Nurses)

  • Purpose: The purpose of this study was to identify the frequency and severity of role conflict experienced by nurses in the hospital. Methods: For this survey a self-report questionnaire on nurses' role conflict was used for data collection. Participants were 472 nurses in hospitals with over 500 beds. The questionnaire had 82 items classified into five categories (role activity, relationships between: nurse-nurse, nurse-patient/caregiver, nurse-doctor, and nurse-other department staff). The questionnaire was developed through focus group interviews with nurses according on their work experiences and literature reviews that were validated by the researchers. Data were analyzed using descriptive statistics for frequency and severity of nurses' role conflict. Cronbach's ${\alpha}$ for the questionnaire was .95 (frequency), .97 (severity). Results: Mean score for nurses' role conflict was 1.64. The highest frequency for nurses' role conflict was in the category, relationship between nurse and patients or their families ($2.00{\pm}0.46$). The highest severity for nurses' role conflict was in the category; relationship between nurses and doctors ($1.96{\pm}0.56$). Conflict about nurses' role activity showed both high frequency ($1.99{\pm}0.39$) and severity ($1.95{\pm}0.43$). Conclusion: Results indicate a need to develop programs to improve interpersonal relationship so as to reduce role conflict and encourage nurses' professional satisfaction and achievement.
