A Survey on the Treatments used in Oriental Obsterics & Gynecology (한방부인과질환에 사용되는 치료방법에 대한 조사연구)

  • Purpose: In order to know therapies used in the field of Obsterics & Gynecology of Oriental medicine. Methods: This survey investigated papers about clinical study, literature investigation for therapeutic methods and treatment reports published from 1999. 2${\sim}$2008. 11. Results: In the clinical studies, acupuncture is more frequently used than herbal medicine. But in the treatment reports, herbal medicine is most frequently used except some cases. Besides acupuncture and moxibustion was used frequently. In the treatment reports various acupuncture methods were used except vaginal bleeding or abdominal pain cases. Whole body acupuncture was used mainly. Pharmacopuncture, Sa-am Acupuncture, auricular acupuncture was also used frequently. And Dong's acupuncture, Hwa acupuncture was used sometimes. Moxibustion was tend to be used for dysfunctional uterine bleeding, menopausal disorder, urinary incontinence, vaginal bleeding or abdominal pain during pregnancy period, hyperemesis, recurrent miscarriage, postpartum disease, lochiorrhea. Also other treatment methods was used such as external therapy, aromatherapy, herbal retention enema, fumigation, electric lipolysis acupuncture, Chuna manual medicine, obesity management, acupuncture at uterus cervix, magnetic innervation therapy, exercise, cupping and physical therapy. Conclusion: These results suggests that acupuncture, moxibustion and herbal medicine are most frequent therapy in the field of Obsterics & Gynecology of Oriental medicine. And other therapy are used such as external therapy, aromatherapy, herbal retention enema, fumigation and so on.

A Delphi Study of Korean Medicine for Anorexia in Children for Clinical Practice Guidelines (소아 식욕부진의 한의표준임상진료지침 개발을 위한 델파이 연구)

  • Objectives This study aimed to develop consensus-based recommendations for establishing standard clinical practice guidelines for pediatric anorexia through the utilization of a Delphi study. Methods We analyzed existing randomized controlled trials for pediatric anorexia treatment using the Delphi method-a structured process for achieving consensus among a panel of experts. A questionnaire was distributed among a select panel of nine specialists in the field. Results The initial Delphi round led to consensus on 30 distinct recommendations; however, consensus was not reached for 19 other recommendations, prompting a second Delphi round. In the subsequent round, adjustments were made based on feedback from the initial round, and deliberations were held on recommendations that previously lacked consensus. Following these adjustments, consensus was achieved on all recommendations. Additionally, a third Delphi iteration was conducted to address three specific queries that required amendment due to a reevaluation of the evidence levels of certain recommendations. In total, three Delphi rounds were carried out to produce informed recommendations related to the diagnosis, treatment, and general management of anorexia. Conclusions This investigation successfully generated evidence-based recommendations for the diagnosis and treatment of pediatric anorexia. The recommendations encompassed various practices, including herbal medicine, acupuncture, moxibustion, cupping, and Chuna manual therapy, which can be integrated into clinical settings.

Analysis of the Research Trends of Pelvic Malposition in Korean Clinical Research (골반부와 관련된 국내임상연구동향 고찰 -골반부의 변위를 중심으로-)

  • Objectives The purpose of this review was to analyze the research trends in clinical research related to pelvic malposition published in Korean medicine journals. Methods We searched articles in Korean databases (OASIS, NDSL, RISS, and KTKP), and sorted the articles by publishing date, pelvic malposition type, field of study, and measurement index. Results 1. 25 original articles and 9 case report articles were reviewed. 2. In recent years, this topic has been studied more frequently, with an increasing number of original articles published. 3. 16 articles analyzed pelvic malposition type. And half of the articles focused on ilium. 4. The original articles have figured out the correlation between pelvic malposition measurement indices and other measurement indices or diseases. 5. Treatment tools, such as chuna therapy, the mckenzie method, devices for correcting malposition, acupuncture were used in the case report articles. 6. 29 measurement indices, such as Ferguson's angle, Ilium shadow measurement, and Iliac height difference were used. Conclusions According to the literature, pelvic malposition is related to other diseases and measurement indices, and manual medicine on pelvic malposition can be effective. However, due to the lack of sufficient empirical evidence from medical articles to support this, further clinical research should be conducted.

The Research Trends of Pharmacopuncture Therapy on Cervical Pain Caused by Traffic Accidents

