A Study for Reorientation of Home Care Service at Community (일개 보건소 방문간호사업의 업무 분석)

  • The purpose of this study is to evaluate the community based home care service, and make reorientation for better service. The data was collected from the public health center, which was operated for one year, 1997. This case is evaluated and reoriented according to five elements of public health care system; system of resources for public health, organization and administration, health care delivery system of financing, management. In resources for public health, available health personnels are 15 physicians, 17 nurses, 11 nurse aides and 2 other persons. One professional health personnel take care of 609 clients, The equipments used for elderly and the disabled are 6 wheelchairs, 4 walkers, 1 hairwashers and 30 viberations. But these equipments are not enough to deal all clients. In organization and administration, planning and setting goals for community home care are made by the director, supervisor of family department and public health nurse. So there is no regular commitee for home care services in this community. The form of delivery of health care is focused on preventive health care. The important works of public health nurse are health education, preventive care for hypertension, D.M. and vaccination of communicable desease. In finaning system, funds come from central government(8.3%), local goverment(16.7%) and health center itself(3.8%), The services consist of health education, vaccination, clinical test and equipment. There are several local volunteers, which are local hospitals, a college, a christian association, a catholic association, a drivers association and a disabled association. The volunteer groups give physical and mental support to the clients. In management, this health center has three evaluation methods. One is done by local government, one is done by health center itself, and the other is done by clients with questionair. But the evaluation tools are deferent between agency. Home care services must be planned and evaluated. This public health center has to have more personnel, equipments, education for professional kowledge and meetings with community volunteer agencies.

Effects of Walking Speed on Foot Joint Motion and Peak Plantar Pressure in Healthy Subjects (정상인에서 보행속도가 발관절의 관절각과 발바닥 최대 압력 분포에 미치는 영향)

  • Many factors affect foot and ankle biomechanics during walking, including gait speed and anthropometric characteristics. However, speed has not been taken into account in foot kinematics and kinetics during walking. This study examined the effect of walking speed on foot joint motion and peak plantar pressure during the walking phase. Eighty healthy subjects (40 men, 40 women) were recruited. Maximal dorsiflexion and excursion were measured at the first metatarsophalangeal joints during walking phase at three different cadences (80, 100, and 120 step/min) using a three dimensional motion analysis system (CMS70P). At the same time, peak plantar pressure was investigated using pressure distribution platforms (MatScan system) under the hallux heads of the first, second, and third metatarsal bones and heel. Maximal dorsiflexion and excursion and excursion at the ankle joint decreased significantly with increasing walking speed. Peak plantar pressure increased significantly under the heads of the first of the first, second, and third metatarsal bones, and heel with increasing walking speed: three was no change under the hallux. There were no significant changes in maximal dorsiflexion or excursion at the first metatarsophalangeal joint. The results show that walking speed should be considered when comparing gait parameters. The results also suggest that slow walking speeds may decrease forefoot peak plantar pressure in patients with peripheral neuropathy who have a high risk of skin breakdown under the forefoot.

Management of IgA vasculitis nephritis (Henoch-Schonlein purpura nephritis) in Children

  • Immunoglobulin (Ig)A vasculitis nephritis (IgAVN), also referred to as Henoch-Schönlein purpura nephritis, is a relatively benign disease in children. However, two 24-year European cohort studies have reported high sustained rates of hypertension, severe proteinuria, and renal dysfunction in patients with IgAVN. Notably, the incidence and exacerbation rates of proteinuria, hypertension, and renal dysfunction during pregnancy were high even in women who recovered from IgAVN before pregnancy. Patients with IgAVN need lifelong care. Trials have been performed to investigate early biomarkers and genes associated with poor prognosis to identify high-risk patients in whom IgAVN may progress to severe renal disease. Urinary IgA/cr, IgM/cr levels, and HLAB35 and angiotensinogen gene expression were shown to be predictors of progression of IgAVN to severe renal dysfunction. The 2019 Single Hub and Access point for paediatric Rheumatology in Europe (SHARE) initiative group published guidelines for pediatric IgAVN, following the Kidney Disease: Improving Global Outcomes (KDIGO) guidelines established in 2012. Compared with the KDIGO guidelines, the SHARE guidelines recommend earlier corticosteroid administration in cases of mild proteinuria (>0.5 g/d). Clinical trials of targeted budesonide delivery to the distal ileum, monoclonal antibody targeting C5, eculizumab and anti-CD20 monoclonal antibody administration, among others are currently underway in patients with IgA nephropathy. It is expected that newer therapeutic agents would become available for IgAVN in the near future. This review summarizes IgAVN with emphasis on recently published literature, including possible preventive strategies, predictive biomarkers for progression of IgAVN, and various treatments.

