Cigarette Smoking and Pancreatic Cancer Risk: A Revisit with an Assessment of the Nicotine Dependence Phenotype

  • Background: Cigarette smoking is a well-established risk factor of pancreatic cancer (PC). Although an association between nicotine dependence phenotype, namely time to first cigarette (TTFC) after waking, and the risk of several smoking-related cancers has been reported, an association between TTFC and PC risk has not been reported. We assessed the impact of smoking behavior, particularly TTFC, on PC risk in a Japanese population. Materials and Methods: We conducted a case-control study using 341 PC and 1,705 non-cancer patients who visited Aichi Cancer Center in Nagoya, Japan. Exposure to risk factors, including smoking behavior, was assessed from the results of a self-administered questionnaire. The impact of smoking on PC risk was assessed with multivariate logistic regression analysis adjusted for potential confounders to estimate odds ratios (ORs) and 95% confidence intervals (CIs). Results: Cigarettes per day (CPD) and/or smoking duration were significantly associated with PC risk, consistent with previous studies. For TTFC and PC risk, we found only a suggestive association: compared with a TTFC of more than 60 minutes, ORs were 1.15 (95%CI, 0.65-2.04) for a TTFC of 30-60 minutes and 1.35 (95%CI, 0.85-2.15) for that of 0-30 minutes (p trend=0.139). After adjustment for CPD or smoking duration, no association was observed between TTFC and PC. Conclusions: In this study, we found no statistically significant association between TTFC and PC risk. Further studies concerning TTFC and PC risk are warranted.

  • Objectives: The purpose of this study was to assess the validity of self-reported cigarette smoking status and investigate factors associated with the accuracy self-reported and measured urinary cotinine in Korean adults. Methods: We used data from the $1^{st}$ Korean National Environmental Health Survey (2009-2011) among adults aged ${\geq}19$ years (N=6,246). The survey examined self-reported smoking status, and urinary cotinine was regarded as the biomarker of exposure to tobacco smoke. Urinary cotinine was analyzed using a gas chromatography-mass spectrometry (GC/MS) and data analysis was conducted using IBM SPSS version 20.0, which uses the sample weight and calculates variance estimates to adjust for the unequal probability of selection into the survey. Results: We calculated a cut-off point (53.3 ug/L) by using a ROC (Receiver Operating Characteristic) curve. The smoking prevalence was 24.6% based on self-reported data and 28.2% based on urinary cotinine concentrations. When we assessed the agreement between self-reported and urinary cotinine, we found an average agreement of 97.7% among self-reported smokers and 94.5% among self-reported non-smokers. Among self-reported smokers, factors affected the discrepancy were age, household economic status and average number of cigarettes smoked per day. On the other hand, gender, former smoking experience, and exposure to SHS (second hand smoke) were associated with discrepancies among self-reported non-smokers. Conclusion: These results suggest that self-reported data on smoking status provide a valid estimate of actual smoking status. In future research, we will conduct a continuous monitoring study for reliability verification of the data to reduce potential interpretation errors.

Reasons and Motivations for Cigarette Smoking and Barriers against Quitting Among a Sample of Young People in Jeddah, Saudi Arabia

  • Background: Cigarette smoking is one of the leading causes of death in the world. Tobacco consumption has grave negative consequences for health so that it is important to understand the reasons and motivations towards cigarette smoking and barriers against quitting smoking among the young generation for developing effective policies to control this widespread problem. Materials and Methods: This cross-sectional survey was carried out at the Faculty of Medicine, Rabigh, King Abdulaziz University, Jeddah, Saudi Arabia. A total of 438 young smokers participated from the University and the general population. Data were collected through anonymous, self-administered questionnaires in the Arabic language that contained questions about the reasons and motivations towards cigarette smoking and barriers against quitting smoking. The questionnaire also contained several questions regarding knowledge and attitude of the participants towards cigarette smoking. The data was analyzed on SPSS-16. Results: The mean age of the respondents was $22.9{\pm}3.48$, out of 438 subjects 87 (19.9%) were married, and 351 (80.1%) were unmarried, and 331 (75.6%) belonged to urban areas while 107 (24.5%) were from the rural areas. Responding to a question about a number of cigarettes smoked per day, 31% answered 11-20, 29% answered 21-30, and 25% answered 1-10. Questioned about smokers in the family, 34.5% responded more than one, with 19% for brother and 13% for father. About the reasons for not quitting smoking, 26% described lack of willpower, 25% had no reason, 22% said that people around me smoke, and 15.3% responded stress at home/work. The major motivation for smokers was smoker friends (42%), for 33.8% others, for 12% father/brother and 7.8% media. Conclusions: There are several avoidable and preventable reasons and barriers against quitting smoking. However, knowledge and attitude about smoking were good, and the majority of the smokers were well aware of the associated hazards. Therefore, there is a need to search out ways and means to help them to quit this addiction.

