Health Problems and Health Services Utilization of Infants Born Prematurely in the U.S.

  • Purposes: The purposes of this study are to describe the health problems experienced by VLBW premature infants and their health care services utilization during the first year of life Method: Eighteen mothers of VLBW premature infants completed a survey questionnaire, asking socioeconomic/demographic information, health/developmental problems experienced by their infants, and their use of health care services. Results: Of the 18 infants, 78% experienced respiratory problems such as cold/running nose and wheezing during the first year and 33% experienced gastrointestinal problems such as vomiting, diarrhea and constipation. Twelve (67%) infants visited the emergency department at least one time and 10 infants (56%) were hospitalized at least once during their first year of age. Interestingly, infants without chronic lung disease visited the emergency department more than infants without chronic lung disease (p=.213). Infants living in non-disadvantaged neighborhoods were hospitalized more than infants living in disadvantaged neighborhoods (p=.000). Conclusions: Health care providers should initiate educating mothers, particularly those living in disadvantaged neighborhoods, about post-NICU discharge health care needs of their VLBW premature infants while their infants were still in the NICU so that unnecessary visits to the emergency department and rehospitalizations can be possibly prevented.

  • The influence of nutrition during early life on physical growth as well as mental development has been thoroughly discussed in the literature. The physical dimensions of the body are greatly influenced by nutrition, particularly during the period of rapid growth in early childhood. Nutritional status affects every pediatric patient's response toillness. Good nutrition is important for achieving normal growth and development. It is indicated that permanent impairment of the central nervous system may result from dietary restriction of imbalance during certain periods of life. If children under 3 years of age show a good nutritional status, it may be assumed that they are well nourished. Several common diseases of children such as iron deficiency, chronic constipation and atopic dermatitis are known food related diseases. Patients with chronic illness and those at risk of malnutrition should have detailed nutritional assessments done. Components of a complete nutritional assessment include a medical history, nutritional history including dietary intake, physical examination, anthropometrics (weight, length or stature, head circumference, midarm circumference, and triceps skinfold thickness), pubertal staging, skeletal maturity staging, and biochemical tests of nutritional status. The use of age, gender, and disease-specific growth charts is essential in assessing nutritional status and monitoring nutrition interventions. Nutrition assessment and dietary counseling is helpful for the cure of disease, and moreover, the prevention of illness.

A Research on Clinical Use of the Xiexiafa (사하법의 내과적 임상응용)

  • Xiexiafa is the one of eight methodes of treatment, the purgatives of oriental medicine was originated from Shanghanlun written by Zhangzhongjing. Traditionally Xiexiafa has been used to treat constipation. But All kinds of researches on Xiexiafa made purgatives possible to use variously for the disease of internal medicine. In the research on clinical use of Xiexiafa, the results were as follow. Xiexiafa is available on emergency medicine because of instant relief. It has an ability of preventive medicine and economical effect. It was considered to treat hard to cure and chronic diseases made by Sushi, Rexie, Shuijie, Tanzhi, Xuxie etc. Considering strong side effects of Xiexiafa and patient conditions, it must be used carefully.

  • Congenital segmental dilatation of the colon is a very rare entity of unknown etiology, characterized by a localized dilatation of a bowel segment of the colon of variable length and an abrupt transition between the normal and dilated intestine. It can affect any part of the colon, with the rectosigmoid colon being the most commonly affected site. The clinical and radiological features may resemble that of Hirschsprung disease, but differ in that the normal ganglion cells are found in the dilated and normal segment of the colon. We performed laparoscopic-assisted transanal endorectal pull-through for segmental dilatation of rectosigmoid colon in an 8-year-old boy with chronic constipation since the age of 5 months.

  • A hiccup is an involuntary, spasmodic contraction of the diaphragm accompanied by a sudden closure of the glottis, which is reported commonly in patients with brain stem disease such as ischemic stroke, dolichoectatic basilar artery, tumor, encephalitis, and multiple sclerosis. 1) Intractable hiccup is an uncommon, chronic and incapacitating disturbance defined as a hiccup bout lasting more than 48hours or recurring despite various treatments and affecting male subjects more than female. 2) Constipation and hiccup are common symptoms in stroke patients and purgation therapy has been often used. We discovered two patients who had a hiccup symptom after purgation therapy(diarrhea) and so reported course and result of treatment.

