Effect of Sling Exercise with PNF Basic Procedure for Pain and Balance Ability of Patients with Chronic Low Back Pain (PNF기법을 적용한 슬링운동이 만성요통환자의 통증과 균형능력에 미치는 영향)

  • Purpose : The purpose of this study was to investigate the effect of sling exercise with PNF basic procedure in patients with chronic low back pain. Methods : This study included 14 patients with chronic low back pain, who were performed sling exercise combined PNF basic procedure. The exercise program comprised 3 sessions of 30 minutes per week for 8 weeks. The VAS(Visual analogue Scale) and BBS(Berg Balance Scale) were evaluated before and after training. All data were analyzed using SPSS 12.0. Results : Significant differences were observed the chronic low back pain patient for VAS, BBS. Chronic low back pain patient improved all test. Conclusion : Sling exercise with PNF basic procedure about chronic low back pain patient is very useful and effective. It is effective in clinical practice.

The Effect of Horse-Riding Exercise on Pain and Body Flexibility for the Patient with Chronic Low Back Pain (승마 운동이 만성요통환자의 통증 및 체간 유연성에 미치는 영향)

  • Purpose : The purpose of this study was to investigate the effect of horse riding exercise for pain and body flexibility in the patient with chronic low back pain. Method : 26 subjects in H-equestrian were randomly divided two group, ball exercise(BE) group and horse-riding exercise(HE) group. Each group carried out 40 minutes exercise three times a week for 8 weeks. VAS were measured for pain, and evaluation of body flexibility in position with forward and backward. Result : The results were as follows, the pain scales of VAS(visual analog scale) between ball exercise and horse-riding exercise groups in post-test, were significantly different in measures(p<.05). And there were significant in two group after exercise(p<.05). The flexibility scales of body between ball exercise and horse-riding exercise groups in post-test, were significantly different in measures(p<.05). And there were significant in two group after exercise(p<.05). Conclusion : These finding revealed that horse-riding exercise was effective on pain and body flexibility of patient with chronic low back pain so that these exercise can be new altematives exercise for pain and body flexibility in the patient with chronic low back pain.

  • In this study, we treated a patient with psychogenic chronic low back pain for 2 years with herbal medication treatment and psychotherapy. The case was a 17 years old female patient complained chronic low back pain for a long time. She received the spine operation, epidural injection, many kind of oriental medicine treatment about low back pain, but there was no improvement. We diagnosed her pain as a psychogenic pain disorder, and treated her with herbal medication for decrease her stress, and did a psychotherapy such as the progressive muscle relaxation, and counseled about trouble with her family. After treatment, her chronic low back pain was improved so much. We think that the herbal medication treatment and psychotherapy can be helpful to treat patients who have psychic and physical disorders.

Comparison of Hip Joint Strength between Young Woman Patient with Chronic Low Back Pain with Lumbar Instability and Normal Subjects

  • Objective: The purpose of this study was to compare the hip joint muscle strength of patients with chronic back pain with lumbar instability and normal subjects. Design: A case control study. Methods: Five types of lumbar instability test were conducted on forty young women with chronic low back pain for more than six months, and those who had 3 or more positive tests were selected as subjects. To select chronic back pain patients with lumbar instability group, aberrant movement patterns during lumbar flexion test (FMT), prone instability test (PIT), posterior-anterior mobility test (PAT), passive lumbar extension test (PLE), and pressure bio-feedback (PBF) were applied. In addition, a digital muscle strength meter was used to measure the hip flexor, extensor, adductor, and abductor muscles of chronic low back pain patients with lumbar instability group (n=20) and normal subject group (n=20). Results: As a result of comparing the hip joint muscle strength between the chronic back pain patient group with lumbar instability and the normal group, there were significant differences in the hip extensor, abductor, and adductor muscles (p<0.05). Conclusions: Patients of chronic back pain with lumbar instability were found to have weak hip joint muscle strength. Therefore, this study suggest that include hip joint strength exercise for functional recovery of chronic back pain patients.

  • Treatment and management of chronic low back pain (CLBP) should be tailored to the patient's individual context. However, there are limited resources available in which to find and manage the causes and mechanisms for each patient. In this study, we designed and developed a personalized context awareness system that uses machine learning techniques to understand the relationship between a patient's lower back pain and the surrounding environment. A pilot study was conducted to verify the context awareness model. The performance of the lower back pain prediction model was successful enough to be practically usable. It was possible to use the information from the model to understand how the variables influence the occurrence of lower back pain.

  • The object of this study is in the comparison of level of depression between patients with chronic low back pain & normal adult, of the relation between reduction of pain & level of depression and in the examination of the influence of traditional physical therapy against the reduction of pain & level of depression of the patients with chronic low back pain for the new recognition of psychological factors to the physical therapists as well as patients. This study was targeted for 40 patients with chronic low back pain who were diagnosed as lumbar sprain & herniated intervertebral disk and compared & analyzed how traditional physical therapy has an influence to their reduction of pain and level of depression by questioning with visual an analog scale of Beck's depression inventory & pain inventory to them. The results are as follows. 1. Traditional physical therapy has an effect in the reduction of pain of the patients with chronic low back pain. 2. Although traditional physical therapy relieved a little the level of depression of patients with chronic low back pain. there was no evident effect statistically. 3. In the relation between the reduction of pain and level of depression, level of depression also showed somewhat relief according to the reduction of pain and appeared that they have some relationship. However, it's degree was not big. 4. In the comparison of the level of depression between the normal adult and the patient with chronic low back pain, the level of depression of the patient with low back pain are high rather than that of normal adult.