지역 주민의 음주 및 흡연 실태에 관한 연구 (A Study on Drinking and Smoking Habits of the Community Residents)

  • Objectives: To investigate the drinking and smoking habits among the community residents of a city. Methods: We surveyed 1.973 adults from October 1, 2005 to November 16, 2005. The data collected on alcohol drinking habits involved frequency, age of initiation, amount of alcohol consumed, number of attempts to reduce consumption and reasons for reducing it. The data collected on smoking habits involved amount, age of initiation, duration, situations, and the number of attempts to quit smoking. These were analyzed with frequency, percentage, mean, standard deviation, $x^2$-test, t-test and ANOVA. Results: The rate of drinking was 67.4%; 83.1% among males, and 52.5%among females. Among all drinkers, the largest group (29.8%) drank once or twice a week. The mean age of initiation was 21.2 years; 19.9 among males, and 22.8 among females. The average consumed amount was 5.8 glasses; males consumed 7.4 glasses, and females 3.3 glasses. The consumption was the highest among those in fifties. The rate of attempts to reduce drinking was 33.4%, mainly for health reasons. The rate of smoking was 38.0%; 72.4% among males, and 5.5% among females. Among all smokers, the largest group (44.1%) smoked between 11 and 20 cigarettes per day. The mean of the age of initiation was 20.7 years; 20.3 among males, 25.6 among females. The mean smoking duration was 20.9 years; 21.3 years among males, and 16.2 years among females. The main reason for the initial attempt was curiosity. The most common situation for smoking was after dinner. The rate of attempts to quit smoking was 77.1%, mainly out of concern for one's health. Alcohol consumption among smokers was higher than among any other group of nonsmokers. Also, the age of drinking initiation among smokers was lower than among any other group of nonsmokers. Conclusion: This study highlighted the actual data on drinking and smoking habits among community residents. The study can be utilized for creating programs aiming at reducing the consumption of alcohol and tobacco or stopping it altogether. Therefore, it is now possible to assess the indicators for interventions in each population group. Finally, the groups of heavy drinkers and smokers are expected to be given priority for intervention program.

성인 남성에서 흡연력이 건강 관련 대사적 지표 및 식사의 질에 미치는 영향: 2007~2009 국민건강영양조사 자료를 바탕으로 (Relationship among Pack-Years of Smoking, Metabolic Biomarkers, and Diet Quality in Male Adults: From the Korean National Health and Nutrition Examination Surveys, 2007~2009)

  • The purpose of this study was to investigate the relationship between pack-years of smoking, metabolic biomarkers, and diet quality in male adults. We analyzed data from the combined 2007-2009 Korean National Health and Nutrition Examination Survey (KNHANES). The extent of smoking was quantified in pack-years (py), with 1 py equivalent to 20 cigarettes per day for 1 year. Subjects (n=1,318) were recruited and divided into two groups according to age (30~49 years age group, n=695; ${\geq}50$ years age group, n=623). In ach age group, subjects were divided into three groups according to the extent of smoking, which included non-smokers (30~49 years, n=156; ${\geq}50$ years, n=217), light smoker (30~49 years, n=269; py<16; ${\geq}50$ years, n=189, py<31), and moderate smoker (30~49 years, n=270; $py{\geq}16$; ${\geq}50$ years, n=217, $py{\geq}31$). The Nutrient Adequacy Ratio (NAR) and the number of foods (Dietary Variety Score, DVS) were analyzed by using the data from the 24-recall method. The results showed that moderate smokers were significantly more likely to drink alcohol compared with the other two groups. Triglyceride and cholesterol levels were significantly higher in smoker groups. The non-smoker group consumed significantly higher levels of vitamin C and potassium compared to the smoker groups. In the diet quality, vitamin C NAR of non-smoker group was significantly higher than those of smoker groups. In the ${\geq}50$ age group, the MAR of nonsmoker, light, and moderate smoker groups were 0.81, 0.78, and 0.78 respectively. The smoker group consumed significantly greate sugar and oils compared to the non-smoker group. Py showed a significantly positive relationship with serum triglyceride level, and a negative relationship with MAR after adjusting for potential confounding factors. In conclusion, male moderate smokers take too little vitamin C, partly due to low diet quality. Therefore, a well-planed diet must be used to replace the nutrients lost from excluded food groups in smoker.