  • Notable progress has been made in the therapeutic and research applications of cyclic peptides since our previous review. New drugs based on cyclic peptides are entering the market, such as plecanatide, a cyclic peptide approved by the United States Food and Drug Administration in 2017 for the treatment of chronic idiopathic constipation. In this review, we discuss recent developments in stapled peptides, prepared with the use of chemical linkers, and bicyclic/tricyclic peptides with more than two rings. These have widespread applications for clinical and research purposes: imaging, diagnostics, improvement of oral absorption, enzyme inhibition, development of receptor agonist/antagonist, and the modulation of protein-protein interaction or protein-RNA interaction. Many cyclic peptides are expected to emerge as therapeutics and biochemical tools.

  • A Literatural study on the etiological factors, classification, prescription of hemorrhoids and hemorrhoids complicated by anal fistula following results were obtained. 1. The cause of hemorrhoids are long time sit, long time gate, overfatigue, overeating, imbalance of stool( constipation or diarrhea), pregnant fertility(overfatigue after childbirth, insufficiency of middle warmer energy), uncontrol sexual excess, pathgenic factors of wetness, heat, wind, dry, genetic cause, excess of anxiety, pile up of heat poison, weakness of entrails and viscera. The cause of hemorrhoid complicated by anal fistula are attack of external wind, heatness, dry, fire, wetness(pathgenic factors), inapporiate treatment and chronic disease, greasy diet, excess of anxiety, constipation, uncontrol sexual excess, obstacle of circulation of vital energy and blood on anal site. 2. Classification of hemorrhoids are female hemorrhoids, male hemorrhoids, pulse hemorrhoids, intestines hemorrhoids, vital energy hemorrhoids, wine hemorrhoids, blood hemonhoids, flowing hemorrhoids. Classification with other method are external hemorrhoids, internal hemorrhoids, mixed hemorrhoids, excrescence hemorrhoids, nipple homorrhoids. External hemorrhoids is classified of varicosis of hemorrhoidal vein, connective tissue form, thrombus form. Classification of hemorrhoid complicated by anal fistula are simple lower hemorrhoid, lower mixed hemorrhoid, deep hemorrhoid, outer of one hole hemorrhoid, a horseshoe hemorrhoids. Once more classificated of four are space of sphincter muscle form, penetration sphincter muscle form, upper of sphincter muscle form, outer of sphincter muscle form. 3. Therapy method of hermorrhoid and hemorrhoid complicated by anal fistula are internal method, fumigation method method, ointment, method of close with medicine, necrotizing method, hot medicated compress( gxternal method), injection, insertion, bind, (operation) and acupuncture therapy (the others method) 4. Herb medicine for many used of internal method are Scutellaria baikalensis George(黃芩), Coptis japonia Makino(黃連), Rehmania giutinosa Liboschitz ex Fischer & Meyer(生地黃), Poncirus trifoliata Refinesque(枳殼), Sanguisorba officinalis Linne(地楡), Sophora japonica L.(槐花), Cnidium officinale Makino (川芎), Astragalus membranaceus Bunge(황기), Angelica gigas Nakai (當歸). 5. Herb medicine for many used of fumigation are Schlechtendalia Chinesis J. A Case of Cauda Equina Syndrome in Early-Onset Chronic Inflammatory Demyelinating Polyneuropathy Clinically Similar to Charcot-Marie-Tooth Disease Type 1

