  • Relapsing polychondritis (RP) is a rare inflammatory disorder of unknown etiology, causing recurrent inflammatory and degenerative reactions involving the cartilaginous structures throughout the body. Generally, RP is known as multiorgan disease presented as auricular chondritis, arthritis, nasal chondritis, ocular inflammation, audiovestibular damage and respiratory tract inflammation. Major airway involvement occurs in more than 50% of the patient and has been reported to be the primary cause of death. Rarely, it may be presented with only respiratory symptoms without typical clinical manifestation of RP. We experienced a 64-year-old male patient with RP involving diffuse airway tract without other characteristic clinical manifestation and present here with a review of literatures.

A Case of Subcutaneous Salmonella Abscess Developed on Gunshot Wound Area In Lung Cancer Patient (폐암환자의 총상부위에 발생한 피하조직 살모넬라 농양 1예)

  • Salmonellosis is one of communicable disease and still occur in sporadic in Korea frequently. They are four main clinical manifestations in salmonellosis. They are gastrocnteritis, typhoidal or septisemia syndrome, focal manifestation and carrier state. Among them, focal manifestation is rarely seen. Saphra, et al. reported that localized salmonella infection is about 5%. Localized salmonella infection frequently occur during salmonella bacteremia, but may also occur with enteric fever or gastroenteritis. Gray, et al reported 3390 cases of minor gunshot wound infection that bacterial isolates from infected wounds consisted of Staphylococcus aureus(90%), Streptococcus sp.(6%), and mixed organisms(4%). Incresed host susceptibility to infection secondary to lowered resistance due to debilitating disease is an important determinant of Salmonella infection. Since salmonella is seldom suspected as a cause of soft tissue infections, there is usually a dangerous delay in the institution of appropriate antimicrobial therapy and isolation procedure. We experienced one case of subcutaneous salmonella abscess developed on gunshot wound area in lung cancer patient, which was confirmed by pus culture.

Psychological state of the patients according to skeletal class III malocclusion symptoms (골격성 III급 부정교합자의 임상적 특성에 따른 심리상태)

  • Objectives: The purpose of the study is to investigate the psychological state of the patients according to skeletal class III malocclusion symptoms. Methods: The subjects were 200 skeletal class III malocclusion patients. The questionnaire consisted of general characteristics, clinical manifestation, and T score of Korean version of self-rated Symptom Checklist-90-Revision modified by Jae-hwan Kim. The data were analyzed using SAS version 9.2 and t-test, ANOVA, and ANCOVA were used. Clinical manifestation included subjective recognition and radiological analysis. The subjective recognition of the patients consisted of self-satisfaction of the appearance, phonation, mastication, and temporomandibular joint pain. T score consisted of somatization, obsessive-compulsive, interpersonal sensitivity, depression, anxiety, hostility, phobic anxiety, paranoid ideation, and psychoticism. Results: The most serious factor in 9 scales was the severe TMJ pain by ANCOVA. Male patients had a higher T score in phobic anxiety, psyochoticism, somatization and depression than female patients. Those having low appearance satisfaction had the problem in interpersonal sensitivity. Obsessive-compulsive symptoms were conspicuous in phonation difficulty and temporomandibular pain. Conclusions: The patients with skeletal class III malocclusion have more satisfaction with appearance, pronunciation, and phonation than those with skeletal class III malocclusion and overjet. Proper dental treatment will improve the communication and quality of life.

A Case of Scar Sarcoidosis (피부반흔에서 발생한 유육종증 1례)

  • Purpose: Sarcoidosis is a systemic noncaseating granulomatous disease of an unknown origin, and can involve any organ including the skin. The infiltration of sarcoid granuloma in an old cutaneous scar is an uncommon cutaneous manifestation of sarcoidosis. This paper reports a 35-year old female who presented with cutanesous nodules in previous facial scars. Methods: A 35-year-old female presented with cutaneous nodules for 2 months in previous scars of the forehead and lower lip that she had acquired in the childhood. An excisional biopsy of the lower lip mass, serologic examinations and radiologic studies were performed. Results: The excisional biopsy revealed noncaseating granulomas consistent with sarcoidosis. Chest CT revealed both hilar and paratracheal lymphadenopathy with nodular densities in both lung fields. Routine laboratory tests, serologic tests, serum angiotensin converting enzyme level, sputum for acid-fast bacilli, ophthalmoscopic examination, TB-PCR and NTM-PCR showed normal findings. Therefore, the patient was diagnosed with sarcoidosis. The patient refused fiberoptic bronchoscopy and medication with oral steroid. Currently, the clinical manifestation and progress are being monitored closely, and treatment is expected to start with oral steroid according to the progress. Conclusion: For the proper management of cutaneous sarcoidosis, particularly scar sarcoidosis, plastic surgeons should be aware of the many clinical and histopathological features of sarcoidosis and recommend a systemic evaluation for early diagnosis and proper treatment.