Effect of Social-Economic Factors and Related factors of Health and Life-Style on Health Foods Intakes among the Middle Aged in the Jeonbuk Region (전북 일부지역 중년의 사회 경제적 요인, 건강 및 생활 관련인자가 건강식품 섭취에 미치는 영향)

  • This study examined health foods intakes and related variables among the middle aged(150 men and 159 women) in the Jeonbuk region. Health foods were classified into 4 groups including Chinese medicine(CM), toner foods(TF), vitamin or mineral supplements(VMS), and other manufactured health food supplements(MHFS). The number of people taking health foods were higher for those in their 50's than in their 40's. The consumption rate of TF in men was the highest among health foods, this result had significance. The reasons for taking health foods were recovery from fatigue, supplement of nutrients and making smooth body activity in general, but TF was used to increase of vigor. The consumption rate of health foods was a little different according to social-economic factors, namely, the consumption rate of CM was higher in people in rural than urban areas, those who graduated from middle school than university, blue color & self-employed as opposed to housewives and service workers, low level income than high level income, and Buddhism and no religion than Christian. The consumption rate had a correlation with the habits of smoking, alcohol drinking and exercise ; namely, the consumption rate of CM was higher than VMS and MHFS on smokers and alcohol drinkers. The more the frequency of exercise, the higher the consumption rate of TF, but the duration of the exercise was not correlated. This study suggests that middle aged people need nutritional education for the right recognition and selection of health foods and to consider the nature of each type of health foods.

Elimination of Mother-to-Infant Transmission of Hepatitis B Virus: 35 Years of Experience

  • Chronic hepatitis B viral (HBV) infection remains a major health threat, especially in high-prevalence areas. Most infants infected by mother-to-infant HBV transmission become chronic carriers. In Taiwan, many important preventive interventions have been implemented to block the perinatal transmission of HBV in the past 35 years. The first nationwide universal HBV vaccination program was launched in Taiwan in July 1984. The three-dose HBV vaccine completion rate reached 98.1% in 2018. The prevalence of Hepatitis B surface antigen (HBsAg) decreased from 9.8% in pre-vaccinated period in 1984 to 0.5% in the vaccinated cohort in 2014. The incidence of hepatocellular carcinoma in children aged 6-9 years significantly declined from 0.52 to 0.13 per 100,000 children born before and after 1984, respectively. Furthermore, we have performed a maternal HBV screening program during pregnancy since 1984, with the screening rate peaked at 93% in 2012. The HBsAg- and HBeAg-seropositive rate in pregnant women declined from 13.4% and 6.4% in 1984-1985 to 5.9% and 1.0% in 2016, respectively. To closely control perinatal HBV infection, we have administered hepatitis B immunoglobulin immediately after birth and checked the serum level of HBsAg and anti-HBs in high-risk babies born to HBsAg-seropositive mothers, irrespective of their HBeAg status, since July 2019. We have also adopted short-term antiviral treatments such as tenofovir 300 mg daily in the third trimester for highly viremic mothers and reduced the perinatal infection rates from 10.7 to 1.5%. Through all these efforts, we expect to meet the global goal of eliminating HBV infection by 2030.

A Basic Study on Strategy Development for Improving the Management of Multicultural Family Support Centers (다문화가족지원센터 운영활성화를 위한 전략개발 기초연구)

  • This paper proposes strategy development to improve the management of Multicultural Family Support Centers by analyzing four Multicultural Family Support Centers (MFSCs) in Chungcheongnam-do. The research was conducted from September 2012 to January 2013. Future directions for the four MFSCs are established based on a SWOT analysis on both the internal and external environments of the centers. Various strategic directions for the four MFSCs were identified based on these analyses. For center A (urban type, college contract type), the strategic directions included the specialization strategy, systemization strategy, empowerment strategy, standardization strategy, and achievement maximization strategy. The strategic directions for center B (urban-rural combined type, social welfare center contract type) included the improvement of multiculture awareness, specialization strategy, human resource development strategy, and networking strategy. For center C (farming and fishing community type, direct management type by local authority), the strategic directions included the specialization strategy, activation strategy, qualification strategy, and networking strategy. For center D (farming and fishing community type, social welfare center contract type), the strategic directions included the specialization strategy, business standardization strategy, human resource development strategy, and network expansion strategy. The suggested management strategy development for the four MFSCs can provide implications for the management of MFSCs in other areas in Korea.

  • The purpose of this study was to analyse the effects of youth depression and aggression and the effects of peer alienation in the process. To that end, the hierarchical regression analysis was used for 869 male teenagers and 800 female teenagers based on the data 7th wave of Korean Youth Panel Survey. Filly, In order to verify the meadiating effect of Peer Reject, bootstrapping method was used. The first seven panels Major analysis results are as follows. First, depression directly affected school adjustment of male and female adolescents. it turns out that the more depressed the young men and women are, the more aggressive they are. Second, In terms of depression and aggression among young men, peer alienation plays a role as a medium and that it does not work in the case of female. It also demonstrated that men's peer rejection has a media-effect in both gender depression and aggression and recommended discriminatory intervention based on gender.

