The increased potential for the lifespan of a child with cancer is largely due to advances in drug treatment, radiation treatment, and surgical techniques. In this generation cancer has become associated with chronic illness. Therefore supportive nursing intervention for children with cancer is needed to promote normal growth and development. This study was designed to develop and test the supportive nursing intervention program for promoting body image and self-esteem of children with cancer The supportive nursing intervention program involved emotional, physical, informational, and social support. The subjects were 41 children with cancer(20 in intervention group, 21 in control group) in K city. Measurements were taken concerning body image and self-esteem from both groups during pre and post test The data were analyzed using Cronbach's alpha, x$^2$-test, paired t-test and t-test. The results were as follows : The intervention group had a more positive body image as measured by the Body Cathexis Scale (t=2.436, p=.020) and a more self esteem as measured by the Cpopersmith self-inventory (t=2.768, p=.009) than the control group at post test. According to this study, the supportive nursing intervention program was effective for promoting the body image and self-esteem of children with cancer. Repeated research is needed to develop a refined supportive nursing intervention program for children with chronic illness.
Parents of handicapped children are experiencing difficulties in their children's care, social isolation, change of life style and lirnited leisure time. Because the parents should take care of the children's daily life, they have lots of psychological and physical stress. Chronic stress of parents puts stress to the other family members and affects the development of children with handicap. The purpose of this study were to identify the level of stress in each of parents of children with motor problem, the characteristics of the children and general information related with the children, and to analyse the stress by reasons. Specially organized questionnaire were used for an investigation method. "Test of stress in mother who has children with chronic illness" by Kim Hee-soon were modified and used. The questionnaire answered by 43 mothers and 35 fathers were analyzed. Data analysis includes frequency analysis, Pearson correlation coefficients, paired-samples t-test and MANOV A by SPSSWIN. The results were as follows: 1) Degree of handicap was most moderate (46.5%), level of motor development was most pull to walk (34.9%), and combined handicap was 69.8%. 2) Sexual distribution represented that 51.2% male and 48.8% female. The cost of physical therapy was 69.8% in no more than 100,000 won. 3) The mean of age, for the mother was 32.8 years and 35.3 years. Level of motor development that mother and father expect was 88.4%, 83% walk alone. 4) Both mother and father experienced stress in other of Part II (changes in father was the illness status of the child and difficulty in taking care of child), Part III (prognosis of the child's condition), Part I (social-personal relationships and the responsibility of the care givers). In the total score of stress, mother's stress is indicated higher level than father's stress. 5) There was no correlationship between characteristics and stress of mother and father. 6) There was no statistically significant difference between characteristics and related general information of children with handicap and stress of mother and father. As a results, the mother of children with handicap are experiencing more stress than the father. Both of parents have the most difficulties in the changes in the illness status of the child and difficulty in taking care of child. This study can be used as resources of education, therapy and counselling for children with handicap and their parents. This study, also, can be used to encourage the quality of Iife for the children with handicapped and their family.

Objectives: To identify factors related to physical health monitoring of patients with schizophrenia spectrum disorder. Methods: A total of 172 patients with schizophrenia spectrum disorder registered in mental health welfare centers and rehabilitation facilities in Gwangju were recruited. Physical health monitoring was defined by two health behaviors; fasting blood tests within recent 2 years in all participants and routine medical check-ups covered by national insurance within recent 5 years in participants aged 40 years or older. Demographic and clinical characteristics including overweight, metabolic syndrome and knowledge about physical illness were compared according to physical health monitoring. Results: Prevalence of overweight and metabolic syndrome were 62.8% and 40.1%, respectively. The rates of fasting blood tests and routine medical check-ups were 34.9% and 67.9%, respectively. The rates of fasting blood tests were significantly higher in general hospital and university hospital compared to mental hospital or private clinic. Rates of routine medical check-ups were significantly lower in individuals using daily rehabilitation service and smokers. Knowledge about cancer and chronic illness were significantly better in individuals receiving routine medical check-ups compared with those not receiving it. Conclusion: Education about physical health should be integrated to mental health service in community mental health center.
