A Review of Clinical Studies with Herbal Medicine for Depression - Based on Randomized Controlled Clinical Trial - (우울증에 대한 한약물 치료 문헌적 고찰 - 무작위 대조군 임상연구를 중심으로 -)

  • Objectives : This study aimed to investigate frequently used herbal materials among herbal prescription for depression focusing on randomized controlled trial. Methods : Every article relevant to depression was initially obtained from China National Infrastructure(CNKI), Korean database and book hand-searching. Searching keywords were 'depression', 'herbal medicine' and 'randomized controllled trial(RCT)'. Results : Among comorbidity with depression, the most accompanied disease was that of circulatory system. Among sixty-five articles, depression with cerebral vascular disease was twenty-eight. Article about mood disorder was twenty-four. High frequently used herbal materials were Bupleuri Radix(41times), Curcumae Radix(34 times), Acori Graminei Rhizoma and Cnidii Rhizoma(24 times). Conclusions : According to this study, we could know select frequent-used herbal medicine. In a clinical treatment, herbal materials can be added herbal prescription related to depression. As these results, it can be helpful to develop new drugs.

  • All health problems that occur in the circulatory system are refer to cardiovascular illness, such as heart and vascular diseases. Deaths from cardiovascular disorders are recorded one third of in total deaths in 2019 worldwide, and the number of deaths continues to rise. Therefore, if it is possible to predict diseases that has high mortality rate with patient's data and AI system, they would enable them to be detected and be treated in advance. In this study, models are produced to predict heart disease, which is one of the cardiovascular diseases, and compare the performance of models with Accuracy, Precision, and Recall, with description of the way of improving the performance of the Decision Tree(Decision Tree, KNN (K-Nearest Neighbor), SVM (Support Vector Machine), and DNN (Deep Neural Network) are used in this study.). Experiments were conducted using scikit-learn, Keras, and TensorFlow libraries using Python as Jupyter Notebook in macOS Big Sur. As a result of comparing the performance of the models, the Decision Tree demonstrates the highest performance, thus, it is recommended to use the Decision Tree in this study.

A Bibliographic Study on the Therapeutic Effects of Achyranthis Radix in Arthritis (우슬(牛膝)의 관절염(關節炎) 치료(治療) 효능(效能)에 관(關)한 서지학적(書誌學的) 고찰(考察))

  • According to herbalogical bibligraphy and literature, therapeutic effects of Achyranthis Radix on arthritis was as follows, 1. Achyranthis Radix of Amaranthaceae is divided into five species-Achyranthes japonica, A. bidentata, A. longifolia, A. fauriei, Cyathula capitata, C. officialis and the characteristic, taste, channel entry, effects and main treatments were alike. 2. Winefrying stood for repairing treatement method for Achyranthis Radix Before repairing treatment method, Achyranthis Radix had three tastes(bitter, sour, and sweet) and calm and not poisoning characteristic. After repairing treatment method, the bitter taste was disappeared, and calm and not poisoning characteristic was changed into warm characteristic. 3. Effects of Achyranthis Radix were quicking the blood and dispelling stasis, liver-kidney supplement and strengthening musculo-skeletal system. Main treatments were relief of lumbar and knee joint pain, static menstrural block and wind-cold- damp impediment. 4. Contraindication of Achyranthis Radix was sympthom caused by spleen-kidney yang vacuity, upper burner disease and lower burner hemorrhage etc. Being used in pregnant woman, it could incur abortion. 5. Contraindication of Achyranthis Radix was beef, milk and mutton. It's fear was Radix Cynanchi Stauntonii and Semen Plantaginis. It's aversion was the firefly. Herba Taraxaci, Carapax Amydae, Carapax Testudinis and Radix Cynanchi Stauntonii. From above results, I suppose Achyranthis Radix has enough herbalogical foundation and could be used to treat arthritis and it is necessary to make a profound study of it.

