Trends of Korean Medicine Conservative Treatment Using Low Back Pain Patients Registry Data: Analysis of Medical Records of 7 Korean Medical Hospital (요통 환자 레지스트리 데이터를 통한 한의 보존적 치료 경향: 7개 한방병원 의무기록 분석)

  • Objectives The purpose of this study is to investigate characteristics of low back pain patients and trends of Korean medicine conservative treatment effects of low back pain patients by registry data. Methods A total of 78 patients were analyzed, statistical analysis was performed using a generalized linear model. The comparison between groups was performed using independent two samples t-test or Wilcoxon rank sum test according to the distribution of normality. Before and after the treatment effects, ANOVA and Kruskal-Wallis method were used for the comparison, Sheffe method was used for the post test. Results The average age of hospitalized patients was 31.8 years old and out-patient department (OPD) patients was 44.9 years old. Lots of OPD patients and hospitalized patients were classified to 'Lumbago due to Coexist Qu Stagnation and Bloos Stasis'. Both OPD patients and hospitalized patients classified to 'Lumbago due to Coexist Qu Stagnation and Bloos Stasis' showed statistically significant decrease in RMDQ (Roland-Morris disability questionnaire) and NRS (numeric rating scale) on 1 month and 3 month after clinical research start date. Comparing the treatment effects of Chuna manual therapy, both Chuna group and non-Chuna group showed statistically significant decrease in RMDQ and NRS on discharge date, 1 month and 3 month after clinical research start date. Conclusions We expect that a larger scale registry study will be carried out to provide a higher level of basis for the Korean medical treatment of low back pain.

Sensory change after implant surgery: related factors for recovery

  • Objectives: This retrospective study aimed to analyze data on nerve damage in patients who complained of sensory changes after dental implant surgery, the clinical results according to proximity of the implant fixture to the inferior alveolar nerve (IAN) canal, and the factors affecting recovery of sensation. Materials and Methods: The electronic medical records of 64 patients who had experienced sensory change after implant surgery were reviewed. Patients were classified by sex, age, implant installation sites, recovery rate and the distance between the implant fixture and IAN canal on computed tomography (CT). The distance was classified into Group I (D>2 mm), Group II (2 mm≥D>0 mm), and Group III (D≤0 mm). Results: The 64 patients were included and the mean age was 57.3±7.3 years. Among the 36 patients who visited our clinic more than two times, 21 patients (58.3%) reported improvement in sensation, 13 patients (36.1%) had no change in sensation, and 2 patients (5.6%) reported worsening sensation. In Group II, symptom improvement was achieved in all patients regardless of the removal of the implant fixture. In Group III, 8 patients (40.0%) had reported symptom improvement with removal of the implant fixture, and 2 patients (33.3%) of recovered patients showed improvement without removal. Removal of the implant fixture in Group III did not result in any significant difference in recovery (P=0.337), although there was a higher possibility of improvement in sensation in removal cases. Conclusion: Clinicians first should consider removing the fixture when it directly invades the IAN canal. However, in cases of sensory change after dental implant surgery where the drill or implant fixture did not invade the IAN canal, other indirect factors such as flap elevation and damage due to anesthesia should be considered as causes of sensory change. Removal of the implant should be considered with caution in these situations.

  • Hue, value, and chroma are the fundamental colour components used in colour property research to identify colour in the digital media environment. In the Munsell colour system, which is based on the characteristics of visual perception, the basic properties are classified into hue, value, and chroma. The methods for recording these three properties can be divided into the colour appearance system and the colour mixing system: in the former, they are documented based on a colour chart that focuses on visual perception, and in the latter, accurate numerical records are kept without concern for discolouration. Colour terminology is crucial for conveying and expressing colours, and colours can be classified and defined according to the combination of hue, value, and chroma. With the development of various media, it has become possible to represent a range of colours previously unachievable, necessitating basic research into the characteristics of colour perception by further subdividing digital-oriented colour studies. In this study, we conducted psychophysical experiments to identify and analyse the categories of value and chroma needed to recognise each colour among the ten representative colours of the Munsell colour system, based on visual perception on a display. This study analyses the results of these experiments, defines their significance as foundational research data on colour perception characteristics, and suggests directions for future research.