The physical therapy that is in use nowadays, especially thermotherapy, aquatherapy and manual therapy, has been around since humans started to live on the earth. In primitive times, when men were injured while hunting, they washed the wound in clean water and dried it out in the sunshine. They also soaked their bodies in hot springs to decrease physical fatigue. This primitive Korean traditional physical therapy was in common use among people throughout the Koguryo, Koryo and Chosun dynasties. Historical records show that some kings in those days recommended to their men these methods to treat any chronic disease or illness. This was often thought of as a positive means of treatment. Korean traditional physical therapy has been developed, and the scientific effects of therapy have been proven. Korean traditional physical therapy is used extensively in hospitals and clinics to treat many kinds of diseases and illnesses. Unfortunately some methods that have not been medically substantiated as an effective treatment are being used by practitioners. In this situation there is an urgent need for scientifical research and data regarding any unproven physical therapy techniques.
The lack of a comprehensive theory describing the mechanism of adaptation scientifically has been one of the limiting factors for the development of nursing intervention of patients with chronic illness. Since Roy's theory provides the general conceptual framework depicting adaptation process with structural stimuli and control mechanism, it is appropriate to understand the process of adaptation. But in Roy's theory, the propositions about cognator and regulator as control mechanisms are not clearly defined. For this reason, most of the previous researches applying the Roy's theory have disregarded the study of cognator and regulator. For the patients with chronic illness such as rheumatoid arthritis, it was reported that adaptation states were different for the same stimuli due to the difference of the control mechanism. Moreover in nursing it is important to identify the control mechanism which can be and must be intervened by nurses. It was the Lazarus and Folkman's theory that proposed the control mechanism. They suggested that individual differences in the reaction against the perceived stress are due to the difference in appraisal and coping. Therefore, the synthesis of Roy's and Lazarus and Folkman's propositions might help to clearly understand the mechanism of adaptation. From this point, a theoretical framework has been developed and tested. The subjects were the 297 patients who had been diagnosed rheumatoid arthritis and attended the outpatient clinic. A hypothetical prediction model of adaptation was tested by the covariance structure analysis with PC-LISREL 7.13. As a result, the overall fit was good($x^2$=78.83, p=0.00 ; GFI=0.96 : AGFI=0.90 ; RMR=0.04) for the hypothetical model. In the final model added GA(5, 1), the overall fit was increased ($x^2$=57.82, p=0.003 ; GFI=0.97 ; AGFI=0.93 : RMR=0.036). Except the fact that illness symptoms affected physical adaptation directly, it was supported that focal and contextual stimuli affected physical and psychosocial adaptation through appraisal and coping. Therefore, it was asserted that the synthesis of two theory's propositions was appropriate. So this model would be useful for the theoretical framework in the nursing practice. And this study synthesizing and testing the theory might contribute to establish nursing's scientific base.
The effects and roles of social support and stressors (stressful life events and chronic strains) on psychosocial distress were examined in a model with data from a sample of the white-collar workers (N=416) in Korea. The findings of this study present that mean levels of psychosocial distress were higher in the female, youth, unmarried and divorced, and low economic status (monthly income). The analysis also shows that, as expected, stressful life and chronic strains as stressors are positively related to occurrence of psychosocial distress, and social support is negatively related to psychosocial distress. Further, the contribution of social support to predicting psychosocial distress is greater in magnitude than that of stressful events, chronic strains, and seciodemographic variables. There is no evidence that social support buffers the impacts of stressors on psychosocial distress, rather social support takes a role of mai or direct effects on psychosocial distress. Therefore, the strength of the social support is an important and meaningful strategy to prevent from stress and mental, physical illness.