  • To diagnose the vascular diseases from the viewpoint of hemodynamics, we need detailed quantitative hemodynamic information of related blood flows with a high spatial resolution of tens micrometer and a high temporal resolution in the order of millisecond. For investigating in-vivo hemodynamic phenomena of vascular circulatory diseases, a new diagnosing technique combining a medical radiography and PIV method was newly developed. This technique called 'Angiographic PIV system' consists of a medical X-ray tube, an X-ray CCD camera, a shutter module for generating double pulse-type X-ray, and a synchronizer. Through several preliminary tests, the feasibility of the Angiographic PIV technique was verified. For in-vivo applications to real blood flows, we developed tracer microcapsules, which were optimized to this system, made of a contrast material of iodine and a matrix material of PVA (polyvinylpyrrolidone). In near future, the Angiographic PIV technique will be used for understanding hemodynamic phenomena of vascular diseases and for their early detection.

An Analytical Study on the Disease Pattern of a Rural Population in Korea (하기(夏期) 무의촌진료(無醫村診療)에서 나타난 우리 나라 일부(一部) 농촌주민(農村住民) 의 질병양상(疾病樣相))

  • This study was made with 1394 clinical records obtained by a mobile charity health team in a rural Korea for 10 days from July 23 to Aug. 1, 1977 to see the disease pattern of the rural population in Korea. The health team consisted of 7 physicians in different clinical departments of the St. Mary's Hospital tried to cover as many as patients in that area by giving full informations and instructions before starting health services. which supposed to minimize the omissions of patients visits in that area. The proportion of each disease as well as its order in terms of the number of patients was reviewed by matching with age and sex distribution of patients, and with the duration of illness (complaints). Major findings obtained in this analysis were as follows; 1. Among 1394 patients, 536 (38.5%) was male and 858(61.5%) was female. As to the age distribution of the patients. those who are in the age group of 10-19 showed the highest proportion of 15.6% and those who are in the age group of 5-9 and 40-49 was the next with the proportion of 15.4% and 15.2% respectively. 2. The most prevalent cause of general morbidity were diseases of digestive system, diseases of nervous system and sensory organs, diseases of respiratory system, and diseases of skin and subcutaneous tissue in order. And their proportions among all patients were 21.8%, 20.7%, 14.5%, and 12.4% in respectively. The order of the diseases ranked by the number of patiens was slightly different in female population from that of total population, i. e. diseases of nervous system and sensory organs was the first. diseases of digestive system was the second, and the diseases of circulatory system was the third. 3. 23.2% of all patients were found to have had symptoms relating to their illness for more or less 5 years, and 18.3%, for more or less 6 months. Looking at the duration of illness by diseases, 28.6% of digestive tract disease patients and had the relating symptoms for about 1 week while 24.3% had had the symptoms for about 5 years, and in diseases of nervous system and and sensory organs, many(33.7%) had had symptoms for relatively long period (more than one year). On the other hand, in diseases of respiratory system, those who had had the relating symptoms for about 1 month was 24.3 % among all patients in this category and those who had had the symptoms for less than 1 week was 32.2%. 4. The duration of illness (complaints) was longer in females than in males and shorter in younger age groups than in older age groups.

Utilization Trends of Health Subcenter for Primary Medical Care in a Korean Rural Area (일개 농촌 면단위지역 주민의 보건지소 의료 이용 추이)