자율성 증진 금연 프로그램이 흡연 여고생의 일일 흡연량, 타액 코티닌, 지각된 동기 및 자율성에 미치는 효과 (The effectiveness of a Autonomous Regulation Improvement Smoking Cessation Program on the Amount of Daily Smoking, Perceived Motivation, Cotinine in Saliva, and Autonomous Regulation for Girls High School Students who Smoked)

  • 본 논문은 특성화고 여학생을 대상으로 자율성 증진 금연 프로그램을 적용하여 일일 흡연량, 타액 코티닌, 지각된 동기 및 자율성에 미치는 영향을 규명하기 위한 유사 실험연구이다. 연구 대상자는 경상북도 G시와 K시의 흡연 여학생 47명이었으며, 자율성 증진 금연 프로그램은 주 2회 총 8회 동안 실시되었다. 자료 수집 기간은 2013년 4월 1일부터 4월 26일까지 약 1개월이였다. 수집된 자료는 SPSS/WIN 18.0 프로그램을 이용하여 유의수준 5%하에서 서술적 통계, T-검정을 시행하였다. 연구결과 자율성 증진 금연 프로그램을 제공받은 실험군은 제공받지 않은 대조군보다 일일 흡연량이 유의하게 줄어들고(p=.044) 타액 코티닌 음성 반응이 유의하게 많아졌으며(p=.048), 지각된 동기(p=.001)와 자율성(p=.027) 점수는 유의하게 높아졌다. 이는 자율성 증진 금연 프로그램이 학교 금연 프로그램으로 흡연 여고생의 금연율을 높이기 위한 방안으로 적용가능하다는 것을 시사한다.

남자흡연대학생의 비만과 심장자율신경조절의 관련성 (The Relationship between Obesity and Cardiac Autonomic Regulation in College-Aged Male Smokers)

  • 본 연구는 담배흡연자에서 비만과 심장자율신경조절의 관련성 규명을 목적으로 수행되었다. 의료기록 및 외관상 건강한 총 85명의 남자 대학생 흡연자들을 대상으로 자료를 수집하였으며, 모든 참가자들의 안정 시 심장자율신경조절 평가를 위해 단기간 표준프로토콜에 따른 심박변이도(heart rate variability, HRV) 지표들을 산출하였다. 한편 체질량지수, 체지방률, 허리둘레 및 허리/엉덩이 둘레비는 표준 비만지표로서 활용되었다. 연구결과 허리/엉덩이 둘레비를 기준으로 분류된 상위집단은 하위집단에 비해 부교감신경활동을 반영하는 HRV의 rMSSD, pNN50, HF 및 SD1에서 유의하게 낮은 값을 나타내었다. 그러나 HRV의 교감신경활동지수 LF/HF에서는 집단 간 통계적 차이가 없었다. 또한 상관분석 결과 시간, 주파수 및 비선형영역의 HRV 부교감신경 지수들이 체질량지수와 관련하지 않는 반면 허리둘레, 허리/엉덩이 둘레비 및 체지방률과 같은 비만지표들과는 유의한 상관관계를 나타내었다. 특히 허리둘레와 체지방률에 비해 허리/엉덩이 둘레비가 부교감신경활동과 더 밀접하게 관련하는 경향을 나타냈다. 이상의 결과들은 남자 대학생 흡연자들에서 전반적인 비만도를 반영하는 체질량지수와는 독립적으로 중심부 비만이 심장부교감신경 저하와 관련하는 주된 상관 요인임을 뒷받침한다.

