  • The purpose of this study was to evaluate the effect of dentinal sclerosis and tubular orientation on Class V restoration bonded with three dentin bonding agents using confocal laser scanning microscope(CLSM). V 급 와동 변연부 형태 및 복합레진 종류에 따른 변연 누출에 관한 실험적 연구 (An Experimental Study on the Effects of the Margin Designs and Composite Resins on the Marginal Leakage of Class V Resin Restorations)

  • The primary aim of this study was to assess the effects of the margin designs and composite resins on the marginal leakage of Class V resin restorations. 60 Class V cavities with $90^{\circ}$ butt joint, 60 with $45^{\circ}$ bevel joint, and 60 with concave joint were prepared on the labial surfaces of 38 extracted anterior teeth and the buccal and lingual surfaces of 71 premolars, and classified as the butt group, bevel group and concave group, respectively. After completion of the cavity preparation, Heliosit${(R)}$ was filled in 20 butt joint cavities, 20 bevel joint cavities, and 20 concave cavities. And Hi-Pol$^{(R)}$ and Palfique$^{(R)}$ were filled in the same manner, respectively. After finishing, all specimens were subjected manually to 50 thermal cycles at $4^{\circ}C$, $37^{\circ}C$, and $60^{\circ}C$. They were immersed in 0.5% methylene blue solution for 24 hours, and buccolingually sectioned with sectioning machine. The sectioned specimens were examined under the light microscope. The results indicated the following: 1. The bevel group showed the least amount of marginal leakage compared with the other two design groups, but the statistical analysis showed no significant difference at the 95% confidence level. 2. Hi-Pol$^{(R)}$ group showed the most severe marginal leakage compared with the other composite resin materials, but there was no significant difference in the groups. 3. The bevel-Heliosit$^{(R)}$ subgroup, which was filled with Heliosit$^{(R)}$ in the beveled cavities, showed the least amount of marginal leakage, but there was no significance in all the subgroups.

  • The purpose of this study was to evaluate the marginal adaptability of the amalgam restorations in applying the cavity varnish (Copalite$^{(R)}$) and dentin bonding agent (Scotchbond 2$^{(R)}$) under the scanning electron microscope. For this study, eighteen sound extracted human molars were selected. Class I cavities in 12 teeth and class V cavities in 6 teeth were prepared using an air turbine with No. 701 tungsten carbide bur and finished using a low speed handpiece with No. 557 fissure bur. The prepared specimens were then divided into three groups including 4 class I cavities and 2 class V cavities in each group and restored as follows ; Group I. All the prepared cavities were restored with amalgam only (Control). Group II. Two layers of Copalite$^{(R)}$ cavity varnish were applied to the cavities with a gentle stream of air after each application and cavities were restored with amalgam. Group III. The enamel cavity margins were etched with 37% phosphoric acid gel for 60 sec., rinsed for 30 sec. and dried. One layer of visible lightcured Scotchbond Dental Adhesive$^{(R)}$ was applied and immediately cured for 20 seconds with visible light-cure unit and cavities were restored with amalgam. All the specimens were cut at the neck of the teeth and the occlusal halves of specimens were sectioned buccolingually in the longitudinal axis centering the amalgam restorations, using the disk. The cut specimens were ground with sandpapers (400, 600, 800, 1000 grit), and cleaned for 5 minutes in the ultrasonic cleaner (Brason Co. U.S.A.). In the cut surfaces, the amalgam - tooth interfaces were examined under the scanning electron microscope (JSM, 35C type, JEOL). The obtained results were as follows ; 1. The amalgam-tooth interfaces were reduced more significantly in the Copalite$^{(R)}$ and Scotchbond 2$^{(R)}$ application group than in the control group. 2. In the class I cavities, the Scotchbond 2$^{(R)}$ application group showed the findings similar to the Copalite$^{(R)}$ application group in the cavity floor, and the marginal adaptability was better in the side wall than in the cavity floor. 3. In the class I cavities, the Scotchbond 2$^{(R)}$ application group showed better marginal adaptability in the occlusal margin than in the gingival margin. 4. The marginal adaptability was in the order of the Scothbond 2$^{(R)}$ application group, the Copalite$^{(R)}$ application group and the control group.