  • This study was analyze through the reports which published on the subject matter of Su Dong-Myun from year of 1972-1993, and analysis of data in 1994 was performed with medical records on the health subcenter by PC-SAS program. The result are as follows: The number of population in Su-Dong Myun(study area) was 5,707 in 1995, 4,641 in 1985 and 5,424 in 1975. In the composition rate of population "0-14" of age group only showed markedly decreasing tendancy from 42.8% in 1975 to 19.1% in 1995. However, "65 and over" showed markedly increasing tendancy from 5.7% in 1975 to 9.8% in 1995. Annual utilization rate showed rapidly increasing tendency from year of 1972 to 1978, such as 314 per showed rapidly decreasing tendency, such as 708 in 1981, 485 in 1984, 272 in 1987, 309 in 190 and in 1993. In the annual age-specific utilization rate, the age group of "0-14" showed the highest rate of 621 per thousand population in 1975, 1159 in 1980, 1021 in 1985 and 538 in 1990. However the age group of "65 and over" showed the highest rate of 481 in 1994. Age specific annual utilization rate showed markedly decreasing tendency in the age group of "0-14" and "15-44", however showed slightly decreasing tendency or same level in the group of "45-64" and "65 and over" from year of 1980 to 1994. In the age specific utilization rate, the lower down the age was, the higher tendency the rate showed, such as 621 per 1,000 population in "0-14" of the age group, 543 in "15-44", 406 in "45-64" and 294 in "65 and over" in 1975. However, the higher up the age was, the higher tendency the rate showed in 1980, 1985 and 1994, except "0-14" of age group. The 5 major diseases were disease of Respiratory system, Gastrointestinal system, Skin and Subcutaneous tissue, Accidents, Poisoning and Violence and Nervous system and Sense organ, in 1975, 1980 and 1985. However, in 1990 and 1994, the 5 major disease were disease of Respiratory system, Gastrointestinal system. Skin and Subcutaneous tissue, Musculoskeletal system and, Connective tissue and Circulatory system. In Composition rate of patient in Su Dong-Myun Health Subcenter by Charged Medical Fee, medical insurance showed almost all the highest rate of 93.9% in year of 1994 and C.H.D.A. of 100% in 1975. Proportion of insurance showed increasing tendency such as 6.6% in 1980, 21.3% in 1985, 69.0% in 1990 and relatively C.H.D.A. showed decreasing tendency.

A Study on Cooperrative Medical Treatment System between traditional Chinese and Western Medicine in China (중국의 한양방협진 현황 (중국중서의결합잡지(中國中西醫結合雜誌)를 대상으로 분석))

  • Objectives: Recently a renovation of the medical-welfare system to reflect the changes of disease spectrum with the demographic changes of society, the increase in income level, and marked concerns for health promotion has been demanded. In accordance with this, attempts have been made to actively integrate traditional medicine based on symptom-differentiated treatment and Western medicine based on disease treatment so that they can complement each other. China has already tried a complementary medical treatment system integrating traditional Chinese and Western medicine. So, this article reviewed major advances in research on integrated traditional Chinese medicine and Western medicine in China. Methods: The authors analyzed data from clinical articles and experimental works in the ' Chinese Journal of Integrated Traditional and Western Medicine' Results and conclusions: Each department attempted to integrate Traditional Chinese Medicine(TCM) and Western Medicine in treatment of various diseases such as malaria, AIDS, and intoxication (rarely found in Korea clinically). Especially in the departments of surgery, dentistry, radiology, and anesthesiology we could see the frequent use of combined treatment. TCM and Western medicine complemented each other very successfully, and the effect of the combined therapy was superior to that of traditional therapy alone. There were diverse methods for therapy in integrated TCM and Western medicine; bath-Tx, physical-Tx, manipulative-Tx, drug -acupuncture, Tibetan medicine, etc. were available in therapy as well as traditional methods such as acupuncture, moxibustion, and negative- Tx. The way of producing Chinese medications were diversified and formulated; making new prescriptions, compounding various kinds of new medicine called' Zhong Cheng Yao' (中成藥) which were easily made, stored, and taken. 'Diagnosis Criteria', 'The effect of TCM Treatment Criteria' were made by committee and broadly used for objectifying diagnosis, discriminating effects of treatments and treatment development, and developing new medical products.

A Study on Utilization Patterns of Oriental Medical Care (한방의료 이용실태에 관한 조사 연구 - 전국 한의원 이용자를 중심으로 -)

