Replication of Early B-cell Factor 1 (EBF1) Gene-by-psychosocial Stress Interaction Effects on Central Adiposity in a Korean Population

  • Objectives: Central obesity plays a major role in the development of many chronic diseases, including cardiovascular disease and cancer. Chronic stress may be involved in the pathophysiology of central obesity. Although several large-scale genome-wide association studies have reported susceptibility genes for central adiposity, the effects of interactions between genes and psychosocial stress on central adiposity have rarely been examined. A recent study focusing on Caucasians discovered the novel gene early B-cell factor 1 (EBF1), which was associated with central obesity-related traits via interactions with stress levels. We aimed to evaluate EBF1 gene-by-stress interaction effects on central adiposity traits, including visceral adipose tissue (VAT), in Korean adults. Methods: A total of 1467 Korean adults were included in this study. We selected 22 single-nucleotide polymorphisms (SNPs) in the EBF1 gene and analyzed their interactions with stress on central adiposity using additive, dominant, and recessive genetic modeling. Results: The four SNPs that had strong linkage disequilibrium relationships (rs10061900, rs10070743, rs4704967, and rs10056564) demonstrated significant interactions with the waist-hip ratio in the dominant model ($p_{int}$<0.007). In addition, two other SNPs (rs6556377 and rs13180086) were associated with VAT by interactions with stress levels, especially in the recessive genetic model ($p_{int}$<0.007). As stress levels increased, the mean values of central adiposity traits according to SNP genotypes exhibited gradual but significant changes (p<0.05). Conclusions: These results suggest that the common genetic variants for EBF1 are associated with central adiposity through interactions with stress levels, emphasizing the importance of managing stress in the prevention of central obesity.

  • We examined the effects of the vibration stress on cortisol secretion and hematological characteristics in soft-shelled turtle, Pelodiscus sinensis. For the stressed group vibration of $45{\sim}78 dB(V)$ from electric vibrator applied for 30 min with 2-h intervals during daytime ($08:00{\sim}18:00$) up to 28 days. Using the blood samples collected from ten turtles held once a week after vibration stress, we measured hematocrit, hemoglobin, red blood cells, cortisol, glucose, lactic acid, osmolality, $Na^+,\;K^+,\;Cl^-$, aspartate aminotransferase (AST), and alanine aminotransferase (ALT). The results have showed that P. sinensis received vibration stress exhibit the 'typical' stress-induced physiological responses (cortisol, glucose, lactic acid, osmolality, ions, hematocrit and hemoglobin) induced by vibration stress. Our data suggested that chronic vibration stress caused substantial stress in the animal, and in particular, the persisting elevated levels of AST and ALT would be highly correlated with the adverse effects of the stress. The high hematological characteristics during entire experimental period showed that the P. sinensis could not adapt to chronic stimuli provoked by vibration stress.

만성 족관절 외측 불안정성에 대한 $Brostr{\ddot{o}}m$ 변형 술식 (The Modified $Brostr{\ddot{o}}m$ Procedure for Chronic Lateral Ankle Instability)

  • Purpose: The purpose of this study is to evaluate the surgical results of modified $Brostr{\ddot{o}}m$ procedure for chronic lateral ankle instability and to assess whether or not associated injuries may affect postoperative satisfaction. Materials and Methods: Twenty- four patients with chronic lateral ankle instability were evaluated retrospectively from August 1998 to March 2002. Average age was 29.3 years. All patients were performed pre & postoperative ankle anteroposterior and lateral view, stress anterior drawer and varus test using Telos device, MRI and intraoperative arthroscopic evaluations before ligament reconstruction. Results: Of the 24 cases, 23 cases was improved more than average 12 points on AOFAS scales. On modified scales of Hamilton, 3 excellent, 20 good, 1 fair results. On stress view, average 2.2 mm difference was improved on anterior drawer test and average 1.7 degree on varus test. Associated injuries were 8 osteochondral defects, 4 anterior impingements, 2 loose bodies, 2 os subfibulare, 2 os submalleolare and 2 partial ruptures of peroneus brevis. 8 cases with no associated injuries rated excellent or good. Conclusion: The modified $Brostr{\ddot{o}}m$ procedure is believed to be an effective and successful method for chronic lateral ankle instability that didn't respond to conservative treatment. Because associated injuries in chronic lateral ankle instability may affect postoperative satisfaction, appropriate detection and treatment may need for postoperative satisfaction.