Various MRI Findings of Toxocara canis Myelitis (개회충 척수염의 다양한 자기공명영상 소견)

  • Purpose To describe magnetic resonance imaging (MRI) findings in 10 cases of Toxocara canis myelitis and to analyze these findings to aid in the diagnosis of this condition. Materials and Methods From June 2015 to March 2018, we retrospectively analyzed the electronic medical records and MR images of patients who were diagnosed with Toxocara canis myelitis. The analysis of the MR images was based on a discussion between an experienced spinal radiologist and a radiology resident. Results This study classified MRI findings into the following two types. Type 1 was defined as central and diffuse T2 high signal intensity on the axial plane, which was observed in 50% of all cases. All lesions showed avid enhancement, mostly in the posterolateral or posterior region (4 cases, 80%). Type 2 was defined as wedge-shaped or focal T2 high signal intensity in the posterolateral or posterior region and corresponded to the remaining 50% of the cases. In this case, the extent of the lesion was relatively small and contrast enhancement was observed in only one case. Conclusion This study revealed that various imaging findings could be observed in cases of Toxocara canis myelitis, and the findings were classified into two types.

  • Orthopraxia & Dermatology in traditional Korean medicine has taken a large part of our medicine from the ancient times. However, studies that have been carried out so far were mainly concerned with specific disease or treatment but studies on the history of orthopraxia & dermatology in traditional Korean medicine have not been done largely. Subsequently, it is confirmed that diseases belonging to the category of orthopraxia & dermatology in traditional Korean medicine are carried with no less weight in the books other than literatures specialized in orthopraxia & dermatology while Korean medical literature is being researched with interests in the history of orthopraxia & dermatology in traditional Korean medicine. This paper mainly introduced and analyzed medical literatures containing diseases in orthopraxia & dermatology among Korean medical literatures which were classified into books on emergency measures, general medical documents and surgically specialized records prior to DongUiBoGam.

A Study of Clinical features and classifications of alopecia patients in Korean medicinal clinic (탈모증 환자의 한의학적 임상 유형에 대한 연구)

  • Objectives : This study was planed to evaluate clinical status of the alopecia patients who had visited Korean medicine clinic. And the result from this study would provide a standard in Korean medical diagnostic and classification method of alopecia. Methods : Clinical records of 183 patients with alopecia seen from January 2004 to April 2005 at Korean medical clinic was examined. They were classified into 4 different types according to chief complains besides alopecia by 2 Korean medical doctors. Results and conclusions : We made clinical analysis of patients of alopecia from January 2004 to April 2005. Among the alopecia patients who visit Korean medical clinic, people age between 20 and 30 had high ratio. The duration from the recognition of initial hair loss to the time of the first visit to the Korean medical clinic was less than 12 months in 20.8%(38/138), and less than 60 months in 72.2% (132/183). The condition of alopecia was more worse than other alopecia patients who visit the west medical clinic. Also the ratio with increased temperature of face or scalp is chief complaint except alopecia in alopecia patients was high in men and the ratio with dysfunction of digestive system or chronic weakness was high in women. Among the incidence of alopecia, the androgenic alopecia was most in number; 43.7%(80/183) and the sex distribution showed 83 men and 100 women.

Implant success rates in full-arch rehabilitations supported by upright and tilted implants: a retrospective investigation with up to five years of follow-up

  • Purpose: The aim of this retrospective study was to investigate the cumulative success rate, the implant survival rate, and the occurrence of biological complications in implants supporting full-arch immediately loaded rehabilitations supported by upright and tilted implants. Methods: The clinical records and periapical radiographs of patients who attended follow-up visits were collected, and information was recorded regarding marginal bone loss resorption, the occurrence of peri-implant infectious diseases, and the implant survival rate. Implants were classified as successful or not successful according to two distinct classifications for implant success. Results: A total of 53 maxillary and mandibular restorations including 212 implants were analysed, of which 56 implants were studied over the full five-year follow-up period. After five years, the cumulative success rate was 76.04% according to the Misch classification and 56.34% according to the Albrektsson classification. The cumulative implant survival rate was 100%, although one implant was found to be affected by peri-implantitis at the second follow-up visit. Conclusions: The cumulative success rate of the implants dropped over time, corresponding to the progression of marginal bone resorption. The prevalence of peri-implantitis was very low, and the implant survival rate was not found to be related to the cumulative success rate.

