The purpose of this study was to evaluate the dentofacial characteristics and the fost-treatment dentofacial changes of those treated by four premolar extractions and to investigate the factors affecting extraction decision. The sample consisted of 35 patients (27 females, and 8 males) with no more than 7.0mm crowding, diagnosed as Class I protrusion. Pre-treatment and post-treatment lateral cephalograms were evaluated. Computerized statistical analysis was carried out using SPSS/PC+ program. The results were as follows. 1. There was no significant change in skeletal pattern after treatment while there was significant change in dentoalveolar and soft tissue pattern. 2. In pre-treatment skeletal pattern, a tendency toward vertical discrepancy was found. 3. In pre-treatment dental pattern, interincisal angle was $113.11^{\circ}$, U1 to FH was $117.78^{\circ}$ and L1 to A-Pog was 7.94mm. Pre-treatment upper and lower lip position was 2.88mm and 5.43mm to E line. 4. After treatment, interincisal angle increased $14.46^{\circ}$ and upper and lower lip moved back 2.45mm and 3.2mm to E line.(p<0.001) 5. The EI was 138.71 before treatment and 148.2 after treatment.
Objective: To examine the differences in lateral cephalometric characteristics between patients with dental protrusion and crowding in order to determine what factors affect dental protrusion or crowding while both malocclusion types are caused by large tooth size. Methods: Twenty nine individuals with dental protrusion and 22 individuals with dental crowding were enrolled in this study. All subjects had larger teeth than average and Class I molar relationships. Craniofacial characteristics and hyoid bone positions were determined from lateral cephalograms and compared between the two groups. Results: In the comparisons of craniofacial characteristics, the measurements indicating maxillary length and facial convexity showed greater values in the protrusion group than in the crowding group. Comparisons of hyoid bone positions showed that the hyoid bone was positioned more anteriorly and superiorly in the protrusion group than in the crowding group. Conclusions: The results of the present study indicate that some craniofacial characteristics and tongue position may affect the development of dental protrusion or crowding; when an individual has large teeth, dental protrusion or crowding might be determined according to maxillary growth and tongue position.
Anchorage plays an important role in orthodontic treatment especially in the maxillary arch. In spite of many efforts for anchorage control. it was difficult for clinicians to predict the result of treatment because most of the treatment necessitated an absolute compliance of patients, But recently, skeletal anchorage has been used widely because it does not necessitate patient compliance but produces absolute anchorage. In addition titanium miniscrews have several advantages such as ease of insertion and removal. possible immediate leading and use in limited implantation spaces. In this case, a skeletal Class I bialveolar protrusion Patient was treated with standard edgewise mechanics using indirect active P.S.A. (palatal skeletal anchorage). The miniscrews in the paramedian area of the hard palate provided anchorage for retraction of the upper anterior teeth and remained firm and stable throughout treatment This indicates that the PSA can be used to reinforce anchorage for orthodontic treatment in the maxillary arch Consequently, this new approach can help effective tooth movement without patient compliance, when used with various transpalatal arch systems.

A total of 580 patients, who visited and received an orthodontic diagnosis in the Department of Pediatric Dentistry, Seoul National University Dental Hospital from 2017 to 2019, were investigated in this study. The aim of this study was to evaluate skeletal patterns of pediatric orthodontic patients determined with lateral cephalometric analysis and to analyze the relationship between skeletal pattern and probable associated clinical features. Also, the modality of orthodontic treatment for each skeletal classification was investigated to aid in therapeutic decisions. Patients aged 7 year accounted for the largest age group; 54.2% of patients showed a skeletal class I pattern, 22.2% showed a skeletal class II pattern, and 23.6% showed a skeletal class III pattern. Bi-maxillary retrusion for skeletal class I, retruded mandible with normal positioning of the maxilla for skeletal class II, and retrusion of the maxilla with protrusion of the mandible for skeletal class III were the largest subgroups by skeletal pattern. Brachyfacial type accounted for 55.0% of patients, followed by 31.9% of mesofacial type and 13.1% of dolichofacial type. The prevalence of anterior crossbite in the study was 43.3%, higher than that in previous studies.
