Patient's Selection for Extracorporeal Shock Wave Lithotripsy for Treatment of Common Bile Duct Stones Resistant to Endoscopic Extraction (체외충격파쇄석술 적용을 위한 총담관결석의 선택)

  • Background/Aim : Common bile duct (CBD) stones may cause jaundice, cholangitis, or pancreatitis. Extracorporeal shock wave lithotripsy (ESWL) may be needed whenever endoscopic procedure are failed to extract common bile duct stones. The aim of this study is to provide the standard for patient's best choice on ESWL for treatment of CBD stones resistant to endoscopic extraction. Materials and Methods : Fourty-six patients failed in endoscopic stone extraction including mechanical lithotripsy were treated by ESWL. In all patients, endoscopic sphincterotomy and nasobiliary drainage tube was done before ESWL using the ultrasonography for stone localization with a spark-gap type lithotriptor. Patients were sedated with an intravenous injection of 50 mg of Demerol. None were treated under general anesthesia. Results : Overall complete clearance rate of CBD stone was 89.1% (41/46). In 82.6% of the patients, the stones were extracted endoscopically after ESWL, and spontaneous passage was observed in 6.5%. In the clearance rate after ESWL, there were no noticeable differences with regard to number (single: 82.8%, two or three: 100%, more than three: 100%) and size of the stone (less than 33mm: 92.9%, 33 mm or larger: 83.3%), whereas there were significant differences with regard to the ratio of sum of long-axis length of the all stones to sum of long-axis length of the CBD excluding stone (1:2.4, 1:2.1) and diameter of the largest stone to diameter of CBD excluding stone (1:0.9, 1:0.4) for patients with complete clearance compared with those without. Conclusion : We propose that stones without the fragments are travelable sufficient space in CBD or extractable sufficient diameter of CBD regardless of stone size and number should be treated by other technique to prevent time and cost consuming, such as percutaneous transhepatic cholangioscopylithotomy.

  • Damage of sea-crossing bridge by ship collision is related to estimate frequencies of overloading due to impact, and bridge accordingly must be designed to satisfy related acceptance criteria. Another important aspect is the management on increment of collision risk during the service period. In this study, related plan, main span length, air draft clearance and collision risk are analyzed for the interim assessment of Incheon Bridge focusing on the ship collision problem. In particular, for the increment of collision risk, the optimized navigation speed is proposed by reviewing the research findings and navigation guidelines etc. as a temporary expedient. Also basic procedure for reasonable prediction of target vessel and passage is established and probabilistic prediction method to embrace the uncertainty of the prediction is proposed as a fundamental solution. It is necessary to conduct further research on collision risk management and promptly carry out interim assessments of other marine bridges.

Gross Alpha Analysis of Nasal Smear Samples and Internal Dose Assessment Procedure in Radiation Emergency (방사선비상시 비강스메어 시료의 전알파 분석 및 내부피폭선량평가 절차)

  • The gross alpha analysis of nasal smear samples for the radiation emergency and the additional follow-up steps were established. Cotton swab sticks using in local hospitals for nasal smear in Korea were used for the verification. The measurement results of standard samples spiked with certified reference source were well agreed within ${\pm}20%$ compared with reference values. The clearance ratio of smear samples conducted with wet smear condition showed higher value. To eliminate the quenching effect of liquid scintillation samples, dry of smear samples should be followed up before counting samples. Based on the measurement results, medical decision levels and internal dose assessment were established for the victims in the beginning of radiation emergency.

  • Magnetic survey flights by helicopters are usually parallel to the topographic surface, with a nominal clearance, but especially in high-resolution surveys the altitudes at which observations are made may be too variable to be regarded as a smooth surface. We have developed a reduction procedure for such data using the method of equivalent sources, where surrounding sources are included to control edge effects, and data from points distributed randomly in three dimensions are directly modelled. Although the problem is generally underdetermined, the method of conjugate gradients can be used to find a minimum-norm solution. There is freedom to select the harmonic function that relates the magnetic anomaly with the source. When the upward continuation function operator is selected, the equivalent source is the magnetic anomaly itself. If we select as source a distribution of magnetic dipoles in the direction of the ambient magnetic field, we can easily derive reduction-to-pole anomalies by rotating the direction of the magnetic dipoles to vertical.