광주, 전남지역 일부 대학생들의 식생활 형태와 식품 선호에 관한 연구 (A Study of Dietary Pattern and Food Preference of Unversity Students in Gwangju and Chonnam Province)

  • This study examined the cultural dietary habits as well as attitudes toward food, within other life pattern elements, of students living in Gwangju City and Chonnam Province, Korea. Questionnaires from 1,000 student respondents were analyzed. The survey consisted of questions regarding physical condition and health status, dietary consciousness, food preference, knowledge of food and nutrition, and dietary culture. The results showed that 1.6% of the students considered their own physical condition to be extremely poor, and 2.7% and 2.1% also considered their father's and mother's physical conditions as extremely poor, respectively. Among the respondents, 18.3% were smokers and consumed an average of 14.8 cigarettes per day. With regard to their dietary habits, the students answered that they preferred to eat meals with friends rather than with family members, fruit was chosen for eating over health food supplements, and there was very little participation or interest in various food and cultural festivals. The female students had a tendency to alleviate mental stresses by eating, while the male students performed more physical activity to deal with stress. The female students also preferred cereal, fruit, fast food, and sweetened foods more than the male students. Between the smokers and non-smokers, significantly more non-smokers chose fruit (p<0.01), ethnic foods (p<0.05), and sweetened foods (p<0.05) as compared to the smokers. Body mass index (BMI) had significant positive correlations with soft drink (p<0.01), health food supplement (p<0.01), and alcoholic beverage (p<0.001) consumption, while BMI was negatively correlated with cereal (p<0.01), fruit (p<0.001), and sweetened food (p<0.01) intake. The health status of students was positively correlated with their father's health status (p<0.01), mother's health status (p<0.001), and BMI (p<0.05), as well as cereal (p<0.001), high protein side dish (p<0.01), fruit (p<0.01), vegetable (p<0.01), and traditional food (p<0.001) intake. The average body weight for female students was approximately 5 kg less than the Korean Nutrition Society's standardized weight, therefore, it is strongly recommended that measures be taken to develop a systematic nutrition education program that would help those students who often unintentionally skip breakfast or go on extreme diets to improve body image.

Effects of a supportive workplace environment on the success rate for smoking cessation camp

  • Background: This study was conducted to identify the success rate for smoking cessation over time after participation in a therapeutic smoking cessation camp, and to identify how participant characteristics, including a supportive workplace environment for smoking cessation (SWESC), affect the success rate for smoking cessation. Methods: In all, 296 participants at smoking cessation camps in Ulsan between 2015 and 2020 were investigated. The success rates of smoking cessation after weeks 4, 6, 12, and 24 at camp were investigated. The participants were grouped as workers with an SWESC, and workers without an SWESC, and variables (age, education, household income, marital status, drinking, exercise, body mass index, morbidity, job, number of counseling sessions, cigarettes smoked per day and smoking initiation age) were investigated. Multiple logistic regression analysis was conducted at each time point. In addition, Cox regression analysis was performed to evaluate the variables affecting the success rate for smoking cessation over time. Results: The smoking cessation success rate of workers with an SWESC at week 24 (90.7%) was higher than that for workers without an SWESC (60.5%). Multiple logistic regression was performed to determine the relationship between each variable and the success rates for smoking cessation at week 6, 12, and 24. SWESC was confirmed as significant (p < 0.05) variables for increased success rate for smoking cessation at all 3 time points. After adjusting for all variables, the Cox proportional hazards survival analysis showed a hazard ratio of 6.17 for SWESC (p < 0.001,; 95% confidence interval: 3.08-12.38). Conclusions: At a professional treatment smoking cessation camp, participants with an SWESC showed a significantly higher success rate for smoking cessation. Supportive workplace environment for workers' health is expected to be an important factor for smoking cessation projects as well as other health promotion projects at workplace.

