임상간호사의 보상공정성 지각의 조직몰입 및 직무만족에 미치는 영향 (The Effectiveness Organizaitonal Commitment, Job Satisfaction by Clinical Nurse's Compensation Justice)

  • Purpose: The purpose of this research is to find out how clinical nurse's recognition is effected to nursing organization through compensation justice, and then will support important basic data to management of nursing organization. Methods: Data collection was held through April 1st to 30th in 2003, The Subjects were 375 who were working at 5 hospitals in Seoul and has experiences at least over one year. Result: The average score of nurses' organizational commitment was 3.95 on a 7 point scale, and job satisfaction was 2.80 on a 5 point scale, and distribute justice was 2 on a 5 point scale, and procedural justice was 2.32 on a 5 point scale. We realize the distribute justice of compensation justice showed outstanding difference by age, education back-ground, experience, status of job, religious and types of hospital foundation, comparing the procedural justice only showed the difference by marriage status and type of hospital foundation, Through the study of how compensation-justice effect to organizational commitment, distribute justice never effect instead of procedural justice made effect 30.4% overall transition, as well as procedural justice explain 31.5% of job satisfaction. Conclusion: Finally we have data on the clinical nurse's recognition of compensation justice distribute justice and procedural justice are generally low, job satisfaction and organizational commitment are average. Job satisfaction and organizational commitment are depend upon age and educational level seriously.

간호대학생의 진로탐색과정: 예비간호사의 진짜 간호사 찾아가기 (Career Exploration Process of Nursing Student in Clinical Practice: A Voyage to Find the Authentic Nurse of Pre-registration Nurses)

  • Purpose: The purpose of the study is to understand the experiences of career exploration among nursing students during their clinical practice and to develop a theoretical framework for the career exploration process. Methods: From February 2 to 28, 2015, data were collected through focus group interviews from 24 participating fourth-year nursing students. The data were analyzed using grounded theory. Results: The central phenomenon of 'confusion between the ideal and the reality of nursing' occurred in contexts for various reasons of choosing one's major, nurses in the ideal, a vague fear of the field, arduous nursing assignments in the field, and severe adversity in the nursing field. Nursing students tried to deal with their confusion by using interaction strategies such as pursuing the intrinsic value of nursing, finding role models, trying to project oneself, and preparing for adapting to reality. The career exploration process in clinical practice was a voyage to discover the authentic nurse of the pre-registration nurse. Conclusion: This result shows that nursing students were to gain confidence in their career choices by finding the value of nursing and interacting with patients. Therefore, opportunities for learning experiences about interaction with patients and various positive experiences in clinical practice should be provided for nursing students.

한국형 응급환자 분류도구 및 환자의존도에 따른 환자군별 간호시간과 간호사 배치수준 (Nursing Hours and Nurse Staffing according to Korean Triage Acuity Scale and Patient Dependency)

  • Purpose: The purpose of this study was to analyze nurse staffing according to patients' acuity and dependency by measuring nursing hours. Methods: The study sample included patients who visited the adult emergency departments (EDs) of three tertiary referral hospitals and nurses who worked on shifts for 48 hours from October 24 to 26, 2019. Hourly patient census and nurse staffing were analyzed. Patient acuity was measured using the Korean Triage and Acuity Scale (KTAS), ranging from Level 1 (highest) to Level 5 (lowest). Patient dependency was measured using six items (e.g., clinical attention and communication) and classified into four groups. Nursing activities were observed every 10 minutes and nursing hours per patient and nurse staffing were analyzed according to acuity and dependency. Results: Nurse-to-patient ratio ranged from 1:1.8 to 1:4.2 during the 48 hours of observation. The average work hours of nurses, excluding breaks and meals, was 8.57 hours; 42.5% of which was spent providing direct care. Higher acuity and dependency were associated with higher nursing hours and staffing level. Patients with KTAS Level 1 were provided 74.3 minutes per hour, 5.02 times higher than Level 5 (14.8 minutes). Patients in the highest dependency group were provided 87.4 minutes per hour, 5.75 times higher than the lowest group (15.2 minutes). Newly arrived patients received more nursing hours than continuously stayed patients within the same KTAS Levels. Conclusion: Large variations were found in hourly patient census, acuity, and dependency. Nurse staffing in EDs should be determined based on patient acuity and dependency.