  • This study shows a development procedure and results of noise reduction for a new developed FF 8th speed automatic transmission. Based on planetary gear operating frequency analysis using PTA(planetary transmission analysis) program developed in 2012, It is expected that gear noise of the rear planetary gear set could be recognized easily in the concept design stage. Therefore, pRMC (planetary run many cases) analysis program that is developed in 2012 was applied to minimize the planetary gear noise level and noise distributions versus torque. To minimize noises coming from oil pump and final gears of a new transmission, several changes were applied, such as changing the clearance of double angular ball bearing, the oil pump rotor tooth number from 9 to 11 and the oil pump type from parachoid to megafloid and so on. Besides, stiffness values of the transmission case and the mount bracket were measured and reinforced properly. Finally, The total noise of the new FF 8th speed automatic transmission was developed successfully. Furthermore, E.O.L. testers also have been adapted to control the noise quality of automatic transmission assembly in the manufacturing factory. This paper could provide practical solutions to the automatic transmission NVH problems.

THE COMPARATIVE STUDY OF THERMAL INDUCTIVE EFFECT BETWEEN INTERNAL CONNECTION AND EXTERNAL CONNECTION IMPLANT IN ABUTMENT PREPARATION (구강내에서 임플랜트 지대주 형성 시 내부연결방식과 외부연결방식간의 열전달 효과 비교)

  • Statement of problem: The cement-type abutment would be needed for the reduction of its body in order to correct the axis and to assure occlusal clearance. In the case of intraoral preparation, there is a potential risk that generated heat could be transmitted into the bone-implant interface, where it can cause deterioration of tissues around the implant and failed osseointegration. Purpose: The purpose of this study was to assess the difference of the heat transmitting effect on external and internal connection implant types under various conditions. Material and method: For evaluating the effects of alternating temperature, the thermocoupling wires were attached on 3 areas of the implant fixture surface corresponding to the cervical, middle, and apex. The abutments were removed 1mm in depth horizontally with diamond burs and were polished for 30 seconds at low speed with silicone points using pressure as applied in routine clinical practice. Obtained data were analyzed using Mann-Whitney rank-sum test and Wilcoxon / Kruskal-Wallis Tests. Result: Increased temperature on bone-implant interface was evident without air-water spray coolant both at high speed reduction and low speed polishing (p<.05). But, the difference between connection types was not shown. Conclusion: The reduction procedure of abutment without using proper coolant leads to serious damage of oral tissues around the implant irrespective of external and internal connection type.

  • International trade has been changed from traditional trade to e-trade due to the fast expanding of information technology like e-marketplace, EDI (Electronic data interchange) using Internet since mid of 1990's. e-Trade, as a new trade method, could handle every trade procedure such as market research, contract, customs clearance, logistics and payment using IT like internet without restriction of time and space. The evolution of transaction-based business model is upon us. The business models of many e-Marketplace in their early stages have typically been based on transaction fees. Many e-Marketplaces have even called out transaction revenues as a core element of their business plans. The transaction business represents the most simple of business models, but it does not provide a long-term sustain able advantage. For buyer's convenience, wide selection and test price hold appeal. For suppliers, the extended global market reach and direct access to customers and consortiums of customers is powerful. To maximize leverage of these new e-marketplace, you must from both a buyer perspective as well as a supplier perspective. Also required is a strategy that takes in account all of the various e-Marketplace transaction standards and one that allows the easy accomodation to new e-marketplace as the market change. These new e-marketplace will need to be factored into the sales channel strategies. To be successful, integration with these e-marketplaces should occur at a complete business process level. This study would suggest on the role of buyers and sellers for e-trade which could maximize effect of e-trade in order to cope with rapid changing IT environment and global trade environment. Therefore, this study suggests top priority tasks for implementing on the specialization strategy of e-trade process.