The purposes of present study were to evaluate changes in models and lateral cephalometric head films during orthodontic treatment and to compare the amount of incisal retraction and anterior movement of molars with the two approaches of the retraction method of canine(sectional canine retractions vs sliding canine retractions) and the anchorage management(head gears vs no head gears, transpalatal arches vs no transpalatal arches and lingual arches vs no lingual arches) and to evaluate changes during orthodontic treatment in models with relation to lateral cephalometric head films. 67 Korean women with Angle's Class I bialveolar protrusion were selected, whose initial chronologic age was above 16 yrs. Models and lateral cephalometric head films were taken before and after orthodontic treatment with four bicuspid extraction. the results were obtained as follows. 1. Significant decreases were observed in intermolar width, arch length and arch perimeter of maxilla and mandible but significant difference was not observed in intercanine width of maxilla and mandible during treatment period. 2. The linear change of the upper incisor to upper lip was 2.84:1 and the linear change of the lower incisor to lower lip was 1.45:1 3. There were no significant differences between the two groups(sectional canine retractions vs sliding canine retractions), the !we groups(transpalatal arches vs no transpalatal arches) and the two groups(lingual arches vs no lingual arches) in the amount of incisal retraction and anterior movement of molars. There were a greater amount of maxillary incisal retraction and a lesser amount of anterior movement of maxillary molars with the use of head gears than no use of head gears. 4. Changes during orthodontic treatment in models with relation to lateral cephalometric head films were obtained as follows : 1) Maxilla Central incisors were moved 3.79mm backward, canines were moved 0.22mm laterally and 3.70mm backward, and molars were moved 0.535mm medially and 2.29mm forward. 2) Mandible Central incisors were moved 3.04mm backward, canines were moved 0.145mm laterally and 3.92mm backward, and molars were moved 0.755mm medially and 1.77mm forward.
With socioeconomic development and change of esthetic recognition, the demand for orthodontic treatment and number of orthodontic patients has been increasing so rapidly. And frequency of malocclusion was changed. So this study was done in an attempt to provide an epidemiologic study so that we can accomodate their orthodontic needs adequately and to obtain the reliable quantitative information regarding the characteristics of orthodontic patients. Distribution and trends were examined in 3,070 malocclusion patients who had been examined and diagnosed at Department of Orthodontics, Dental Hospital, Chosun University over 10 year-period from 1990 to 1999. The results were as follows : 1. The number of patients per year was increasing trend and higher visiting rate in female(56.5%) than in male(43.5%). 2. Age distribution had shown 7${\sim}$ 12 year-old group being the largest(37.9%) and each percentage of 13${\sim}$18, 19${\sim}$24, above-19, 0${\sim}$6 year${\sim}$old group was 32.0%, 19.6%, 7.1%, 3.4%. 3. Hellman dental age IVa which is completion of the permanent dentition showed the highest percentage in male and female. 4. Geographic distribution showed a majority of patients in Kwang Ju(71.0%). Group within the distance 10km from Chosun Dental Hospital was 56.3% and group within 20km was 14.7%. 5. Anterior cross bite showed the highest percentage in chief complaints and percentage of Mn. prognathism and protrusion of Mx. teeth was 12.6%, 12.2%. 6. Distribution in the types of malocclusion according to the Angle's classification had shown; 38.9% for Class I, 20.7% for Class II division 1, 2.0% for Class III division 2, 38.4% for Class III. 7. In the dental vertical dysplasia according to the Angle's classification, deep bite was the most frequent in Class II div.1 and div. 2(24.3%, 56.7%) and open bite in Class III(21.4%). 8. In the skeletal sagittal dysplasia, 39.3% of skeletal Class II was due to the undergrowth of the mandible and 46.3% of skeletal Class III was due to the overgrowth of the mandible. 9. Distribution in orthodontic treatment acceding to the extraction and nonextraction had shown 66.9% for nonextraction case, 33.1% for extraction case, and four first bicuspids have been extracted in the highest percentage(38.6%). 10. Patients who had orthognathic surgery comprised 7.9%, with an increasing trend.
Over the Past decades, the number of Patients seeking orthodontic treatment has increased markedly with socioeconomic development and change of recognition on appearance. The purpose of this study was to provide an epidemiologic data base related to the orthodontic treatment need. We could take an adequate information regarding the characteristics of orthodontic patients, and the changing trends about treatment mordality. Distrubution and treands were Investigated in 676 patients who had been examined and diagnosed at Department of orthodontics, Dental Hospital, Seoul National University from January to June in 1992 and 2002. 1. Sex distribution of patients changed from 1:2.1 to 1:1.5 (male female). 2. In 2002, are distribution had shown $7\~12$ year-old group being the largest$(32.0\%)$ and percentage of $19\~24,\;13\~18,\;over\;25,\;4\~6,\;0\~3$ year-old group were $24.0\%,\;21.6\%,\;14.2\%,\;5.8\%,\;2.4\%$ respctively. Compared with data in 1992, the number of adult patients highly increased. 3. With regard to Angle classification, each percentage of Class I, Class II div 1, Class II div 2, and Class III malocclusion were $25.0\%,\;20.9\%,\;3.4\%,\;and\;48.1\%$ respectively in 2002. 4. Geographic distribution showed that most of the patients visited $(37.0\%)$ lived in northeast of Seoul in 2002. 5. Mandibular prognathism showed the highest percentage in chief complaints. The percentages of crowding and facial asymmetry were $14.2\%\;and\;11.8\%$ in 2002. Patients with facial asymmetry increased significantly. 6. Percentages of patients treated with fixed appliance and orthognathic surgery were $38.0\%\;and\;25.0\%$ in 2002. Patients needed to observe the growth pattern comprised $13.0\%$ with increasing trends. The use of chin cap reduced and the percentage of ortognathic surgery and growth observation increased significantly.
