Relationship among Chronic Disease, Health Status and Health Related Lifestyle of Rural Elderly by Gender (농촌노인의 성별에 따른 만성질환과 건강상태 및 건강생활양식)

  • Purpose: The purpose of this study was to investigate the chronic disease, health status and lifestyle, and to test the chronic disease and health status and lifestyle of rural elderly by gender. Method: The interview survey was performed in September 2004 with structured questionnaires(Scale of Long-Term Health and Welfare Need Survey) to 770 of the elderly who lived in Muan-Gun of Chunnam Province. The percentage, Chi-squire test and regression method were used for some of the cross-sectional data. Results: The 770 elderly respondents were composed of 51.3% male and 48.7% female. 59.1% of the elderly had chronic disease. About the subjective health status that 54.3% of the respondents have been answered not good health status, 87.9% of the respondents have been health examination. The related variables of chronic disease and general characteristics were education and religious level in male, age, marital status, type of social security, education and religious level in female, and health status variables were subjective health status, cognitive function, ADL, IADL, and lifestyle factors was exercise in male, examination in female. Conclusion: These results suggested that special health promotion and education programs of the health habits such as physical exercise and health examination were necessities for the elderly of rural area.

Factors Related to Health-promoting Behaviors and Chronic Diseases in the Elderly (노인들의 건강증진행위와 만성질환과의 관련요인)

  • Objectives: The purpose of this study is to investigate the relationship between health promotion behaviors and chronic disease prevalence of Korean elderly and to provide information for preventing chronic diseases and improving health conditions of the elderly. Methods: A subset of 584 cases in the fourth Korea National Health and Nutrition Examination Survey was used for secondary analysis. Chi square test was used to compare chronic diseases prevalence by general characteristics and health promotion behaviors. Logistic regression analysis was used to identify the factors associated with chronic diseases. Results: Gender, occupation, subjective health status, smoking, and alcohol drinking have significant association with chronic diseases. Conclusions: Gender-specific health education for the elderly should be implemented at the health center, and opportunities for social participation can be enhanced through job creation for the elderly. Active campaigns on smoking cessation and moderate drinking are needed to prevent and manage chronic diseases of the elderly.

An Exploratory Study of Factors associated with the Health Behavior of Working Elderly with Chronic Diseases (만성질환이 있는 일하는 노인의 건강행위 관련요인에 관한 탐색적 연구)

  • Purpose: This study aimed to identify the health behaviors of working elderly Koreans aged 65 over and examined the socio-demographic and disease-related factors by health behaviors. Methods: This study used data obtained from the 2nd basic survey of the 2008 Korean Longitudinal Study of Ageing. We selected 381 working elderly having one or more of the diseases hypertension, diabetes, heart disease or cerebrovascular disease. Results: 78.9% out of the subjects have hypertension, 31.2% have diabetes, 12.3% have heart disease, and 6.3% have cerebrovascular disease. Compared to the health behaviors of the general elderly, the rate of the practice of regular exercise among the subjects was lower, but the smoking and drinking rate were higher. The significant variables associated with health behavior practice rates were gender, type of work, subjective health status and chronic diseases. Conclusion: The type of work of the elderly with chronic diseases was significantly associated with health behaviors. Consequently, this study found that continuous care programs for the working elderly with chronic diseases should be developed and provided as an occupational health service when the jobs are offered to them.

  • Recently, several researches indicated the relationship between oral condition and respiratory disease such as pneumonia and chronic obstructive pulmonary disease (COPD). Respiratory disease is known as common chronic disease in the elderly increasing mortality and morbidity. In this study, we have reviewed the association between oral disease and respiratory disease in the elderly. The related data were searched and collected from abroad and domestic studies. The studies included the randomized controlled clinical trials (RCTs), longitudinal, cohort, case-control, and systematic review studies. With the data from the studies, we concluded that poor oral hygiene or periodontal disease can influence the pneumonia in the elderly. Further studies will be needed to investigate the association between oral disease and COPD.

Experiences of Medication Misuse and Abuse Behaviors in Elderly People with Chronic Disease in Community (지역사회에 거주하는 만성질환 노인의 약물 오·남용행위 경험)

  • The purpose of this study was to understand the essential meanings in experiences of medication misuse and abuse behavior in elderly people with chronic disease in community. The participants were 10 patients with chronic disease over 65 ages who were selected by convenience sampling. Data were collected through individual in-depth interviews from February to May 2014, and were analyzed using Colaizzi (1978)'s phenomenological analysis method. As a results, the essential meanings and structure of medication misuse and abuse behavior experience in elderly people with chronic disease were derived into five clusters of themes as follows: 'Burden on the medications', 'Anxiety about the side effect of the medications or the disease exacerbation', 'There is no alternative other than the medications', 'Expectations of the medications effect for my criteria', 'Lack of understanding on the correct medication use'. The results of this study contributed to promoting a deeper understanding of medication misuse and abuse behaviors in elderly people with chronic disease, and provided for the basic data of strategic development for the prevention of medication misuse and abuse behaviors from the perspective of elderly people with chronic disease in community.

