강직성척추염 환자의 1례 증례보고 (A Case Report on the Ankylosing Spondylitis Patient)

  • Objectives : Ankylosing Spondylitis, inflamatic disease, appeared symostosis, pain and dyscinesia in spine and sacroilic joint. It repeated an improvement and a depravation. We observed one patient with Ankylosing Spondylitis. Methods : This patient was juged incurable in points of western medical's View. But he was improved by treating oriental medical therapy. The therapies which used in this patient was acupunture, bee venom, korean chuna therapy and kinesiatrics etc. We measured the R.O.M, special test and degree of pain every day. Results : The R.O.M of extension in lumbar ${vertebrae(0_{\circ}\;{\rightarrow}5_{\circ}\;)\;and\;patrick\;sign(++/-\;{\rightarrow}\;-/-)}$ was improved a little. Degree of pain was improved better than admission. Conclusions : This case has demonstrated that the Ankylosing Spondylitis treated by oriental medical therapies proved good results.

  • Objectives: This study is performed to evaluate the clinical effect of chuna therapy on the neck pain associated with kyphotic cervical curvature. Methods: This study carried out on three patients with neck pain & kyphotic cervical curvature who have received treatment in Depar1ment of Oriental Rehabilitation Medicine, Daejon Oriental Hospital of Daejon University from 28th July 2006 to 3th November 2006. Pre and post treatment, We evaluated the cervical angle, Jackson's angle, Jochumsen method, Ishihara index, VAS and effective score of treatment. Results & Conclusions: Two patients who received Chuna treatment recovered cervical curvature and improved neck pain. But One patient who dosen't received Chuna treatment was no improvement in cervical curvature & neck pain.

  • Objectives : This study was performed to evaluate the effects of oriental medical treatments and Chuna treatment on patients with pain and limited range of motion(LOM) about shoulder subsequent to scapular fracture. Methods : Two patients suffered from pain and LOM about shoulder after scapular fracture, one is scapular body fracture with multiple rib fractures, the other is only intraglenoid fracture were treated with Chuna therapy, acupuncture, herbal medicine, physical therapy and measured by VNRS(Verbal numerical rating scale) and ROM (Range of motion). Results : After oriental medical treatments and Chuna treatment, we found out a recovery from two patients suffered from pain and LOM about shoulder subsequent to scapular fracture. Conclusions : Through this study, we suggest that oriental medical treatments and Chuna treatment were effective to cure patients with pain and LOM subsequent to scapular fracture.

림프부종에 대한 추나요법의 효과: 체계적 문헌고찰과 메타분석 (Chuna Manual Therapy for Lymphedema: A Systematic Review and Meta-analysis)

  • Objectives Purpose of our study is to investigate the preventive and therapeutic effect of Chuna manual therapy (CMT) for lymphedema. Methods A study search of 10 databases was performed. We included the randomized controlled trials (RCTs) which performed CMT for lymphedema in this study. The keywords used were 'chuna' or 'tuina' and 'lymphedema'. Two independent authors rated study quality and risk of bias using the Cochrane risk of bias tool. Results 9 appropriate RCTs were remained after screening. The therapeutic effects of the experimental group was statistically higher than that of the control group with functional exercise or taking western medicine. Subjective symptom score was also lower in the CMT group. Conclusions These results suggests that CMT has sufficient evidence that it is more effective in prevent or alleviating symptoms of lymphedema than conventional treating methods. However, due to the high risk of bias of included studies, further researches are needed with higher quality of evidence.

신바로 약침의 국내 임상 연구 동향: 스코핑 리뷰 (Domestic Clinical Research Trends of Shinbaro Pharmacopuncture: Scoping Review)

  • Objectives This study aimed to investigate domestic clinical research trends in Shinbaro pharmacopuncture, identify diseases managed using Shinbaro pharmacopuncture, and suggest the direction for future studies to increase its clinical utilization. Methods This study used five steps proposed by Arksey and O'Malley and the PRISMA-extension for scoping reviews checklist. We examined published literature on Shinbaro pharmacopuncture studies reported until June 5, 2023, in the following eight databases (Research Information Sharing Service, Science ON, Oriental Medicine Advanced Searching Integrated System, KMBASE, The Society of Internal Korean Medicine, PubMed, EMBASE, and the Cochrane Library). The search terms used were 'Shinbaro' or 'Sinbaro'. Results A total of 47 studies were included in our analysis. Of these, 37 (78.7%) were interference time series studies. Shinbaro pharmacopuncture was the most frequently used treatment for lumbosacral disease (n=15). In the facial area, ST4 and ST6 were used in five out of six studies, and in the shoulder area, TE14 and LI15 were used in all studies. Nine of the 15 studies in the lumbosacral area used the EX-B2. The other parts mostly used the pressure points. Compared to other pharmacopuncture methods, the treatment effect was similar to that of bee venom, and faster than that of jungsongouhyul. Conclusions This is the first scoping review of Shinbaro pharmacopuncture therapy in South Korea. Studies with a high level of evidence based on sole treatment, large capacities, and standardization of Shinbaro pharmacopuncture need to be conducted to increase its clinical utilization.