종양간호사의 암 환자 상담경험 (Oncology Nurses’ Experiences of Counseling with Cancer Patients)

  • Purpose: To explore and describe the essence of oncology nurses' experiences of counseling with cancer patients. Methods: Qualitative research with a phenomenological study. Participants were 6 oncology nurses who had worked as a clinical nurse specialist or an education-counseling nurse. Data were collected through individual semi-constructed interviews and analyzed with a thematic approach according to Colaizzi's method. Results: Seven themes emerged from the data: harmony of education and counseling, burdening, useless self-blaming, getting more matured, experiencing rewards and meanings, internalized strategies for counseling, and needs for self growth. Conclusion: The study highlights the significant impact of counseling with cancer patients by oncology nurses. Oncology nurses experienced both positive and negative aspects of being involved in cancer care. They stated that they were personally growing and getting maturated with the counseling experience. Sometimes they felt sad when the patient's status was progressed. They learned and internalized counseling skill during the communication with cancer patients. They wanted to learn about the practical course of counseling and communication method and the psychology of cancer patients.

수술실 간호사의 임상등급에 따른 간호역량 (A Study on the Clinical Competence according to Clinical Ladder of Operating Room Nurses)

  • Purpose: This study was to explore the clinical competence according to clinical ladder of operating room nurses. Method: The subjects were 125 the operating room nurses working at five university hospitals in four city. The instrument to measure the clinical competency and clinical performance of OR nurses was developed by researcher. It consisted of 12 domains of clinical competence and 23 items of clinical performance. Results: Clinical competence and clinical performance of OR nurses group devided into 4 groups by clinical ladder such as 0-12month, 13-36 mon. 37-84 mon. over 85 month were significantly different. More experienced nurses performed higher level of clinical performance and competency in 23 items and 12 domains. Conclusion: It will be needed to add concrete behavioral patterns and behavioral indicators of nursing competencies, per stage of the clinical ladder, by repetitive studies on nurses of various hospitals and to confirm the validity.

한의 정보 표준화를 위한 공통 임상 기록 서식 개발 연구 (A Development Study of Common Clinical Document Forms for Traditional Korean Medicine Information Standardization)

  • Objectives: The clinical document forms, a format for collecting clinical data, is the most fundamental object of standardization. Doctors must have a mutual understanding of the clinical chart. Methods: Clinical document forms were developed by investigating existing conditions in hospitals and conducting demand surveys, doing literature research, and seeking expert advice for the improvement of version 1.0. In addition, an organization of a network of 19 Oriental medical doctors and nurses, 190 patients, and users of collected and assessed data was formed to come up with version 2.0. Results: The overall format was divided into different portions that the patient, nurse, and doctor must fill out, respectively. The patient's section consists of demographic data, lifestyle details, history, and symptoms. The data to be supplied by the nurse include the patient's vital signs and anthropometric parameters. As for the doctors, they are to supply data regarding the patient's palpitation, the detailed symptoms of the patient's head, ophthalmological and otorhinolaryngological symptoms (mouth), respiration, circulatory organ and chest conditions, digestive-organ conditions (thirst), neuropsychiatric conditions, reproductive system, musculoskeletal system, skin (depilation), etc. Conclusions: Common clinical chart development is the prior question to Traditional Korean Medicine standardization. A web-based clinical document format should be developed to support diagnosis and treatment, and furthermore EMR (electronic medical record system) and EHR (electronic health record) developed. Clinical information could be shared through a network of medical institutions and be useful Traditional Korean Medicine for evidence-based medicine.

병원간호사의 간호인성과 고위험 병원체 감염환자 간호의도와의 관계: 간호전문직관의 매개효과 (Mediating Effect of Nursing Professionalism in the Relationship between Nurse's Character and Nursing Intention Caring for High Risk Pathogen Infected Patients in the Tertiary Hospital Nurses)