  • The presbyphonia is a combination of physiological and structural changes due to aging of the larynx in elderly patients with voice problems. Some of these changes are inevitable, while others may be avoidable or reversible. The fatigue of phonation is the most common clinical symptom of the aging voice. The voice problems with aging are produced from variable causes including the organic lesions of the larynx. It is essential that the curers understand physiologic and pathologic changes of aging voice for minimizing glottal incompetence and improving vocal performance and quality of life of the elderly.

  • Objectives & Methods: This research may accounts for Self Targeting Therapy (STT) which is a peculiar operation of Mountain Ginseng Pharmacopuncture. The objectives include analysis of its mechanism and reporting of clinical cases. Results: 1. STT of Mountain Ginseng Pharmacopuncture refers to the function of curing and making a diagnosis of the disease by oneself. 2. The mechanism of STT is to pursue the latent illness and make the remedial action by oneself. In this process, the symptom of latent disease is outwardly expressed. 3. The most popular symptom expressed is manifestation of pecular sensation along the specific meridians. Then followed headache, dizziness, powerless and etc. Diverse symptoms can be expressed by a patient. 4. If the revealed symptoms are carefully observed, the cause of the disease or the patient state can be diagnosed easily. Conclusion: The mechanism of STT of Mountain Ginseng Pharmacopuncture can be a great asset in clinical trials. Other studies on STT of Mountain Ginseng Pharmacopuncture should be followed in the future.

Cardiac Rupture Clinical Analysis of 10 -Operative Cases- (심장 파열 수술치험 10례에 대한 분석)

  • From January, 1990 to December, 1994, we have operated 10 patients with cardiac rupture. The patients are divided into two groups according to their clinical manifestation ; five patients in each hemorrhage and tamponade group. The patients in both groups could maintain their vital signs with closed thoracostomy and fluid resuscitation. The effect of pericardiocentesis was especially dramatic in three patients of tamponade group. The average time from injury to admission was 101 minutes and that of the patients who came our hospital via one or two other hospitals was 170 minutes comparing 31 minutes of those who came directly. The average time from admission to operation was 211 minutes. Considering 98 minutes for the diagnosis and preoperative management and another 30 minutes for the preparation for operation, operations were delayed by 83 minutes to get permission. We conclude that this delaying time for transport and operation of heart-ruptured patients should be shortened in order to manage them more effectively.

  • Simultaneous subarachnoid hemorrhage and infarction is a quite rare presentation in a patient with a spontaneous dissecting aneurysm of the anterior cerebral artery. Identifying relevant radiographic features and serial angiographic surveillance as well as mode of clinical manifestation, either hemorrhage or infarction, could sufficiently determine appropriate treatment. Enlargement of ruptured aneurysm and progressing arterial stenosis around the aneurysm indicates impending risk of subsequent stroke. In this setting, prompt treatment with stent-assisted endovascular embolization can be a reliable alternative to direct surgery. When multiple arterial dissections are coexistent, management strategy often became complicated. However, satisfactory clinical results can be obtained by acknowledging responsible arterial site with careful radiographic inspection and antiplatelet medication.

  • Pertussis is a highly contagious respiratory tract disease caused by $Bordetella$ $pertussis$ infection. The clinical manifestation of this infection can be severe enough to cause death. Although pertussis has been supposed to be a vaccine-preventable disease ever since the widespread vaccination of children against pertussis was started, since the 1990s, cases of pertussis and related fatalities are on the rise, especially in countries with high vaccination coverage. In Korea, there have been no deaths due to pertussis since 1990, and the vaccination rate continues to be approximately 94%. However, the number of pertussis cases reported to the Korea Center for Disease Control and Prevention has tended to increase in the 2000s, and in 2009, there was an obvious increase in the number of pertussis cases reported. This review aims to present the latest information about the pathogenesis, diagnosis, treatment, and prevention of pertussis.

  • The venom gland of Toad contains large quantities of cardiac glycosides, and toad venom poisoning is similar to digitalis toxicity and carries a high mortality. Sometimes after ingestion of aphrodisiac pills which contain dried toad, a patient develops gastrointestinal symptoms and bradycardia, psychoneurologic symptoms. We have experienced 2 cases of toad venom intoxication, who ingested asian toads. Patients were presented to our ED with nausea, vomiting, and abdominal pain. The patients were peformed monitoring and conservative treatment and were fully recovered. Toad venom intoxication should be considered in patients with clinical manifestation of gastrointestinal irritation, cardiac arrhythmias, hyperkalemia, and detectable serum digoxin level without current medication of digoxin.
