COPD는 비가역적인 기류제한을 특징으로 하는 질병의 상태이다. 이러한 기류제한은 유해입자나 가스등에 대한 폐의 비정상적인 염증반응과 관련되어 있고 자연경과 중 지속적으로 진행하는 양상을 띤다. 비정상적인 염증반응 및 단백분해효소와 항단백분해효소간의 불균형과 산화 스트레스 등의 기전에 의해 점액의 과분비, 섬모의 기능장애, 소기도의 섬유화와 협착, 폐실질의 파괴, 폐혈관손상 등이 발생한다. 이러한 병리학적 변화에 의해 기류제한이 발생하고 가스교환 장애, 폐고혈압, 폐성심, 전신적인 염증이나 골격근의 기능장애 등이 유발된다. COPD 환자의 일반적인 증상인 기침, 객담, 호흡곤란 등은 이러한 병태생리학적 변화로 설명할 수 있다.
Purpose: To develop and test the validity and reliability of the Korean version of the Family Management Measure (Korean FaMM) to assess applicability for families with children having chronic illnesses. Methods: The Korean FaMM was articulated through forward-backward translation methods. Internal consistency reliability, construct and criterion validity were calculated using PASW WIN (19.0) and AMOS (20.0). Survey data were collected from 341 mothers of children suffering from chronic disease enrolled in a university hospital in Seoul, South Korea. Results: The Korean version of FaMM showed reliable internal consistency with Cronbach's alpha for the total scale of .69-.91. Factor loadings of the 53 items on the six sub-scales ranged from 0.28-0.84. The model of six subscales for the Korean FaMM was validated by expiratory and confirmatory factor analysis (${\chi}^2$ <.001, RMR<.05, GFI, AGFI, NFI, NNFI>.08). Criterion validity compared to the Parental Stress Index (PSI) showed significant correlation. Conclusion: The findings of this study demonstrate that the Korean FaMM showed satisfactory construct and criterion validity and reliability. It is useful to measure Korean family's management style with their children who have a chronic illness.
크로마틴 리모델링은 후성유전기전을 통해 유전자 발현을 조절한다. 비정상적인 히스톤 변형이 우울증 발생에 관여하는 것으로 알려져 있다. p11 (S100A10)은 인간과 설치류에서 우울증의 병태생리에 관여한다고 보고되었다. 본 연구는 우울증 동물모델인 장기간 예측 불가능한 스트레스가 마우스 해마에서 p11 유전자 promoter의 히스톤 변형에 미치는 영향을 조사하고자 하였다. C57BL/6 마우스에 21일 동안 스트레스를 가하고, 강제수영검사를 수행하여 우울 유사 행동 양상을 측정하였다. Real time PCR 및 Western blotting 분석법으로 p11 발현 변화를 조사하였으며, 염색질 면역침전분석법을 수행하여 p11 promoter의 히스톤 H3 아세틸화 및 메틸화 양을 측정하였다. 장기간 예측 불가능한 스트레스는 강제수영검사에서 부동시간을 증가시켜 우울 유사 행동을 나타내었으며, 해마의 p11 mRNA 및 단백질 발현을 유의하게 감소시켰다. 또한 p11 promoter의 히스톤 H3 아세틸화(Ac-H3) 및 H3-K4 트리메틸화(H3K4met3)를 유의하게 감소시켰으며, H3-K27 트리메틸화(H3K27met3)를 증가시켰다. 본 연구결과는 만성 스트레스가 해마에서 p11 유전자의 후성유전적 억제를 야기하여 p11 유전자의 발현을 감소시킴을 시사한다.

Non-alcoholic fatty liver disease (NAFLD) is one of the most common liver diseases. NAFLD can further progress to irreversible liver failure such as non-alcoholic steatohepatitis (NASH) fibrosis and cirrhosis. However, specific regulator of NASH-fibrosis has yet to be established. Here, we found that glutathione peroxidase 7 (GPx7) was markedly expressed in NASH fibrosis. Although GPx7 is an antioxidant enzyme protecting other organs, whether GPx7 plays a role in NASH fibrosis has yet to be studied. We found that knockdown of GPx7 in transforming growth factor-β (TGF-β) and free fatty acids (FFA)-treated LX-2 cells elevated the expression of pro-fibrotic and pro-inflammatory genes and collagen synthesis. Consistently, GPx7 overexpression in LX-2 cells led to the suppression of ROS production and reduced the expression of pro-fibrotic and pro-inflammatory genes. Further, NASH fibrosis induced by choline-deficient amino acid defined, high fat diet (CDAHFD) feeding was significantly accelerated by knockdown of GPx7, as evidenced by up-regulated liver fibrosis and inflammation compared with CDAHFD control mice. Collectively, these results suggest that GPx7 might be a novel therapeutic target to prevent the progression and development of NAFLD.