  • Purpose: This study aims to explore the mediating effect of the nursing professionalism in the relationship between nurse's character and nursing intention caring for high risk pathogen infected patients in the tertiary hospital nurses. Methods: This study used a crosssectional descriptive design. The participants were 129 nurses from two tertiary hospitals. The questionnaire consisted of tools measuring clinical nursing personality, nursing professionalism and nursing intention caring for high risk pathogen infected patients in the tertiary hospital nurses. The data were analyzed using independent sample t-test, one-way ANOVA, Pearson correlation coefficient, and hierarchical multiple linear regression. Results: There were significant relationships between nurse's character and nursing professionalism (r=.59, p<.001), nurse's character and nursing intention caring for high risk pathogen infected patients (r=.54, p<.001), and nursing professionalism and nursing intention caring for high risk pathogen infected patients (r=.54, p<.001). In the relationship between nurse's character and nursing intention caring for high risk pathogen infected patients, nursing professionalism had a partial mediating effect. Conclusion: This current study suggests that strategies for improving nursing professionalism in nurses should be considered when developing an educational program for enhancing their nursing intention caring for high risk pathogen infected patients.

요양병원 간호인력 배치수준이 환자결과에 미치는 영향에 관한 경로분석 (Path Analysis of the Effects of Nurse Staffing on Patient Outcomes in Long-Term Care Hospitals)

  • Purpose: The study aimed to examine the effects of nurse staffing levels on patient outcomes in long-term care hospitals in South Korea using path analysis. Methods: We conducted a secondary analysis of national data, which included the 2021 hospital assessment results of long-term care hospitals from the Health Insurance Review and Assessment Service. Data collection was performed between June 19 and June 27, 2023. The study sample consisted of 1,215 hospitals, and we analyzed the data using SAS 9.4 and Mplus 8 software. Results: The average numbers of patients per registered nurse, certified nursing assistant, and nursing staff, including both registered nurses and certified nursing assistant, was 10.00, 7.43, and 4.00, respectively. Path analysis revealed that the number of patients per registered nurse had direct effects on improvements in activities of daily living and indirect effects on indwelling catheterization, pressure ulcer improvement, and weight loss. The number of patients per certified nursing assistant had direct effects on new pressure ulcer development and pressure ulcer improvement, with no indirect effects. The number of patients per nursing staff had direct effects on pressure ulcer improvement and no indirect effects on other patient outcomes. Conclusion: The findings suggest that establishing policies to evaluate staffing levels of registered nurse and certified nursing assistants separately is necessary in order to improve registered nurse staffing levels and patient outcomes in long-term care hospitals.

임상실습교육에서 간호학생의 적극성에 영향을 미치는 요인 (Factors Affecting Nursing Students'Activeness in Clinical Education)

  • Purpose: The purpose of this study is to analyze the factors influencing students' active practice in clinical nursing education and to find ways to improve them. Methods: In this study, the factors affecting students' practice in nursing clinical practice were reviewed and their opinions were discussed. Results: A limited clinical practice environment, lack of a role model of leading and independent nurse interferes with the active practice of nursing students and negatively affects the image of nurses. On the other hand, semantics that links practice to students'life supports active practice. Conclusion: Clinical practicum educators should actively carry out the role of introducing students to rational of their practice, and arrange meetings with nurses and students who carry out nursing care independently with leadership. In addition, the clinical nursing education should include courses that allow students to discuss the various issues they have in the nursing practice.

임상시험 연구간호사의 업무 실태와 직무만족에 관한 연구 (Job Status and Job Satisfaction among Clinical Research Coordinator)

  • Purpose: The purpose of this study was to examine job status and job satisfaction among clinical research coordinators. Method: Subjects were 91 clinical research coordinators participating in clinical trials at hospitals in Korea. Data were collected from July to August, 2008 using a structured questionnaire that include informed consent, and question as pertaining to, general characteristics, job related characteristics, and job satisfaction. The collected data were analyzed using SPSS 12.0 version. Result: Almost half of the subjects were called as nurse researcher. Major tasks included participant screening (100%), participant scheduling (100%), and obtaining informed consent from participants (98.9%). The most difficult aspect of work was salary (28.6%). The mean score on the job satisfaction was 80.21 (range: 54.00-109.00). Item related to promotion ranked lowest in job satisfaction. There were significant differences in job satisfaction according to the type of hospital (t=-3.640, p<.001), title (t=7.894, p=.001), and route for information of CRC (t=4.970, p=.001). Conclusions: Subjects feel poorly compensated, unrecognized, and perceive a limited career path. These factors are likely to diminish job satisfaction over time. Attention to these aspects through education program and criteria for clinical research coordinators could be helpful.