A Study on multifidus muscle activation by Needle EMG during shoulder flexion in Chronic Low Back Pain Patients (침 근전도로 측정한 만성 요통 환자의 어깨 굴곡시 나타나는 다열근 활성도 비교)

  • Purpose : The purpose of study is activation of lumbar multifidus muscle by needle EMG during shoulder flexion in chronic low back pain patients. The subject were consisted of 10 women patients with chronic low back pain and healthy asymtomatic subject 10 women. Methods : 10 women patients with chronic low back pain and healthy asymptomatic subject 10 women is voluntary participated for the research. Subjects were positioned in standing. The needle EMG were measured activation of multifidus. Needle electrode was used to 28 gauge. The shoulder flexion movement used to activate the multifidus was then measured. Results : Results of the analysis showed that asymptomatic subjects had significantly larger multifidus muscle activation compared with CLBP subjects during shoulder flexion. Conclusion : This study will be used as multifidus measurement method of patient with chronic LBP. The multifidus muscle in chronic LBP patient clinical significance. Most of chronic LBP patients have multifidus contraction pattern. Therefore chronic LBP patients necessary multifidus activation measurement with needle EMG.

Acceptance versus catastrophizing in predicting quality of life in patients with chronic low back pain

  • Background: The aim of this study was to investigate the relationship between pain catastrophizing, acceptance, and quality of life in relation to chronic low back pain in Jakarta, the capital city of Indonesia. We also analyze the effect of personality in catastrophizing and acceptance. Methods: A total of 52 chronic low back pain patients were enrolled as participants from 2 hospitals in Jakarta (43 females, 9 males, mean age 54.38 years). Participants completed a set of self-reported questionnaires: the NEO Five Factor Inventory (NEO-FFI), Chronic Pain Acceptance Questionnaire-Revised (CPAQ-R), Pain Catastrophizing Scale (PCS), and Pain Discomfort Module (PDM). Results: This study showed that acceptance increased the patient's quality of life by giving physical relief from pain. In contrast, pain catastrophizing decreased the quality of life, and increased the patients' tendency to get frustrated, irritated, and anxious about the pain. From a personality perspective, the trait neuroticism may lead to a higher level of pain catastrophizing. Conclusions: This study showed that catastrophizing, compared with acceptance, had a greater impact on the patient's life by reducing its quality.

A Qualitative Analysis on Paraspinal Muscles in Patients with Acute Low Back Pain and Chronic Low Back Pain (급성 요통환자와 만성 요통환자에서 척추주변근육의 정량적 분석)

  • This study was conducted to measure and analyze the changes in paraspinal muscles of acute and chronic low back pain patients using MRI, and to provide clinical basic data for diagnosis and treatment for low back pain. For this purpose, 20 patients with acute low back pain frome August 2012 to January 2013 which occurred within 12 weeks, and 20 patients with chronic low back pain that progressed over 12 weeks, were chosen as subjects, and their MRI measurements were compared with one another. As a result, in relation to in the fatty degeneration ratio of the left spine and right spine, there were significant differences in erector spinae and multifidus(p<.001), and in relation to the Fat Infiltration ratio between all the groups, there were significant differences in psoas major, erector spinae and multifidus between the acute low back pain patient group and the chronic low back pain patient group(p<.001). In the post-hoc test, multifidus and erector spinae in the acute low back pain group and chronic low back pain group showed the highest Fat Infiltration ratio. The serious Fat Infiltration of multifidus and erector spinae in the chronic low back pain group led to weakened strength of muscles that stabilize the spine. In conclusion, it is considered that this study would present important data and basis in making acute and chronic low back pain patients pay more attention to multifidus and psoas major during rehabilitation exercise, and selecting a rehabilitation exercise program.

Effects of the Latest Robotic Horse-riding for Low Back Pain : Narrative Review

  • Purpose: The purpose of this study is to review health-care program using the recent robotic horse-riding technology and its clinical effects for chronic low back pain. Research design, data and methodology: Recent clinical articles were determined under three inclusion criteria for in-depth review: 1) article that is published within 1 year, 2) article that includes the detailed explanation of health-care program using robotic horse-riding, 3) the article that deals with chronic low back pain during more than 6 months. Results: As a result, the finally-determined two articles demonstrated the clinical effects of robotic horse-riding statistically on pain intensity, low back muscle strength, spinal alignment, and fear-avoidance belief. Conclusions: After in-depth review, I concluded that health-care program using robotic horse-riding for chronic low back pain needs to be provided at low-intensity (e.g. less than 6km/h horse walking program) in the beginning of health-care for improving their motor control ability, then, at the increased intensity for strengthening core muscles.