  • The objectives of this study were to examine the utilization patterns of oriental medical care and to discover problems in its delivery. The data for this study were collected from a questionnaire survey mailed out from March 10 to April 9 1999 to 6.346 oriental medical clinic. The questionnaires were then distributed to two patients in each clinics. Of these questionnaires. 670 were completed and returned. The major statistical methods used for the analysis were the t-test. ANOVA, and x$^2$-test. The major findings are as follows: 1. Respondents reported visiting oriental doctors twice as often as they visited western doctors(All those completing the survey received the questionnaire at oriental medical clinics). 2. The number of reported visits to oriental physicians according to among gender, age, marital status, education, income and residence. Males, married respondents, the elderly and the residents of rural areas visited oriental physicians more frequently than females, singles, younger respondents and urbanites. Those people belonging to the middle income class and middle education level also more frequently visited oriental physicians. 3. There are several factors that restrict the utilization of oriental medical care, such as the limitation of the scientific diagnostic instrument use commonplace reliance upon western medical techniques, and the perception of high price for oriental medical care. It is very important to focus oriental medical care onto the fields of acupuncture, circulatory system disease, musculoskeletal system ailments, etc. to improve the utilization of oriental medical care. Other policies for the improvement of oriental medical care include the standardization of price, quality and quantity of oriental medicine.

Multiplexed targeting of microRNA in stem cell-derived extracellular vesicles for regenerative medicine

  • Regenerative medicine is a research field that develops methods to restore damaged cell or tissue function by regeneration, repair or replacement. Stem cells are the raw material of the body that is ultimately used from the point of view of regenerative medicine, and stem cell therapy uses cells themselves or their derivatives to promote responses to diseases and dysfunctions, the ultimate goal of regenerative medicine. Stem cell-derived extracellular vesicles (EVs) are recognized as an attractive source because they can enrich exogenous microRNAs (miRNAs) by targeting pathological recipient cells for disease therapy and can overcome the obstacles faced by current cell therapy agents. However, there are some limitations that need to be addressed before using miRNA-enriched EVs derived from stem cells for multiplexed therapeutic targeting in many diseases. Here, we review various roles on miRNA-based stem cell EVs that can induce effective and stable functional improvement of stem cell-derived EVs. In addition, we introduce and review the implications of several miRNA-enriched EV therapies improved by multiplexed targeting in diseases involving the circulatory system and nervous system. This systemic review may offer potential roles for stem cell-derived therapeutics with multiplexed targeting.

A Literary Study on Combination of Yeolgyeol $(LU_7)$ and Johae $(KI_6)$ of Eight Confluent Acupoints (팔맥교회혈(八脈交會穴) 중(中).열결(列缺) 조해(照海)의 배합(配合)에 관한 문헌(文獻) 연구(硏究))

  • Objectives : The aim of this study was to analyze how to treat various symptoms through the combination of Yoelgyoel $(LU_7)$ and Johae $(KI_6)$, according to reviewing the contents and data since Ling Shu (靈樞經) to recent literatures including thirty-five medical books. Methods : It was arranged and considered that the location, needling, and symptoms of each acupoint were described in various literatures before the publication of Chim Kyung Ji Nam (鍼經指南). Through various literature since the Publication of Chim Kyung Ji Nam, it was examined how to be recognized and be referred about Yoelgyoel $(LU_7)$ and Johae $(KI_6)$. Results and Conclusions : The location of Yoelgyoel is the superior 1.5cun at wrist joint striation, medial of extensor carpi radialis longus; the location of Johae is the depression part under foot medial condyle. Yoelgyoel is often used for respiratory organ disease, urinary organ disease, neuopsychiatory disease, musculoskeletal system disease; Johae is often used for urinary organ disease, circulatory organ disease. At Chim Kyung Ji Nam, Yoelgyoel is often used for thoraco-abdominal Pain, gynecological disease, digestive organ disease; Johae is often used for abdominal pain, gynecological disease, digestive organ disease. Therefore, these points are used together for general internal disease. As well, these are not directly continuous with Previous literatures from Chim Kyung Ji Nam. The combination of Yoelgyoel and Johae have been developed to the three categories as follows; it is quoted from as it is; it is reconstructed in the form of song; it has new symptoms enlarged. Consequently, the combination of Yoelgyoel and Johae was not bind to the rule of Up-Bottom harmony (上下配合), but asserted for the rule of Ju-Eng harmony (主應配合), which add specific acupoints to e combination of Yolgyol and Chohae as the complication of the symptoms.