A Narrative Inquiry of Experience of the Elderly's Living with Chronic Disease in Hospital-based Home Care Services (병원중심가정간호를 받는 만성질환 노인의 삶의 경험에 관한 내러티브 탐구)

  • Purpose: The purpose of this study is to document the experiences of elderly persons with chronic diseases who are under hospital-based home care services and to find out how they describe the experiences using a narrative inquiry method. Methods: The participants are 7 elderly patients over the age of 70. Data collection and analysis were conducted through Jeong Gwang Soon's six-stages: observing phenomena, selecting participants, talking, talking again, writing, and confirming. Results: The results of data analysis were classified into 4 themes of overall experience related to chronic diseases: response to home health care, overcoming chronic disease, life reconstruction, and 22 categories as common denominators extracted from the elderly persons' lives. Conclusion: This research makes the experiences of the elderly living with chronic diseases more understandable, and it can be utilized as a effective nursing praxis to improve the quality of elderly persons with chronic diseases in hospital-based home care services.

Factors Influencing Medication Adherence and Status of Medication Use of the Elderly with Chronic Disease Taking Non-opioid Analgesics (만성질환 관련 비마약성 진통제 처방 노인의 약물사용 실태 및 약물복용이행에 영향을 미치는 요인)

  • Purpose: This study investigates the status of medication use of the elderly with chronic disease taking non-opioid analgesics and attempts to identify factors influencing medication adherence. Methods: Data were collected from September 1 to October 19, 2016. A structured questionnaire was used for face-to-face interview with a convenience sample of 161, elderly people with chronic disease taking non-opioid analgesics. The survey included questions about status of medication use, medication adherence, symptom experience, depression and family function. Data were analyzed using descriptive statistics, t-tests, ANOVA, Pearson's correlation coefficients, and stepwise multiple regression with IBM SPSS 23.0 program. Results: The mean score of medication adherence of the elderly with chronic disease was $4.48{\pm}2.35$. Experiences of side effects (${\beta}=.31$, p< .001), use of over-the-counter pain medication (${\beta}=.19$, p= .009), and family function (${\beta}=.16$, p= .031) were identified as significant predictors. The final model explained 18.0% of the variation of medication adherence of the elderly with chronic disease taking non-opioid analgesics (F= 12.30, p< .001). Conclusion: Therefore, as a strategy to improve medication adherence of the elderly with chronic disease, therapeutic intervention should be developed to improve family function and to manage with personalized plans considering experiences of side effects and use of over-the-counter pain medication.

Concept Analysis of Health Equilibrium in the Community-dwelling Elderly With Chronic Diseases (만성질환 노인의 건강 평형감에 대한 개념분석)

  • Purpose: The purpose of this study was to define and clarify the concept of 'equilibrium' in community-dwelling elderly persons with chronic diseases. Methods: A hybrid model was used for development of the concept of equilibrium. The model included a field study conducted in Seoul, Korea. Participants in this study included 12 elderly persons with chronic diseases who were taking medicine regularly. Results: The concept of equilibrium was found to be a complex phenomenon having meaning in two dimensions: the family-friendly and social relation-oriented dimensions. Four attributes and nine indicators were defined. Conclusion: Equilibrium was defined as the state of having health recognition, will power, and motive power (family-friendly dimension) and having a feeling of participation (social relation-oriented). Therefore, nurses who work with elderly persons with chronic disease in the community should be aware of the attributes and indicators of equilibrium in order to enhance the equilibrium of their elderly clients with chronic diseases.

Coping with Experiences in Multiple Chronic Diseases in the Rural Elderly (농촌 노인의 복합만성질환에 대한 대처 경험 - 문화기술학적 방법으로 -)

  • Purpose: The purpose of this study was to describe how the rural elderly cope with experiences in multiple chronic diseases. Method: Data were collected through participant observation and in-depth interview using ethnography. The participants were 9 women and 2 men who aged over 65, were living in rural community, and had experienced two or more chronic diseases. Results: According to the results of this study, those who had experienced multiple chronic diseases went through the stages of 'recognizing of revealed symptoms', 'discovering of disease', 'overcoming', 'neglecting', 'discovering another disease', 'being frustrated' and 'living with suffering'. Conclusion: The results of this study are expected to be utilized as basic materials to develop a nursing intervention program for effective management of chronic diseases.

  • This study was to suggest the roles of social enterprises in maintaining healthy life of the socially vulnerable by creating a customized care environment through chronic disease diet management targeting the elderly at home in need of care in the community. As for the subjects of this study, 102 in-home elderly people aged 65 or older (14 males & 88 females) who needed care in the community were provided with a diet management lunch box delivery service customized for chronic diseases. Theoretical education and cooking class on chronic diseases were provided to 15 volunteers with 10 sessions, and customized lunch boxes were provided to the elderly with chronic diseases at home on that day. In conclusion, it is believed that only by increasing the level of knowledge about chronic diseases and nutritional knowledge of the elderly will it be possible to establish a proper meal plan. Since the role of volunteers is important, it is necessary to systematically seek nutritional education methods for volunteers.