  • Objectives: The purpose of this study is to comprehensively review Korean domestic studies and investigate the research trends of pharmacopuncture therapy on cervical pain caused by traffic accidents. Methods: Domestic studies between February 1999 and May 2020 from four Korean databases were searched with combinations of keywords 'cervical pain', 'traffic accident', 'whiplash injury', and 'pharmacopuncture'. Results: 17 studies were selected for review, including 7 randomized controlled trials, 5 retrospective observational studies, 3 case reports, and 2 non-randomized controlled trials. Each study was reviewed by published year, study type, types of pharmacopuncture solutions, selected acupuncture points, dosage of pharmacopuncture solutions, frequency of treatment, concurrent treatments, outcome measurements, and the effectiveness of pharmacopuncture therapy. The results are as follows: (1) It showed that the total number of published studies had increased slightly in the last 10 years compared to the previous decade. (2) The pharmacopuncture solutions used in the studies were in the following order: Bee-Venom (蜂毒), Jungsongouhyul (中性瘀血), Hwangryunhaedoktang (黃連解毒湯) and Soyeom (消炎). Frequently used acupuncture points were GB20, GV21, A-shi point, GV16, EX-B2, and SI15 in order. (3) The most commonly used total injection dosage was 1.0 cc at a time, and the frequency of treatment was twice a week. (4) Concurrent treatments such as acupuncture, herbal medication, physical therapy and Chuna manual therapy were performed in all 17 studies. (5) Pharmacopuncture therapy showed positive effects on cervical pain caused by traffic accidents in all 17 studies reviewed. Conclusion: Pharmacopuncture therapy was effective in cervical pain caused by traffic accidents in all 17 studies selected. Further studies will be needed using more larger scales and more objective data to confirm the effectiveness of pharmacopuncture therapy and to generalize its application.

  • Cervical dystonia (CD) is a disorder characterized by sustained or intermittent, involuntary muscle contractions which cause twisting, repetitive movements and abnormal postures. In this case report, a CD patients was treated with transfer energy capacitive and resistive (TECAR) therapy and conventional Korean medicine, which consists of acupuncture, Chuna manual medicine, pharmacopuncture and herbal medicine. For outcome measures, this study evaluated Cobb's angle, EuroQol five dimension scale (EQ-5D), numerical rating scale (NRS), neck disabillity index (NDI) and cervical range of motion. As a result, improvements were found in Cobb's angle ($18.65^{\circ}$ to $15.90^{\circ}$), EQ-5D score (0.808 to 0.862), NRS (5 to 3), NDI (16 to 8) and angle of cervical lateral bending ($15^{\circ}$ to $25^{\circ}$). In conclusion, this study shows that combined therapy of conventional Korean medicine treatment and TECAR therapy can be effective for CD patients.

Effects of Korean Medicine Treatment for a Patient with Lumbar Radiculopathy Diagnosed with Bertolotti's Syndrome: A Case Report (베르톨로티 증후군으로 확인된 요추 신경병증 환자에 대한 한의학적 치료 효과: 증례보고)

  • Bertolotti's syndrome is a rare spinal disease and it is known to cause low back pain due to a lumbosacral transitional vertebra. There has been rare study of Bertolotti's syndrome. This study reports the effects of Korean medicine treatment on the patient who was diagnosed on Bertolotti's syndrome. The patient was treated with Korean medicine treatment including acupuncture, pharmacopuncture, herbal medicine, and Chuna manual therapy during 40 days. Numerical rating scale (NRS), Euroqol five dimension (EQ-5D) index, Oswestry disability index (ODI), lumbar range of motion were used for assessment. After treatment, low back pain NRS decreased from 3 to 2, and low limb pain NRS decreased from 5 to 2. EQ-5D index, ODI and lumbar range of motion also were improved. This study shows Korean medicine treatment can be an effective care for Bertolotti's syndrome.

A Case Report of Combined Korean Medicine Treatment of Spinal Stenosis with Lipomatosis (지방종을 동반한 척추관협착증 환자의 복합한방치료 치험 1예)

  • Spinal epidural lipomatosis is a rare disorder characterized by overgrowth of fat in the extradural space, causing spinal stenosis with compression of the neural elements. This study reports on the effectiveness of Korean medicine treatment on a patient who was diagnosed on lumbar stenosis with lipomatosis. The patient was treated with Korean traditional medicine including pharmacopucture, acupuncture, Chuna manual treatment, and Korean herbal medicine, cupping. Numerical rating scale (NRS), Oswestry disability index (ODI), Euroqol five dimension (EQ-5D) index, lumbar range of motion were used as objective tools for evaluating the patient's progress. Back NRS decreased from 6 to 3. In the case of lower limbl radiation NRS, 5 was reduced to 3 upon discharge. EQ-5D index also increased from 0.751 to 0.766. For ODI, the score dropped from 26.67 to 24.44 on hospitalization. As a result, clinical improvements were found in a patient. In conclusion, this study shows that Korean medicine treatment can be considered as effective conservative care for spinal stenosis with lipomatosis.