  • The purpose of this study was to evaluate the marginal leakage of composite resin. Preparing 144 class V cavities on freshly extracted noncarious teeth, composite resins were prepared and inserted by one dentist according to the manufacturer's instructions. The experiments were performed in two different groups; In group I; Class V cavities with $90^{\circ}$ cavosurface angle, In group II; Class V cavities with $135^{\circ}$ cavosurface angle. And each group was divided 2 subgroups; In control group; composite resin restoraions without acid etch technique. In experimental group; composite resin restorations with acid etch technique. All specimens were immersed in 0.05% crystal violet solution. Before examination, the restored teeth were subjected to thermal stress. The specimens were sectioned occlusogingivally through the center of the restorations with a diamond disk. The sections were examined under a reflected light microscope at 1 day, 7 days and 30 days after immersing the specimens in dye solution. The results were as follows; 1. Control group of group I and group II showed marginal leakage. 2. The degree of marginal leakage in experimental group was greater reduced than control group. 3. In control group, the degree of marginal leakage in group I was greater than group II. 4. In experimental group, there is not statistical differences of the degree of marginal leakage between group I & group II.

Microleakage of Class V cavity restored with flowable and microfill composite resins after load cycling.

  • Flowable and microfill composites have been recommended for Class V cavities. But the use of flowable composites is controversial because of its physical properties. Objectives: The aim of this study was to evaluate the microleakage of 6 composites (2 hybrids, 2 microfills, and 2 flowable composites) with/without load cycling. 5급 와동에서 저유동성과 고유동성 레진의 변연 미세누출 비교 (COMPARISON OF MARGINAL MICROLEAKAGE BETWEEN LOW AND HIGH FLOWABLE RESINS IN CLASS V CAVITY)

  • 이 연구는 1단계 자가부식 접착제와 저유동성 및 고유동성 레진을 이용한 5급 와동 수복물에서 법랑질과 상아질 변연의 미세 누출 차이를 상호 비교하기 위하여 시행하였다. 40개의 대구치의 협, 설면 치경부에 5급 와동을 형성하고, 8개씩 무작위로 선택하여 사용된 유동성 레진에 따라 5개의 군으로 분류하여 각 군의 와동을 유동성 레진으로 충전하였다 (대조군: Filtek Z 350, M-LF 군: Metafil Flo $\alpha$, U-LF 군: Unifil LoFlo Plus, M-HF군: Metafil Flo, U-HF군: Unifil Flow). 각 군의 치아를 2% methylene blue용액에 24시간 동안 침적시킨 다음, 협면과 설면의 5급 수복물의 중앙부가 통과되도록 절단하였다. 각 군의 절단 시편에서 유동성 레진 수복물의 법랑질과 상아질 변연부를 광학 입체현미경 하에서 색소침투 정도를 관찰하여 미세누출 점수를 평가하였다. 본 연구의 결과, 1단계 자가부식 접착제와 유동성 레진을 이용하여 5급 와동을 충전한 경우, 고유동성 레진보다 저유동성 레진이 낮은 변연 미세누출을 나타내어 5급 와동에서 저유동성 레진의 사용이 추천된다.

  • The purpose of this study was to evaluate the adaptation of light cured glass ionomer cement and composite resin using all- etch technique to tooth structure. In this study, class V cavities were prepared on the buccal surfaces of 10 extracted human premolar teeth with cementum margin and teeth were randomly assigned 2 groups of 5 teeth each. The cavities of glass ionomer cement group were filled with the light cured glass ionomer cement(Fuji II LC) and the cavities of composite resin group were filled with the light cured composite resion(P - 50) using all- etch technique with All- Bond 2. The restored teeth were stored in 100 % relative humidity at $37^{\circ}C$ for 48 hours. And then, the roots of the teeth were removed with the tapered fissure bur and the remaining crowns were sectioned occlusogingivally through the center of restorations. Adaptation at tooth - restoration interface were assessed occlusally, gingivally, and axially by scanning electron microscope. The results were as follows : 1. The adaptation to enamel walls of composite resin restorations using All - Bond 2 showed better than glass ionomer restorations. 2. The adaptation to gingival and axial walls of glass ionomer restorations showed better than composite resin restorations using All - Bond 2. 3. In both groups, occlusal margins of restorations showed better adaptation than gingival margins of restorations.