한의 의료행위 급여 항목 확대 방안 모색을 위한 전자우편 설문조사 (An E-mail survey for expanding the basic benefit package of Korean medicine in Korean national health insurance)

  • Objectives: This study aimed to investigate the opinions of Korean medical doctors on how to elucidate possible remedial measures for expanding the health insurance benefits coverage item of Korean medicine (KM). Methods: An online survey was conducted to all members who had registered e-mail address in the association of Korean medicine from 1 to 17 November, 2016. Statistical analysis was performed and odds ratio with 95% confidence interval were calculated by each subgroup. Results: A total of 743 members answered the questions and the response rate was 4.1%. The priorities for expanding health insurance benefits were as follows: thermographic imaging, Sasang constitution typing test, and pulse wave among examinations; pharamacopuncture, embedding acupuncture, and acupotomy among procedures; Chuna manual therapy, manual therapy for meridian muscle, and Daoyin exercise therapy among manual therapies; low-frequency electrical therapy, traction, paraffin bath, and light therapy among physical therapies; and aromatherapy, enema therapy, and color therapy among activities of KM. Conclusions: It should be covered by the national health insurance (NHI) of KM that thermographic imaging, pharmacoacupuncture, Chuna manual therapy, low-frequency electrical therapy, aromatherapy as a top priority. We also suggest that basic medical tests, such as blood, urine, or imaging, should be included in the coverage of the NHI of KM. It is necessary to review the expertise and public opinions about the plans and priorities for the conversion of the desired medical services to be covered by the NHI.

봉독요법(蜂毒療法)의 근골격계질환(筋骨格界疾患) 치료기전(治療機轉)에 대한 문헌적(文獻的) 고찰(考察) (The Study of Literature Review on Mechanism of Bee Venom Therapy for Musculo-skeletal Disorder)

  • Objectives : There have been many studies of the effect of Bee Venom therapy about arthritis, but no one study was reported about its whole functional mechanism to musculo-skeletal system. This study was designed to investigate the effect, Indication, and side effect of Bee Venom therapy on musculo-skeletal disease by literature review of articles. Results : The effects of Bee Venom therapy to musculo-skeletal system are divided to Anti_inflammatory effect and Anti-nociceptive effect. Anti_inflammatory effect is achieved through competitive chemotaxis, immuno-regulation, increasing of cortisol secretion by stimulating hypothalamus-pituitary gland-adrenal cortex axis. Anti-nociceptive effect is achieved by Anti-inflammatory mechanism and it works similar to anti-nociceptive effect of the acupuncture acting on central and peripheral nociceptive transduction system. The Bee Venom therapy could cause severe side effect, for example, hypersensitivity and anaphylaxis, injury to central nerve system and cardiovascular system, peripheral blood system, and renal dysfunction. Conclusions : With its Anti-inflammatory and Anti-nociceptive mechanism, Bee Venom therapy is considered that has good effects to autoimmune disease, chronic inflammation of various musculo-skeletal disease and various pain syndrome. But the clinician must be careful for its side effects.

경근요법(經筋療法)을 통한 근원성 신경 포착 증후군의 치료 (Investigation on the Meridian-Muscle Therapy for Myogenic Nerve Entrapment Syndrome)

  • Objectives : Scalenus anticus syndrome and Piriformis syndrome is representative of myogenic Nerve entrapment syndrome, and their clinical symptoms are similar to HIVD(herniated Intervertebral disc) of cervical or lumbar spine. But, distinguished by muscle test, these syndrome apply to Meridian-muscle therapy. Methods : Meridian-muscle therapy consists of Ashi(阿是)-point therapy, taping therapy, myofascial release technique, manipulation, their based on the traditional meridian-muscle theory. This theory is similar to myofascial pain syndrome in western medicine. The study population consisted of 9 patients who were already diagnosed as Nerve entrapment syndrome with radiological examination & physical examination and muscle test. The evaluation of clinical outcome was done by Visual Analogue Scale (VAS) and Pain Assesment Questionnaire(PAQ). Results and Conclusions : After treatment, All patient's VAS is decreased as $2.11{\pm}1.59$ and the evaluation of clinical effect was excellent(6 cases) or good(3 cases) according to PAQ. Conclusively, Meridian muscle therapy is efficacious against Nerve entrapment syndrome.

매선요법 단독치료로 호전된 만성 요통 환자 4례 임상증례보고 (A Clinical Four Case Studies on Chronic Low Back Pain Treated by Needle Embeeding Therapy)

  • Objectives : The aim of this study was to evaluate clinical effects of Needle embeeding therapy on Chronic low back pain. Methods : The patients have been treated only with Needle embeeding therapy for once a week for 4 weeks. To estimate the efficacy of Needle embeeding therapy and functional changes of patients we have checked VNRS and ODI every week. Results : The Needle embedding therapy reduced VNRS and ODI score. Conclusion : The result suggests Needle embeeding therapy can be effective treatment on patients with Chronic low back pain.

외상 후 슬관절 강직에 대한 복합 한의치료의 경과: 증례보고 (Combination of Korean Medicine Therapy to Treat Posttraumatic Knee Stiffness: A Case Report)

  • The purpose of this study is to illustrate the potential for clinical improvement in treating a patient with a case of Posttraumatic Knee Stiffness with a combination of Korean medicine therapy. We treated the patient with acupuncture, electroacupuncture, CHUNA manual therapy and Interferential Current Therapy from $5^{th}$ December 2016 to $13^{th}$ January 2017 (total 29 times) by evaluating knee function with VAS score. After six weeks of treatment, this patient achieved effective outcome following the technique, showing that clinical symptom as able to walked and pain was relieved, VAS changed from 10 to 2 and the knee flexion ROM (active/passive) changed from 30/60 degrees to 120/140 degrees. This result shows that Korean Medicine therapy may be an effective option for Posttraumatic Knee Stiffness. Further clinical studies are needed to clarify the effect of Korean Medicine therapy on Posttraumatic Knee Stiffness.