Incidence and Trends of Malignant and Benign Pancreatic Lesions in Yazd, Iran between 2001 and 2011

  • Background: Despite recent valuable steps in initiating a cancer registry in Iran, data depicting prevalence, incidence, and clinical picture of pancreatic tumors in the country are exceedinglyly sparse. With the aim of filling this knowledge gap, we reviewed cases in the pathology archive of Shahid Sadoughi hospital (Yazd, Iran), between 2001 and 2011. Materials and Methods: Medical records of 177 patients are reported in the present study. In cases for which paraffin-embedded blocks were available, the specimens were evaluated by two independent pathologists blinded to the primary diagnosis. We extrapolated the frequency of malignant lesions in our study to the population of Yazd province, derived from national census data, to generate cancer incidence rates. Results: Final diagnosis of malignancy was made in 117 cases (66.1%), and the remainder (60 lesions, 33.9%) were classified as benign. Adenecarcinoma and neuroendocrine tumors were the two most common histological types of malignancy identified in 88 (75.2%) and 11 (9.4%) specimens, respectively. Crude annual incidence of pancreatic cancer was 0.55 per 100,000 person in 2001 and increased to 1.68 in 2011. Age standardized incidence rates in 2001 and 2011 were 0.75 and 2.68, respectively. A significant increasing trend in cancer incidence was observed during the 11 years of the study period (r=+0.856, p=0.009). Sex-stratified analysis, confirmed the observed trend in men (r=+0.728, p=0.034), but not women (r=+0.635, p=0.083). Conclusions: Over the past decade, incidence of pancreas malignancies has risen steadily in Yazd, Iran. Nevertheless, these figures are still substantially lower than those prevalent in developed nations.

A Retrospective Analysis of Liver Function Test in Cancer Patients Taking Rhus Verniciflua STOKES Decoction(Chijongtang) with Anti-cancer Drugs (항암제와 함께 옻나무 전탕추출물(치종탕)을 복용한 암환자의 간기능 검사에 대한 후향적 조사)

  • Objectives : The aim of this study was to evaluate safety of Rhus Verniciflua STOKES as a combination therapy of Oriental and conventional cancer treatment. So, We investigated liver function test(especially total bilirubin, AST, ALT, and ALP) levels in outpatients at an oriental medical clinic, to know whether Rhus Verniciflua STOKES decotion (Chijongtang) was hepatotoxic. The outpatients were treated with Rhus Verniciflua STOKES decotion(Chijongtang) during intravenous or oral chemotherapy. Methods : We surveyed whether Chijongtang caused liver injury in patients, who visited and took Chijongtang at Hana oriental medical clinic, from March, 2008 to November 2011. We looked over all the liver function tests of those patients, done during medication. Patients who had no records of liver function tests or with basal liver disease were excluded. And patients were classified into normal and abnormal liver function groups. Results and Conclusions : 48 patients were enrolled, and their mean dosage was 300ml/day, and the mean medication period was 9.3 (2-42) months. During and after medication, there were no abnormal liver function test results at all, in the normal liver function group. and 10 out of 11 patients in the abnormal liver function group showed normal liver function test levels. Roughly speaking, according to the above results, it seems that taking Chijongtang did not cause hepatotoxicity. And it seems Chijongtang has hepatoprotective effect. But we still need more laboratory and clinical research to reach a more definitive conclusion.

A SURVEY OF THE TREATMENT IN FREE DENTAL CLINIC (치과 자원봉사 기관에서 시행한 장애인 치과 치료에 대한 통계)

  • The purpose of this study were to investigate the chief complain and dental treatment needs in handicapped patient. This study examines treatment records of 1025 patients in free dental clinic for handicapped patients during 10 years from 1999 to 2008. The results were as follows : 5.8 average visit per patient; mean patient age was 25; 544 patients was younger than 20. Handicapped patients classified according to types of disability. Crippled disorder were 19.1%, brain disorder were 4.5%, visual disorder were 3.1%, auditory disorder were 4.1%, speech disorder were 0.9%, mental retardation were 67.1%, and developmental disorder were 25.1%. Performed treatments were 322 scaling, 13 fluoride varnish, 727 preventive resin restoration, 1296 resin restoration, 600 amalgam restoration, 46 GI restoration, 612 extraction, 289 pulp treatment of primary teeth, 75 pulp treatment of permanent teeth and 138 stainless steel crown restoration. Many handicapped patients have some difficulty to dental treatment. They have limited access to dental care, which is compounded by a shortage of skilled dental professionals who are willing to treat these population and financial problems.