Objective : The purpose of this study was to analyze the effect on psychosocial status in elderly women with chronic low back pain after practicing lumbar stabilizing exercise using the gym ball, and to developed and activated on exercise program for healthy senile life. Methods : The subjects were 23 elderly women with chronic low back pain. Mean age of subjects was $67{\pm}13$ years. The lumbar stabilizing exercise using the gym ball was performed 3 times a week during the 8 weeks study period. The research tool were ODI(Oswestry Disability Index), VAS(Visual Analogue Scale), SES(Self-esteem Scale), GDSSF-K(Geriatric Depression Scale Short Form Korea Version), PSS (perceived stress scale), Life satisfaction, and MMSE-K. The analysis of data was processed by the statistical program SPSS 12.0 and paired t-test and frequency analysis were used to compare psychosocial status between the pretest and post tests. Result : The lumbar stabilizing exercise using the gym ball increased significantly SES and Life satisfaction of subjects, and ODI, VAS(trunk flexion & trunk extention), GDSSF-K, and PSS were decreased significantly(p<.05). While VAS(trunk rotation) were not significantly. Conclusion : The lumbar stabilizing exercise using the gym ball improved psychosocial status in elderly women with chronic low back pain.
Background: The treatment of trigeminal neuralgia remains a challenging issue. Stem cells from human exfoliated deciduous teeth (SHED) provide optimized therapy for chronic pain. This study aimed to investigate the mechanisms underlying the attenuation of trigeminal neuralgia by SHED. Methods: Trigeminal neuralgia was induced by chronic constriction injury of the infraorbital nerve. The mechanical threshold was assessed after model establishment and local SHED transplantation. Endoplasmic reticulum (ER) morphology and Caspase12 expression in trigeminal ganglion (TG) was evaluated as well. BiP expression was observed in PC12 cells induced by tunicamycin. Results: The local transplantation of SHED could relieve trigeminal neuralgia in rats. Further, transmission electron microscopy revealed swelling of the ER in rats with trigeminal neuralgia. Moreover, SHED inhibited the tunicamycin-induced up-regulated expression of BiP mRNA and protein in vitro. Additionally, SHED decreased the up-regulated expression of Caspase12 mRNA and protein in the TG of rats caused by trigeminal neuralgia after chronic constriction injury of the infraorbital nerve mode. Conclusions: This findings demonstrated that SHED could alleviate pain by relieving ER stress which provide potential basic evidence for clinical pain treatment.
Purpose: This study reports on a series of patients with chronic lateral ankle instability that underwent the Brostrom procedure with suture tape augmentation and allowed early unrestricted weight-bearing in a simple stirrup brace. Materials and Methods: This retrospective study was conducted on 36 patients (22 males and 14 females of mean age 34 years [range 23~48 years]) with chronic lateral ankle instability treated using the Brostrom procedure using suture tape augmentation and inferior extensor retinaculum reinforcement with a fiber-wire connected to a SwiveLock screw inserted into the talus. When possible, patients started unrestricted weight-bearing in a stirrup brace from the third postoperative day. Demographics and functional outcomes, including American Orthopaedic Foot and Ankle Society (AOFAS) ankle-hindfoot, visual analogue scale (VAS), and satisfaction scores, were recorded. In addition, varus stress radiographs obtained before and 24 months after surgery were compared. Patients were followed for a mean 29 months (range 25~40 months). Results: Mean AOFAS ankle-hindfoot scores increased from 51 points preoperatively to 92 points at final follow-up, and mean VAS decreased from 6.8 to 1.2 points. Mean patient satisfaction scores were 8.7 at 12 months and 9.6 at 24 months. Stress radiographs demonstrated that talar tilt decreased from a mean 18 degrees preoperatively to 7 degrees at 24 months. Conclusion: Early unrestricted weight-bearing in a stirrup brace following the Brostrom procedure with suture tape augmentation is a successful protocol for treating chronic lateral ankle instability.