The seeds of addiction are typically sown years prior to the onset of addictive substance use or engagement in addictive behaviors, due to the priming of the reward pathway (RewP) by alterations in the mechanism of stress-signaling from the hypothalamic-pituitary-adrenal axis (HPA) and related pathways. Excessive stress from a single-event and/or cumulative life experiences during childhood, such as those documented in the Adverse Childhood Experiences Study, is translated into neurobiological toxicity that alters the set-point of the HPA axis and limbic system homeostasis [suggested new term: regulation pathway (RegP)]. The resultant alteration of the RegP not only increases the risk for psychiatric and physical illness, but also that for early onset and chronic addictions by dysregulating the RewP. This paper reviews the interface of these symbiotic pathways that result in the phenotypic pathology of emotional dysregulation, cognitive impairment, and compulsive behaviors, as well as morbidity and shorter life expectancy when dysregulated by chronic stress.
While the threat from traditional communicable diseases have been decreasing non communicable chronic diseases are increasing due to the aging of population and change in life pattern of the people such as over intake of cholesterol and lack of physical exercise etc. On the other hand, since 1980s, AIDS is spreading rapidly throughout the globe and environmental pollution, accidents, addictive diseases such as drug abuse and alcoholism are becoming serious factors to hinder the health promotion of the people. In order to improve general public health and promote individual health status, existing program for communicable disease control by the government such as tuberculosis, leprosy, STD and acute communicable diseases should be effectively continued. In principle, effort should be placed on eradication of source of infection, reduction of communicability of source in infection, treatment of source of infection as well as increase of individual registance to the diseases through immunization and improvement of physical status. Since the pattern of illness is being shifted from communicable diseases to non communicable chronic diseases such as cancer, cerebral hemorrhage, heart disease and hypertention etc., special emphasis should be placed on the prevention and control of those adult diseases. Early detection of the patients, registration and treatment of patients and health education should be systematically developed for effective control of the diseases. In addition, program activities on MCH, nutrition, dentistry, mental health, environmental health, accident prevention. medical delivery system, health insurance. and all other health issues should be improved in order to achieve our goal of health promotion.
Purpose: The purpose of the study was to identify and describe the caregiving difficulties that mothers of children with spina bifida experience from their own perspectives. Methods: A qualitative descriptive study was designed. Data were collected from five mini-focus group interviews and four individual interviews using open-ended questions. Nineteen mothers of children with mild spina bifida participated in the study during 2014-2015. Data were analyzed using qualitative content analysis to identify major difficulties perceived by the mothers. Results: Five domains were identified with 12 subdomains. "Daily routine continence management" describes difficulties arising in bladder and bowel management for the child with spina bifida. "Management of school life of child" deals with difficulties in relation to the school facilities, such as the toilets, as well as teachers and friends. "Relationship with family and neighbors" illustrates problems in relations with their spouse, normal children, relatives and neighbors. "Maintaining physical and psychological health" includes physical and psychological problems of the mothers. "Finance" describes economic burden that the mothers face in the management of child's illness. Conclusion: The findings from this study provide insight into the practical issues related to the management of chronic conditions of children with spina bifida from the mothers' perspectives.
Purpose: The aim of this study was to develop a substantive theory on self-management conducted by the adolescents with chronic kidney disease from their lived experience. Methods: Data was collected through in-depth interviews from May to December in 2015 with thirteen adolescents with chronic kidney disease. The data collected were analyzed on the basis of Strauss and Corbin's grounded theory. Results: The core of the category found in this study was "overcoming the unstable sense of self- control and integrating disease experience into their life". The causal conditions triggering the central phenomenon were "restriction in daily life" and "manifestation and aggravation of symptom". The central phenomenon in the experience of self-management within the adolescents with chronic kidney disease was "unstable sense of self control". The intervening condition for unstable self control were "micro system support" and "motivational resources". This study found that the adolescents with chronic kidney disease followed a series of strategies when they faced the central phenomenon, including; passive coping, reappraisal of illness, active coping, compliance with treatment, controlling physical activity, and adjusting school life. With these strategic approaches, the adolescents with chronic kidney disease could maintain their active lifestyles and achieve their health behaviors. The process of self-management by these adolescents passed through four phases; limited experience caused by diseases, effort for normalization, reorganizing their daily lives, and integration with daily lives and self-management. Conclusion: This Study explored the process and experience of self-management of adolescents with chronic kidney disease. These findings can be used for basis for developing substantive theory and nursing intervention strategy for adolescents with chronic kidney diseases.