Movies that seek after the truth and hierophanic time - Focused on and (구도의 영화와 히에로파니적 시간 - <희생>과 <꽃섬>을 중심으로)

  • This writing, focused on by Tarkovsky and by Song, Il-gon, examines moments of sage which is what Eliade described as 'hierophanic time'. These productions, which can be seen as movies that seek after the truth, show important paradoxical ideas and expressions in those moments. This is because symbolic messages from unrealistic and out of ordinary images are specifically shown. If they had to be compared, through the film by Tarkovsky, the technical civilization of contemporary society in an apocalyptic view is criticized and saved, or the prophetic will to not give up the desire for salvation is shown. In the process, the short shots which forces to show hierophanic time not only diversely visualized the author's ideological self-consciousness towards the conversion of the new world, but it also fulfills the metahistorical meanings mentioned by Eliade. However, in the film by Song, Il-gon, the realistic personal sufferings of three women is specifically shown first in extreme. They overcome their unsolvable problems through mysterious rituals and belief in miracles. In the scenes that include hierophanic time, there are many cases that cover Christian faith and the meaning of salvation. In other words, we can say that hierophanic time in , are related to conviction which led to moments of miracles in order to overcome reality. Therefore, even though there is a difference in the way authors use hierophanic time, the scenes that show it in the two movies display individuality of mythical imaginations disclosed by Eliade. This is because the general hope for reproduction, or meaningful symbols related to the ideal conversion of affairs are revealed.

A Study on Pregnancy, Delivery, and Infant Rearing Knowledge and Educational Need of Marriage Immigrant Women (국제결혼 이주여성의 임신·출산 및 영유아 양육 지식과 교육요구도)

  • Objectives: This study was performed to understand pregnancy, delivery, and infant rearing knowledge and educational need of marriage immigrant women to be utilized as a basic data for developing a suitable educational program. Methods: The subject of the study was composed of one hundred twenty two married immigrant women who live in South Korea. The method used in determining the population was the convenience sampling method. The data were gathered by means of personal interviews using questionnaires. Results: The mean score of pregnancy and delivery knowledge was 3.35, educational need was 3.64 out of 5, and infant rearing knowledge was 3.16, educational need was 3.66 out of 5. The pregnancy and delivery knowledge of the subject showed significant differences based on their present residential location and presence of children and their educational need varies according to their country of origin. The infant rearing knowledge on the other hand, showed notable differences according to presence of children while homeland and existence of children were the two major factors that greatly influenced the significant variation for educational need. Conclusions: The score of pregnancy, delivery and infant educational need were higher than knowledge. The score of pregnancy, delivery, and infant rearing educational need garnered similar results. Thus it is suggested that further researches should be conducted for the development, application, and verification of pregnancy, delivery, and infant rearing education programs that consider knowledge and educational need of immigrant married women in South Korea.

The Effect of Spiritual Well-being on the Mental Health of the Cho-Sun Tribal Women Residing in P.R. of China (중국거주 조선족 여성의 영적 안녕정도가 정신건강에 미치는 영향)

  • Background: Spirituality has been an important part of Transpersonal Psychology and is believed to have a large effect on the mental health because it has been systematized. The aim of this study was to determine the level of spiritual disposition on human beings along with its effects on one's mental health. Materials and Methods: The study targeted 400 women residing in Youn-Gil city of JiLin Prov., which is a district of the Cho-Sun tribe in China. Their spiritual well-being was studied using the Spiritual Well-being Scale-Korean Version. The spiritual well-being scale consists of 2 sub-scales of religious well-being and existential well-being. The study was evaluated using a lie scale, psychotic trend, and a combined anxiety-depression scale. The results were considered to be factors of one's mental health. The correlation between the spiritual well-being and each tendency was analyzed by regression analysis. Results: The total score of the Cho-Sun tribal women according to the spiritual well-being scale was 68.29 which was much less than the 100.65 of Korean Christian women. There was no significant correlation between the spiritual well-being and the Lie trend. However, it was found that 86%(344) of Cho-Sun tribal women scored above 70 in the Lie trend with a mean score of 74.57 which is higher than normal populations. Regarding the correlation between the spiritual well-being and psychotic trend, the psychotic trend became significantly higher when the religious well-being was at a high level. On the other hand, the psychotic trend became significantly lower when the existential well-being was at a high level. Regarding the correlation between the spiritual well-being and anxiety, the anxiety was significantly higher when the religious well-being was at a high level. However, the anxiety level was significantly low when the existential well-being was at a high level. Regarding the correlation between the spiritual well-being and depression, the depression level was somewhat significantly high when the religious well-being was at a high level. However, the depression level was significantly low when the existential well-being was at a high level. Conclusion: This study evaluated the effects of spiritual well-being on a person's mental health among Cho-Sun tribal women in Youn-Gil city of JiLIn Prov., P.R. of China. The results found that the religious well-being, which is a sub-scale of spiritual well-being, had negative effects while the existential well-being had positive effects on the mental health. These results proved that a person's religious disposition had negative effects on their mental health in a communitarian society.