Purpose: This study examined relationships among stress, stress coping strategies, and somatization in mothers-in-law from multi-cultural families in a rural area. Methods: Elderly mothers-in-law (n=227) living with foreign daughters-in-law completed a self-reporting questionnaire. Data were collected from April to August 2009. Questions related to stress (Visual Analog Scale, VAS), coping strategies (Coping Strategy Scale) for stress, and somatization (Symptom Check List 90, Revised). SPSS/WIN 12.0 program was used for descriptive analysis, t-test, one-way ANOVA, Pearson correlation, and multiple regression analyses. Results: Subjects had a moderate level of stress (5.03). There were significant differences in stress level according to age, educational level, religion, chronic disease, health status, number of children, agreement of an international marriage of her sons, satisfaction in living with a foreign daughter-in-law, and family conflict. Stress showed a significant positive correlation with offensive coping strategy, passive strategy, and somatization. Stress, offensive coping strategy, and passive coping strategy affected the level of somatization. Conclusion: In a family situation involving co-habitation of mother- and foreign daughter-in-law, increased stress experienced by the mother-in-law can lead to increased offensive and passive coping strategies, and increased somatization. More effective means of stress reduction are needed for mothers-in-law from multi-cultural families.
본 연구는 넙치를 인공사육 시설에서 육성할 때 나타나는 무안측 흑화(착색증)의 스트레스 관련성을 확인하기 위해, 무안측에 체색이 없는 것(정상 그룹)와 체색이 있는 것(흑화 그룹)들 사이 스트레스 인자 비교 실험을 실시하였다. 본 실험에 사용된 실험어는 평균 전장 21.5±0.42 cm, 평균 무게 87.5±6.1 g였으며, 실험어의 무안측 착색면적률은 정상 그룹 0.63±0.12%, 착색증 그룹 16.7±4.7%였다. 분석한 스트레스 관련 인자는 혈중 GLU 및 TP, 코티졸 및 갑상선호르몬(FT3 & FT4) 농도 및 어체 성분(수분, 조단백, 조지방, 조회분)이었다. 실험 결과, 정상과 무안측 착색 넙치에 있어 혈중 GLU 및 TP의 농도는 정상 형질보다 착색증 형질에서 유의하게 높게 나타났다. 혈중 호르몬 농도에 있어 갑상선호르몬의 경우 정상과 착색증 그룹 사이에는 유의차를 찾아볼 수 없지만, 코티졸은 착색증 그룹이 정상 그룹에 비해 유의하게 높게 나타났다. 어체 중 영양 성분의 경우, 수분과 회분은 그룹 간 유의차가 없었지만, 조단백은 착색증 그룹이 정상보다 다소 낮으며, 조지방은 정상보다 유의하게 높았다. 이상의 스트레스 인자를 통한 정상과 착색증 그룹 간 비교를 통해 알 수 있듯, 양식산 넙치의 착색증은 만성적이 약한 스트레스에 관련되어 있을 가능성이 높은 것으로 확인되었다.
현대사회에서 스트레스와 긴장감은 피할 수 없는 요인이다. 다양한 피부질환은 스트레스를 일으키는 중요한 요인으로 언급되고 있다. 피부질환을 가진 환자들은 수면상태가 원활하지 않아 전반적으로 수면 효율이 낮다. 또한 피부질환으로 인해 심리적 스트레스 수치가 높아지고, 이와 같은 과정은 반복적으로 발생하고 있다. 피부질환과 스트레스는 상호적으로 연관되어 있으며, psychodematology에 대한 연구가 증가하고 있다. 이에 본 연구에서는 피부질환을 저하 시킬 수 있는 호박, 작약, 타트체리 복합물을 활용하여 피부 각질 형성 세포에서 스트레스로 인한 만성 피부질환을 개선할 수 있는 소재를 개발하고 효능을 입증하고자 하였다. HaCaT 각질형성세포에 복합 추출물은 12.5, 25, 50, 100 ㎍/mL 농도 의존적으로 TNF-α, IL-1β, IL-6, MDC, TARC 발현량이 저해되었으며 특히 IL-1β의 경우, 100 ㎍/mL의 농도에서 40% 이상 저해하는 우수한 효능을 확인하였다. 또한 AQP-3, HA, filaggrin의 생성량 농도 의존적으로 유의미한 증가를 보이며 TNF-α/IFN-γ로 증가된 p-ERK, p-JNK, p-p38의 단백질 발현은 복합 추출물의 처리로 유의하게 감소시 키는 것으로 나타났다. 이를 통하여 해당 복합 추출물은 피부질환을 치료 및 예방할 수 있는 소재로서 활용가치가 있는 것으로 판단되며, 이는 피부질환과 스트레스 간의 상호 관계의 악영향을 낮춰 줄 것으로 판단된다.