  • Innate immunity is a first line defence system in the body which is for sensing signals of danger such as pathogenic microbes or host-derived signals of cellular stress. Pattern recognition receptors (PRR's), which present in the cell memebrane, are suspect the infection through pathogen-associated molecular patterns (PAMP), and activate innate immunity with response to promote inflammation via inflammatory cells such as macrophages and neutrophils, and cytokines. Inflammasome are protein complexes which are part of innate immunity in inflammation to remove pathogens and repair damaged tissues. What is the important role of inflammation in disease? In this review, we are focused on the action mechanism of NLRP3 inflammasome in inflammatory diseases such as asthma, atopic dermatitis, and sepsis.

지역사회 만성정신질환자의 건강위험행위 영향요인 (Factors Influencing Health Risk Behaviors of the Chronic Mental Illness in the Community)

  • 본 연구는 지역사회 만성정신질환자를 대상으로 건강위험행위와 스트레스생활사건, 정신증상, 병식, 우울간의 관계 및 건강위험행위에 영향을 미치는 요인을 파악하여 효율적인 만성정신질환자의 건강증진프로그램 개발을 위한 기초자료를 마련하고자 시도되었다. 연구의 대상은 D시와 C도에 위치한 정신보건센터와 사회복귀시설을 이용하고 있는 성인 만성정신질환자 255명이며 자료수집기간은 2011년 8월 1일에서 9월 20일까지였다. 수집된 자료는 실수, 백분율, t-test, ANOVA, Pearson 상관계수, 다중회귀분석을 이용하여 분석하였다. 연구결과 성별, 진단명, 스트레스 생활사건, 병식, 우울이 통계적으로 유의한 영향요인으로 나타났으며, 이들 전체 요인들은 만성정신질환자의 건강위험행위를 24% 설명하였다. 따라서 지역사회 만성정신질환자의 성별과 진단을 반영하여 병식교육과 스트레스 및 우울관리를 통해 건강위험행위를 효율적으로 관리할 수 있는 건강증진프로그램이 개발되어야 할 것이다.

만성 두통환자에 대한 두개천골요법의 효과 (The Effects of Craniosacral Therapy on Chronic Headache)

  • The Purpose of this study is to explore the effects of Craniosacral therapy(CST) on the chronic headache patients, and the research was used the one-group pretest-posttest design. The subjects were 31 chronic headache patients, who received Craniosacral therapy by once per week for 6 weeks. The data were collected from July 1, 2003 to August 28, 2003. In order to evaluate the effects of craniosacral therapy, Visual Analogue Scale(VAS) for headach intensity, blood pressure, pulse rate, respiration rate, craniosacral rhythm rate, depression and stress perception were measured before and after treatment. The results are as follows : 1) The mean score of intensity of headache (VAS) significantly decreased after each session of Craniosacral therapy(F=86.14, p<.001). 2) The mean score of systolic blood pressure significantly decreased after each session of craniosacral therapy(F=6.99, p<.001), and the mean score of diastolic blood pressure significantly decreased after 3rd session of Craniosacral therapy(t=5.710, p<.001). The means of pulse(F=6.19, p<.001), respiration (F=7.93, p<.001) and craniosacral impulse rate(F=22.84, p<.001) significantly decreased after each session of Craniosacral therapy. 3) The means score of depression significantly decreased after Craniosacral therapy(t=6.627, p<.001) and stress also significantly decreased after Craniosacral therapy(t=5.987, p<.001). Therefore, these results of this study suggest that Craniosacral therapy could be an effective nursing intervention to reduce the intensity of headache.