  • To present a case of cauda equina syndrome (CES) caused by chronic inflammatory demyelinating polyneuropathy (CIDP) which seemed clinically similar to Charcot-Marie-Tooth disease type1 (CMT1). CIDP is an immune-mediated polyneuropathy, either progressive or relapsing-remitting. It is a non-hereditary disorder characterized by symmetrical motor and sensory deficits. Rarely, spinal nerve roots can be involved, leading to CES by hypertrophic cauda equina. A 34-year-old man presented with low back pain, radicular pain, bilateral lower-extremity weakness, urinary incontinence, and constipation. He had had musculoskeletal deformities, such as hammertoes and pes cavus, since age 10. Lumbar spine magnetic resonance imaging showed diffuse thickening of the cauda equina. Electrophysiological testing showed increased distal latency, conduction blocks, temporal dispersion, and severe nerve conduction velocity slowing (3 m/s). We were not able to find genetic mutations at the PMP 22, MPZ, PRX, and EGR2 genes. The pathologic findings of the sural nerve biopsy revealed thinly myelinated nerve fibers with Schwann cells proliferation. We performed a decompressive laminectomy, intravenous IgG (IV-IgG) and oral steroid. At 1 week after surgery, most of his symptoms showed marked improvements except foot deformities. There was no relapse or aggravation of disease for 3 years. We diagnosed the case as an early-onset CIDP with cauda equine syndrome, whose initial clinical findings were similar to those of CMT1, and successfully managed with decompressive laminectomy, IV-IgG and oral steroid.

Anti-oxidative and Anti-inflammatory Effect of Fractionated Extracts of Cynomorium Songaricum (쇄양의 항산화 및 항염증 효과)

  • Oxidative stress and inflammation are important events in the development of chronic inflammatory diseases including arthritis, atherosclerosis, diabetes, hypertension. Cynomorium songaricum (CS) has been used as a traditional Korean herbal medicine, and it is currently used in traditional clinics to treat frequent urination, spermatorrhea, weakness of the sinews and constipation in the folk medicine. The aim of this study was to determine whether fractionated extracts of CS inhibit free radical generation such as DPPH radical, superoxide radical, nitric oxide and peroxynitrite, production of nitrite an index of NO, $PGE_2$, iNOS, COX-2 and pro-inflammatory cytokines in lipopolysaccharide (LPS)-treated RAW 264.7 macrophages. Cytotoxic activity of extracts on RAW 264.7 cells was measured using 5-(3-caroboxymeth-oxyphenyl)-2H-tetra-zolium inner salt (MTS) assay. Our results indicated that the most superior extract which scavenged DPPH radical, reactive oxygen species (ROS) and RNS was CS ethyl acetate extract (CSEA). Moreover, CSEA significantly inhibited the LPS-induced NO, $PGE_2$ production and iNOS, COX-2 expression accompanied by an attenuation of TNF-$\alpha$, IL-$1{\beta}$ and IL-6 formation in macrophages. Furthermore, CSEA treatment also blocked LPS-induced intracellular ROS production and the activation of NF-${\kappa}B$. These findings indicate that CSEA inhibits the production of pro-inflammatory mediators and cytokines via the suppression of ROS production and NF-${\kappa}B$ activation. Take together, these results indicate that CSEA has the potential for use as an natural anti-oxidant and an agent of anti-chronic inflammatory diseases.

Assessment of the Effectiveness of Biofeedback Therapy in Children with Pelvic Floor Dyssynergia (소아 골반저 근실조에서 바이오피드백 치료의 유용성)

  • Purpose: Recently well-developed anorectal function tests have revealed that there is an obvious pelvic floor dyssynergia (PFD) pattern in pediatric patients with constipation, as well as in adult's. The use of biofeedback therapy (BT) has been widely implemented in adult PFD patients; however, this approach has only rarely been considered for pediatric PFD patients. Therefore, we assessed the effectiveness of BT in children with PFD. Methods: We studied 70 children with PFD, who were referred to the department of pediatrics at the Asan Medical Center for the management of soiling or chronic constipation from September 2002 to February 2005. Diagnosis of PFD and assessment of the efficacy of BT for PFD treatment were carried out along with several ano-rectal function tests (cine-defecography, ano-rectal manometry, balloon expulsion test and intra-anal EMG); in addition, a questionnaire was administered. The BT based intra-anal EMG was performed. A follow-up telephone interview was performed more than 6 months later. Results: Most of the symptoms and results of the ano-rectal function tests were statistically improved after BT. In comparisons between the BT and control groups (BT refusal group due to poor compliance), the symptoms were statistically improved at follow-up. Therefore, for the shortterm improvement of symptoms, BT was better than conservative therapy alone. The negative feelings associated with ano-rectal function testing and BT were directly associated with failure or success of therapy. Conclusion: Pediatric patients with constipation or soiling that presented with an obvious PFD pattern showed that BT was a useful therapeutic tool for rapid improvement of symptoms.