Survey on Satisfaction and Symptom Improvement of Korean Medicine Treatment in 122 Cases by Traffic Accident (교통사고 환자 122례에 대한 한방치료 만족도 및 호전도 조사)

  • Objectives: This study conducted a survey research to characterize traffic accident(TA) patients seeking Korean medicine treatment and to analyze the symptom improvement and satisfaction scores. Methods: A survey was conducted in 122 outpatients, who visited OO University Korean Hospital due to TA-associated symptoms from November, 2017 to May, 2018. The questionnaire included information on patient demographic characteristics, accident circumstance details, pain levels, reason for treatment selection, treatment methods, treatment purpose, symptom improvement and satisfaction. All statistical analyses were performed using Windows SPSS version 20.0 Results: The characteristics of traffic accident patients using Korean medical institution were usually accidents that occurred while driving slowly, and minor injuries like a sprain. The most important consideration in choosing Korean medical institution was its past experience. The biggest reason for switching treatment from Western medicine to Korean medicine was for diverse treatments. Satisfaction with Korean medicine was measured between very satisfaction and satisfaction. Patients showed the highest satisfaction with acupuncture, followed by pharmacopuncture and chuna manual therapy. Most treatments were measured between very satisfaction and satisfaction. After treatment, 95.90 percent of the patients said they would recommend Korean treatment. Conclusions: Although this study has limitations as research in survey format, we intended to analyze determining factors for the use of Korean medicine treatment through satisfaction, symptom improvement, and Numeric rating scale (NRS) change.

Effects of Conservative Korean Traditional Medical Treatment on Lumbar Intervertebral Disc Herniation in 12 Adolescents : A Retrospective Study (청소년기 요추간판탈출증에 대한 한방 보존적 치료 12례의 후향적 분석)

  • Objectives : This study was performed to assess the effectiveness of conservative Korean traditional medical treatment on lumbar intervertebral disc herniation in 12 adolescents. Methods : Data were collected from adolescent patients diagnosed with lumbar intervertebral herniation by MRI, hospitalized at Jaseng Korean Medicine Hospital from January 1, 2014 to December 31, 2015. A total of 12 patients were included in the study. Patients were treated by acupuncture, pharmacopuncture, electroacupuncture, herbal medicine, chuna manual therapy, physical therapy during hospitalization period. To measure treatment outcomes, a verbal numerical rating scale (VNRS), Oswestry disability index (ODI), range of motion (ROM), Straight leg raising test (SLR), and EQ-5D were used. Results : The average age of the participants was $16.05{\pm}1.62$. The average of duration of symptom was $19.25{\pm}26.83(weeks)$ and the average length of hospitalization was $23.8{\pm}16.4(days)$. 2 patients (17 %) suffered from only lumbar pain, while 10 patients (83 %) suffered from lumbar and leg pain. No participant suffered from only leg pain. 8 patients (67 %) were diagnosed with disc herniation in one segment, and 4 patients (33 %) were diagnosed with disc herniation in multiple segments. A total of 17 disc segments (28 %) were herniated. The most herniated lumbar level was L4-5. 3 segments were diagnosed with bulging (17 %), 6 with protrusion (35 %), and 8 with extrusion (47 %). Extrusion was the most frequent herniated type. After treatment, the average VNRS of lumbar pain significantly decreased from $5.58{\pm}1.62$ to $2.91{\pm}1.56$ (p<0.001) and the average VNRS of leg pain significantly decreased from $5.16{\pm}2.51$ to $3.08{\pm}1.8$ (p<0.001). ODI significantly decreased from $48.87{\pm}18.72$ to $28.57{\pm}15.05$ (p<0.05), and EQ-5D significantly increased $0.58{\pm}0.31$ to $0.80{\pm}0.12$ (p<0.05). Range of flexion significantly improved from $61.25{\pm}32.62$ to $68.33{\pm}26.22$ (p<0.05). Conclusion : Conservative Korean Traditional Medical Treatment may be effective in the treatment of lumbar intervertebral disc herniation in adolescence. However, more case reports and clinical research are needed.

Correlation between Lumbar Malposition and Disc Herniation in Lumbar Disc Herniation Patients: Focused on L4-L5, L5-S1 (요추 추간판 탈출 환자에서 나타나는 요추부 변위와 추간판 탈출의 상관관계: L4-L5, L5-S1 요추 간 추간판 탈출을 중심으로)

  • Objectives This study was conducted to find out the correlation between lumbar malposition and lumbar disc herniation. Methods Among the patients who visited Gwangju Jaseng Hospital of Korean Medicine from January 2019 to January 2021, taking 92 patients under age 60 who had the records of X-rays and magnetic resonance imaging (MRI) images as the research subjects, Cobb's angle was measured in anterior-posterior (AP) view and lateral (LAT) view, the number of displacements was scored by listing categories defined by The Korean Society of Chuna Manual Medicine for Spine and Nerves. The degree of lumbar intervertebral disc herniation was expressed as a percentage according to the method of Kato, etc., and the correlation between each factor was analyzed. Results AP curvature and MRI herniation index showed significant positive correlation in L4-L5 level but there was no significant difference in L5-S1 level. LAT curvature and MRI herniation index had no correlation in L4-L5 level, but there was a significant negative correlation in L5-S1 level. Malposition score and MRI herniation index had a significant positive correlation in L5-S1 level, whereas there was no correlation in L4-L5. Conclusions As a result of the study, AP curvature and MRI herniation index showed a significant positive tendency in L4-L5 lumbar spine, and LAT curvature and MRI herniation index showed a significant negative tendency in L5-S1 lumbar spine. Malposition score and MRI herniation index were found to have a significant positive tendency in L5-S1 lumbar spine.