Analysis of Propeller-WIG Interaction and Performance in Potential Flow (포텐셜 유동에 의한 프로펠러-WIG선의 상호작용 및 성능해석)

  • The interaction between forward mounted propeller and wing in ground effect, and its aerodynamic performance are analyzed by potential flow approximation. A Vortex Lattice Method(VLM) for the propeller analysis and a potential based panel method for the WIG are used together with an image method by assuming the free surface as a rigid wall. The interaction of propeller and wing in the proximity of the ground is taken into account by an iterative procedure where the boundary conditions are satisfied with the given convergence criteria. The program developed is first checked by comparing its numerical results with the experimental data and other numerical results for the propeller MP101-rudder MR21 system. Then, the propeller-WIG interaction and its performance versus ground clearance are investigated by changing parameters such as propeller position, diameter and speed of revolution. It is shown that the forward mounted propeller increases the lift forces of the wing and also enhances the height stability, depending on the design parameter. Therefore, the appropriate selection of the design parameter such as propeller diameter, revolution, the longitudinal and vertical position of propeller is necessary.

Accuracy of Bite Registration Using Intraoral Scanner Based on Data Trimming Strategy for Fremitus Teeth

  • Purpose: This study aimed to evaluate the accuracy of bite registration using intraoral scanner based on data trimming strategy for fremitus teeth. Materials and Methods: A reference model was designed by Medit Model Builder software (MEDIT Corp., Seoul). Tooth number 24 and 25 were separated as dies and tooth number 26 was prepared for full-coverage crown. Those were printed using a 3D printer (NextDent 5100). The scanning procedure was performed by a single trained operator with one intraoral scanner (i700; MEDIT Corp.). The scanning groups were divided as follows: group 1 (G1), no fremitus; group 2 (G2), 0.5 mm buccal fremitus in the maxillary left first and second premolar; and group 3 (G3), 1.5 mm buccal fremitus in the maxillary left first and second premolar. Each group was scanned 10 times and were analyzed using the reference model data. Surface-based occlusal clearance was analyzed at the prepared tooth to evaluate accuracy. Result: Mean values of control group (G1) were 1.587±0.021 mm. G2 showed similar values to those from the control group (1.580±0.024 mm before trimming strategy and 1.588±0.052 mm after trimming strategy). G3 showed significantly greater values (1.627±0.025 mm before trimming strategy and 1.590±0.024 mm after trimming strategy) and the differences were found between trimming strategy (P=0.004). Conclusion: Bite trimming strategy for fremitus teeth is a reliable technique to reduce inaccuracies caused by the mobility at maximum intercuspation.

A Theoretical Study and Empirical Analysis of New Obstacle Limitation Surface (OLS) - The Case of Incheon International Airport - (신 장애물제한표면에 관한 이론적 고찰과 실증분석 - 인천국제공항을 중심으로 -)

  • Obstacle Limitation Surface (OLS) is conceptual surface establishing the airspace around aerodromes to be maintained from obstacles to ensure safe aircraft operations. Despite advances in the technologies for aircraft, navigation systems and the development of new flight procedures, the criteria defining OLS have not been amended since its initial establishment, resulting in the overestimation of areas for height restriction. As there were requests to examine OLS at the 12th Air Navigation Conference and the 38th ICAO Assembly, the research on the OLS revision began in earnest and ICAO has proposed Obstacle Free Surface (OFS) and Obstacle Evaluation Surface (OES) as an alternative of the existing OLS. OFS is surfaces where obstacles shall not be permitted, and OES is ones where obstacles be evaluated with an aeronautical study and could be permitted under some conditions. The purpose of this study is to preemptively assess the efficiency and safety of OFS and OES by applying them to the second runway (15L/33R) of Incheon International Airport. The results show that OFS and OES are capable of serving the instrument flight procedure safely with a smaller obstacle clearance area compared to the existing OLS.