제 5급 와동에서의 단일용기 상아질 접착제와 자가 산부식 접착제의 상아질에 대한 침투도 평가 (A CONFOCAL MICROSCOPIC STUDY ON DENTINAL INFILTRATION OF ONE-BOTTLE ADHESIVE SYSTEMS AND SELF-ETCHING PRIMING SYSTEM BONDED TO CLASS V CAVITIES)

  • Objective : The purpose of this study was to evaluate the resin infiltration into dentin of one-bottle adhesive systems and self-etching primer bonded to Class V cavities using confocal laser scanning microscope(CLSM). Material and Methods : Forty Class V cavities were prepared from freshly extracted caries-free Human teeth. These teeth were divided into two groups based on the presence of cervical abrasion: Group I, cervical abrasion : Group II, wedge-shaped cavity preparation. Resin-dentin interfaces were produced with two one-bottle dentin bonding systems-ONE COAT BOND(OCB; Coltene$^R$) and Syntac$^R$SPrint$^{TM}$(SS; VIVADENT)-, one self-etching priming system-CLEARFIL$^{TM}$ SE BOND (SB : KURARAY)- and one multi-step dentin bonding system-Scotchbond$^{TM}$Multi-Purpose (SBMP, 3M Dental Products)-as control according to manufacturers' instructions. Cavities were restored with Spectrum$^{R}$(Dentsply). Specimens were immersed in saline for 24 hours and sectioned longitudinally with a low-speed diamond disc. The resin-dentin interfaces were microscopically observed using CLSM. The quality of resin-infiltrated dentin layers were evaluated by five dentists using 0~4 scale. Results : Confocal laser scanning microscopal investigations using primer labeled with rhodamine B showed that the penetration of the primer occurred along the cavity margins. Statistical analysis using one-way ANOVA followed by Duncan's Multiple Range test revealed that the primer penetration of the group 2(wedge-shaped cavity preparation) was more effective than group 1(cervical abrasion) and that of the gingival interfaces was more effective than the occlusal interfaces. In the one-bottle dentin bonding systems, the resin penetration score of OCB was compatible to SBMP, but those of SS and self-etching priming system, SB were lower than SBMP.

제 5급와동에서 광중합 그래스 아이오노머 수복물의 미세변연누출에 관한 연구 (THE MICROLEAKAGE OF LIGHT-CURED GLASS LONOMER RESTORATIVE MATERIALS IN CLASS V CAVITIES)

  • The purpose of this study was to evaluate the microleakage of light cured glass ionomer restorative materials in class 5 cavities. In this in vitro study, class 5 cavities were prepared on buccal and lingual surfaces of forty extracted human premolars and molars on cementum margin. These specimen were randomly divided into four groups of 10 each : Group 1 was Fugi II (control), Group 2 was Fugi II LC, Group 3 was Vitremer, and Group 4 was Dyract. Group 2 was also divided once more into 2 groups of 5 each : Group 2-1 was pretreated with dentin conditioner and Group 2-2 was not. All teeth were restored according to the manufacturer's instructions. After 500 thermocycling between $5^{\circ}C$ and $55^{\circ}C$, the 40 teeth were placed in 2 % Methylene blue dye for 24hr, then rinsed with tab water. The specimen were embedded in clear resin, then sectioned buccolingually through the center of restoration with a low speed diamond saw. The dye penetration on each of the specimen were then observed with a stereomicroscope at 20. The results of the study were statistically analyzed using the Student-Newman-Keuls Methods and the Mann-Whitney Rank Sum Test. Tooth restorative interfaces were evaluated using SEM analysis. Results were as follows, 1. Compared to conventional glass ionomer restoratioqs, all light cured glass ionomer restorations were fairly resistant to microleakage (P<0.05). 2. Groups 3 (Vitremer) and Group 4 (Dyract) were found to be the most resistant, Group 2 (Fugi II LC) fairly resistant, and Group 1 (Fugi II) least resistant to microleakage(P<0.05). 3. No significant differences were found between Group 2-1 and Group 2-2 (P>0.08). 4. With the backscattered SEM analysis, the degrees to which tight bonding occurred were also observed in all the groups except for Group 1. Group 4 showed the highest degree of tight bonding than any other materials used in this study.