일 지역 성인의 흡연실태 (A Study on the Actual Condition of the Adult-smoking in a Region)

  • To investigate the actual condition of the adult-smoking in Chinan County. I used self-reporting questionnaires among 923 residents living in nine districts selected at random among the sites of eleven eup-myons from December 28. 1998 to January 6. 1999. collected data and analyized using SPSS. The smoking rate of adults in Chinan County was $36.9\%$. There was a significant difference in smoking rate according to the age. gender, education and job among general characteristics. The smoking rate in group of above 40s was about $40\%$ and that of the male took $55.5\%$ which was higher than that of female and the smoking rate of the people having high-school education was the highest$(46.2\%)$. With regard to the career. the smoking rate of the farmers ranked first$(46.1\%)$. Therefore anti-smoking business for smokers should be focused on above 40 years old, males. people having high school education and farmers. As the result of the survey of smoking habits of 340 people who smoke currently, the average smoking begining age was 21.8 and $81.5\%$ among them was between 16-25 years old. $4.4\%$ was under 15. during around the elementary school. Most motives to smoke were as followed; curiosity or taste$(33.6\%)$, relief of stress $(31.2\%)$, peer presure$(26.5\%)$ And considering the amount of the cigarette which is smoked per day, the case which is less than a pack of cigarettes was highest as $75.5\%$ and the people who smoke over one pack of cigarettes took $24.5\%$. As for the kind of tobacco which is smoked, how to smoke and the desire for the smoking, most case was toxin was moderate$(47.8\%)$ or mild$(46.9\%)$. shallowly$(49.0\%)$ or deeply$(46.3\%)$ and under stress$(33.4\%)$. after meals$(27.8\%)$, during drink$(15.7\%)$ and so on. The highest point marked among the factors of smoking motives was 'the reduce of negative emotion' $(3.27\pm1.00)$. followed by 'uncomfortable habits' $(2.87\pm1.02)$, 'addiction' $(2.84\pm1.06)$. 'habit' $(2.74\pm1.12)$. 'pleasure' $(2.70\pm1.04)$. 'stimulus' $(2.59\pm.90)$, 'sensation-exercise satisfaction' $(2.42\pm.97)$. Smokers smoke to reduce the negative emotions when angry in most common case. depressed. anxious. uncomfortable. lone. ashamed or embarrased. and intend to solve the certain problem. etc. Other motives are uncomfortable habit. addiction. habit. pleasure and the pursuit of stimulus. The level of nicotine dependence of adults m Chinan County was 10.57 which amount to 'high' wholly. As the resulf of the level of nicotine dependence score. the people who are low in the level of nicotine was $33.5\%$. the people who are high was $48.2\%$. very high was $18.4\%$. The approach for anti-smoking for smokers should be conducted differently according to the level of the nicotine. For the people who are in low level of nicotin dependence the prohibition of the smoking should be guided through the approach to foster strong will. for those who are in 'high' by acquiring proper method for the prohibition of smoking. and for those who are 'very high' the anti-smoking should be induced by providing proper program because of the possibility of the suffer from abstinence syndrome. The difference of the level of nicotine with the general characteristics of the objects had not statistically significant difference. The difference of the level of nicotine dependence accompanied by smoking habit had statistically significant difference according to the amount of smoke, the kind of tobacos. smoke inhale habit. In other words, the group of heavy smokers had higher level of the nicotine dependence than that of the light smokers relatively and the group which smoke strong taboaco has higher level of nicotine than that of which smoke mild or moderate. And the group of smokers who smoke deeply has higher level of nicotine than that who smoke shallowly or nonswallow. Aa a result of the analysis of the correlation between smoking motive factors and the level of nicotine, there was the indication that people who smoke for the decrease of the negative emotion. habit, pleasure. stimulus. sensation-exercise satisfaction had high level of the nicotine dependence. As the result of the anti-smoking will of smokers. $65.0\%$ of them had prohibition of smoking will. $29.3\%$ had no will to quit smoke. The most important reason for anti-smoking was health. $67.9\%$ had experience to try to quit smoke and the biggest reason to fail to quit smoking was the lack of the will power to keep anti-smoking. $52.8\%$ of them were advised to stop smoking from their spouses or children. only $2.8\%$ were by medical. The people who have the opinion to need anti-smoking education were $69.6\%$. Therefore when the business for the hygine of the mouth for adult is set. it should be centered on the people who have intention of prohibition of the smoking and help to quit smoking by way of other affirmative counter-program not smoking under stress.