Thymoquinone Prevents Myocardial and Perivascular Fibrosis Induced by Chronic Lipopolysaccharide Exposure in Male Rats - Thymoquinone and Cardiac Fibrosis -

  • Objectives: Thymoquinone (TQ) is one of the active ingredients of herbal plants such as Nigella sativa L. (NS) which has beneficial effects on the body. The beneficial effects of TQ on the cardiovascular system have reported. This study aimed to investigate the effect of TQ on cardiac fibrosis and permeability, serum and tissue concentration of inflammatory markers and oxidative stress status in chronic lipopolysaccharide exposure in male rats. Methods: Seventy male Wistar rats were randomly divided into five groups as follows: (1) control; (2) LPS (1 mg/kg/day); (3-5) LPS + TQ with three doses of 2, 5 and 10 mg/kg (n=14 in each group). After 3 weeks, serum and cardiac levels of $IL-1{\beta}$, $TNF-{\alpha}$ and nitric oxide (NO) metabolites, and cardiac levels of malondialdehyde (MDA), total thiol groups, catalase (CAT) and superoxide dismutase (SOD) activities, permeability of heart tissue (evaluated by Evans blue dye method) and myocardial fibrosis were determined, histologically. Results: LPS administration induced myocardial and perivascular fibrosis and increased cardiac oxidative stress (MDA), inflammatory markers and heart permeability, while, reduced anti-oxidative enzymes (SOD and CAT) and the total thiol group. Administration of TQ significantly attenuated these observations. Conclusion: TQ improved myocardial and perivascular fibrosis through suppression of chronic inflammation and improving oxidative stress status and can be considered for attenuation of cardiac fibrosis in conditions with chronic low-grade inflammation.

거골하 관절의 만성 불안정성 - 1예 보고 - (Chronic subtalar joint instability - One case report -)

  • Purpose: Chronic subtalar instability is not common and similar to chronic ankle instability and the incidence and cause chronic subtalar instability are not well known. Recently we have experienced chronic subtalar instability without chronic ankle instability which was treated with modified Brostrom procedures. Materials and Methods: The patient is 46 year old man who has suffered from left ankle sprain for 30 years and recently aggravated more than twice a day. On subtalar stress view, 14 degree angulation of subtalar joint was noted and on anterior drawer view, 8 mm anterior displacement of left ankle was seen. Results: In operation, there was no anterior talofibular ligament abnormility but calcaneofibular ligament loosening was found. Ligament reconstruction was performed using modified Brostrom procedure. At 12 months after operation, the patient complains no pain and no limit of motion and no instability. Conclusion: We experienced chronic subtalar instability without ankle instability treated with modified Brostrom procedures. No instability was found after treatment without complication.

성별에 따른 1인 가구 성인의 만성질환 유병 관련 요인 (Factors Associated with Chronic Disease Occurrence in One-person Household Depending on Gender)

  • Purpose: This study was conducted to assess the effect of household types on the occurrence of chronic diseases depending on gender and to identify the factors associated with chronic disease occurrence in one-person households. Methods: Multivariate logistic regression analysis was conducted using the data of 15,949 adults with the age of 19 years or older from the sixth Korea National Health and Nutrition Examination Survey (2013~2015). Results: For male, the risk of chronic disease occurrence was higher in one-person households than in multi-person households, and the same held true after adjusting for the confounding factors. For female, no significant relationship was found between household types and chronic disease occurrence. Factors associated with chronic disease occurrence were age, employment state, marital state, smoking, perceived stress, and depression in male, and age, employment state, physical activity, and obesity in female. Conclusion: It is necessary to monitor the disparity in health depending on household types in male. Additionally, providing a gender specific chronic disease prevention and health enhancement program is required.

만성질환을 가진 성인의 미충족 의료 영향요인 (Factors Affecting Unmet Healthcare Needs among Adults with Chronic Diseases)

  • Purpose: In adults with chronic diseases, unmet healthcare needs are likely to increase the risk of worsening the illness and complications. This study was conducted to explore factors affecting unmet healthcare needs among adults with chronic diseases. Methods: A cross sectional study was performed using the data from the seventh Korea National Health and Nutrition Examination Survey (KNHANES VII), 2016-2017. The sample of this study consisted of 6,104 adults with chronic diseases. The data were analyzed by Rao-Scott 𝑥2 test and logistic regression using complex samples analysis. Results: The proportion of subjects who experienced unmet healthcare needs was 10.9% of adults with chronic diseases. The factors affecting unmet healthcare needs of adults with chronic diseases were age, gender, employment status, household income, subjective health status, activity limitation, and perceived stress. Conclusion: The findings of this study suggest that various factors may be associated with their unmet healthcare needs. Finally, the development of strategies to reduce unmet healthcare needs for people with chronic diseases should consider the associated factors presented in the study.
